Involvement of Selective Alpha-2 Adrenoreceptor in Sympathetically Maintained Pain

  • Objective : Peripheral nerve injury often leads to neuropathic pain, which is characterized by burning pain, allodynia, and hyperalgesia. The role of the sympathetic nervous system in neuropathic pain is a complex and controversial issue. It is generally accepted that the alpha adrenoreceptor (AR) in sympathetic nerve system plays a significant role in the maintenance of pain. Among alpha adrenoreceptor, alpha-1 receptors play a major role in the sympathetic mediated pain. The primary goal of this study is to test the hypothesis that sympathetically maintained pain involves peripheral alpha-2 receptors in human. Methods : The study was a randomized, prospective, double-blinded, crossover study involving twenty patients. The treatments were : Yohimbine (30 mg mixed in 500 mL normal saline), and Phentolamine (1 mg/kg in 500 mL normal saline) in 500 mL normal saline at 70 mL/hr initially then titrated. The patients underwent infusions on three different appointments, at least one month apart. Thus, all patients received all 2 treatments. Pain measurement was by visual analogue scale, neuropathic pain questionnaire, and McGill pain questionnaire. Results : There were significant decreases in the visual analogue scale, neuropathic score, McGill pain score of yohimnine, and phentolamine. Conclusion : We conclude that alpha-2 adrenoreceptor, along with alpha-2 adrenoreceptor, may be play role in sympathetically maintained pain in human.

  • Objectives : The purpose of this study was to investigate two subjects: the diagnostic value of bilateral lowering of electrical activity at point H4,5,6 of Ryodoraku and the mechanism for Ryodoraku phenomena. Methods : Electrical activities of Ryodoraku test and electrogastrography recorded simultaneously and monitored continuously from 16 cases of functional dyspeptic patients were collected and their variations were grouped by the topics of discussion which were peculiarity, stability, lagging, alterability, and anomaly. Ryodoraku recordings obtained from 6 patients with different gastrointestinal diseases and 1 normal healthy person were used as control. The results are discussed with Nakatani's suggestion, theory of sympathetic nerve and Meridian Principle, respectively. Finely, coincidence of stomach arrangement between anatomy and meridian system in Ryodoraku was also evaluated. Results : Time-course variation showed a regular relationship between the typical pattern of Ryodoraku at point H4,5,6 and gastric myoelectrical activity. However, an irregular relationship and atypical pattern of Ryodoraku occasionally appeared. A literature search suggested that electrical response at the Ryodoraku point H4,5,6 may be dependent on an afferent sympathetic spinal reflex transmitted from the stomach. However, there was no evidence for making clear whether bilateral lowering of electrical activity at this point was induced by hypofunction of local sympathetic nerve in the skin itself or of signals transmitted from the gastric sympathetic nerve or not. The coincidence of 19% could not provide a visceral arrangement of the stomach between anatomy and meridian systems. Conclusions : Bilateral lowering of electrical activity at Ryodoraku point H4,5,6 has value as a diagnostic index for gastric dysmotility of functional dyspepsia. This phenomenon is associated with spinal reflex transmitted from the afferent sympathetic nerve in the stomach but not that of meridian function.

Heat-stress 온도 적용이 교감신경활동에 미치는 영향 (Influence of Heat Stress Temperature on Sympathetic Nerve Activities)

  • The purpose of this study were to investigate influence of heat stress temperature on sympathetic nerve activities. Subjects were 8 normal adults (4 men, 4 women, 21.36 years old). First sympathetic nerve activities were measured at the point that increase of core temperature stops at the state of applying normal thermic temperature (NIT; $34^{\circ}C$). After measurement, temperature of bathtub was increased to heat stress temperature (HST; $46^{\circ}C$) and sympathetic nerve activities were remeasured at the point that temperature increase stops. Sympathetic skin response (SSR) were analyzed using EMG, IR thermometer, and auto stethoscope. SSR latency showed significant differences at both palms by electrical stimulation to median nerve (p<.05). Electrical stimulation to forehead showed significant difference at left palm (p<.05) and electrical stimulation to navel showed significant difference at right palm (p<.05). Median nerve in changes of SSR amplitude showed significant differences at both palms in HST (p<.01). Electrical stimulation to navel showed significant difference at left palm (p<.05). Ts of forehead and xiphoid process showed significant differences (p<0.01). Tc of oral (p<0.05) and inner ear (p<0.01) showed significant differences. Pulse rate showed significant difference (<0.05). This study showed that immersion in HST had significant decrease of excitability in sympathetic nervous system compared to immersion in NTT.

