The reliability of catenary system is very important for uninterrupted train operation as it supplies electric power to train without redundant facilities. This paper provides a systematic approach to the reliability analysis of the catenary system based on FMECA procedures defined by global standards such as MIL Std 1692a and IEC 60812. Field failure data collected from the operation and maintenance of high-speed railway catenary system for 9 years are used to derive critical failure modes and to evaluate the criticality of the failure modes. Evaluation of the criticality are carried out by quantitative procedures defined by MIL Std 1692a and by criticality matrix defined by IEC 60812. FMECA results suggests that three critical failure modes should be checked carefully during maintenance work, which include strand break of dropper and voltage equalizing wire, power supply failure of feeder. Maintenance procedure of catenary system in order of importance is suggested too. These results can be applied to maintenance planning and design of catenary system to improve the reliability of electric railway system.

Reliability Centered Maintenance (RCM) introduces a systematic method and decision logic tree for utilizing previous operating experience focused on reliability and optimization of maintenance activities. In this paper RCM methodology is applied on safety injection system for APR-1400. Functional Failure Mode Effects and Criticality Analysis (FME&CA) are applied to evaluate the failure modes and the effect on the component, system and plant. Logic Tree Analysis (LTA) is used to determine the optimum maintenance tasks. The results show that increasing the condition based maintenance will reduce component failure and improve reliability and availability of the system. Also the extension of the surveillance test interval of Safety Injection Pumps (SIPs) would lead to an improved pump's availability, eliminate the unnecessary maintenance tasks and this will optimize maintenance activities.
Stability of vertical escarpments has been the subject of discussion for long time. However, available literature provides scarce knowledge about the effect of the formation of undercut and surface cracks on the stability of a vertical escarpment. The present study deals with a systematic analysis of the effect of surface cracks and undercut on slope stability using finite element based lower bound limit analysis. In the present analysis, the non-dimensional stability factor (${\gamma}H/c$) is used to inspect the degrading effect of undercut and cracks developed at different offset distances from the edge of the vertical escarpment. Failure patterns are also studied in detail to understand the extent and the type of failure zone which may generate during the state of collapse.
It is inevitable to control the systematic failure to obtain the software safety integrity of embedded software installed in rolling stock. Because it is not possible to assess systematic failure integrity by quantitative methods, SILs are used to group documentation, methods, tools and techniques throughout software development lifecycle which, when used effectively, are considered to provide an appropriate level of confidence in the realization of a system to a stated integrity level. Normally, safety approval process is through generic product, generic application and specification application for. For safety approval on generic application of software based system, it is required to apply the certified software processes from the planning stage for the assigned SIL. As such, we will develop project specific application with high safety integrity within time limit of contractual delivery schedule through software assessment to the modified area with the re-use of certified software module and documentation. At this point, Hyundai Rotem has developed software processes applicable to support SIL 4 based on EN50128 which was assessed and certified by TUV SUD. This paper introduces the Hyundai Rotem's detailed approach and prospective action to achieve software safety integrity level.

Railway vehicle is very important to implement the effective maintenance in proper to prevent any failure during operation period. Many railway authorities are making efforts to maintain the railway vehicle through scientific and systematic procedure. To achieve this, Reliability Centered Maintenance(RCM) is partially applied. The efficiency of RCM has proven and its terminology was familiar with nuclear power, military and chemical plant etc. since the commercial aircraft's industries has introduced the maintenance program based on the target of reliability. The application of RCM on railway vehicle can be utilized with systematic analysis method to select the best effective maintenance period and action to prevent the failures by selecting the equipment affecting the its safety and reliability. This paper is presented that the procedure of adequate and effective maintenance for railway vehicle by comparing among the related standards in example IEC60300-3,11, MIL-STD-2173, and technical documents or papers. In accordance with above result, RCM procedure is proposed to apply effectively for maintenance of railway vehicle. That is, (1) Analysis of data and Calculation of criticality per equipment (2) Selection of equipment to analyze (3) Analysis of failure mode and effect (4) Evaluation of maintenance method and period (5) Optimization of maintenance program through renewal of maintenance method and period.

