Secondary amyloidosis complication of Crohn disease treated with infliximab (크론병에 동반된 속발성 아밀로이드증에서 infliximab 치료)

  • Secondary systemic (AA) amyloidosis is a severe complication of progressed Crohn disease (CD) characterized by the deposition of amyloid A in body organs and tissues. Various therapeutic approaches have been recommended, however there is still no effective treatment. Recently, several case reports have demonstrated the effects of anti-tumor necrosis factor-${\alpha}$ therapy in patients with AA amyloidosis associated with CD. We report on a 35-year-old female patient with CD complicated by AA amyloidosis in the gastrointestinal tract and renal involvement, who was treated with infliximab. The infliximab therapy improved the gastrointestinal symptoms and decreased the serum creatinine.

The Analysis of Factors on the Service-Linkage of Long-term Care Workers for the Elderly (일부 노인 장기요양보호기관 종사자간의 서비스연계 조사)

  • Purpose: This study was to analyze the factors that affect the service relationship of long term care workers for the elderly and to provide basic resource for the successful connection of long term care services. Methods: 259 subjects who were engaged in long term care units completed a self-administered questionnaire that measured the extent of service linkage among one another. The Cronbach's ${\alpha}$ score determined the internal consistency of the acquired data and the discriminated validity was estimated by Pearson's correlation coefficient. Multiple regression analysis was conducted to investigate the influence of the known factors on the service linkage. Results: Acceptance and participation negatively influenced on the service linkage. Reliance, comprehension, recognition on service, and frequent contact with others positively activated the service linkage of long term care workers. Conclusion: The establishments of systemic training courses providing education that emphasizes reliability and recognizes other services, including work environment to contact easily are needed to improve the service-linkage of long-term care workers for the elderly.

Postoperative Radiation Therapy in Resected N2 Stage Non-Small Cell Lung Cancer (N2 병기 비소세포 폐암의 수술후 방사선치료)

  • A total of forty patients with resected N2 stage non-small cell lung cancer treated with postoperative adjuvant radiation therapy between Jan. 1975 and Dec. 1990 at the Department of Radiation Oncology, Yonsei University College of Medicine, Yonsei Cancer Center were retrospectively analysed to evaluate whether postoperative radiation therapy improves survival. Patterns of failure and prognostic factors affecting survival were also analysed. The 5 year overall and disease free survival rate were $26.3\%,\;27.3\%$ and median survival 23.5 months. The 5 year survival rates by T-stage were $T1\;66.7\%,\;T2\;25.6\%\;and\;T3\;12.5\%.$ Loco-regional failure rate was $14.3\%$ and distant metastasis rate was $42.9\%$ and both $2.9\%.$ Statistically significant factor affecting distant failure rate was number of postitive lymph nodes(>=4). This retrospective study suggests that postoperative radiation therapy in resected N2 stage non-small cell lung cancer can reduce loco-regional recurrence and may improve survival rate as compared with other studies which were treated by surgery alone. Further study of systemic control is also needed due to high rate of distant metastasis.

Inhallation of e-Cigarette Cartridge Solution Aggravates Allergen-induced Airway Inflammation and Hyper-responsiveness in Mice

  • Electronic cigarettes (e-cigarettes) are becoming increasingly popular worldwide and their cellular effects warrant further evaluation. In this study, we investigated the effects of an e-cigarette cartridge solution on allergen related asthmatic airway inflammation (AI) and airway hyperresponsiveness (AHR), when it is delivered by intratracheal route in mice. Asthmatic AI and AHR were induced by systemic sensitization to ovalbumin (OVA) followed by intratracheal, intraperitoneal, and aerosol allergen challenges in BALB/c mice. The cartridge solution of e-cigarette (containing 16 mg/ml nicotine) was diluted 50 times and $100{\mu}l$ of the diluted solution was intratracheally instilled to OVA-sensitized (OVA-S) mice two times a week for 10 weeks. Long-term e-cigarette inhalation elicited no remarkable changes in the activities of alanine aminotransferase (ALT), aspartate aminotransferase (AST), lactate dehydrogenase enzymes in serum, however, increased infiltration of inflammatory cells including eosinophils, into airways from blood, aggravated the asthmatic AI and AHR, and stimulated the production of cytokines such as interleukin (IL)-4, IL-5 and IL-13, and OVA-specific IgE production. Our data suggest that the inhalation of e-cigarette solutions can function as an important factor to exacerbate the allergy-induced asthma symptoms. Further studies are needed to address the effects of e-cigarette solutions on human health.

