Surgical Experience of Idiopathic Hypertrophic Subaortic Stenosis - Report of A case - (특발성 비후성 대동맥판하 협착증의 수술적 치험 1례 보)

  • A 23-y-o male patient was suffered from intermittent fainting and dyspnea on exertion [NYHA Class IIIIV]. 2-D - Echocardiogram and cardiac catheterization with cineangiogram showed typical IHSS findings those were asymmetrical septal hypertrophy [ASH], systolic anterior motion of anterior mitral leaflet [SAM] which induced mild mitral regurgitation [Seller Grade I/IV] and pressure gradient about 60 mmHg between left ventricle and the aorta. Medical treatment with 8-adrenergic blockade [propranolol] and Ca" channel antagonist [Verapamil] had no response. So, we performed trans-aortic ventricular septal myotomy and mymectomy. Resected rectangular muscle bar was 1 Cm x 1 Cm x 4.5 Cm. Post-operative pressure gradient between the left ventricle and the aorta was less than 10 mmHg and SAM. was disappeared with decreased mitral regurgitation grade. Post-operative course was smooth and his symptoms and signs were free without any medication during 12 months follow-up.w-up.

Surgical Treatment of Pulmonary Atresia with VSD - A Report of 2 Cases - (심실중격결손증을 동반한 폐동맥 폐쇄증 [Pulmonary Atresia] 의 외과적 치료-2예 보고-)

  • Pulmonary atresia with VSD is uncommon congenital anomaly with high mortality in neonatal period. Recently we experienced surgical correction of 2 cases of pulmonary atresia with VSD. The first case was 7-year old female patient and diagnosed as pulmonary atresia with VSD combined PDA. So, total correction was undertaken which consisted of PDA ligation, patch repair of VSD, transannular enlargement of RVOT with woven Dacron vascular graft, and closure of PFO. Postoperative systemic Rt. ventricular and radial artery pressure ratio was 0.44 and her postoperative course was uneventful. The second case was 6-year old male patient diagnosed as pulmonary atresia with VSD and large systemic-pulmonary collateral arteries. There were two large systemic-pulmonary collaterals, one was simply controlled by ligation, but the other was considered to supply Rt. upper lung. So end to side anastomosis was performed to the RVOT patch. Postoperative systolic Rt. ventricular and radial artery pressure ratio was 0.54. During the follow up period he showed clinical picture of Rt. heart failure, which is relatively well controlled with anticongestive therapy.

  • The focus of this study is to investigate how personal display systems - a VDU (Visual Display Unit) and an HMD (Head Mounted Display) physiologically affect the body in virtual environments, and to evaluate differential effects of using the VDU and the HMD on physiological responses to mental stressful tasks (virtual reality flight simulation). As physiological variables, autonomic measures (heart rate, blood pressure), immune cells (leukocyte, neutrophil, lymphocyte), and hormones (catecholamine) were measured before and after experiments. Physiological data were measured in order to compare a level of mental stress on the VDU and the HMD. Increments in blood pressure (systolic (p<0.05), diastolic (p<0.1)), norepinephrin (catecholamine) (p<0.005), and neutrophils (p<0.2) of the group using the HMD showed a significant difference with the group using the VDU. Although, the heart rate was not statistically significant between two environments, differences of them quietly increased on the HMD more than on the VDU.

  • Pathophysiological implications of the vascular nitric oxide (NO)/cGMP pathway in hypertension were investigated. Sprague-Dawley rats were made deoxycorticosterone acetate (DOCA)-salt hypertensive for six weeks. The protein expression of endothelial constitutive NO synthase (ecNOS) and the tissue content of NO were determined in the thoracic aorta. The protein expression and catalytic activity of soluble guanylyl cyclase (GC) were also determined. Systolic blood pressure measured on the day of experiment was significantly higher in the experimental group than in the control. The hypertension was associated with decreases in the vascular tissue content of NO metabolites, concomitantly with the expression of ecNOS proteins. The protein expression of GC was not affected, while its catalytic activity was significantly decreased in hypertension. These results indicate that the high blood pressure is associated with a decreased activity of vascular NO/cGMP pathway in DOCA-salt hypertension.

