수술적 방법으로 유도된 3단계 고도(高度) 발목염좌 모델에서 혈위(穴位)에 따른 전침효과의 비교 (Comparison Study for Analgesic Effects of Electroacupuncture on Surgical Ankle Sprain Model Classified as Grade 3 in Rats)

  • Objectives : The purpose of this study was to determine whether electroacupuncture(EA) is effective in reducing pain on the severe ankle sprain classified as grade 3 in rats. Methods : The severe(grade 3) ankle sprain model was induced surgically by ankle ligament injury(the anterior talofibular, the calcaneofibular and the posterior talofibular) in the Sprague-Dawley rats(180~250 g). The effects of EA on weight bearing forces(WBR) of the affected foot were examined in a rat model of ankle sprain. EA was applied to either SI6, ST37, GB34, GB39 or GB42 acupoints by trains of electrical pulses(2 Hz, 1 ms pulse width, 2 mA intensity) for 15 min. Results : Cutting of the lateral ankle ligament complex produced the severe ankle sprain symptoms as grade 3. EA of the contralateral SI6 resulted in more analgesic effect than one of ipsilateral SI6 even though there was significant effect. EA of the ipsilateral GB34 and GB39 produced potent analgesic effects on the surgical ankle sprained pain behaviors. However, there were no significant analgesic effects in the contralateral GB34 and GB39 EA groups. In addition, both side of ST37 and GB42 did not result in analgesic effect on the surgical ankle sprained rat. Conclusions : The data suggest that EA induced analgesia shows point specificity on the severe ankle sprained pain model classified as grade 3.

  • Disposable blade is widely used for palatal and oral mucosal incision in oral and maxillofadal surgery nowadays, But its design and durability need for improvement, Especially, there are so many hard tissues intraoral area, such as bone and tooth, therefor the sharpness of the surgical blade was easily destroyed, The purpose of this study was to make basic data for developing new design of surgical blade using in oral and maxillofacial area including for the patients who have cleft lip and palate deformities, Some questionnaires about the usefulness of currently used surgical blades were sent to 150 dentists, the 54 of them made a reply, Secondly, The used-once blade and fresh new blade were examined under the scanning electron microscope with the 4000-times magnification, Lastly, the tissue reaction following the surgical incision with a fresh-new and a used blade on rat buccal cheek mucosa and hard palate was evaluated with light microscope with hematoxilin-eosin staining, The time interval from the surgical trauma to taking a sample were 1 day, 3 days, 7 days, and 14 days, At each time schedule, 2 Sprague-Dawley rats were sacrificed, Many dentists were agreed to need for changing the design of the surgical blades and also demand to improve the durability of the blades, They were also eager to adopt the new design of blade if it was available, The blade used in surgical extraction procedure was heavily damaged in its sharpe edge of number 15 blade, The histological differences were not prominent, but the delayed healing was detected in buccal mucosal defects especially in the surgical group with used blade, There are slight different changes in hard palatal defects between a used and a new blade group, In this study, we could find that there are imperative demanding on improvement of surgical blade design and durability for oral and maxillofadal area, The blade currently using in surgical extraction was easily damaged, The animal model of this study was not perfect for the purpose of this study.

A Prototype of Robotic External Fixation System for Surgery of Bone Deformity Correction

  • A robotic external fixation system for the surgery of bone deformity correction was developed to simulate the execution process of mal-unioned femur by the adjustment of the joints of the fixation system. An inverse kinematics analysis algorithm was developed to calculate the necessary rotations and translations at each joint of the robotic system. The computer graphic model was developed for validation of the analysis result and visualization of the surgical process. For given rotational and angular deformity case, the surgical execution process using the robotic system was well matched with the pre-operative planning. The final residual rotational deformities were within $1.0^{\circ}{\sim}1.6^{\circ}$ after surgical correction process. The presented robotic system with computer-aided planning can be useful for knowledge-based fracture treatment and bone deformity correction under external fixation.

전방십자인대의 최적 재건을 위한 등장성 해석 (Analysis of Isometry of the Anterior Cruciate Ligament for Optimal Ligament Reconstruction)

  • The anterior cruciate ligament (ACL) is liable to a major injury that often results in a functional impairment requiring surgical reconstruction. The success of reconstruction depends on such factors as attachment positions, initial tension of ligament and surgical methods of fixation. The purpose of this study is to find isometric positions of the substitute during flexion/extension. The distance between selected attachments on the femur and tibia was computed from a set of measurements using a 6 degree-of-freedom magnetic sensor system. A three-dimensional knee model was constructed from CT images and was used to simulate length change during knee flexion/extension. This model was scaled for each subject. Twenty seven points on the tibia model and forty two points on the femur model were selected to calculate length change. This study determined the maximum and minimum distances to the tibial attachment during flexion/extension. The results showed that minimum length changes were $1.9{\sim}5.8mm$ (average $3.6{\pm}1.4mm$). The most isometric region was both the posterosuperior and anterior-diagonal areas from the over-the-top. The proposed method can be utilized and applied to an optimal reconstruction of ACL deficient knees.