Effect of Rivaroxaban on Fibrinolytic Therapy in Massive Pulmonary Embolism: Two Cases

  • The risk of dying from a pulmonary embolism (PE) is estimated to be about 30% if inotropic support is required and no cardiopulmonary arrest occurs. Fibrinolysis in massive PE is regarded as a life-saving intervention, unless there is a high risk of bleeding following the use of the fibrinolytic therapy. Rivaroxaban is an oral factor Xa inhibitor, however its anticoagulation effects before or after administration of fibrinolytics in massive PE are still unknown. Two patents were admitted: 61-year-old woman with repeated syncope, and a 73-year-old woman was admitted with dyspnea and poor oral intake. Systemic arterial hypotension with radiologic confirmation led to a diagnosis of massive PE in both patients. Rivaroxaban was administered before in one, and after firbrinolytic therapy in the other. One showed similar efficacy of rivaroxaban with currently used anticoagulants after successful fibrinolysis, and the other one without antecedent administration of the fibrinolytic agent showed unfavorable efficacy of rivaroxaban.

Renotropic Action of Intracerebroventricular Prostaglandin $F_{2{\alpha}}$ in the Dog (개에 있어서 측뇌실내(側腦室內) Prostaglandin $F_{2{\alpha}}$의 신장작용(腎臟作用))

  • It has recently been reported that prostaglandin $(PG)F_{2\alpha}$ produces diruesis and natriuresis when given directly into a lateral ventricle of the brain in rabbits (Kook et al). In this study attempts were made to elucidate the mechanism of the natriuresis utilizing dogs. In mongrel dogs of both sexes weighing 9-l2kg, a lateral ventricle of the cerebrum was cannulated and the agent was delivered in less than 0.2ml of 0.9% saline. $PGF_{2\alpha}$, $100\;{\mu}g$, ivt, elicited mild diuresis, while glomerular filtration rate and systemic blood pressure remained unchanged. In 8 chloralso-anesthetized, hydrated dogs undergoing water diuresis, ivt $PGF_{2\alpha}$ produced antidiuresis concomitant with marked natriuresis, which resembles the action of large doses of Arginine vasopressin in doses of 0.3-1.5mu/kg/min produced marked natriuresis. In 9 experiments, $PGF_{2\alpha}$ was given intraventricularly during the maximal diuresis induced by ADH. The kidney responded with significant natriuresis though less marked and transient than during water diuresis. It is thus concluded that beside ADH still other natriuretic factor(s) may be involved in the natriuresis induced by ivt $PGF_{2\alpha}$ in the dog.

  • Endotoxemia induces production of inflammatory mediators and acute phase proteins, leading to multiorgan injury and systemic inflammation. Hypothalamic-pituitary-adrenal (HPA) axis activation and glucocorticoids (GCs) release modify endotoxemia-induced inflammatory responses. In the present study, we investigated whether pre-exposure of GCs influences endotoxin-induced production of inflammatory mediators in hepatocytes. Hepa1c1c-7 cells were pretreated with low concentrations of corticosterone for 24 h and then cultured without corticosterone in the presence or absence of LPS. Our results demonstrated that LPS alone significantly enhanced production of IL-6 and CRP but reduced vascular endothelial growth factor (VEGF) compared to controls. Combination of corticosterone pretreatment and LPS significantly upregulated production of IL-6, IL-$1{\beta}$, and VEGF but downregulated CRP compared to those in LPS alone. These findings suggest that in low concentration of corticosterone-preexposed hepatocytes, endotoxemia may induce upregulation of IL-6, IL-$1{\beta}$, VEGF and but downregulation of CRP.

