Cuffless Blood Pressure Estimation Based on a Convolutional Neural Network using PPG and ECG Signals for Portable or Wearable Blood Pressure Devices (휴대용 및 웨어러블 측정기를 위한 ECG와 PPG 신호를 활용한 합성곱 신경망 알고리즘 기반의 비가압식 혈압 추정 방법)

  • In this paper, we propose an algorithm for estimating blood pressure using ECG (Electrocardiogram) and PPG (Photoplethysmography) signals. To estimate the BP (Blood pressure), we generate a periodic input signal, remove the noise according to the differential and threshold methods, and then estimate the systolic and diastolic blood pressures based on the convolutional neural network. We used 49 patient data of 3.1GB in the MIMIC database. As a result, it was found that the prediction error (RMSE) of systolic BP was 5.80mmHg, and the prediction error of diastolic BP was 2.78mmHg. This result confirms that the performance of class A is satisfied with the existing BP monitor evaluation method proposed by the British High Blood Pressure Association.

  • In order to figure out the normal range, lower limit of hypertension and upper limit of hypotension of the blood pressure of Korean, authors had measured blood pressure according to Korotkow's method for 31,897 healthy persons as samples who were occupied different levels of the social class except cases who would seem to be the essential hypertension and had the diseases affecting to secondary hypertension. A following conclusion was induced by actual measurement and statistical reasoning. 1. The normal range and limits of hypo- and hypertension by sex and age groups of Korean were demonstrated in Table 1 and Figure 1. 2. The more aging the higher value of blood pressure in both sexes, especially women rather than men and systolic as to diastolic. 3. Generally, blood pressure values of female were lower than male, after 55 years of age, however, the crossing phenomenon was recognizable. 4. To settle the normal and abnormal ranges of the blood pressure of Korean, it was attempted that $M{\pm}1.282{\sigma}$ as normal range, $M+2{\sigma}$ as lower limit of hypertension and $M-2{\sigma}$ as upper limit of hypotension were calculated, and regression lines were drawn to adjust the biological variables and derive continuity from each age class. The blood pressure levels were becoming elevated as to getting increased of the body weight, particularly diastolic value at 40-49 age group in male and systolic value at 30-39 age group in female.

Effects of Uwhangchungsimwon(牛黃淸心元) on Cerebral Blood Flow and Systemic Blood Pressure in Humans (우황청심원이 정상인의 뇌혈류 및 혈압에 미치는 영향)

  • Uwhangchungsimwon(UC) has been used in the treatment of a wide variety of conditions including stroke, hypertension, arteriosclerosis, autonomic imbalance, mental instablity, etc in Korean traditional hospitals, In particular it is often initialy chosen for emergency care of acute stroke. The aim of this study was to evaluate the effect of UC on cerebral hemodynamics. Using transcranial Doppler ultrasound, we studied changes of mean flow velocity and pulsatility index(PI) of middle cerebral arteries (MCAs) from 11 health young volunteers who were administrated with 1 pill UC and 11 health controls who were not. We obtained hypercapnia with breath-holding and evaluated cerebrovascular reactivity with breath-holding index(BHI). Systolic blood pressure, diastolic blood pressure, and heart rate were measured using ambulatory blood pressure monitoring(ABPM). In UC administration group, the evaluation was performed during basal condition. and repeated at 20, 40, and 60 min after administration. In controls, the evaluation was performed at corresponding time intervals. Mean flow velocity in middle cerebral artery, systolic blood pressure, diastolic blood pressure, and heart rate did not change during the observation period and were not different between these two groups. However, administration of UC was associated with decreases in PI by $3.6{\sim}12.4%$ in BHI by $17.9{\sim}24.8%$ compared with pre-administration period. Decreases in PI and BHI with UC were significantly different compared with control group (p<0.05). These results indicate that UC decreases PI and BHI in cerebral artery, which is due to a dilation of cerebral resistance vessels.

  • The purpose of this research was to determine the relationship of the dietary nutrients to blood pressure among preschool children in Seoul and to concurrently study the effect of seasonal variance on the aforementioned relationship. The subjects of the study consisted of 203 preschool children aged four to six years. Anthropometric measurements of height, weight, pulse rate and blood pressure, urinary excretion of five cations(Na, K, Ca, P, Mg), creatinine and urea nitrogen and dietary questionaires concerning sodium, potassium calcium and phosphorus were taken during the two periods of summer(Aug. 1986)and winter(Feb, 1987). The results obtained are summarized as follows: 1) The daily urinary excretion of five cations, creatinine and urea nitrogen is summer and winter was as follows; The sodium content was 57.8 mEq in the summer and 59.4 mEq in the winter ; potassium 20.4 mEq and 23.0 mEq, respectively ; calcium, 5.5 mEq and 3.6 mEq, respectively ; and phosphorus, 27.4 mEq and 19.9 mEq, respectively. Only calcium and phosphours excretions in the urine showed significant differences per season(p<0.05). 2) The average dietary intake per day of sodium was 2349mg in the summer and 2155mg in the winter ; potassium consumption was 1425mg in the summer and 1448mg in the winter ; intake of calcium was 472mg in the summer and 500mg in the winter ; and phosphours consumption was 642mg in the summer and 634mg in hte winter. The sodium-to-potassium consumption ratio 1.6 and 1.5, respectively, in the summer and in the winter and the calcium-to-phosphorus ration was 0.7 in the summer and 0.8 in the winter. The dietary calcium intake showed significant differences between the seasons. 3) The principal source of sodium consumption among preschool children was from seasoning-including talbe salt, soy sauce and instant sauce-which accounted for higher then 45% of the sodium intake in both seasons. The main source of potassium was frutis and vegetables which accounted for 29.6% of the potassium intake in the summer and 25.7% in the winter. Relationship between Blood Pressure and Impairment of Cognitive Function In Some Rural Residents Aged 60-64 (일부 60-64세 농촌 거주자에서 혈압과 인지기능 장애와의 관계)

