It is well known that concentration of C-reactive protein(CRP) in the serum increase as nonspecific reaction of the various tissue injury. The CRP, synthesized in the hepatocyte, is one of 'acute phase proteins' in the serum. The main signal patterns of this protein are regulated by synthesis of interleukin-I secreted from macrophage in the area of tissue injury. Many studies were performed for quantitative analysis for CRP according to various surgical operation, but the study for fracture patients associated with trauma, especially in mandible, are rare. The mandible fracture have intrinsic danger for infection in oral bacteria associated with open wound in oral cavity, and, are difficult for detection of tissue reaction between surgical swelling and infection by facial swelling. In this study, quantitative analysis for CRP associated tissue injury in mandibular fracture and surgical intervention was done, the results were as follows: 1. After initial mandibular trauma, the value of serum CRP diminished sequentially, most high value was presented in post-traumatic 2 days. 2. The CRP was diminished significantly 2 days after surgical intervention, and maintained normal value in 5 days after surgery. 3. The change of CRP are higher value in surgical intervention than initial trauma, it suggested that tissue injury from surgery was severe than trauma. 4. The high value of CRP was obtained in mandibular fracture combined soft tissue injury than no associated soft tissue injury. 5. In measurement of CRP according to surgical approach, highest serum value in patients of combined intra-oral and extra-oral approach was showed, and intra-oral approach, extra-oral approach, in sequential orders. 6. The CRP value are more higher in patient of 2 fracture site than only one fracture site. From the results obtained in this study, CRP has showed different values in mandibular fracture associated with severity of tissue injury and surgical intervention, and quantitative analysis of CRP value in serum can be applied to the clinical management of mandibular fracture.
The hyaluronic acid (HA) with excellent biocompatibility can be combined with the monomer polylactide with good biodegradability to produce biocompatible materials which can control the period of degradation in a human body. By freeze drying method, HA and the lactic acid, monomer of polylactide, or lactide, the ester dimer of polylactide, were crosslinked with crosslinking agent, l-ethyl-3-(3-dimethyl aminopropyl) carbodiimide. The analysis of infrared spectroscopy showed that the ester linkage was formed and the analysis of nuclear magnetic resonance (NMR) spectroscopy showed that the ester linkage was due to the reaction of lactic acid and HA. The conversion (6∼32%) and degree of crosslinking (4∼19%) increased but the selectivity was almost constant at 62% as the mole ratio of LA to HA increased from 1 to 10 in the crosslinking reaction. The brittleness became more pronounced and the rate of degradation became faster with more addition of lactic acid resulting from the higher ratio of LA to HA, and the swelling ratio was in the range of 500 to 2000%.

Electroacupuncture(EA) is commonly used in various diseases. In the present study, the effect of EA in the allergic mouse model was examined. Allergy is generated via immunological mechanism and non-immunological mechanism. Mast cells activated dy those mechanisms get to release various substances such as histamine, leukotrienes, prostaglandin, TNF-$\alpha$, IL-4, IL-6, etc. which induce allergic reactions and the following inflammatory responses. To evaluate the anti-allergic effects of EA, mortality, ear swelling response, vascular permeability and cytokine secretion were investigated in EA group and non-EA group of which mice were compound 48/80-induced allergy model or PCA model. Compound 48/80 induces allergic reaction via non-immunological mechanism and PCA model is generated through the same mechanism with immediate-type(Type1) allergic reaction, one of immunological allergic reactions. EA inhibited compound 48/80-induced ear swelling response but did not inhibit the systemic anaphylaxis. EA also inhibited passive cutaneous anaphylaxis(PCA) activated dy anti-dinitrophenol IgE. In addition, EA inhibited IL-6 and TNF-$\alpha$ secretion from 48 h PCA in mice. These results indicate that EA may be used for the treatment of mast cell-mediated allergic diseases, especially immediate-type(Type 1) allergy and non-immunologically mediated allergy.

Synthesis of the non-swelling property micas was carried out by hydrothermal method. In order to artificially induce the diffusion of ions, a rotating system was attached to the hydrothermal apparatus and by adding 0.7 mm zircon beads, synthesis of the non-swelling property micas could be performed in a low temperature area. The hydrothermal conditions for the preparation of micas were a reaction temperature of $260^{\circ}C$, for 72 hrs, using $1K_2O,\;1Al(OH)_3,\;4Mg(OH)_2\;and\;6SiO_2$ as the starting materials and a 8M-KOH solution as the hydrothermal solvent. The micas obtained under these conditions were a plate shape with a size of $2.89{\mu}m$ and showed a whiteness of over 97 %. Also, through the FT-IR analysis, because the absorption peak of the $Mg_3OH$ vibration was observed at approximately $3700cm^{-1}$, it could be known that it was phlogopite of non-swelling property showing the chemical composition of $KMg_3AlSi_3O_{10}(OH)_2$. This result was very consistent with the EDS analysis where O (41.34 %), Mg (3.88 %), Al (11.45 %), Si (17.62 %) and K (25.71%) elements were detected.
