성향정기산을 포함한 한방치료로 호전된 뇌경색 환자의 감각이상 및 위약감 치험 1례 (A Case Study of Cerebral Infarction Patients with Dysesthesia and Weakness Treated with Sunghyuangjunggi-san (Xingxiang zhengqi san))

  • Objectives: The purpose of this study was to address a case of cerebral infarction with dysesthesia and weakness. Methods: A patient diagnosed with cerebral infarction with dysesthesia and weakness was treated with Korean medicine therapies including herbal medication (Sunhyunagjunggi-san) for 8 days. To evaluate the therapeutic effect, we used the National Institute of Health Stroke Scale (NIHSS) and Numerical Rating Scale (NRS). Results: After the treatment, the NIHSS and NRS scores dramatically decreased, and the symptoms no longer disturbed the patient. Although the symptoms of dysesthesia did not disappear completely, fine therapeutic effects were noted. Conclusion: These results suggested that Korean medicine therapies with Sunghyuangjunggi-san have a beneficial effect on the dysesthesia and weakness of cerebral infarction.

Rapid Atypical Progression of Neuro-Behçet's Disease Involving Whole Brainstem and Bilateral Thalami

  • We present a case of Neuro-Behçet's disease with an unpredictable clinical course. A 47-year-old man was admitted to the neurosurgery department of our hospital with a mild headache. Three days after admission, his consciousness suddenly decreased and respiratory distress progressed rapidly. A brain MRI revealed that the previously observed abnormal signal had extended markedly to both the thalamic areas and the entire brain stem, and the surrounding brain parenchyma were compressed by cerebral edema. Based on the patient's symptoms of recurrent oral and genital ulcers, skin lesions, and uveitis, a rheumatologist made a diagnosis of Behçet's disease with CNS involvement. The patient was treated with high-dose methylprednisolone with respiratory assistance in the intensive care unit for 9 days and his neurologic symptoms improved remarkably. Neuro-Behçet's disease must be considered in the differential diagnosis in rapidly deteriorated young neurological patients along with a stroke, low-grade glioma, multiple sclerosis, and occlusive venous disease.

장종정(張從正) 토법(吐法)의 경험례 (Experience in Jang Jong-jeong's Vomiting Treatment)

  • Jang Jong-jeong thought that, because all diseases come from miasma, for treating diseases, it is important to attack or remove miasma infiltrating into the body. Thus, he treated diseases using three methods, namely, vomiting against miasma on the top, sweating against miasma on the surface, and diarrhea against miasma on the bottom. Among them, vomiting is rarely used in contemporary Oriental medicine, but still one of major disease treatment methods. Vomiting is quite meaningful in today's clinics and particularly effective for acute diseases, the miasma of which is in the upper chest. According to the present researcher's experiences, it is effective in cases that the pathological symptoms are observed in the upper chest such as early stroke, dyspepsia, chest congestion and congestion heat. For vomiting, the researcher applied Dokseongsan, Samseongsan, Gwachesan, Gangyeomtang, Bangpungtongseongsan, etc. Dokseongsan was used for vomiting in a cool way, and Samseongsan and Gwachesan were used for vomiting in a warm way. Bangpungtongseongsan was used when inner heat was choked up by the attack of external miasma and when the pathological symptoms of Bangpungtongseongsan were observed.

Effect of Korea Red Ginseng on the Symptoms and Hemodynamics in Healthy Elders

  • Clinical study on hemodynamics of healthy elders before and after tread mill. Using doubleblind, placebo-controlled study design. Seventy-five 50-70 years old volunteers without organic disease were divided into two groups, Ginseng group and control group. Each subject was received 3 g Korea Red Ginseng (KRG) capsules or placebo per day for 4 weeks. Before and after administration 4 weeks, the symptoms were asked and hemodynamics parameter such as pump function, systolic function, preload and afterload were recorded before and after tread mill 1,5, 10 minutes by the method of thoracic impedance cardiograph. The result showed that Ginseng could improve quality of life, had obvious effect of increasing PEP (pre-ejection period), PEP/LVET (pre-ejection period/left ventricular ejection period), PCWP (wedged pressure pulmonary capillary). The change rate of HR (heart rate), SV (stroke volume) in KRG group were much lowered, while EF (ejection fraction), LVET (left ventricular ejection period), LVEDP (left ventricular end diastolic pressure) was recovered much quickly. The circulation showed Ginseng could improve the quality of life though its promoting circulation function which are increasing both the systolic function and the preload.

