Peripheral Vestibular Syndrome in a Cat with Foreign Body Otitis Media/Interna

  • A 15-month-old, intact male, domestic short hair cat was presented with ataxia, protrusion of the right third eyelid, and anorexia. Clinical signs were firstly noted after ear cleaning at home. Symptoms of the Homer's syndrome were evident. However, postural and proprioceptive reaction deficits were not detected on neurologic examination. Otoscopic examination revealed foreign body in right ear canal. A diagnosis of peripheral vestibular syndrome was made based on results of physical examination including neurologic and otic examination, blood work, and magnetic resonance imaging (MRI). Based on these examinations, the present patient was definitely diagnosed as otitis medial interna induced by foreign body. The vestibular signs were resolved 10 days after removal of cotton tips in right ear canal. This case report indicates that home-care cotton swab can iatrogenically induce otitis media/interna in cats.

Topical Application of 0.1% Tacrolimus Ointment for Treatment of Sinus Refractory Pododermatitis Secondary to Atopy in a Dog

  • An 8-year-old, castrated male Shih-tzu was referred due to relapsing pododermatitis and generalized pruritus. On physical examination, right forepaw showed swelling and serosanguinous exudates from sinus tract on dorsal paw. There were no remarkable findings on complete blood count (CBC), serum chemistry, and radiologic examination. On cytological examination of exudates from sinus tract, phagocyted bacteria and numerous degenerative neutrophils were noted. Results of deep skin scraping and plucking hair examination were unremarkable. Skin biopsy was performed and ruled out other skin diseases. Histopathology showed epidermal hyperplasia and diffuse mononuclear cell inflammation in dermal layer. Moreover, pyogranulomatous inflammation was demonstrated in subcutaneous layer. This case was clinically diagnosed as pododermatitis secondary to atopic dermatitis (AD). Clinical signs of pododermatitis were gradually improved following topical application of tacrolimus 0.1% ointment. This case report describes that tacrolimus ointment can be applicable for the treatment of refractory pododermatitis.

Community-acquired Legionnaires' Disease in a Newly Constructed Apartment Building

  • Objectives: Legionnaires' disease (LD) is a severe type of pneumonia caused by inhalation of aerosols contaminated with Legionella. On September 22, 2016, a single case of LD was reported from a newly built apartment building in Gyeonggi province. This article describes an epidemiologic investigation of LD and identification of the possible source of infection. Methods: To identify the source of LD, we interviewed the patient's husband using a questionnaire based on the Legionella management guidelines from the Korea Centers for Disease Control and Prevention. Water samples from the site were collected and analyzed. An epidemiological investigation of the residents and visitors in the apartment building was conducted for 14 days before the index patient's symptoms first appeared to 14 days after the implementation of environmental control measures. Results: Legionella pneumophila serogroup 1 was isolated from the heated-water samples from the patient's residence and the basement of the apartment complex. Thirty-two suspected cases were reported from the apartment building during the surveillance period, yet all were confirmed negative based on urinary antigen tests. Conclusions: The likely source of infection was the building's potable water, particularly heated water. Further study of effective monitoring systems in heated potable water should be considered.

Global Sex Differences in Cancer Mortality with Age and Country Specific Characteristics

  • Background: The cancer research literature suggests that women, especially premenopausal women, have lower cancer mortality rates than men. However, it is unclear if that is true for populations at all age levels in all countries and what factors affect such sex differences. This paper attempts to fill that gap. Materials and Methods: Sex- and country-specific cancer mortality data were statistically analyzed with particular attention to geographic, social, and economic factors that may affect the sex differences. Results: The sex differences were age and country specific, rather than universal. Premenopausal women actually tend to have a disadvantage compared to men or postmenopausal women. Male cancer mortality appears to be the affecting factor in explaining variations in sex differences. Latitude of residence and literacy rate are the affecting factors in cancer mortality and sex differences. African and Latin American countries tend to have a female disadvantage, while East Asian and Eastern European countries are more likely to have a female advantage. Conclusions: The findings challenge the cancer mortality literature and indicate that the sex differences and their possible causes are more complicated than the current literature suggests. They also highlight the urgency of adapting age- and country- specific health systems and policies to better meet the needs of younger women.

