A 69-year-old Woman with Anomalous Origin of Left Coronary Artery from the Pulmonary Artery: Surgical Repair Using a Trap-door Flap

  • We present a patient with anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA), which was diagnosed and corrected in her 60 s. The patient is the oldest documented survivor of ALCAPA who underwent a surgical repair. ALCAPA should be corrected surgically to restore the dual coronary system at any age and this case shows that the surgical procedure may be performed safely even in an elderly patient.

  • Aortoesophageal fistula (AEF) is a rare and potentially fatal disease that causes massive gastrointestinal bleeding. Therefore, early diagnosis and treatment are essential to prevent mortality. Controlling the massive bleeding is the most important aspect of treating AEF. The traditional surgical treatment was emergent thoracotomy, but intraoperative or perioperative mortality was high. We report a case of a patient presenting with hematemesis who was successfully treated by a staged treatment, in which bridging thoracic endovascular aortic repair was followed by delayed surgical repair of the esophagus and aorta.

  • Damage to the endotracheal tube (ETT) during oral and maxillofacial surgeries is a rare but serious intraoperative complication. Herein, we present a case of a partially damaged ETT that was repaired using Dermabond surgical adhesive (Ethicon, Sommerville, NJ, USA) during a Lefort osteotomy. Dermabond surgical adhesive can be a simple and viable tool to repair partially transected ETTs where ETT exchange carries a high risk of airway loss. Our case adds to one of the several techniques for managing damaged ETT in an intraoperative setting.

  • Situs inversus totalis is a rare condition of visceral transposition in thoracic and abdominal cavities. Computed tomography (CT)-based morphometric analysis of the cardiovascular system prior to the surgery helps to describe vessel topography and size, choose the right surgical insertion site, avoid vessel trauma, and prevent hemorrhage during surgical intervention. We present a case report of situs inversus totalis detected incidentally in a 74-year-old male with the acute abdominal syndrome. Appropriate detailed aorta measurements are used to choose an adequate size of the aortic prosthesis during open surgical repair or endovascular aneurysm repair. An accurate assessment of the vessels on CT scans assists in consideration of the catheter diameter and the most reliable cannulation site. Vessel size correlates with morphological conditions (kinking, stenosis, occlusion), which may be considered a risk of organ malperfusion. The anatomical analysis prior to surgery in different anatomical variations may ensure patient safety and predict complications.

Ulnar collateral ligament repair in professional baseball players

  • Background: While initial reports of surgical repair are promising, literature detailing post-repair outcomes is lacking. This study explores the effectiveness of ulnar collateral ligament (UCL) repair in returning professional baseball players to their pre-injury level of play. Professional baseball players undergoing UCL repair will have successful outcomes and high return to sport rates after surgery. Methods: From 2016 to 2021, publicly available databases were utilized to search for professional baseball players who underwent UCL repair. Players were included if they underwent primary UCL repair with an internal brace. Players were excluded if they underwent revision UCL repair, UCL reconstruction, or were amateur baseball players. Results: Of the 11 pitchers that underwent UCL repair, MiLB pitchers returned to sport at an average of 17.5 months. MiLB pitchers had similar ERA, games played, innings pitched, and walks plus hits per inning pitched (WHIP) before and after surgery. Four MLB pitchers (80%) returned to sport at 9.55 months. MLB pitchers had fewer games played and innings pitched but similar ERA and WHIP in after versus before surgery. Pitch velocity and spin rates after surgery varied based on pitch type. The 7 positional players who underwent UCL repair showed no differences in batting or fielding performances before versus after surgery. Conclusions: UCL repair can successfully return both pitchers and positional players at both the MiLB and MLB level back to play at pre-injury levels. Repair can be considered as an option in the right type of injury for players hoping to maximize performance after surgery with minimal recovery time.

