중풍의 아형분류에 관하여 (Survey of Stroke Subtype Classification)

  • In Korea, Korean Medicine is the first choice in treating stroke. Because stroke prognosis, risk of recurrence, and choices for management are influenced by stroke subtype, correct analysis of stroke subtype is important. Until now, the studies on differentiation of the symptoms and signs for the stroke were a few and the Korean standard differentiation of the symptoms and signs for the stroke don't exist. To develop the Korean standard differentiation of the symptoms and signs for the stroke, we go through subtype of stroke in traditional chinese medicine, Korean Medicine, western medicine.

지역사회 당뇨병 유병자와 비유병자의 심근경색증 및 뇌졸중 조기증상과 대처방안 인지도 비교: 성향점수매칭 분석 (Awareness of Early Symptoms and Emergency Responses to Myocardial Infarction and Stroke in People with Diabetes Mellitus Compared to Non-diabetic Population in the Community: A Propensity Score-Matched Analysis)

  • Background: We determined the differences in awareness of myocardial infarction and stroke according to the presence or absence of diabetes mellitus in the community. Methods: The 2018 Community Health Survey identified 20,812 people with diabetes mellitus aged 40-79 years. Using 1:1 matching by propensity score, 20,812 people without diabetes mellitus but with similar sociodemographic characteristics were selected as a comparison. Outcome variables were awareness of early symptoms of myocardial infarction and stroke and awareness of coping strategies in case of occurrence. Results: There was no significant difference between nondiabetic and diabetic people in terms of recognizing all early symptoms of myocardial infarction (nondiabetic, 42.7%; diabetic, 43.0%; p=0.43) and stroke (nondiabetic, 49.4%; diabetic, 49.4%; p=0.91). In addition, no significant difference was found between nondiabetic and diabetic people in the proportion of knowing correct emergency response to myocardial infarction (nondiabetic, 84.6%; diabetic, 84.4%; p=0.56) and stroke (nondiabetic, 81.3%; diabetic, 81.4%; p=0.77). Conclusion: Since people with diabetes are at greater risk of cardiovascular disease than the general public, it is important to lower the risk of disability and death by improving their awareness of early symptoms and correct emergency response to myocardial infarction and stroke.

한의 중풍변증 표준안-I에 대한 보고 (Report on the Korean Standard Differentiation of the Symptoms and Signs for the Stroke-1)

  • To develop the Korean standard differentiation of the symptoms and signs for the stroke(KSDS), the committee on Stroke Diagnosis Standardization of traditional Korean medicine(TKM) was organized dy nineteen experts in college of Korean medicine. On July 9th 2005, the second consultation meeting was held in Daejeon, Korea. Fifteen experts of the committee attended the meeting and they discussed the KSDS and came to a consensus. The 15-member committee consensus was as follows: First, board members defined the stroke on the basis of TKM. Second, they divided the symptoms and signs of stroke into five categories- fire and heat, dampness and phlegm, blood stasis, qi deficiency, yin deficiency. Third, the symptom indicator of each differentiation type for the stroke was recommended. KSDS-1 will be applied to the clinical practice and revised.

치매, 중풍 노인의 장기요양서비스 이용현황과 이용수준 관련 요인 (The Long-term Care Utilization of the Elderly with Dementia, Stroke, and Multimorbidity in Korea)

  • Background: This study examined the relationships of dementia, stroke, and combined multimorbidity with long-term care utilizations among older people in South Korea. Methods: A nationally representative sample of 10,130 older adults who used long-term care services in 2010 were analyzed. We used the 5% sample of aged 65 years or older linked with National Health Insurance Corporation registry data of long-term care insurance system. The sample was categorized into three groups: dementia only (47.6%), stroke only (36.3%), and both dementia and stroke (16.1%). We estimated the use of institutional care, home care, and total expenditure of long-term care services, adjusting for the severity of each function (such as daily life, behavior or cognitive change, nursing care needs, and rehabilitation care needs) and sociodemographic characteristics. Results: Having dementia symptoms was positively associated with the use of institutional care services, on the other hand, having stroke symptoms was positively related with the use of home care. The total long-term care cost was higher in the group of having both dementia and stroke. Conclusion: Older persons with dementia symptoms and stroke symptoms have different patterns of long-term care utilization, and the multimorbidity increased the overall expenditure of long-term care utilization. These findings imply a need for differentiated management strategy targeting physically and cognitively impaired older persons, and special concerning for persons with multimorbidity conditions for long-term care insurance program in Korea.

