Mode of Action of the Bacteriocin from Lactobacillus sp. GM7311 against Gram Positive Bacteria (Lactobacillus sp. GM7311이 생산하는 박테리오신의 Gram 양성균에 대한 작용형태)

  • The bacteriocin produced by Lactobacillus sp. GM7311 showed strong inhibitory activity against the growth of three Gram positive bacteria, Listeria monocytogenes, Bacillus subtilis, and Staphylococcus aureus. When the bacteriocin was added to the culture at different phases, viable cells of all of the tested strains were decreased, although the most inhibited phase was different. Thereby, when the bacteriocin $(100\;Bu/m{\ell})$ was added to exponential and stationary phase of L. monocytogenes, the rapid reduction of viable cell counts occured. And, in the case of B. subtilis, the highest inhibitory effect occured at lag phase and mid-exponential phase by the addition of the bacteriocin under same condition as mentioned above. Also, we can observe the accelerated reduction of survivors counts for the all of the phase except stationary phase in the S. auresus. Transmission electron microscopic observation of L. monocrogenes and B. subtilis treated with bacteriocin revealed apparent Iysis of the cell wall and excretion of the cell contents, indicating bacteriolysis. Also, the amino acids and fatty acids compositions were different from controls. However, the Iysis of cell wall didn't occur in S. aureus, though the cytoplasmic materials were reduced. This result indicates that the bacteriocin inhibits the synthesis of nuclear materials such as DNA, RNA and proteins.

Abdomino-perineal Organ Injuries Caused by Cultivators (경운기 사고에 의해 발생한 복부 및 회음부 장기 손상)

  • Purpose: Cultivator accidents are frequent and often lead to abdomino-perineal organ injury and, if severe, to death. This study presents the clinical characteristics, outcomes, and factors associated with mortality in patients who sustained an abdomino-perineal organ injury in cultivator accidents. Methods: We retrospectively analyzed the records of 53 patients who visited the emergency department of a tertiary hospital with abdomino-perineal organ injuries caused in cultivator accidents from April 2005 to March 2010. Results: All 53 patients had visited other medical institutions before visiting our hospital. Their mean age was $64.0{\pm}11.1$ (range, 20-80) years and 32 (60.4%) patients were 65 or older. The male-to-female ratio was 46:7. The chief complaint was abdominal pain (38 cases, 71.7%). The 53 patients included 41 cultivator operators (77.4%), 11 passengers (20.8%), and 1 passerby (1.9%). The causes of the injuries included a direct impact of the handlebar in 20 cases (37.7%), a rollover in 21 cases (39.6%), a fall in 10 cases (18.9%), and a wheel in two cases (3.8%). Several of the 53 patients had injuries to multiple abdomino-perineal organs, and the injured organs included the liver (23 cases, 26.4%), spleen (16 cases, 18.4%), pancreas (7 cases, 8.0%), small bowel (7 cases, 8.0%), mesentery (6 cases, 6.9%), adrenal gland (5 cases, 5.8%), and other organs. According to the abbreviated injury scale (AIS) dictionary, a thoracic injury was the most frequent co-injury (33 of 53 cases, 62.3%). Abdomino-perineal surgery was performed in 31 cases (58.8%) and angio-embolization was performed for six liver and two kidney injuries. Thirteen patients died (24.5%); all were males. The Injury Severity Scale (ISS) was lower in the survivors ($17.8{\pm}8.5$ vs. $27.0{\pm}16.0$; p=0.010). Conclusion: With the aging of agricultural workers, safety education programs should be implemented. Furthermore, the patient transfer system in agricultural areas must be improved.

Comparison between Gel Pad Cooling Device and Water Blanket during Target Temperature Management in Cardiac Arrest Patients

  • Background: Target temperature management (TTM) improves neurological outcomes for comatose survivors of out-of-hospital cardiac arrest. We compared the efficacy and safety of a gel pad cooling device (GP) and a water blanket (WB) during TTM. Methods: We performed a retrospective analysis in a single hospital, wherein we measured the time to target temperature ($<34^{\circ}C$) after initiation of cooling to evaluate the effectiveness of the cooling method. The temperature farthest from $33^{\circ}C$ was selected every hour during maintenance. Generalized estimation equation analysis was used to compare the absolute temperature differences from $33^{\circ}C$ during the maintenance period. If the selected temperature was not between $32^{\circ}C$ and $34^{\circ}C$, the hour was considered a deviation from the target. We compared the deviation rates during hypothermia maintenance to evaluate the safety of the different methods. Results: A GP was used for 23 patients among of 53 patients, and a WB was used for the remaining. There was no difference in baseline temperature at the start of cooling between the two patient groups (GP, $35.7^{\circ}C$ vs. WB, $35.6^{\circ}C$; P=0.741). The time to target temperature (134.2 minutes vs. 233.4 minutes, P=0.056) was shorter in the GP patient group. Deviation from maintenance temperature (2.0% vs. 23.7%, P<0.001) occurred significantly more frequently in the WB group. The mean absolute temperature difference from $33^{\circ}C$ during the maintenance period was $0.19^{\circ}C$ (95% confidence interval [CI], $0.17^{\circ}C$ to $0.21^{\circ}C$) in the GP group and $0.76^{\circ}C$ (95% CI, $0.71^{\circ}C$ to $0.80^{\circ}C$) in the WB group. GP significantly decreased this difference by $0.59^{\circ}C$ (95% CI, $0.44^{\circ}C$ to $0.75^{\circ}C$; P<0.001). Conclusions: The GP was superior to the WB for strict temperature control during TTM.

