비행절차설계과정에서 적용 가능한 위험평가기준 연구 (A Study on the Risk Assessment Criteria to be applicable for Establishing Flight Procedure Design Process)

  • Risk assessment should be processed when physical circumstances of airspace such as establishment of new flight procedures or reconstruction of existing airspace are planned to change and one of the alternatives for the change have to be selected. This is an effort to find the best alternative which is able to maintain at or above the acceptable risk level. ICAO and its contracting states provide specialized guidance material for 'Safety Management System' relating to handing airspace matters. These manuals include a conceptual framework for managing safety as well as some of the systemic processes and criteria used to meet the objectives of a State's safety programme. This criteria must be established in compliance with the State's laws and regulations for air safety and for the fulfillment of the State's safety goals. This study is to carry out the risk assessment criteria through literature reviews relevant to the safety management, and to propose the results of criteria to be applicable for establishing flight procedure design process.

공과계열 대학생을 위한 맞춤형 테크노 리더십 캠프 프로그램 개발 (Development of Customized Technological Leadership Camp Programs for Engineering Students)

  • 연구의 목적은 공과계열 대학생들이 미래의 조직에서 테크노 리더가 되기 위한 기초소양을 쌓을 수 있는 맞춤형 테크노 리더십 캠프 프로그램을 개발하는 것이다. 테크노 리더십 캠프 프로그램은 체제적 교수설계 방법론의 절차에 따라 개발되었다. 문헌분석과 사례분석을 통해 테크노 리더십 프레임웍이 개발되었고 중요도-수행도 분석법을 활용한 요구분석을 통해 핵심개발 테크노 리더십 역량이 도출되었다. 요구분석 대상은 A대학교 공과계열 대학생 202명과 동일대학과 산학협력관계에 있는 기업체관리자 22명이었다. 도출된 핵심개발 테크노 리더십 역량을 중심으로 기본과정과 심화과정의 수준별 테크노 리더십 캠프 프로그램이 개발되었고 2회에 걸친 전문가 검토의 과정을 통해 수정 보완되었다.

후두백반증으로 발현된 후두 칸디다증 1예 (A Case of Laryngeal Candidiasis Presented as Laryngeal Leukoplakia)

  • Candida exists in the oral cavity as normal flora, which is cultured in 7% of the population. And the development of candidiasis is usually related to the systemic or local immunosuppressed status such as diabetes, long-term antibiotics, steroid, radiation therapy or chemotherapy. However, isolated laryngeal candidiasis in immunocompetent patients is a rare entity with fewer than 40 cases reported in the world. Symptoms of laryngeal candidiasis are variable such as hoarseness, dysphagia, or odynophagia according to its extent, but it has clinical importance because of its resemblance with laryngeal premalignant or malignant lesions. Diagnosis is made by biopsy under direct laryngoscopy with special staining to identify the hyphae. In this article, we report a case of laryngeal candidiasis presented as leukoplakia localized on vocal fold with literature reviews.

자세와 균형 조절에 관한 연구 (A Study on Control of Posture and Balance)

  • The very definitions of posture and balance have changed, as has our understanding of the underlying neural mechanisms, In rehabilitation science, there awe at least two different conceptual theories to describe the neural control of posture and balance : the reflex/hierarchical theory and system theory. A reflex/hierarchical theory suggests the posture and balance result from hierarchically organized reflex responses triggered by independent sensory systems. The systems approach suggests that action emerges from an interaction of the individual with the task and environment. That is to say, the systems approach implies that the ability to control our body's position in space emerges from a complex interaction of musculoskeletal and neural systems, collectively referred to as the postural control system. The specific organization of postural systems determined both by the functional task and the environment in which it is being performed, The postural control system is divided into three basic functional components for assessment : 1) musculoskeletal components, 2) motor coordination components, and 3) sensory organization components. It is proposed that a systemic functional understanding of human balance is critical to effective programs for balance rehabilitation. Thus, this article briefly reviews the basic functional components to consider in designing treatment plan and for the benefit of the balance assessment.

치과진료실에서 흔한 긴급상황의 처치 (How to cope with medical emergencies in the Dental Clinic)

  • Conceptually, the emergency is classified as an urgency and an emergency. The Urgency is not immediately life threatening, but could become so if not resolved promptly. So, it requires prompt patient care same as in the emergency situation. However, the emergency is immediately life threatening and requires immediate action, such as calling 119 and basic life support. Most medical emergencies in the dental clinic cases are urgencies. The incidence of true emergencies is approximately 1/1,000,000. Adequate managing medical urgencies are important because the chance of encountering medical emergencies in the dental clinic is high and higher these days especially because of rapidly aging Korean society. Many dentists indicate that many dentists feel difficulties to recognize and treat medical problems. This paper reviews the concept of medical emergencies and how to cope with commonly occurring urgencies in the dental clinic such as loss of consciousness, hypoglycemia, hyperventilation syndrome. The best treatment for medical urgencies and emergencies in the dental clinic is prevention. Also, it is required to make preparation for emergency situations such as CPR education for dentists and being well-acquainted with equipment and drugs for the emergency care as well as to systemic medical evaluation, patient monitoring, and sedations for controlling patient's anxiety and pain. In this paper, simple algorithms based on guidelines for common urgencies in the dental clinic are suggested. In conclusion, every dentist has competencies to do the urgency care adequately and basic life support. Also, advanced cardiac life support is strongly recommended when sedation is performed in the clinic.