  • 관절경적 회전근 개 복원술 시에 시행하게 되는 골 처치에는 견봉 성형술, 원위 쇄골 절제술, 건 부착부(대결절 부위) 처치 및 오구 성형술 등이 있다. 각각의 골 처치는 여러 이론적 근거에 의해 적응증을 가지며, 수술방법 또한 다양하게 알려져 있다. 이에 관절경적 회전근 개 복원술 시에 사용되는 여러 골 처치의 적응증 및 방법에 대해 기술하고자 하였다.

Traumatic lumbar hernia: clinical features and management

  • Purpose: Traumatic lumbar hernia is rare, thus making diagnosis and proper treatment challenging. Accordingly, we aimed to investigate the clinical manifestations and proper management strategies of traumatic lumbar hernias. Methods: The medical records of patients with traumatic lumbar hernia treated at Gachon University Gil Hospital from March 2006 to February 2015, were retrospectively reviewed. Results: We included 5 men and 4 women (mean age, 55 years; range, 23-71 years). In 8 patients, most injuries were caused by motor vehicle collisions, including those wherein a pedestrian was struck (5 cases of car accidents, 2 falls, and 1 involving penetrating materials); in 1 patient, the probable cause was severe cough. Eight patients underwent hernia repair surgery (5 open and 3 laparoscopic), and a prosthetic mesh was used in 7 patients. Hernia repairs were elective in 7 patients; emergency hernia repair was performed with right hemicolectomy in 1 patient. No severe complication or recurrence was observed. Only 2 patients had mild complications, such as postoperative seroma. Conclusion: Traumatic lumbar hernia is a relatively rare injury of the posteriolateral abdominal wall. Lumbar hernia should be suspected in patients with high-energy injuries of the torso, and all such patients should undergo abdominopelvic computed tomography. After diagnosis, hernia repair can be electively performed without complications in most cases.

  • The facial nerve stimulates the muscles of facial expression and the parasympathetic nerves of the face. Consequently, facial nerve paralysis can lead to facial asymmetry, deformation, and functional impairment. Facial nerve palsy is most commonly idiopathic, as with Bell palsy, but it can also result from a tumor or trauma. In this article, we discuss traumatic facial nerve injury. To identify the cause of the injury, it is important to first determine its location. The location and extent of the damage inform the treatment method, with options including primary repair, nerve graft, cross-face nerve graft, nerve crossover, and muscle transfer. Intracranial proximal facial nerve injuries present a challenge to surgical approaches due to the complexity of the temporal bone. Surgical intervention in these cases requires a collaborative approach between neurosurgery and otolaryngology, and nerve repair or grafting is difficult. This article describes the treatment of peripheral facial nerve injury. Primary repair generally offers the best prognosis. If primary repair is not feasible within 6 months of injury, nerve grafting should be attempted, and if more than 12 months have elapsed, functional muscle transfer should be performed. If the affected nerve cannot be utilized at that time, the contralateral facial nerve, ipsilateral masseter nerve, or hypoglossal nerve can serve as the donor nerve. Other accompanying symptoms, such as lagophthalmos or midface ptosis, must also be considered for the successful treatment of facial nerve injury.

대동맥 결손증 (Type A) 의 외과적 치험 (Surgical Repair of Interruption of the Aortic Arch[Type A] -A Report of 5 Cases-)

  • Between 1981 and 1987, five patients with an interruption of the aortic arch were operated upon. All had a ventricular septal defect and a patent ductus arteriosus as associated anomalies. A two-stage procedure was employed in these cases, the initial procedure being repair of the interrupted arch, ligation of the patent ductus arteriosus, banding of the main pulmonary artery and a lung biopsy which was followed, 5 to 49 months later, by the repair of the ventricular septal defect. Four patients completed the two-stage procedure with one postoperative mortality. The remaining patient is yet to complete the second stage procedure.

  • Eventration of the diaphragm is a rare anomaly, the cause of which still is not understand completely. Recently we were experienced two cases of diaphragmatic eventration which were successfully treated with surgical diaphragmatic plication at the department of thoracic and cardiovascular surgery, college of medicine, chosun university. Specific complication were not noticed after surgical repair of diaphragmatic eventration with good results.
