A Clinical Study of Parotid Gland Tumors (이하선 종양의 임상적고찰)

  • This is a clinical analysis of 24 cases of parotid gland tumor who were treated in the department of Surgery, Chonnam National University Medical School during the past 10years from January, 1976 to December, 1985. According to this analysis of total 24 cases, we concluded as follows; 1) 21 cases were benign tumors and 3 cases were malignant tumors. As the histopathological findings, benign tumor included 88%and malignant tumor 12%. Majority of benign tumor contained mixed tumor(86.7%), and the malignant tumor contained mucoepidermoid carcinoma (66.7%). 2) The peak age incidence was 40th in benign tumor and 50th in malignant tumor. 3) The chief complaints of patient was a painless mass and the duration of illness was average 5.2 years. 4) The mean size of mass was 4.5cm in diameter. 5) The surgical procedures were performed with excision 6 cases, superficial lobectomy 8 cases, wide excision with partial parotidectomy 4 cases, total parotidectomy 3 cases in benign tumors. In malignant tumors, total parotidectomy 2 cases and wide excision with partial parotidectomy 1 case were performed. 6) Major postoperative complications such as facial nerve palsy 7 cases (temporary ; 5 cases, permanent; 2 cases), Frey syndrome 1 case, seroma 1 case, hematoma 1 case, and wound infection 1 case were developed. The recurrence contained pleomorphic adenoma 1 case and mucoepidermoid carcinoma 1 case.

A Literary Study on Embedding Therapy (매선료법(埋線療法)에 대한 문헌적(文獻的) 고찰(考察))

  • Background : This study focuses on the new acupuncture method of embedding method which inserts a substance on the acupuncture points for continuous stimulation. Clinical applications and cautions were examined through literary investigations. Results : Based on the literary consideration of embedding method, the following results were obtained : 1. Embedding method is a combination of traditional and embedding technique to provide longer duration of stimulation on the acupuncture points. 2. To administer the embedding method, one needs to utilize a embedding thread besides acupuncture apparatus. Sheep gut is commonly used in China and the surgical thread is the choice in Korea. 3. Embedding method may vary from the patient to patient, depending on the nature and location of the illness. Piercing, embedding, and tying are some of the possibilities. 4. Embedding method may have different arrangement of threads based on the choice of usage. 5. Embedding method is effective for various chronic illnesses such as aches, functional diseases, and the diseases of internal organs. 6. When using the embedding methods, cautions against infection and side effects due to strong stimulation are mandatory.

A Rare Case of Auricular Endochondral Pseudocyst (이개연골에 발생한 가성낭종 1예)

  • Auricular endochondral pseudocyst is a very rare, benign intracartilaginous cystic lesion which most commonly presents as a cystic mass in the anterior plane of the auricle. We present a case report of a 48-year-old man with a fluctuating lesion of 3 week's duration on the left auricle, with no specific history of trauma or disease. Initial incisional drainage revealed an abundance of serous fluid, which quickly recurred. Surgical removal of the hypertrophic perichondrium forming the pseudocyst anterior wall and ear cartilage curettage was carried out with intraoperative absolute alcohol sclerotherapy, followed by compression dressings. The auricle healed uneventfully, with a good final cosmetic result and no recurrence within a 6-month follow-up period. We report this unusual case as the first in Korean plastic surgery with a review of the literature.

Acute airway obstruction resulting in Pneumonia after palatoplasty: A Case Report (구개성형술후 폐렴을 동반한 급성 기도 폐색: 증례보고)

  • Cleft palate patients with congenital anomalies have an increased risk of airway problems following palatoplasty. Factors that were related included presence of associated congenital anomalies, duration of surgery, age at time of surgery, history of previous airway problem, and excessive pressure exerted on the base of the tongue by Dingman retractor. This report described a complication of post-operative Pneumonia after palatoplasty (Furlow technique), which resulted in a life-threatening acute airway obstruction in an infant with cleft palate. Patient has a history of previous mild airway problems. In addition to this problem, we speculate that Furlow technique involves more extensive surgical dissection than other techniques may increase risk for upper airway obstruction. Awareness of this risk permits identifying those patients prior to surgery so that they can be monitored and managed properly, minimizing the likelihood of major complications or possibility of death.

Superiorly Based Flap Tracheostomy (Superiorly based flap을 이용한 기관절개술)

  • The superiorly based flap tracheostomy(SBFT) has been advocated as an new technique of tracheostomy to manage a wide variety of causes of upper airway obstruction. This technique has particular applicability in patients who require long term tracheostomy such as in bilateral vocal cord paralysis and severe obstructive sleep apnea. SBFT has numerous advantages such as shortening of the gap between the skin and trachea : construction of a self-sustaining tract ; circumferential mucocutaneous junction to reduce infection, granulation tissue, bleeding, and stenosis of the tract : avoidance of the laryngotracheal damage : easy placement of a tracheostomal stent to promote speech, coughing and swallowing. Most of all, this technique can reduces the suprastomal buckling by the support of the superiorly based tracheal flap, and thus prevents the stenosis of suprastomal airway. The disadvantage of SBFT is more time-consuming procedure than the conventional tracheostomy, A retrospective analysis of 8 patients undergoing SBFT between June, 1994 and March, 1995 in Dankook University Hospital was performed to present the surgical technique and com-plication rates. The average duration of follow up was 11 months. The complications were consisted of a wound infection and a sternal granulation. The other complications including wound dehiscence, tracheitis, pneumonia, tracheal granulation, sternal narrowing and subglottic stenosis were not experienced.