The Impact of Secondary Trauma Stress on Occupational Stress Levels of Sexual Violence Support Workers : The Moderating Effect of Work-Related Job Education (성폭력피해자 지원센터 종사자의 이차적 외상 스트레스가 직무 스트레스에 미치는 영향 : 직무교육의 조절효과)

  • This study investigated the impact of the secondary trauma stress experienced by workers who support sexual violence victims on occupational stress level, and verified the moderating effect of the job education from support facilities in this relationship. The data analyzed in this study were sampled from "The Primary Study on The Development of Training Program to Cultivate Professional Workers of Support Organizations for Sexual Violence Survivors" as a secondary data and 482 nationwide workers were selected. By applying SPSS PROCESS macro, the moderation model was analyzed. The analysis results are as follows. First, sexual violence victim support workers' secondary trauma stress has a significantly influence on occupational stress level of workers. Second, the job education from support agencies themselves moderates the relationship between the secondary trauma stress and occupational stress level of support agents. It also showed that the higher level of job education participates, the less the experience of being stressed on the job. Thus, on the basis of the result, the theoretical discussion on the secondary trauma stress and occupational stress level of sexual violence victim support worker was drawn out and the empirical implication on the direction of the education for job related was suggested.

Angiopoietin-1 and -2 and vascular endothelial growth factor expression in ovarian grafts after cryopreservation using two methods

  • Objective: The favored method of preserving fertility in young female cancer survivors is cryopreservation and autotransplantation of ovarian tissue. Reducing hypoxia until angiogenesis takes place is essential for the survival of transplanted ovarian tissue. The aim of this study was to investigate the role of angiopoietin-1 (Angpt-1), angiopoietin-2 (Angpt-2), and vascular endothelial growth factor (VEGF) in ovarian tissue grafts that were cryopreserved using two methods. Methods: Ovarian tissues harvested from ICR mice were divided into three groups: group I (control), no cryopreservation; group II, vitrification in EFS (ethylene-glycol, ficoll, and sucrose solution)-40; and group III, slow freezing in dimethyl sulfoxide. We extracted mRNA for VEGF, Angpt-1, and Angpt-2 from ovarian tissue 1 week following cryopreservation and again 2 weeks after autotransplantation. We used reverse transcriptase-polymerase chain reaction to quantify the levels of VEGF, Angpt-1, and Angpt-2 in the tissue. Results: Angpt-1 and Angpt-2 expression decreased after cryopreservation in groups II and III. After autotransplantation, Angpt-1 and Angpt-2 expression in ovarian tissue showed different trends. Angpt-1 expression in groups II and III was lower than in group I, but Angpt-2 in groups II and III showed no significant difference from group I. The vitrified ovarian tissues had higher expression of VEGF and Angpt-2 than the slow-frozen ovarian tissues, but the difference was not statistically significant. Conclusion: Our results indicate that Angpt-2 may play an important role in ovarian tissue transplantation after cryopreservation although further studies are needed to understand its exact function.

A Case Study on the Documentation in a Disaster Area - On the Basis of Great Hanshin-Awaji Earthquake - (재해 지역의 기록화 사례 연구 - 한신·아와지 대지진 기록관리 사례를 중심으로 -)

  • Records are the social memory storage including collective memory about region, it is impossible to put into and represent all aspects of society with only public records. Japan showed the possibilities of cooperative collecting and the positive records producing and collecting efforts between organizations, even if it was not accomplished by systemic documentation strategy. Some characteristics were found out when this case was reviewed, it is as follows. To begin with, it is the cooperation and share in collecting. Especially, the activities of private organizations look better than public organizations and the cooperative collecting efforts lead to transmit much more social memory and historical records to next generation. Secondly, it is the positive records producing and collecting. The private organizations also left many records of various activities of those one accord. They recognized that recording experience and leaving behind is the survivors' responsibility. We cannot help recognizing the growth of a sense of duty and historical consciousness to record their own experience with undergoing big disaster, earthquake. Thirdly, there was no limit when it comes to the collecting scope. All records related with people and place in disaster area were the target for collecting just like slogan, 'Let's transmit records to next generation as much as possible', 'Common records and resources deserve leaving, because it is important life's information proving situations of the time. We were able to confirm the high will and enthusiasm about 'how, what and why do we transmit something of society to next generation' in this case.