분만경험 측정도구의 체계적 고찰 (A Systematic Review of Birth Experience Assessment Instrument)

  • Purpose: This study aimed to conduct a systematic review and to describe characteristics of the birth experience assessment instrument. Methods: Literature related to the development of the birth experience assessment instrument was examined using a systematic review method. A literature search was conducted using the keywords as '[normal birth]; [$satisfac^*$ OR care quality]; [instrument OR scale] AND (development)' through PubMed, CINAHL, SCOPUS, PsycINFO, and RISS. The search used quality appraisal through QUADAS (Quality Assessment of studies of Diagnostic Accuracy included Systemic reviews) yielding 17 records. Results: The birth experience assessment instrument was categorized for instrumental characteristics: birth satisfaction (n=8), perception of labor experience (n=5), and birth care quality assessment in normal and operative birth experiences (n=4). Important key elements for content characteristics were as follows: nursing practice (n=10), pain control (n=5), environment (n=5), participation (n=4), and support (n=4). Conclusion: This study demonstrated that the birth experience instrument is appropriate for measuring quality of birth care in various clinical conditions. This review of the birth experience instrument reports that an appropriate psychometric tool for enhancing quality of birth care is important.

Recombinant zoster vaccine (Shingrix®): a new option for the prevention of herpes zoster and postherpetic neuralgia

  • Postherpetic neuralgia (PHN) is a challenging condition for pain management specialists. The prevention of herpes zoster (HZ) and subsequent PHN in individuals aged 50 years and older, via the development of new vaccines, is an ongoing research project. The live zoster vaccine (LZV, Zostavax®) was the first proof of concept that vaccination could prevent HZ, but LZV cannot be used in various immunecompromised patients. This led to the development of a new non-live recombinant zoster vaccine (RZV, Shingrix®). This RZV has shown promising results in many clinical trials, with high reactogenicity and similar systemic adverse effects compared to those of LZV. The National Advisory Committee on Immunization has recommended LZV as a standard vaccine for HZ prevention in adults ≥ 50 years of age, but no studies directly comparing the safety and efficacy of RZV and LZV vaccines have been conducted. This article reviews the brief history, efficacy, and safety of the two vaccines and discusses the advantage of RZV over LZV based on the available literature.

Probiotics-Mediated Bioconversion and Periodontitis

  • Novel bioactive metabolites have been developed through a bioconversion of dairy products or other foods using probiotics isolated from dairy products or other fermented foods. These probiotics-mediated bioconversion (PMB) metabolites show antioxidant, anti-inflammatory, antimicrobial, epithelial barrier, and anticancer activities. In addition, the effect of PMB metabolites in periodontitis is recently reported in several studies. Periodontitis is a chronic inflammatory disease caused by infections, and the tooth support tissue is destroyed. Common treatments for periodontitis include scaling and root planning with systemic antibiotics. However, the overuse of antibiotics has led to the emergence of drug-resistant microorganisms and disturbs the beneficial bacteria, including lactobacilli in the oral cavity. For this reason, PMB metabolites, such as fermented milk, have been suggested as substitutes for antibiotics to reduce periodontitis. This paper reviews the recent studies on the correlation between periodontitis and PMB metabolites and classifies the efficacy of major PMB metabolites for periodontitis. The review suggests that PMB is effective for periodontitis, and further studies are needed to confirm the therapeutic effect of PMB metabolites on periodontitis.

국방 안전관리시스템 개선을 위한 주요 외국군 사례분석 연구 (Comparative Analysis of Foreign Armed Forces' Safety Management System for the Development of the R.O.K Defense Safety Management System)

  • Department of Defense, Republic of Korea has put in significant efforts towards safety over the years by developing defense safety management systems, such as establishing safety management organizations and enacting laws and regulations. Therefore, the number of casualties due to military activities has been reduced dramatically. However, many errors and problems are present in the safety management system (SMS) owing to a lack of proper SMS. That brings inefficiency and discontinuity in SMS. Hence, typical phenomena such as lack of safety management professionals and reactive safety management activities have not been properly corrected. This study reviews various academic papers on the SMS and research reports of major foreign institutions to redefine the concept and components of the defense safety management system to ultimately suggest improvement in the R.O.K defense safety management system. Additionally, the safety management system of the U.S. and British forces, considered to be the leading safety management system, were analyzed and compared to R.O.K defense SMS to derive their implications.

Medication-Related Osteonecrosis of the Jaws: A Literature Review

  • Medication-related osteonecrosis of the jaw (MRONJ) is a serious side effect of antiresorptive agents and bone-modifying agents. It is of the utmost importance to know the management of the MRONJ to improve the patient's quality of life. This study comprehensively reviews the current definitions of MRONJs, and antiresorptive medications, clinical manifestation and staging, risk factors, treatment strategies, and prevention methods of MRONJ. The disease is defined as an exposure of bone and osteonecrosis of the jaw in the oral cavity for at least 8 weeks in patients taking antiresorptive drugs or antiangiogenic agents and with no history of radiotherapy treatment of the jaws. Many articles have reported risk factors associated with MRONJ such as systemic diseases, antiresorptive medication, oral infection, and poor oral hygiene. Osteonecrosis and antiresorptive medications including bisphosphonate and denosumab have been strongly associated, but the pathology of MRONJ is only limited. Hence, an effective and appropriate management and treatment for MRONJ is still to be defined. The objectives of MRONJ treatment are to minimize osteonecrosis and relieve symptoms, and many treatments are suggested from conservative treatment to marginal resection, but this remains controversial. Appropriate treatment of MRONJ remained difficult, although many studies are being covered.
