갑상선 수술 후 발생한 성대마비는 삶의 질을 중요시하는 요즘 시대에 제일 치명적 합병증 중 하나이다. 그 결과 수술 중 신경감시시스템의 사용은 보편화되고 있으나 아직 기존의 시스템은 외과 의사가 사용하기에 불편하거나 문제점들이 있다. 그래서 새로운 방식의 신경 탐침과 신경 감시 장치의 개발이 필요한 시점이다. 이에 최근 모든 수술 기구(금속형 기구, 내시경 및 로봇 기구, 에너지 기반 디바이스)에 탈부착이 가능한 신경을 자극하는 신경 탐침 및 후두 떨림을 측정하기 위한 표면압력센서를 이용한 새로운 형태의 수술 중 신경감시시스템의 개발에 대한 연구가 기대된다.
Objectives: This study compared the amount of apically extruded bacteria during the glide-path preparation by using multi-file and single-file glide-path establishing nickel-titanium (NiTi) rotary systems. Materials and Methods: Sixty mandibular first molar teeth were used to prepare the test apparatus. They were decoronated, blocked into glass vials, sterilized in ethylene oxide gas, infected with a pure culture of Enterococcus faecalis, randomly assigned to 5 experimental groups, and then prepared using manual stainless-steel files (group KF) and glide-path establishing NiTi rotary files (group PF with PathFiles, group GF with G-Files, group PG with ProGlider, and group OG with One G). At the end of canal preparation, 0.01 mL NaCl solution was taken from the experimental vials. The suspension was plated on brain heart infusion agar and colonies of bacteria were counted, and the results were given as number of colony-forming units (CFU). Results: The manual instrumentation technique tested in group KF extruded the highest number of bacteria compared to the other 4 groups (p < 0.05). The 4 groups using rotary glide-path establishing instruments extruded similar amounts of bacteria. Conclusions: All glide-path establishment instrument systems tested caused a measurable apical extrusion of bacteria. The manual glide-path preparation showed the highest number of bacteria extruded compared to the other NiTi glide-path establishing instruments.

Background: Awareness and attitudes of nurses regarding end of life care are important factors in providing hospice care. In an extensive literature review, we found no related articles investigating Iranian nurses awareness and attitudes about providing such care. Objectives: The aims of this study were to investigate the awareness and attitudes of Iranian nurses in providing hospice care. Materials and Methods: In this descriptive-correlational study, 240 nurses employed in six educational centers were selected by non-randomized stratified sampling. The data collection instruments included an awareness test and attitudes regarding providing end of life care in hospice questionnaire. The data were analyzed using descriptive statistics and independent sample t-tests, one-way ANOVA, and Pearson correlation tests. Results: The nurses' awareness score was 14.3 out of 29 and 55.7% of them stated that they had not received any education in providing end of life care. Also, by obtaining the score of 91.7 out of 120 the attitudes of participants in providing end of life care in hospices were positive. In addition, the highest attitudes score of nurses were in the dimensions of benefits of implementation and health care team. Conclusions: Considering low awareness of nurses about end of life care in hospices, continuing education should be provided for them in this regard. Especially, by considering the positive attitude of nurses, providing such programs could help develop hospice care in Iran.
In this study, the effect of the shape of the specimen and the feedrate of the dental ultrasonic surgical instrument on the cutting force and surface roughness of the specimen is analyzed. Experimental specimens were made of SAWBONES artificial bone materials in square and spherical specimens. In addition, the cutting feedrate of the surgical instrument was controlled through the developed moving system. The cutting force generated when cutting the specimen was measured through a force sensor. After the experiment, the cutting surface of the specimen was observed through a three-dimensional optical microscope and the surface roughness was measured. Through one-way ANOVA, the effect of each specimen shape and feed rate on surface roughness was analyzed. As a result of the experiment, the cutting force increased proportionally in the initial feed rate increase stage, but the increase in cutting force decreased as the feed rate continued to increase. Also, the cutting force showed a difference according to the shape of the specimen. The spherical specimen with a relatively small cutting surface area had less cutting force than the square specimen. However, as a result of one-way ANOVA, it was found that the specimen shape and feed rate did not affect the surface roughness. In future studies, it is expected to be used for comparative analysis of ultrasonic surgical instruments and correlation analysis between cutting factors.