Clinical Study of Tracheocutaneous Fistula (기관피부누공에 대한 임상적 고찰)

  • The tracheocutaneous fistula(TCF) may develop infrequently as a complication after tracheostomy. Prolonged tracheostomy tube dependence increases the risk of TCF developing, and in growth of stratified squamous epithelium lines the furrow connecting the tracheal mucosa and the skin, accounting for persistence of the fistulous tract. Such fistulas are a nuisance and create nursing and social problems including poor hygiene, aspiration, difficulty with speech, and depletion of pulmonary reserve. Surgical closure has generally been successful by primary closure, fistulectomy with primary closure, and closure by secondary intention following excision of the tracheocutaneous fistula. No large series compares the efficacy of these techniques and each has its own merits. Recent literature has purposed to minimizing complications. For ten years, from January 1985 to December 1994, the authors experienced 25 cases of TCF which were analyzed in respect to incidence and interval of cannulation, duration between decanulation and fistular closure, precedent disease, closure methods, and complications of TCF repair.

Changes of Xenograft According to Extracted Time with Chloroform-methanol Solution in freeze-dried Cortical bone of Pig Transplanted to Dogs (개에서 동결건조한 돼지 치밀골의 Chloroform-methanol Solution 축출시간에 따른 이식골의 변화)

  • It has been known that periods of absorption varies allografts or xenografts of transplantations of freeze-dried cortical bone(FDCB). In this study changes of absorption of FDCB in xenograft transplantations were evaluated according to extracted time with chloroform-methanol solution(CM sol.). The FDCB from pig was removed soft tissue by surgical knife. Fat of the FDCB was removed with treatments of CM sol. for 2, 6, and 10 days, then the treated FDCB was freeze-dried at $-80^{\circ}C$ and sterilized with ethylene oxide gas. The FDCB was transplanted to fifteen millimeter artificial-defected regions of 6 dogs on fibular diaphyses. This was biweekly examined by radiograph for 18 weeks. In result new bone formation with FDCB treated for 6 days was higher than the other bones treated for 2 and 10 days. Duration of absorption with FDCB treated for 6 days was longer than the others. The remain with FDCB treated for 10 days was more than the others.

The Analysis of Nursing Research on Music Therapy in Korea (음악요법에 관한 국내 간호학 연구 분석)

  • Purpose: The purpose of this study was to review and analyze the trend and issues of nursing research on music therapy. This study was aimed to help direct developing better music therapy programs in the future. Method: We reviewed 57 dissertations and published articles from 1983 to Feb. 2005 that were related to music therapy in nursing. Result: Among the 57 published researches on music therapy, 47 (82.5%) were dissertations and 10 (17.5%) were published articles. Among the 57 researches, 55 (96.5%) were experiential researches and 2 (3.5%) were non-experimental researches. Surgical patients were the most popular subject for researches. The number of subject was a minimum of 7 people and maximum of 143 people. Listening to music 41 (74.5%) was the most popular program for music therapy. Among 55 researches, 33 (60.0%) had a music therapy session of 25-60 minutes and this was the most popular time duration of therapy. Among all the 108 dependent variables, 57 (52.8%) were psychological variables, 40 (37.0%) were physical variables and 11 (10.2%) were social-cognitive variables. Conclusion: There is an urgent need to develop systematic music therapy programs with different nursing interventions, as based on theories and principles while considering the physical, psychological, and social characteristic of the subjects.

Development and Feasibility of a Video Recording-Based Standardized Handoff Program for Ward Nurses: A pilot study (비디오 녹화 기반 병동 간호 인수인계 프로그램 개발 및 적용가능성 : 예비연구)

  • Purpose : This study develop a Video Recording-Based Standardized Handoff (VRSH) program at shift change for ward nurses. Methods : The study was conducted in five medical, three surgical, and one comprehensive nursing care service wards affiliated with a secondary general hospital. In this methodological study, the VRSH program was developed between April and December, 2017. It is noted that 65 nurses who were involved in the VRSH program participated in this study. Results : In line with the modified Situation, Background, Assessment, Recommendation tool, the VRSH program consisted of three phases. In the VRSH program, the average time for handoff duration was 3-5 minutes per patient. More than 90% of the ward nurses were satisfied with the VRSH program since it benefited them by reducing overtime work and improving the performance, as well as effective communication, of nurses. The content analysis of nurses' VRSH program experience, revealed three categories and eight sub-categories. Conclusions : This study provides evidence that the VRSH program improves effective nursing performance and, the handoff communication and relationships between nurses. Future studies on large sample sizes and multiple settings are required to substantially evaluate the impact of the VRSH program on clinical outcomes.

Schwannoma of the tongue: a case report with review of literature

  • Background: Schwannomas (or neurilemmomas) of the tongue are benign, usually solitary, encapsulated masses derived from Schwann cells. Clinical evidence indicates that schwannoma is painless and slow growing. In general, schwannoma is treated by surgical excision. Here, we describe a case of schwannoma of the tongue, include a review of the literature from 1955 to 2016, and provide data on age, gender, location, presenting symptoms, size, and treatment methods. Case presentation: A 71-year-old female patient presented with a swelling at the base of the tongue of unknown duration. Magnetic resonance images (MRI) showed a large well-circumscribed solid mass and no significant lymph node enlargement. The mass was excised without removing overlying mucosa. Conclusions: The authors report a case of lingual schwannoma that was completely removed intraorally without preoperative biopsy. No sign or symptoms of recurrence were observed at 12 months postoperatively.