Factors Influencing Stroke in Community-dwelling Adults : Focusing on Health-related Quality of Life (지역사회거주 성인의 뇌졸중 영향 요인 : 건강관련 삶의 질을 중심으로)

  • Objective : The purpose of this study was to investigate the factors influencing stroke in community-dwelling adults. Methods : This study used raw data from the community health survey in 2016. Among the 228,452 subjects who participated in the survey, 225,003 (98.5%) of them were included in the analysis of this study. The sociodemographic characteristics were selected as gender, age, region, income, marital status, and comorbidity was selected as osteoporosis, hypertension, diabetes, dyslipidemia, myocardial infarction and arthritis. Health-related quality of life was assessed by EuroQol-5 Dimension(EQ-5D) and the subcategory of EQ-5D (mobility, self-care, usual activities, pain/discomfort, anxiety/depression) were included in the analysis. Dependent variables were stroke, and the independent variables were the 16 variables described above. Statistical analysis was performed using binomial logistic regression analysis. Results : In sociodemographic variables, stroke was predicted by men, aging, and lower income levels. In comorbidity variables, stroke was predicted by hypertension, diabetes mellitus, hyperlipidemia, and myocardial infarction. In sub-domains of health-related quality of life, stroke was predicted by self-care, usual activities, mobility, anxiety/depression, and pain/discomfort. Conclusion : These finding suggest that it is needed development of a customized health promotion program for the improvement of self-care and activities of daily living in community-dwelling stroke survivors.

The Effects of Convegenced Integration Support Program for Home-Based Cancer Patients (재가암환자를 위한 융합적 통합지지 프로그램의 적용 효과)

  • This study is an experimental study on the single group pre-post test design for the convegenced of an integration support program for home-based cancer patients and the verification of its effects. Targeting total 33 home-based cancer patients in J city, total 30 sessions of the convegenced integration support program composed of cancer-related knowledge education, stress management, cognitive behavioral approach, and exercise for revitalization were provided three times a week for ten weeks. In the results of this study, the convegenced integration support program reduced stresses of the subjects and also improved their hopes and self-esteem. Thus, this program is considered as a onvegenced integration support program suitable for home-based cancer patients. In the future, it would be necessary to seek for the nursing interventional strategies for the expansion for this program to regional health centers or community institutes performing the convegenced integration support program for cancer survivors, so that they could continuously participated in the program.

Analysis of Affecting Factors on Exposure Assessment Errors and Characteristics of Applicants for Damage by Usage of Humidifier Disinfectants (가습기살균제 사용에 따른 피해 신청자들의 특성 및 노출평가 오류 영향요인 분석)

  • Objectives: The lung injuries by exposure to the humidifier disinfectants (HDs) were reported in 2011, Korea. For the HD victims, environmental exposure level and clinical diagnosis were conducted to determine the levels of damage by HDs. Methods: The exposure assessment to the HDs from 1st to 4th questionnaire surveys were carried out for 5,245 victims. And the affecting factors of exposure levels were analyzed by characterizing exposure and demographic information. By using of exposure concentration and cumulative time, exposure levels were classified and compared by percentage of clinical diagnosis classes. The high exposure and low clinical diagnosis rating groups, and low exposure and high clinical diagnosis rating groups were analyzed to overcome the limitation of past exposure assessment such as recall bias. Results: Among the all applicants damaged by the humidifier disinfectants, survivors were 4,028 and the dead were 1,217. And male and female were 2,675, and 2,547, respectively. In case of occurrence age of lung disease, under 10 years was majority age group (1,536) and followed by thirties (917). Pregnant women and fetuses were 339 and 439, respectively. And the damages by exposure to the HDs were concentrated on these susceptible populations in groups with low exposure and high clinical diagnosis rating. On the other hand, the groups classified by high exposure and low clinical diagnosis rating were shown different characterization. Conclusions: The questionnaire survey on past exposure may be uncertain due to recall bias. However, the relationship between classified exposure levels and clinical diagnosis ratings might be shown positive correlation if the exposure assessment errors were analyzed and controlled.

Relationship between psychological stress and hippocampus volume through meta-analysis (메타분석을 통한 심리적 스트레스와 해마용적 간의 관계)

  • This study was conducted to define hippocampal volume differences between a stress-laid group and normal individual group via meta-analysis. This investigation included studies that contained hippocampal volume measurements in association with stress from 1990 to 2016. Magnetic resonance imaging (MRI) measurements of hippocampal volumes were used to determine the mean size, standard deviation and sample size. The pooled effect size was measured by the standardized mean difference (SMD) with a 95% confidence interval. This meta-analysis included 13 studies comprising 374 stress subjects and 439 normal subjects. Eight studies targeted war-related veterans and holocaust survivors, while five targeted personal trauma associated with childhood abuse, rape, accident or financial hardship. The overall mean effect size (Hedges' g) was -0.70 (-1.04; -0.37). Overall, the SMD value was -0.70, with a moderate effect size, but high heterogeneity. Depending on the characteristics of the stressor, studies were divided by subgroups of war-related veterans and subjects with personal trauma and then analyzed again. In the war-related subgroup, the Hedges' g was -0.47 (-0.78; -0.16), while in the personal stressor-related subgroup it was -0.91 (-1.57; -0.26). These results indicate that there was a significant negative correlation between psychological stress and hippocampal volume.