This research was performed to provide basic data for the development of infection related dental hygiene studies by surveying the current condition of recognition among students in the department of dental hygiene toward hospital infection management while receiving the following results by using a personal self-administered survey method targeting 303 students in the department of dental hygiene from certain areas. 1. With the current condition of recognition on the sanitization and sterilization of instruments among students in the department of dental hygiene(study), the rate at which surgical instruments are to be sterilized with autoclaves was 79.9% which was relatively higher than other instruments while it was shown that prosthetic instruments for treatment was 56.4%, conservative instruments for treatment was 51.8%, and ultrasonic scaler tip was 51.1% while the way syringe tips(36.1%) and the dental anesthetic apparatus(27.9%) were revealed to require sanitization by alcohol. 2. The 'hand wash' area was the highest with 4.71 while the 'materials and environment management' area and 'equipment management' area appeared high respectively with 4.43 and 4.41. 3. With the current condition of recognition on equipment management, 'equipments used for contagious patients are separately washed after a one-time use and must be sterilized or separated-and-discarded' was the highest with 4.82 while 'sterilization equipments with humidity or water on it are considered contaminated and are not used' showed the lowest recognition level with 3.90. 4. Regarding the current condition of materials and environment management, 'contagious and general trash are separated and discarded' was the highest with 4.70 while 'the refrigerator for medicine storage is cleaned on a regular basis once a month' was revealed as the lowest with 4.11. 5. With the current condition of recognition on hand washing, 'one must wash their hands after coming in contact with contagious patients, was the highest with 4.90 while washing hands after taking off gloves' appeared as the lowest with 4.51 point. Computer aided process planning (CAPP) keeps an important role between the design and manufacturing engineering processes. A CAPP system is a digital link between a computer aided design (CAD) model and manufacturing instructions. CAPP have been researched and applied in manufacturing filed, however, one manufacturing area where CAPP has not been extensively researched is rapid prototyping (RP). RP is a technique for creating directly a three dimensional CAD data into a physical prototype. RP enables to build physical models automatically and to use to reduce the time for the product development cycle as well as to improve the final quality of the designed product. Three-dimensional (3D) printing is one kind of RP that creates three-dimensional objects from CAD models. The paper presents a computer aided process planning system for printing medical products. 3D printing has been used to solve complex medical problems such as surgical instruments, bioengineered products, medical implants, and surgical guides.
Fournier's gangrene is a synergistic necrotizing fasciitis of the perineal, perirectal and urogenital area and can be fatal unless treated in early stage. Perianal and urogenital infections are common causes of the disease but it can occur after artificial procedure on perineal area using by surgical instruments. It is mixed aerobic and anaerobic infection and E. coli is the most common causative bacteria. Untill now many investigators have focused on early diagnosis, preserving hemodynamic stability, broad-spectrum systemic antibiotics and treatment of underlying disease in management of Fournier's gangrene. The authors have experienced five patients of chronic liver disease whose necrotizing perineal infections developed spontaneously and treated them aggressively as described above and reconstructed perineal soft tissue defects using by various surgical methods, then we got good results both functionary and cosmetically. From now on, we would better reconstruct soft tissue defect of perineum with skin graft or pedicled flap in early stage when treat Fournier's gangrene, thereafter we can get an ultimate increase in patient's life quality.
The developmental and subsequent addition of laser technology to existing surgical techniques offered new and exciting Possibilities for improving traditional endoscopic operations and expanding the scope of the specialty of otolaryngology-head and neck surgery. Lasers were found to be Precise, but potentially dangerous, surgical instruments whose use was associated with certain distinct and unprecedented advantages, but also with many unique and potentially serious, and sometimes catastrophic complications. The Purpose of this study was to evaluate the of elation technique of $CO_2$laser, kinds of anesthesia, equipments of operating room, and education of medical staff. Four hundreds 4 cases of $CO_2$laser laryngeal microsurgery were reviewed for the safety of $CO_2$laser in laryngeal microsurgery. Vocal polyp (46,3%) was the most common pathology in our cases. The others were vocal nodule, Reinke`s edema, epiglottic cyst, laryngeal granuloma, laryngeal papilloma, intracordal cysts, laryngeal tuberculosis, laryngeal web, laryngotracheal stenosis, and laryngeal cancer. Following complete equipment controls, treatment area controls, maintenance, service and procedural controls, personel protective equipment, and warning signs, no complications were found in our cases. In conclusion, laryngeal microsurgery with $CO_2$laser could be done safely without complication if complete preoperative preparation and education of medical staff preceded.
Chronic periodontitis is one of the most common disease in clinical dentistry, which needs various surgical interventions to treat the moderate to severe destruction of supporting periodontium. Most patients have fear and anxiety to these surgical procedures and some dentists also have problems to deal with these patients. Applying the conscious sedation technique in outpatient units, periodontists have become to manage their patients successfully without any fear and anxiety and increased the level of patients' satisfaction. Particularly, periodontal treatments require relative long chair-time and patients are usually reluctant to the periodontal instruments used in the oral cavity. This study is focused on the sedation procedure with intravenous midazolam infusioon performed in Department of Periodontology, Ewha Womans University Hospital. The whole procedure of intravenous midazolam sedation is briefly reviewed from patient selection to patient discharge.
Purpose: This study was done to identify nursing interventions clinically applicable to relieve displeasure and anxiety of patients having an operation in which loud noise was unavoidable. Methods: Participants were 34 patients scheduled to have a total knee replacement. They were assigned to either the control (17 patients) or experimental (17 patients) group. Data were collected from September to November 2010. The instruments used for the survey included noise-control earplugs, a noise level meter (TES-1358) and a structured scale measuring degree of noise perception, displeasure and anxiety. Data were analyzed using frequency, percentage, standard deviation, $x^2$-test, and t-test with the SPSS WIN 18.0 program. Results: Hypothesis 1: 'The displeasure levels of the experimental group of patients wearing earplugs will be lower than the control group patients without earplugs' was accepted (t=-20.769, p<.001). Hypothesis 2: 'The anxiety levels of the experimental group of patients wearing earplugs will be lower than the control group patients without earplugs' was accepted (t=-6.298, p<.001). Conclusion: Based on these findings, the wearing of earplugs for patients having a surgical operation with loud noises is recommended as an effective nursing intervention to relieve patient displeasure and anxiety during their surgical operations.