증강현실 기반의 최소침습수술용 인터페이스의 개발 (Development of Immersive Augmented Reality interface for Minimally Invasive Surgery)

  • This study developed a novel augmented reality interface for minimally invasive surgery. The augmented reality technique can alleviate the sensory feedback problem inherent to laparoscopic surgery. An augmented reality system merges real laparoscope image and reconstructed 3D patient model based on diagnostic medical image such as CT, MRI data. By using reconstructed 3D patient model, AR interface could express structure of patient body that is invisible outside visual field of laparoscope. Therefore, an augmented reality system improved sight information of limited laparoscope. In our augmented reality system, the laparoscopic view is located at the center of a wide-angle concave screen and reconstructed 3D patient model is displayed outside the laparoscope. By using a joystick, the laparoscopic view and the reconstructed 3D patient model view are changed concurrently. With our augmented reality system, the surgeon can see the peritoneal cavity from a wide angle of view, without having to move the laparoscope. Since the concave screen serves immersive environments, the surgeon can feel as if she is in the patient body. For these reasons, a surgeon can recognize easily depth information about inner parts of patient and position information of surgical instruments without laparoscope motion. It is possible for surgeon to manipulate surgical instruments more exact and fast. Therefore immersive augmented reality interface for minimally invasive surgery will reduce bodily, environmental load of a surgeon and increase efficiency of MIS.

Cone-Beam CT를 이용한 악교정 수술용 스텐트 제작과 임상 적용 (SURGICAL STENT FABRICATION AND CLINICAL APPLICATION FOR ORTHOGNATHIC SURGERY USING Cone-Beam CT)

  • The application of CT with basis on 3 dimensional-reconstruction is getting more widely practiced. With the data obtained from cone-beam computed tomography(CBCT), not only the diagnosis of the patient with skeletal abnormality but also the virtual simulation of the orthognathic surgery were performed and its application would be popular in orthodontic field. We reported a case, a 19-year old man who was diagnosed mandibular prognathism and required orthognatic surgery. In this case, the virtual orthognathic surgery was simulated and surgical wafer was fabricated by using CBCT data. That wafer was applied the actual orthognathic surgery. After preoperative orthodontic treatment, we prepared surgery as follows. : (l)Acquisition of 3D image data, (2)Reconstruction of 3-dimensional virtual model, (3)Virtual model surgery, (4)Extraction of stere-olithographic image, (5)Check-up for occlusal interference, (6)Fabrication of surgical stent by stereolithography. Bilateral sagittal split ramus osteotomy was operated and used stereolithographic surgical stent. 1 month later, we superimposed CBCT datas of virtual surgery and that of actual surgery, and then compared the result. CT data's application for othognathic surgery yielded satisfactory outcomes.

  • In this study, the accuracy of the orthognathic surgical guides with single-stage split path was upgraded to realize orthognathic surgical guides with two-stage split path and simulated surgery was performed to verify its accuracy. As a result, the average error distance between the simulation model and the scan model was + 0.289 / - 0.468 mm (standard deviation 0.128), which was confirmed to be within ± 0.5 mm, which is a clinically acceptable level. Also, there was no significant difference compared with the average value of + 0.313 / - 0.456 mm (average standard deviation 0.106) of the conventional single-stage split path type device. It is judged that the use of this device can contribute to the reduction of surgical time and increase in accuracy since a separate finishing operation for bone preparation is unnecessary.

QLF의 원리와 임상적 활용 (QLF Concept and Clinical Implementation)

  • The leading paradigm of dentistry had been focused on the rehabilitation treatment that identifies active caries, manages them surgically, and restores their original functions. However, changes in the external environment including the current disease prevalence require dentistry to have a paradigm shift. The new paradigm suggests the detection of caries in their earlier stages over the visual diagnosis of cavities, and the reversal of the incipient caries by non-surgical approach. For this to be achieved, a high-technology detection device recognizing changes in the earlier stages which can not he visually observed is needed. Development of early caries detection device has recently become a major issue in preventive dentistry on account of this need, and QLF(Quantitified Light induced Fluorescence) conspicuously stands out among the newly released devices. In this study, the fundamental concept of QLF(Quantitified Light induced Fluorescence) and the possible clinical applications of the earlier intraoral camera model as well as the recently designed digital camera model will be discussed.

Nasal airflows in deformed nasal cavity models

  • Several studies have utilized physical models of the healthy nasal cavity to investigate the relationship between nasal anatomy and airflow. With our experiences of experimental investigations on nasal airflows in normal and abnormal nasal cavity models, we are going to deal with the topic that may contribute to the diagnosis and treatment of nasal diseases. In this paper, airflows in the normal and artificially deformed models, which simulate surgical treatment, are investigated experimentally by PIV. High-resolution CT data and careful surface rendering of computational model with the help of the ENT doctor provide more sophisticated nasal cavity models. The CBC PIV (Correlation Based Correction PIV) algorithm with window offset is used for PIV flow analysis. Average and RMS distributions in sagittal and coronal sections are obtained for inspiratory and expiratory nasal airflows. Comparisons in nasal airflows for both normal and deformed cases are also appreciated. In case of simulations of surgical operations, velocity and RMS distributions in coronal section changes locally, this may cause some difficulties in physiologic functions of noses and may hurt mucosal surface.

구강암 절제 및 재건 수술에 따른 기도 내 공기 유동 시뮬레이션 (Numerical Simulation of Air Flows in Human Upper Airway for Free Flap Reconstruction Following Resection Surgery in Oral Cancer Patients)

  • Oral cancer surgery changes the morphologic characteristics of the human upper airway. These changes can affect the flow patterns. In this study, computational fluid dynamics (CFD) simulations with transient solver were performed to numerically investigate the air flows in the human upper airways depending oral cancer surgery. 3D reconstructed models were obtained from 2D CT images of one patient. For the boundary condition, the realistic breathing cycle of human was applied. The hydraulic diameters of cross-sections for post-surgical model are changed greatly along streamwise direction, so these variations can cause higher wall shear stress and flow disturbance compared to pre-surgical model. The recirculation flows observed in the protruding region result in the relatively large pressure drop. These results can be helpful to understand the flow variations after resection surgery of oral cancer.