Beidellite, a member of the dioctahedral smectite group, was synthesized hydrothermally from dickite. Organophilic [DEACOOH]-beidellite intercalation complex was formed by the cation exchange reaction between synthetic Na-beidellite and [DEACOOH]Br. The products dried in high vacuum were treated with various organic solvent such as methanol, ethanol, acetone, ether, acetonitrile and caprolactam in order to determine the swelling behaviour of the prepared complexes. After drying under high vacuum, basal spacing of [DEACOOH]-beidellite shows 15.1 ${\AA}$, and it changed to 19.4, 29.9, 15.9, 16.8, 14.8, 26.5 and 14.8 ${\AA}$ under distilled water, methanol, ethanol, acetone, ether, acetonitrile and caporlactam, respectively. Especially, the characteristics of the intercalation complexes and their swelling behavior of the synthetic beidellite and natural montmorillonite were compared.
Subcutaneous cervicofacial, mediastinal emphysemas are complications associated with head and neck surgery, trauma, infectious processes, tooth extraction. Drill cooling stream and dental syringe air ject are the sources of high pressure air that may enter exposed soft tissue. Since the introduction of the high-speed air turbine drill in the 1960s, The incidence of iatrogenic subcutaneous emphysema has increased. Most cases begin to resolve after 2 to 3 days and residual swelling is usually minimal at the end of 7 to 10 days. Surgical approach is not advised because it is likely to be ineffective. The differential diagnosis of neck swelling after dental procedure includes hematoma, cellulitis, angioedema, allergic reaction, subcutaneous emphysema. We report a rare case of patient with subcutaneous cervicofacial emphysema and mediastinal emphysema secondary to third molar extraction.
A 31-year-old male with the complaint of severe limitation of mouth opening was referred to our department of Chonbuk National University Hospital. The physical status of the patient was hyposthenic. Extraoral examination showed no condylar movement of the both temporomandibular joints, no pain, no facial swelling or paresthesia. Intraoral examination showed several cervical caries on the upper anterior teeth, and gingival swelling on the whole dentition. Transcranial view showed no condylar movement, and narrowing of joint spaces. Chest P-A view showed straightening of thoracic, lumbar spine, and squaring of vertebrae of the same spines. Conventional lateral radiograph of cervical spine showed calcification of the intervertebral ligament. Computed tomograph showed extensive bone formation between temporal bone and the condylar head at both sides. Laboratory findings showed positive reaction on HLA-B27 histocompatibility antigen and increased level of IgA, IgG, ESR. Based on the clinical, radiographic, and the laboratary findings, final diagnosis was made as bony ankylosis of the both temporomandibular joints secondary to ankylosing spondylitis.
Cross-linked starch microspheres were prepared using different kinds of cross-linking agents. The influence of several parameters on morphology, size, swelling ratio and drug release rate from these microspheres were evaluated. These parameters included cross-linker type, concentration and the duration of cross-linking reaction. Microspheres cross-linked with glutaraldehyde had smooth surface compared with those prepared with epichlorhydrine or formaldehyde. The particle size increased with increasing the cross-linking time and increasing the drug loading. Swelling ratio of the particles was a function of cross-linker type but not the concentration or time of cross-linking. Drug release from starch microspheres was measured in phosphate buffer and also in phosphate buffer containing a-amylase. Results showed that microspheres cross-linked with epichlorhydrine released all their drug content in the first 30 minutes. However, cross-linking of the starch microspheres with glutaraldehyde or formaldehyde decreased drug release rate. SEM and drug release studies showed that cross-linked starch microspheres were susceptible to the enzymatic degradation under the influence of alpha-amylase. Changing the enzyme concentration from 5000 to 10,000 IU/L, increased drug release rate but higher concentration of enzyme (20,000 IU/L) caused no more acceleration.
Ulmus parvifolia has been used as a traditional folk medicine to treat the carbuncle in deep skin. In this study, the effect of water extract of root bark of Ulmus parvifolia (WUP) on the carbuncle, pain, inflammation and hypersensitivity was evaluated in animal models. The administration of WUP significantly decreased the size of Staphylococcus aureus ($10^8$ cells/mouse)-induced carbuncle, and also exhibited analgesic activity in the HAc-induced writhing syndrome at doses of 50-500 mg/kg. It also showed significant anti-inflammatory activity in the carageenin- and complete Freund's adjuvant-induced inflammation. In the histamine-induced anaphylaxis, it decreased the percent of mortality by protecting mice treated with Bordetella pertussis. In the immune responses in the mice sensitized and challenged with sheep red blood cells, the Arthus reaction determined by swelling of foot pad at 4 h after challenge, HA titer, HY titer and PFC which can be used to evaluate the humoral immune response were significantly suppressed by oral administration of WUP at doses of 100 and 200mg/kg. The cellular immune responses in the same mice such as delayed type hypersensitivity determined by swelling of foot pad at 24 h after challenge and RFC were also significantly suppressed in the same manner.
Lamierre's syndrome is characterized by a rare fulminant condition resulting from primary oropharyngeal infection followed by secondary septic thrombophlebitis of the internal jugular vein and metastatic infection. A forty-year-old man who had been on ventilator due to servere chest trauma, showed severe reddish inflammatory swelling of the right cervical soft tissue and newly developed pneumonia. He went into in septic condition shortly thereafter. Thrombophlebitis with central abscess in the right internal jugular vein was identified by neck CT and MRA(magnetic resonance angiography). Right cervical swelling worsened in spite of clindamycin and heparin therapy. We performed immediate surgery for removal of septic thrombus and resection of internal jugular vein. Patient's septic condition, pneumonia, and local inflammatory reaction were improved within several days after surgery.