뇌졸중 환자들의 지연도착시간에 관한 요인들 (Factors Delaying Hospital Arrival Time after Stroke)

  • Objective: The management for the stroke should ,given as soon as possible to be effect. But Patients with stroke symptoms commonly delay many hours before seeking medical attention. We evaluated the factors which are related to the time of hospital arrival after acute stroke. Method: Data were obtained from 317 patients admitted to our hospital within 72 hours of stroke onset. We assessed demographic variables, stoke subtype. referral routes. history of previous stroke, level of consciousness, distance from the place where stroke occurred to hospital, and the time interval between onset of stroke and arrival at the hospital. Results: Mean patient age was 65.99±9.57 years. The mean time interval between onset of stroke and hospital arrival was 17.26±18.69 hours and 128 (40.38%) patients arrived within 6 hours. The patients whoes stoke subtype was infarction, who arrived our hospital by way of other hospital, who had no suffered from previous stroke and who showed no impairement of consciousness was arrived at the hospital late(p<0.05). Conclusion: The majority of patients arrive at the hospital after prolonged delays for multiple reasons, and patients with milder symptoms, for whom treatment might be more effective, were less likely to arrive in time for therapy. Our study suggest that effective education about stroke to the patients and public would be highly necessary.

  • Type 2 diabetes mellitus (T2DM) is a progressive disorder caused by a combination of insulin resistance and 𝛽 cell dysfunction. Sogal(消渴) is a traditional Korean medical term referring to a condition pertaining 3 major symptoms - thirst, polyphasia, polyuria. Sogal has been reported to have similar characteristics with DM. This case report demonstrates a patient with T2DM complaining of typical Sogal symptoms. We diagnosed him as So-yang person Sogal and treated him with acupuncture and herbal medicine.

응급구급대원에서 뇌졸중 조기증상 인지 및 환자평가 향상을 위한 교육 필요성 (Necessity of Education for Emergency Medical Technicians to Improve Awareness of Early Symptoms of Stroke and Assessment of Stroke Patients)

  • 이 연구는 응급구급대원들을 대상으로 뇌졸중과 관련된 교육에 대한 특성 및 뇌졸중 조기증상 인지수준을 파악함으로써 이와 관련된 교육프로그램 운영의 필요성과 방향을 제시하고자 수행하였다. 연구 대상자는 전라북도 전체 응급구급대원 298명이었으며 설문조사는 직접면접법을 통해 이뤄졌다. 조사기간은 2011년 4월 1일-30일까지 1개월 동안 이루어졌다. 연구결과 뇌졸중 관련 교육 이수율은 63.2%로 나타났으며 주로 소방학교(63.1%)를 통해 이수하고 있었다. 그러나 대상자들의 뇌졸중 관련 교육 실시요구도(85.0%)는 상대적으로 높게 나타났다. 뇌졸중 조기증상에 대한 전반적인 조기인지 수준 및 뇌졸중 환자 상태평가 점수는 여자에서, 30대 미만, 소방사에서, 구급업무를 직접 담당자에서, 응급구조사에서 뇌졸중 환자 상태평가 수준이 높게 나타났다. 뇌졸중 환자 발생 시 우선적 조치사항으로는 전문치료가 가능한 최근접 병원으로의 환자 이송이 82.8%로 나타났다. 뇌졸중 조기증상에 대한 인지수준, 환자 상태평가, 환자처치에 대한 적절성은 뇌졸중 관련 교육 이수자에서 비이수자보다 높게 나타났다. 응급구급대원은 뇌졸중 조기증상에 대한 조기인지 수준이 증가할수록 뇌졸중 환자 처치에 대한 자신감, 처치 만족도, 처치 적절성이 향상되는 것으로 나타났다. 결론적으로 응급구급대원들을 대상으로 하는 뇌졸중 관련 교육 프로그램 개발 및 운영이 필요하다. 교육 프로그램 개발 및 운영 시에는 기존 교육프로그램에 대한 심층분석을 통해 대상자들의 특성을 고려한 교육내용과 적절한 교육과정을 확보함으로써 교육 대상자들의 요구도를 충족시키고, 교육 이수율을 높여 궁극적으로는 응급구급대원들의 실질적인 역량 강화를 이룰 수 있도록 해야겠다.