측두하악장애 치료후 저작능률에 관한 연구 (A Study on the Masticatory Efficiency after Treament for Temporomandibular Disorders)

  • This study was performed to investigate the masicatory efficiency in patients with temporomandibular disorders (TMDs), especially internal derangement of temporomandibular (TM) joint. For this study, 26 patients after treatment and 33 dental students who had no signs and symptoms of TMDs were selected as the patients group and as the normal group, respectively. Mean treatment duration of the patients was. 5.1 months. Verbal rating scale(VRS) and Visual analogue scale(VAS) were used for recording of subjective symptoms. Treatment index (VAS Ti) derived from VAS was calculated for evaluation of treatment progress and clinical examination was also performed for objective symptoms. BioEGN(Bioresearch Inc., Milwaukee, USA) was used for observation chewing movement pattern on peanut, caramel candy, and gum chewing. Chewing time in second and symptoms after chewing were recorded, and pattern of chewing stroke between in affected side and in contralateral side or between in right in left side were compared, and especially, gum chewing pattern between before and after treatment were also compared in the patients group. The data obtained were analysed by SPSS windows program and the results of this study were as follows : 1. Subjective symptoms evaluated by VAS showed no difference between the two groups, but those by VRS showed slight difference for TM joint pain, head not neck symptoms, and chewing ability. 2. There were no difference at the level of subjective symptoms between the subgroups divided by treatment duration of five months in the patients group. However, value of VAS Ti of pain was higher in subgroup of long treatment duration than that of subgroup of short treatment duration. 3. There were no difference in chewing time for peanut or caramel candy between the two groups, but on caramel candy chewing, the patients group complained slight discomfort after swallowing. Chewing velocity and range of motion on gum chewing after treatment in the patients group showed significant difference and greatly improved compared to those to before treatment, and which were not differ from those of normal subjects. In conclusion, treatment of temporomandibular disorders about for five months would greatly improve chewing ability and movement pattern in most of the patients with TMDs.

전환장애 환자에 개결서경탕(開結舒經湯)을 투여한 치험례 (A Case of Conversion Disorder Treated with Gaegyeolseogyeong-tang)

  • Conversion Disorder is a disorder whose predominant feature is a loss or alteration in physical functioning that suggests a physical disorder but that is actually a direct expression of a psychological conflict or need. The Diagnostic and Statistical Manual of Mental Disorders-IV (DSM-IV) guidelines for Conversion Disorder include these definitions: A psychosocial stressor produces a psychological conflict that is believed to help initiate or exacerbate the illness The symptoms are not under conscious control, etc. While functional disabilities are common with conversion disorders, physical and laboratory abnormalities are absent or minor in comparison with the patient's subjective complaints. Symptoms of Conversion Disorder are similar to those of stroke. But the mechanism of Conversion Disorder is similar as that of Stagnation Syndrome of Ki (氣鬱證) in Oriental medicine. Gaegyeolseogyeong-tang has been used to treat women who suffer from Conversion Disorder induced by the Stagnation Syndrome of Ki (氣鬱證). After application of the Gaegyeolseogyeong-tang for 7 days, symptoms and signs improved dramatically.

열다한소탕(熱多寒少湯)과 청폐사간탕(淸肺瀉肝湯)의 활용에 대한 임상적 연구 (Clinical Study on the Utilization of Yeoldahanso-tang and Cheongpyesagan-tang)

  • Objective: The purpose of this study is to examine the characteristics and the improvement in the symptoms of the patients who showed improvement after the administration of Yeoldahanso-tang and Cheongpyesagan-tang. Methods: The patients whose Sasang Constitution were examined by an expert at six Oriental medical hospitals and three regional Oriental-medicine clinics, 89 showed a predetermined symptom improvement when Yeoldahanso-tang and Cheongpysagan-tang were administered to them from November 2007 to July 2008. The traits of the patients and the effects of their treatment were examined. Results and Conclusions: 1. There were about as many male patients as there were female patients, but 21.3% of the patients who were female and who were in their 40's showed the most improvement, which makes them the biggest category in this study. 2. The main syndromes in which the patients showed improvement were hemi-paralysis, vertigo, and headache, and the ordinary symptoms in which the patients showed improvement were diarrhea, fatigue, and dry mouth. 3. The average food intake of the patients was highest and their feces had intermediate hardness (semi-soft). 4. The amount of the patients' sweat was either high(38.2%) or appropriate(38.2%) and they showed a dislike of heat over coldness. 5. Among the diseases that the patients previously had, high blood pressure. The above results show that Yeoldahanso-tang and Cheongpyesagan-tang can be considered beneficial to females in their 40's and to stroke patients whose food intake is normal, whose sweat level is either copious or appropriate, who dislike heat and who are suffering from high blood pressure.