  • Objectives : Face-to-face interviews were conducted to investigate the relationship between blood pressure and the impairment of cognitive function in rural elderly (N=932) aged 60-64 in Dalsung County, April to September in 1996 Methods : Impairment of cognitive function was defined as a score of less than 23 by the Korean version of the Mini-Mental State Examination (MMSEK). Blood pressure was measured once in each subject using a portable automatic sphygmomanometer. Results : By univariate logistic regression on males, no category of systolic blood pressure bore statistical significance. Groups with diastolic blood pressures of, less than 80 mmHg, 90-94 mmHg, and more than 95mmHg had odds ratios of more than one compared with the reference group (80-89 mmHg). This was most significant in the group with blood pressures lower than 80 mmHg, which had a statistically significant odds ratio of 1.68 (95% confidence interval CI; 1.02-2.75). No category of blood pressure was statistically significant in females. Multivariate logistic regression for males, with adjustment for age, educational attainment, smoking, alcoholic drinking, body mass index, atherosclerotic disease, and antihypertensive medication use, did not alter the odds ratios significantly in terms of systolic blood pressure. However, the group with diastolic blood pressure of less than 80 mmHg had an increased odds ratio of 2.01 (95% CI; 1.15-3.52) compared with the reference group. In females, systolic blood pressure did not alter the odds ratio, but the group with a diastolic blood pressure of less than 80 mmHg had a statistically significant odds ratio of 0.57 (95% CI; 0.37-0.89). Conclusions : These results suggest that the relationship between blood pressure and cognitive function status is stronger diastolic than systolic blood pressure and that there is a complex relationship between blood pressure and cognitive function by sex.

  • Purpose: This study was done to examine the effects of abdominal breathing on VAS-Anxiety (VAS-A), blood pressure, peripheral skin temperature and saturation oxygen in pregnant women in preterm labor. Method: The study design was a matched control group interrupted time series. Forty-six women matched to gestational age were assigned to either the experimental group (26) or control group (20). Data were collected between March 2007 and May 2008. For the experimental treatment the women performed abdominal breathing 30 times, which took 5 minutes, and did one set of 5-minute abdominal breathing daily for three days. Data collection was done before and after the abdominal breathing to measure VAS-A, blood pressure, peripheral skin temperature and oxygen saturation. Descriptive, $X^2$, Mann-Whitney U tests were used to analyze the data with the SPSS/PC+Win 15.0 program. Result: For the experimental group there were significant decreases in VAS-A (Z=-4.37, p=.00), systolic blood pressure (Z=-3.38, p=.00), and an increase in skin temperature (Z=-4.50, p=.00) and oxygen saturation (Z=-3.66, p=.00). Conclusion: These findings suggest that abdominal breathing in pregnant women in preterm labor results in decreases in anxiety(VAS-A) including biological evidences such as systolic blood pressure, and increases in peripheral skin temperature and oxygen saturation. Further longitudinal study is needed on the lasting effects and obstetric and neonatal outcomes following abdominal breathing.

The Effects of Chu-ma Therapy on Decreasing Blood Pressure in Essential Hypertension (추마요법이 본태성 고혈압 환자의 혈압하강에 미치는 효과)

  • The purpose of this study is to evaluate the effects of Chu-ma therapy and to suggest that the therapy is an effective nursing intervention tool to reduce blood pressure. The research design employed was the non-synchronized research method with non- equivalent control group. A total of 30 people with essential hypertension, who were from forty to sixty five years old, participated in the study. The Chu-ma therapy was administered by every day for ten or fifteen minutes for eight weeks from 19, April to 13, June in 1999. In order to evaluate the effects of Chu-ma therapy, blood pressure of the two groups were measured once a week, and physiological parameters (epinephrine, norepinephrine, total cholesterol, HDL-cholesterol, triglycerides) were measured before and after the treatment. Collected data was analyzed by SAS package. The results of this study can be summarized as follows : 1) There were significant decrease in systolic blood pressure and diastolic blood pressure in the experimental group. 2) There were no significant changes in epinephrine, norepinephrine of the two groups. 3) There were significant decrease in total cholesterol and triglycerides, and HDL- cholesterol increased significantly in the experimental group. 4) The effect of Chu-ma therapy on the measured time on the blood pressure in experimental group was as follows: Both of systolic and diastolic blood pressures were significantly decreased after 5weeks. The result proved that Chu-ma therapy is an effective nursing intervention tool for clients with essential hypertenion. However further research is still necessary to compare the effect with the different periods and number of times for Chu-ma therapy.