Objectives: The aims of this study were to conduct a systematic review of the microtensile bond strength (${\mu}TBS$) of multi-mode adhesives to dentin and to perform a meta-analysis to assess the significance of differences in the ${\mu}TBS$ of one of the most commonly used universal adhesives (Scotchbond Universal, 3M ESPE) depending on whether the etch-and-rinse or self-etch mode was used. Materials and Methods: An electronic search was performed of MEDLINE/PubMed, ScienceDirect, and EBSCOhost. Laboratory studies that evaluated the ${\mu}TBS$ of multi-mode adhesives to dentin using either the etch-and-rinse or self-etch mode were selected. A meta-analysis was conducted of the reviewed studies to quantify the differences in the ${\mu}TBS$ of Scotchbond Universal adhesive. Results: Only 10 studies fulfilled the inclusion criteria for the systematic review. Extensive variation was found in the restorative materials, testing methodologies, and failure mode in the reviewed articles. Furthermore, variation was also observed in the dimensions of the microtensile testing beams. The meta-analysis showed no statistically significant difference between the etch-and-rinse and self-etch modes for Scotchbond Universal adhesive (p > 0.05). Conclusions: Multi-mode 'universal' adhesives can achieve substantial bonding to dentin, regardless of the used modes (either etch-and-rinse or self-etch).

Failure mode and effect analysis (FMEA) is a systematic approach for identifying potential failures before they occur, with the intent to minimize the risk associated with them. It has been widely used in the various manufacturing industries as a solution to reliability problems. As the importance of the service sector is increasing, however, it has been recently extended to some applications in services. Despite these attempts, FMEA cannot be directly applied to the reliability problems in a service industry. Due to the heterogeneity and customer participation in service process, we cannot perfectly prevent service failures. For this reason, we suggest a new risk priority number with three input parameters that consist of severity, probability of occurrence, and recoverability. In this paper, we propose an approach for assessing service risk and service reliability using the service-oriented risk priority number (S-RPN). An example regarding a hypermarket service process is used to demonstrate the proposed approach.
Difficulties are encountered when the behavior of complex systems (i.e., fuel failure probability) that have unreliable deterministic models is predicted. For more realistic prediction of the behavior of complex systems with limited observational data, the present study was undertaken to devise an approach of combining predictions from the deterministic model and actual observational data. Predictions by this method of combining are inferred to be of higher reliability than separate predictions made by either model taken independently. A systematic method of hierarchical pattern discovery based on the method developed in the SPEAR was used for systematic search of weighting factors and pattern boundaries for the present method. A sample calculation was performed for prediction of CANDU fuel failures that had occurred due to power ramp during refuelling process. It was demonstrated by this sample calculation that there exists a region of feature space in which fuel failure probability from the PROFIT model nearly agree with that from observational data.
Purpose: To develop a probability-based differential diagnosis for pediatric acute liver failure (PALF) based on age and socioeconomic status of the country of origin. Methods: Comprehensive literature search using PubMed, EMBASE, and SCOPUS databases was performed. Children 0-22 years of age who met PALF registry criteria were included. Articles included >10 children, and could not be a case report, review article, or editorial. No language filter was utilized, but an English abstract was required. Etiology of PALF, age of child, and country of origin was extracted from included articles. Results: 32 full text articles were reviewed in detail; 2,982 children were included. The top diagnosis of PALF in developed countries was acetaminophen toxicity (9.24%; 95% CredI 7.99-10.6), whereas in developing countries it was Hepatitis A (28.9%; 95% CredI 26.3-31.7). In developed countries, the leading diagnosis of PALF in children aged <1 year was metabolic disorder (17.2%; 95% CredI 10.3-25.5), whereas in developing countries it was unspecified infection (39.3%; CredI 27.6-51.8). In developed countries, the leading diagnosis in children aged >1 year was Non-A-B-C Hepatitis (8.18%; CredI 5.28-11.7), whereas in developing countries it was Hepatitis A (32.4%; CredI 28.6-36.3). Conclusion: The leading causes of PALF in children aged 0-22 years differ depending on the age and developmental status of their country of origin, suggesting that these factors must be considered in the evaluation of children with PALF.

Objectives: Papillon-Lefèvre syndrome (PLS) is a rare autosomal recessive disorder. These patients lose their teeth at a young age and are in need of prosthetic rehabilitation. The aim of this systematic review was to assess the success of dental implant placement in these patients. Materials and Methods: An electronic search was performed in PubMed Central, Scopus, and Web of Science using the keyword "Papillon-Lefèvre syndrome" AND "dental implant" OR "prosthodontics". Articles reporting implant placement in patients with PLS until July 2019 were included. Results: Assessment of the included 11 articles reporting 15 cases showed 136 implant placements in these patients. Implant failure occurred in 3 patients (20 implants). The peri-implantitis and failure rate was higher in the maxilla. Meta-analysis showed the probability of failure to be 7% (95% confidence interval [CI] 0%-31%) for maxillary implants and 2% (95% CI 0%-9%) for mandibular implants. The follow-up time ranged between 1 and 20 years. Healing after bone graft and implant placement in these patients was uneventful. Conclusion: Dental implants may be a viable treatment option for PLS patients. Implantation can help preserve alveolar bone if the patients' immunological and growing conditions are well-considered and proper oral hygiene and compliance with the maintenance program are continued.