Clinical and Clinicopathological Findings of Naturally Occurring Hyperadrenocorticism in Dogs: A Retrospective Study of 17 Cases

  • A retrospective study of 17 dogs with hyperadrenocorticism (HAC) was performed to evaluate the clinical and clinicopathological outcomes in dogs with pituitary-dependent HAC or adrenal gland-dependent HAC. Sixteen dogs were monitored at day 10, 30 and 90 then every 3 month after first presentation. In each examination, physical examination and ACTH stimulation test and client questionnaire were performed. In 17 cases, eight dogs were successfully treated with mitotane therapy (median dose 16.7 mg/kg; range 5.3 to 50 mg/kg, SID); another one dog showed favorable response, but the clients declined to further treatment before the full control of the disease had been achieved; four dogs showed poor response to treatment. One dog was euthanized due to septicemia. These results showed canine HAC can be medically managed successfully without side effects at the doses used in this study.

축산관련차량 이동에 따른 도로의 에어로졸 발생량 분석 (Aerosol Emission from Road by Livestock Transport Vehicle Movement)

  • Most of livestock houses are concentrated in certain area with mass rearing system resulting in rapid spread of infectious diseases such as HPAI (highly pathogenic avian influenza). The livestock-related vehicles which frequently travel between farms could be a major factor for disease spread by means of transmission of airborne aerosol including pathogens. This study was focused on the quantitative measurement of aerosol concentration by field experiment while vehicles were passing through the road. The TSP (total suspended particle) and PM10 (particle matter) were measured using air sampler with teflon filter installed downward the road with consideration of weather forecast and the direction of road. And aerosol spectrometer and video recorders were also used to measure the real-time distribution of aerosol concentration by its size. The results showed that PM2.5 was not considerable for transmission of airborne aerosol from the livestock-related vehicle. The mass generated from the road during the vehicle movement was measured and calculated to 241.4 ${\mu}g/m^3$ by means of the difference between TSP and PM2.5. The dispersion distance was predicted by 79.6 m from the trend curve.

Assessing Reliability and Validity of an Instrument for Measuring Resilience Safety Culture in Sociotechnical Systems

  • Background: Safety culture, acting as the oil necessary in an efficient safety management system, has its own weaknesses in the current conceptualization and utilization in practice. As a new approach, resilience safety culture (RSC) has been proposed to reduce these weaknesses and improve safety culture; however, it requires a valid and reliable instrument to be measured. This study aimed at evaluating the reliability and validity of such an instrument in measuring the RSC in sociotechnical systems. Methods: The researchers designed an instrument based on resilience engineering principles and safety culture as the first instrument to measure the RSC. The RSC instrument was distributed among 354 staff members from 12 units of an anonymous petrochemical plant through hand delivery. Content validity, confirmatory, and exploratory factor analysis were used to examine the construct validity, and Cronbach alpha and test-retest were employed to examine the reliability of the instrument. Results: The results of the content validity index and content validity ratio were calculated as 0.97 and 0.83, respectively. The explanatory factor analysis showed 14 factors with 68.29% total variance and 0.88 Kaiser-Meyer-Olkin index. The results were also confirmed with confirmatory factor analysis (relative Chi-square = 2453.49, Root Mean Square Error of Approximation = 0.04). The reliability of the RSC instrument, as measured by internal consistency, was found to be satisfactory (Cronbach ${\alpha}=0.94$). The results of test-retest reliability was r = 0.85, p < 0.001. Conclusion: The results of the study suggest that the measure shows acceptable validity and reliability.

Effectiveness of Aromatherapy with Light Thai Massage for Cellular Immunity Improvement in Colorectal Cancer Patients Receiving Chemotherapy

  • Background: Patients with colorectal cancer are usually treated with chemotherapy, which reduces the number of blood cells, especially white blood cells, and consequently increases the risk of infections. Some research studies have reported that aromatherapy massage affects the immune system and improves immune function by, for example, increasing the numbers of natural killer cells and peripheral blood lymphocytes. However, there has been no report of any study which provided good evidence as to whether aromatherapy with Thai massage could improve the immune system in patients with colorectal cancer. The objectives of this study were to determine whether the use of aromatherapy with light Thai massage in patients with colorectal cancer, who have received chemotherapy, can result in improvement of the cellular immunity and reduce the severity of the common symptoms of side effects. Materials and Methods: Sixty-six patients with colorectal cancer in Phichit Hospital, Thailand, were enrolled in a single-blind, randomised-controlled trial. The intervention consisted of three massage sessions with ginger and coconut oil over a 1-week period. The control group received standard supportive care only. Assessments were conducted at pre-assessment and at the end of one week of massage or standard care. Changes from pre-assessment to the end of treatment were measured in terms of white blood cells, neutrophils, lymphocytes, CD4 and CD8 cells and the CD4/CD8 ratio and also the severity of self-rated symptom scores. Results: The main finding was that after adjusting for pre-assessment values the mean lymphocyte count at the post-assessment was significantly higher (P=0.04) in the treatment group than in the controls. The size of this difference suggested that aromatherapy with Thai massage could boost lymphocyte numbers by 11%. The secondary outcomes were that at the post assessment the symptom severity scores for fatigue, presenting symptom, pain and stress were significantly lower in the massage group than in the standard care controls. Conclusions: Aromatherapy with light Thai massage can be beneficial for the immune systems of cancer patients who are undergoing chemotherapy by increasing the number of lymphocytes and can help to reduce the severity of common symptoms.