  • The current study was performed to observe the effects of Panax ginseng (PG) and P. quinquefolium (PQ) on hemodynamics such as blood flow rate (BF), blood flow velocity (BV), heart rate (HR), systolic blood pressure (SBP), and diastolic blood pressure (DBP) and body temperature (BT) in healthy young men. This is a randomized, single-blind study observed during 6 hrs after orally single administration of PG and PQ groups. (omitted)

Penetrating Wound of the Heart: A Report of Three Cases (심장관통자상의 응급수술 치험 3례)

  • Three patients who sustained penetrating stab wound of the heart have been treated successfully by emergency thoracotomy in the Department of Thoracic Surgery, Chonnam University Hospital. There were two knife and one glass wound. The location of the injury were all on the right ventricle, but in one patient, it was penetrated to ventricular septum. All patients were in shock with a systolic pressure under 60 mmHg when admitted to the emergency room. In one of the three patients, blood pressure was not detectable and subsequently cardiac arrest. Two patients required immediate thoracotomy because of intrathoracic hemorrhage and increased pericardial tamponade and the other one required prompt thoracotomy because of sudden onset of cardiac arrest. There were no death postoperatively. Two patients are living without any complication in 4 years and 4 weeks after operation. One who had penetrating wound to ventricular septum, turned to cardiac decompensation, but he is living now in 4$\frac{1}{2}$ years after operation. Exploratory thoracotomy should be performed immediately in all the patients in whom a penetrating wound of the heart or pericardial tamponade following a penetrating wound of the chest wall is suspected.

Fermented ginseng, GBCK25, ameliorates hemodynamic function on experimentally induced myocardial injury

  • In the present study, we investigated whether treatment with GBCK25 facilitated the recovery of hemodynamic parameters, left ventricle systolic pressure, left ventricular developed pressure, and electrocardiographic changes. GBCK25 significantly prevented the decrease in hemodynamic parameters and ameliorated the electrocardiographic abnormality. These results indicate that GBCK25 has distinct cardioprotective effects in rat heart.

  • A 9-month-old intact male Maltese dog (1.52 kg) was diagnosed with a patent ductus arteriosus (PDA). Transcatheter occlusion of the PDA was performed by using the Amplatz canine duct occluder (ACDO). After occlusion, reflex bradycardia occurred and lasted for at least 15 h with normal systolic arterial pressure and slightly increased diastolic arterial pressure. The bradycardia slowly resolved, and the heart rate was normal in re-examinations after 7 and 30 days. This is the first case of reflex bradycardia after ACDO implantation, in which the bradycardia continued for a long time, even after recovery from anesthesia.

  • In order to find out the effects of vinyl house body, the author compared the physical and laboratory examinations between 96 cases of vinyl house farmers and 96 cases of land farmers who lived in 3 Gun's in Kyoungnam Province from July, 1st through September, 30, 1993. The examination included blood pressure, CBC, urinalysis, and blood chemistry. Systolic blood pressure and alkaline phosphates levels were lower in vinyl house farmers than in land farmers. But white blood cell counts were higher in vinyl house farmers and the other results were not different between two groups. It suggests that the environment of vinyl house - small, poorly ventilating, hot and humid environment - affected human body, then those facts should be considered during construction of vinyl houses.

  • Purpose: This study was to analysis the effect of self-foot reflexology(SRM) program on, level of hypertension. Method: A nonequivalent control group pretest-posttest design was used. For the experimental group, SRM was applied three times a week for 8wks. Test was done by measured x2-test, t-test, repeated measures ANOVA test. Result: There were significant decreases in systolic, diastolic blood pressure and difference of pretest and post test and between the experimental group and control group. Conclusion: Findings indicate that this study will contribute to develop nursing strategies for the regulation of the blood pressure.