Regulation of $GABA_A$ Receptor by Protein Kinase A in Sympathetic Neurons of Major Pelvic Ganglia

  • Major pelvic ganglia (MPG) in rats are an unique autonomic ganglia, containing both sympathetic and parasympathetic neurons related with the function of bladder, penis and bowel. It has been widely known that ionotropic $GABA_A$ receptors are the molecular target of $\gamma$-aminobutric acid (GABA), a major inhibitory neurotransmitter in central nervous system. However, their functions and regulations of $GABA_A$ receptors expressed in autonomic ganglia have been poorly understood. 1 examined the modulatory role of adenylyl cyclase (AC) and protein kinase A(PKA) on $GABA_A$-induced inward currents in the neurons of rat MPG. $GABA_A$ receptors were identified using immunofluorescent labeling in the rat major pelvic ganglion. Electrophysiological experiments were performed to record the activities of $GABA_A$ receptors. $GABA_A$ receptors were expressed only in sympathetic neurons. GABA induced marked inward currents in a concentration-dependent manner. Mucimol ($5{\mu}M$), a $GABA_A$ receptor agonist induced inward currents were significantly reduced in the presence of SQ 225361 $20{\mu}M$, a AC inhibitor and myristoylated PKA inhibitor 100 nM. In addition, forskolin ($1{\mu}M$), AC activator, augmented the GABA induced currents. The activation of AC/PKA-dependent pathway could involve in the regulation $GABA_A$ receptors, expressed only in sympathetic neurons of rat MPG. These findings are helpful for the better understanding the function of various pelvic organs innervated by MPG.

  • The purpose of study was to compare change of blood volume on upper limb of stimulus site on interferential current therapy. Twenty university student(twelve females. eight males :mean aged 23.08) with health condition participated this study MP150 system(biopac system) was used to measured blood volume. PPG senser was located thrum finger end The obtain result are as follows. 1. The result of this study were following that stimulate time blood volume were significantly increased sympathetic stimulation group compared with muscle stimulation group(p<.05). 2. The result of this study were following that stimulate time blood volume were significantly increased sympathetic stimulation group compared with muscle stimulation group(p<.05). 3. The result of this study were following that sympathetic stimulation group were significantly increased stimulate time blood volume compared with stimulate time blood volume(p<.05). 4. The result of this study were following that muscle stimulation group were significantly increased stimulate time blood volume compared with stimulate time blood volume(p<.05).

Therapeutic potential of stellate ganglion block in orofacial pain: a mini review

  • Orofacial pain is a common complaint of patients that causes distress and compromises the quality of life. It has many etiologies including trauma, interventional procedures, nerve injury, varicella-zoster (shingles), tumor, and vascular and idiopathic factors. It has been demonstrated that the sympathetic nervous system is usually involved in various orofacial pain disorders such as postherpetic neuralgia, complex regional pain syndromes, and atypical facial pain. The stellate sympathetic ganglion innervates the head, neck, and upper extremity. In this review article, the effect of stellate ganglion block and its mechanism of action in orofacial pain disorders are discussed.

젊은 비만 여성의 심박변이도 특성에 대한 연구 (A Study on Characters of Heart Rate Variability in Young Overweight and Obese Woman)

  • Objectives Obesity is the major risk factor of the heart disease and the metabolic disease. The autonomic nervous system is a key contributor in the regulation of energy balance, so the blunted activity may contribute to the maintenance of the obese state. So we evaluated the function of the autonomic nervous system in young overweight and obese women with heart rate variability. Methods The subjects were 26 overweight and obese young women(BMI〉$23kg/m^2$) and 25 lean women who visited Kangnam Kyunghee hospital for obesity management from March 2006 to April 2008. Bioimpedance Analysis (BIA), short-term spectral analysis of heart rate variability (HRV) were performed. Results HRV of obese young group was lower than the lean young group, but there is no statistical significance. BMI had significantly a negative correlation with Low Frequency(LF) of HRV. LF power is mediated by sympathetic nervous system activity. These results indicate a decrease of sympathetic modulation in overweight and obese young women. Conclusions Overweight and obese young women have decreased sympathetic nervous system activity. In clinical practice, an assessement of HRV would be noninvasive and sensitive methods for sympathetic nervous system of young overweight and obese women.