The Relationship between Pulse Wave Velocity (PWV) and Risk Factors of Cardiovascular Disease including Serum Lipid Values (Pulse Wave Velocity(PWV)와 혈청지질수치를 포함한 심혈관 질환 위험 요인들과의 상관관계 연구)

  • Objectives : There are lots of reports that cardiovascular disease, including hypertension, cerebro-vascular accident, and coronary heart disease, is related to atherosclerotic changes. Increased serum levels of lipids could play a role in these changes. This study aimed to investigate the relationship between PWV and risk factors of cardiovascular disease, including serum lipid values. Methods : This study included 261 subjects ($49.32{\pm}11.79$ years, 112 male) who underwent PWV and serum lipid evaluation. We investigated the correlation between serum lipid values, blood pressure, body mass index (EMI) and PWV. Pearson's correlation and partial correlation analysis were applied to examine the relationship between PWV and risk factors of cardiovascular disease. Results : Serum triglyceride, total cholesterol, systolic blood pressure, and diastolic blood pressure were significantly correlated with PWV. Partial correlation coefficient adjusted by age yielded significant correlation between serum triglyceride, systolic blood pressure, diastolic blood pressure and PWV. Conclusion : In this study, it seems that there are significant relationships between PWV, triglyceride and blood pressure. We could suggest that PWV might have some relationships with Dam-eum and blood stasis in oriental theory.

Effect of Health Behavior and Obesity Indices on Blood Pressure in Peru Low-income Urban Areas (Baseline 조사 결과를 이용한 페루 도시 빈곤지역 주민의 건강행태와 비만이 혈압에 미치는 영향)

  • Objectives: The purpose of this study is analysing the influence of how health behavior such as smoking, drinking, and consuming salt and obesity affects to blood pressure and providing useful data for preventing ischemic heart disease of Peru which is main cause of death since 2012 and promoting healthy lifestyle of community. Methods: At the selected four districts in Lima and Callao region of Peru by KOICA Peru office, survey was conducted among adults over the age of 18. Excepted 19 incomplete people for the missing, 686 people were analysed in this survey. Results: There was no significant relationship between blood pressure and drinking and salt intake. By regression analysis, systolic blood pressure was significantly related with gender, age and abdominal obesity and diastolic blood pressure was significantly related with gender and abdominal obesity. Conclusions: As a result, diastolic and systolic blood pressure are mainly affected by waist circumference and gender. Therefore, It's need to be classifying male into vulnerable groups in the occurrence of hypertension and providing health education and information about hypertension to them. For improving health status of vulnerable group, awareness of hypertension prevention and changing unhealthy lifestyle to healthy lifestyle activities are necessary. In the end, it is necessary to prevent abdominal obesity through periodic health education.

An Effect of Qi Gong Gymnastics Program on the Physiopsychological Parameter in Essential Hypertension (기공체조프로그램이 고혈압 환자의 생리.심리적 지수에 미치는 영향)

  • The purpose of this study is to evaluate the effects of Qigong gymnastics exercise program on the physiopsychological parameter in essential hypertension. The design of the research was a one group pretest-posttest design. A total of 20 pateints with hypertension who were from forty to sixty-five years old participated in the study. The Qigong gymnastics program was carried out twice a week for thirty minutes or forty minutes for six weeks from 22, Sep. to 31, Oct. in 1997. In order to evaluate the effect of the Qigong gymnastics program physiological parameter (blood pressure, pulse rate, FVC, FEV1, Epinephrine, Norepinephrine, Cortisol) and psychological parameter(self-efficacy, Life satisfaction) were measured before and after the exercise program. Collected data was analyzed by the paired t-test with SAS package. The result are as follows : 1) There were significant decrease in systolic blood pressure and diastolic blood pressure, but there was no significant change in pulse rate. 2) There were no significant difference in FVC and FEV1 3) There were significant decrease in epinephrine, norepinephrine and cortisol. 4) There were significant improved in general self-efficacy and life satisfaction. 5) The effect of Qigong gymnastics program by measurement time on blood pressure and pulse rate were as follows : In a systolic blood pressure was gradually significant decreased from 1 week to 6 weeks and diastolic blood pressure was significantly decreased after 4weeks. But there was no significant change in pulse rate. This results suggest that Qigong gymnastics are an appropriate nursing intervention for clients with hypertension. For further research is necessary to reevaluate the effect with a letter experimental design and longer period than 8 weeks for the Qigong gymnastics program.