의학교과서를 통해 본 북한예방의학 내용분석에 관한 연구 (The State of the Art of Preventive Medicine in North Korea With Reference to the Content Analysis of a Medical Textbook)

  • Objectives : The most frequently cited health related slogan in North Korea is that socialistic medicine is preventive medicine. It implies that North Korea puts preventive medicine at the operational center of its national health care system. This study aimed at examining and comparing preventive medicine practices in North Korea with those of South Korea. Methods : Efforts have been made to obtain a textbook for analysis its contents. Many people have iassisted in the study by joining the interview. Some of these people are as follows : a former professor of PyongYang Medical School, NK physicians living in South Korea, WHO staffs, diplomatic officials, etc. The major items of analysis consisted of industrial medicine and hygiene, nutrition, school health, epidemiology, health statistics and heath policy & management. Results : Public health philosophy is finely noted and well integrated in the operation of the North Korean national health care system, particularly in the area of industrial medicine and hygiene. Preventive medicine with a strong health surveillance system spanning a number of broad social organizations is a major tool to improve the health of the people in North Korea. The emphasis on preventive medicine has a close relationship with the 'Juche Philosophy' and the shortage of pharmaceuticals and medical equipment. To cope with the shortage problem, North Korean health workers are encouraged to grow medicinal herbs. We have found that they put little effort into teaching newly emerging diseases, such as AIDS, VDT syndrome, hazards of EMF, and agricultural chemical poisonings. Of the subjects of the preventive medicine text, 78.9% coincide with those of South Korean industrial health manuals and 34.2% with South Korean epidemiology texts. However, an absolute difference was found to exist between the health policies and management systems. Conclusion : In North Korea, the concept of preventive medicine functions as the basic philosophic strategy of the national health care system. It differs greatly from the South Korean system in both practice and educational content. Its contribution to society is simply incomparable to that of South Korea. More communication and further study is called for in order to improve the preventive medicine practices in the future.

손상의 역학과 대책 (Epidemiology and Control of Injury)

  • Injury has recently become a major world-wide health problem. Injury related deaths occur in many actively working young people and produce major social and economical losses. However health related specialists do not recognize the importance of injury and there have not been many studies related to this problem. This research studied the trends of injury related research in Korea, mortality rate and prevalence rate, socio-economical losses and control in Korea and out of the country, based on literature from Korea and without such as statistical yearly reports on causes of deaths and OECD health reports, as well as WHO web sites. Studies in Korea about injury were very few, with 9 in the 1960's, 5 in the 1980's, 4 in the 1990's and 5 in 2000's. Mortality rate of injury was higher in Korea than in England, USA or Japan, especially in car accidents, suicide and falls. In Korea, the yearly trends in mortality rates were highest in car accidents but those rates are falling, suicide is steadily rising, with highest rate in 2003. Falls is in second rank with no change in rates. In 2003, the ten causes of death in Korea were suicide in 5th rank, transport accidents in 7th rank, and falls in 10th rank. Considering age groups, in the teens, transport accidents were 1st rank, in the 20's and 30's, suicide was 1st rank, and although there were some differences, falls, drowning, assault, fire were in the top 10. Prevalence rates of injury could not be known, but in 2001, according to the National Health and Nutrition Survey, lifelong injury was 10%, and yearly major injury was 1.3%, major injury for two weeks was 0.1%, and minor injury was 10%. In other foreign countries, injury has become to be recognized as a major health related problem, and much programs are being set up to reduce injury related deaths and injuries. WHO is putting much effort in prevention of violence and transport accidents, and in the USA, Canada and Europe, there are injury surveillance systems. Recently, as suicide is increasing rapidly and providing much problems, each country are managing suicide prevention programs. In Korea, Ministry of Construction and Transportation is managing and guiding the policies for prevention of transport accidents. In September of 2004, the Ministry of Health and Welfare has set up a 5 year plan of suicide prevention.