The Effect of Manual Lymphatic Drainage on the Stress and Pain in Patient with Postoperative Breast Cancer

  • Objective: This study was at investigated the effects of manual lymphatic drainage (MLD) on stress and pain in patients with postoperative breast cancer. Design: A randomized controlled trial. Methods: A total of twenty-two patients with postoperative breast cancer voluntarily participated in the study. Subjects were randomly assigned to the MLD groups (n=12) and rest groups (n=12). The intervention was conducted in each group for twenty minutes a day, five times a week, for four weeks. Before and after the intervention, the participants measured sympathetic nerve, parasympathetic nerve, and pain by using a sphygmograph and short-form McGill pain questionnaire.An independent t-test was used to analyze pretest and posttest changes between the groups, a paired t-test was used to analyze pre-posttests within each group. Results: After analyzing, the MLD group has been shown a significant decrease in the sympathetic nervous system (p<0.05), a significant increase in the parasympathetic nervous system (p<0.05), and a significant increase in pain (p<0.05). However, the rest group was no significant difference between pre and post. Conclusions: The results of this study confirmed that MLD techniques are an effective method in reducing stress and pain in patients with postoperativebreast cancer. And it is thought that can be used as basic data and to develop programs on stress and pain management reduction for patients with breast cancer.

Effects of Chamaecyparis Obtusa Essential Oil on the Autonomic Nervous System

  • The purpose of this study is to explore the effects of Chamaecyparis obtusa essential oil on the activity of autonomic nervous system. Heart rate variability (HRV), as an indicator of autonomic nervous system activity and blood pressure were measured before and after inhalation of Chamaecyparis obtusa essential oil. The systolic blood pressure was decreased by inhalation of Chamaecyparis obtusa essential oil (p<0.05). The diastolic blood pressure was not changed significantly by Chamaecyparis obtusa. High frequency (HF) power level was not changed but High frequency/Low frequency (HF/LF) ratio was decreased by Chamaecyparis obtusa essential oil (p<0.05), meaning that parasympathetic nervous system activity was not affected but sympathetic nervous system activity was decreased. These results indicate that Chamaecyparis obtusa essential oil has a modulatory effect on the autonomic nervous system activity.

한방병원에 내원한 만성 피로환자와 건강인의 심박변이도 측정을 통한 피로 진단 지표 연구 (Fatigue Diagnostic Measure Research Through The Heart Rate Variability of Chronic Fatigue Patients and Healthy Students in Korean Medical Hospital)

  • This study compared the heart rate variability tests of healthy college students and chronic fatigue patients for 2 years. Study subjects were idiopathic chronic fatigue patients who carried out the HRV tests and students conducted HRV test when health screening test. Study subjects were 250 people, 104 people and men (41.6%), women were 146 patients (58.4%). In patient group, the autonomic nervous system activity was decreased, the activity of the sympathetic nerve and the parasympathetic nerve were significantly reduced compared to the control group(Patient:84.17±16.27, Control:98.33±17.28). Regardless of gender, patient group's autonomic nervous system activity was decreased, the activity of the sympathetic nerve and the parasympathetic nerve were significantly reduced, compared to the control group(Patient Female:84.17±16.27, Patient Male:84.07±14.96/Control Female:98.33±17.28, Control Male:96.45±16.92). Even though same ages, patient group's autonomic nervous activity was reduced compared to control group(Patient:89.36±12.43, Control:97.39±16.91). Fatigue patients' activity of the sympathetic nerve and the parasympathetic nerve were significantly reduced, regardless of the fact that activity of the sympathetic nerve is increased and the parasympathetic nerve is decreased in stress state like an impassioned injury(七情傷). Therefore HRV test is useful to diagnose Fatigue from this study.