조선(朝鮮) 의서(醫書)의 중풍(中風) 침구법(鍼灸法) 비교 (Comparative Study of Stroke Treated by Acupuncture in the Medical Documents Of the Chosun-Korea Dynasty)

  • Objectives : The objective of this article is to compare acupuncture treatment of stroke in the medical documents of the Chosun-Korea dynasty. Through such inquisition, the development process of acupuncture of stroke treatment could be found. Methods : We used Cross-comparison analysis of the contents of five medical books, HyangYakZipSungBang (HYZSB), UiRimChwarYo(URCY), DongEuiBoGam(DEBG), ChimGuGyungHumBang(CGGHB) and Saam-Acupuncture(SA) of the Chosun-Korea dynasty. Results & Conclusion : 1. Distinction between Stroke of Viscera and Stroke of Bowel was important. Firts two books divided stroke into three type. But last three books separated into Stroke of Viscera and Stroke of Bowel. 2. They emphasized Symptoms of Five Visceral Disease. They devised Symptoms of Five Visceral Disease for Stroke of Viscera treatment. This method appears very rarely in clinical medicine. 3. They used common important acupoint for treatment of stroke. Most of these acupoints exist at the limbs. 4. They chose acupoints on the opposite side against the symptom.

계절과 연령은 중풍환자의 증후(證候)에 어떠한 영향을 미치는가? (Do the Symptoms of Stroke Vary by Seasons and Age-groups?)

  • Objective : We wanted to examine the differences of symptoms of stroke by seasons and age groups. Methods : We recorded the symptoms of 254 stroke patients for 1 year and statistically analyzed the results. The Chi-Square test was used for the analysis.(p<0.05). Results : There was no difference of symptoms by age, but the symptoms were different by seasons. In spring and autumn, there was a large amount of Hwa. In summer, there was a large amount of Kihuh, and in winter, there was a large amount of Hwa and Dahm. Conclusions : We have concluded that these differences should be considered in the management and the prevention of stroke.

중풍(中風) 발병전(發病前) 제증상(諸症狀)에 대(對)한 임상(臨床) 연구(硏究) (A clinical study on the prodromal syndrome of cerebrovascular accident)

  • Background and Purpose : The aim of the present study was to investigate the prodromes of stroke in already attacked patients and to prove the traditional hypothesis that some symptoms were to be prodromes of stroke in the oriental medicine. Methods : The questionnaire which was based on symptoms of traditional hypothesis was distributed to subacute stroke patients who were confirmed by Brain CT or MRI in Oriental Medical Hospital, Kyung Hee University, except patients with indistinct onset, symptoms proved any other disease. Results : Eighty-three patients(82.2%) felt some prodromal symptoms and fifty-nine patients (58.4%) underwent a change in their life within one week before onset. Most common prodromal symptoms was the weakness or numbness of limbs, single or unilateral. Conclusion : Our results suggest that the prodromal symptoms before stroke can be regarded as predicting sign. And we think that these research may contribute to preventing stroke and relapse.