대자석의 중의 신경정신과 임상연구 현황 (Research Trends of Traditional Chinese Medicine Containing Haematitum in the Neuropsychiatric Clinical Scene)

  • Objectives: This study was intended to review the research trends of treating neuropsychiatric diseases and symptoms with Traditional Chinese Medicine containing Haematitum. Methods: Articles were obtained through the CNKI (China National Knowledge Infrastructure) by searching with 'Haematitum' as the main key word, and supportive words related with neuropsychiatric diseases and symptoms were selected. There were 61 articles related to clinical fields, which were then classified according to study design. Results: The 61 articles were categorized into the following types of study design: 3 randomized controlled trials, 1 quasi-randomized trial, 3 simple-designed clinical trials, and 54 case studies. Decoctions containing Haematitum were used to treat diseases and symptoms such as vertigo, headache, stroke, epilepsy, neurosis, globus hystericus, fishbilepoisoning, insomnia, mania, post-traumatic brain syndrome, and kinesia. All articles reported a good rate of effectiveness. There was no poor responsiveness regarding the effects of Haematitum in 9 studies, but it was not mentioned in the other 52 studies. Decoctions self-prepared by the authors were used in 28 studies. Modified Seonbokdeja-tang, modified Banhabeakchulcheonma-tang, modified Ondam-tang were used in that order of frequency. The daily dosage of Haematitum provided was 0.2~6 g in powder, and 9~60 g in decoction. Conclusions: Decoctions containing Haematitum are used restrictively in the neuropsychiatric clinical scene. While there were no reports of poor responsiveness of the effects of Haematitum, more research is needed to confirm its clinical stability.

보양환오탕가감방 및 복합한의치료로 호전된 시상 부위 뇌경색 후 우측 편마비 환자 치험 1례 (A Case Report on Right Hemiparesis after Left Thalamus Infarction Treated with Boyanghwanoh-tang-gagam and Complex Korean Medicine Treatment)

  • Objectives: This study reports the case of a patient with right hemiparesis after a left thalamic infarction, in which the patient experienced improved clinical symptoms (hemiparesis, constipation) after treatment with a complex Korean medicine treatment. Methods: A 81-year-old male patient diagnosed with acute left thalamic infarction was treated with daily acupuncture and herbal medicine (Boyanghwanoh-tang-gagam) three times a day. Jowiseunggi-tang extract granules were administered to treat constipation by adjusting the dosage according to the reported symptoms. The patient's clinical course was assessed using changes in Modified Barthel Index (MBI) score, Manual Muscle Testing (MMT) grade, and defecation frequency . Results: The patient's MBI score improved from 35 to 69 and was discharged with the ability to walk under supervision. The MMT grade also improved from 3/4 to 5/5 in the upper/lower extremities. Other clinical symptoms, including constipation, also improved, leading to the discontinuation of magnesium medication. Conclusion: These findings suggest that complex Korean medicine treatments, including herbal medicine and acupuncture, may be an effective treatment for post-stroke hemiplegic patients.

침치료가 뇌졸중으로 인한 구음장애에 미치는 음향적 특성에 대한 증례보고 (Clinical Acoustic Study of Acupuncture Therapy Effects on Post-Stroke Dysarthria)

  • Objectives : The aim of this study is to find the acoustic characteristics of acupuncture therapy effects on post-stroke dysarthria. Methods : Acupuncture therapy was applied for four to six weeks by inserting needles into eight acupuncture points, CV23, CV24, bilateral 'Sheyu' and ipsilateral ST4, ST6 and contralateral LI4, ST36 on facial palsy side. All the speech samples were collected, pre-treatment and post-treatment, using Computerized Speech Lab. VOT and TD of each speech sample and vowel formant(F1&F2) were analyzed on spectrogram. Result : VOT and TD were decreased after treatment. F1 was decreased, and F2 was increased after treatment. Conclusions : This suggests that acupuncture therapy improves symptoms of post-stroke dysarthria by stimulating articulation organs such as tongue, lips, cheeks, larynx and pharynx.