  • While human error happens repeatedly in the semiconductor industry in Korea, which has brought a tremendous loss from manpower, welfare etc., there are limitations to human error prevention activities. When a semiconductor company introduces new machines and facilities from Japan or Germany, the companies often do not consider human factors in the design. Also, semiconductor companies are so occupied with promoting increased productivity, their attention to human errors has been pushed aside. Negative aspects of technical exchange associated with safety management are one aspect of the industry's nature. A semiconductor company recently began acknowledging on the back of TQM(Total Quality Management) that human error has a decisive effect on the safety. There are a number of uncontrollable and hard to handle event sets because the nature of these events with a human error may often be threatened or very intensive. It is strongly required that systemic studies should be performed to grasp the whole picture of a current situation for hazard factors. This study aims to examine the human error approach through the case of human error prevention field activities in a semiconductor industry compared with the activities and experience in nuclear power plants.

BISPHOSPHONATE, IS IT AN EMERGING RISK FACTOR IN ORAL SURGERY? (Bisphosphonate, 구강악안면외과 영역의 새로운 위험 요소인가?)

  • Since the first description of bisphosphonate related osteonecrosis of the jaw (BRONJ) in 2002, the number of report on the disease has rapidly been increasing. Now, BRONJ is considered as a new entity, which is emerging problem in oral and maxillofacial surgery. Bisphosphonates (BPs) can be categorized into 2 groups: nitrogen-containing and non-nitrogen containing, and nitrogen-containing BPs are considered to have more efficacy and toxicity possibly. It is unusual for osteonecrosis to occur in the maxilla but BRONJ is found in both the mandible and the maxilla, which is one of the special features of BRONJ compared with common infectious osteomyelitis of the jaws. Intravenous BPs are usually more likely to cause BRONJ than oral BPs which are frequently prescribed for osteoporosis and osteopenia. Nonetheless, the use of intravenous BPs cannot be prevented because of systemic condition of the patients. Although it is rare that oral BPs cause BRONJ in osteoporosis/osteopenia patients, we should be aware of BRONJ since the population of the patients is exceedingly increasing with the prolonging of life expectancy. So, we'd like to enlighten upon the problems and solutions of BRONJ.

Cardiometabolic Effects of Obstructive Sleep Apnea and Treatment Effects of Oral Appliance: An Updated Review for Dentists

  • Obstructive sleep apnea (OSA) is a relatively common, but greatly underdiagnosed sleep-related breathing disorder, characterized by recurrent collapse of the upper airway during sleep. OSA has been associated with a variety of cardiometabolic disease, such as hypertension, coronary artery disease, cardiac arrhythmia, cerebrovascular disease and metabolic dysfunction. Neurocognitive impairment, including excessive daytime sleepiness, increased risk of motor vehicle accidents, is also related to OSA. Sleep fragmentation and related arousals during sleep lead to intermittent hypoxia, sympathetic activation, oxidative stress, systemic inflammation and metabolic dysregulation which provide biological plausibility to this pathologic mechanism. Extensive studies demonstrated that OSA is a modifiable risk factor for the above mentioned diseases and oral appliances (OAs), although continuous positive air pressure (CPAP) is a first-line therapy of OSA, are not inferior to CPAP at least in mild OSA, and may be an alternative to CPAP in CPAP-intolerant subjects with OSA. The goal of this article is to provide a current knowledge of pathologic link between OSA and cardiovascular disease, focusing on intermittent hypoxia, sympathetic activation, oxidative stress and metabolic dysregulation. Then, previous epidemiologic studies will be reviewed to understand the causal relationship between OSA and cardiovascular disease. Finally, the effects of OAs will be updated via recent metaanalyses compared to CPAP.
