소아에서 항문 내초음파 검사상 정상 항문관의 형태학적 소견 (Anal Endosonographic Features of the Normal Anus in Children)

  • Anal endosonography has recently been popularized in adult patients in order to access staging of rectal cancer and other lesions of the anorectum. This study is not familiar to pediatric surgeons. We performed anal endosonography in 30 children without anorectal disease in order to determine the standard morphology of the anorectum. The internal anal sphincter(IAS) was clearly identified as a homogeneous hypoechoic circular band, extending caudally to a level just proximal to the anal verge. The external anal sphincter(EAS) showed mixed echogenicity and different architecture along the anal canal; the EAS was U-shape in the upper canal and it had a circular pattern in the lower canal. In the upper canal of girls, the perineal body and the vagina were found just anterior of the anal canal, which made the U shape of the EAS. In the male, the sphincter tapered anteriorly into two arcs that met in the midline. The perineal body was prominent in the female. Posteriorly, the anococcygeal ligament was represented by a triangular shadow in both sex. The thickness of the IAS was measured in 3 directions, left, right and posterior, at 3 levels, upper, middle and lower areas of the anal canal. The average thickness was 0.86-2.40 mm between 6 and 18 months of age(Group 1), 0.88-3.20 mm between 19 and 36(Group 2), 1.07-2.20 mm between 37 and 54(Group 3) and 1.18-2.42 mm more than 54-month-old(Group 4). The thickness was correlated with the age of the children only in the right upper(p=0.008) and the left middle portion(p=0.015). We were able to obtain standard morphologic features of the normal anal canal in children with anal endosonography. We believe that this technique is a safe and an effective procedure to evaluate anorectal lesions in children.

개에서 항문낭 절제를 위한 폴리카테터의 이용 (Use of a Foley Catheter for Anal Sacculectomy in Dogs)

  • 개의 오른쪽 또는 왼쪽 항문낭에 화농성 삼출물이 있거나 항문낭이 개방된 8마리의 개를 대상으로 항문낭 적출술을 하였다. 환자의 체중은 3-8 kg이며. 항문낭을 쉽게 절제하기 위하여 폴리카테터(1.5 ml, 6호)를 이용하여 폐쇄법으로 항문 적출술을 하였다. 항문낭 적출술을 하는 동안 모든 환자에서 폴리카테터를 팽창하여 항문낭을 쉽게 식별할 수 있었다. 항문낭 적출술을 실시 한 환자들을 대상으로 1-3개월 동안 임상증상을 추적하였다. 항문낭 적출술이 만성 항문낭염 또는 항문 질환에 좋은 치료법이었다. 폴리카테터의 이용은 항문낭의 적출 시 항문낭을 쉽게 구별할 수 있었고 경제적이었다.

Comparative Study on the Value of Anal Preserving Surgery for Aged People with Low Rectal Carcinoma in Jiangsu, China

  • Objective: To compare the efficacy of anal preserving surgery for aged people with low rectal carcinoma. Methods: Clinical data for a consecutive cohort of 98 rectal cancer patients with distal tumors located within 3cm -7cm of the anal verge were collected. Among these, 42 received anal preserving surgery (35 with Dixon, 3 with Parks and 4 with transanal operations). The local recurrence and survival rates in the above operations were compared with those of the Miles operation in another 56 patients with rectal cancer. Results: The local recurrence and 3-, 5-year survival rates of anal preserving surgery were 16.7%, 64.3% and 52.4%, those of Miles operations were 16.1%, 67.9% and 51.8% respectively (P>0.05). Conclusion: Anal preserving surgery for aged people with low rectal cancer is not inferior to conventional operations in China, with satisfactory long term survival and comparable local recurrence rates.

Assessment of normal anal sphincter anatomy using transanal ultrasonography in healthy Korean volunteers: a retrospective observational study

  • Background: To date, there have been no studies on the normal anatomic values of the anal sphincter in healthy Koreans. Therefore, this study aimed to determine the normal anatomic values of transanal ultrasonography (TAUS). Methods: The thickness of the external anal sphincter (EAS) and internal anal sphincter (IAS) was measured by TAUS from healthy Korean volunteers between September 2019 and August 2021. Results: Thirty-six volunteers with a median age of 37 years (range, 20-77 years) and a median body mass index (BMI) of 23.5 kg/m2 (range, 17.2-31.2 kg/m2) were examined. The median thickness of the EAS at 4 cm and 2 cm from the anal verge was 7.4 mm (range, 5.8-8.8 mm) and 6.5 mm (range, 5.6-8.0 mm), respectively. The median thickness of the IAS at 2 cm from the anal verge was 1.8 mm (range, 0.8-4.3 mm). There were no differences in sphincter muscle thickness between the sexes. However, the EAS tended to thicken as the BMI increased (EAS at 2 cm and 4 cm from the anal verge, Spearman rho=0.433, 0.363; p=0.008 and p=0.029, respectively). Conclusion: In healthy Korean, the median thickness of the IAS at 2 cm from the anal verge was 1.8 mm and the median thickness of the EAS at 2 cm and 4 cm from the anal verge was 6.5 mm and 7.4 mm respectively. There were no differences in anal sphincter thickness between sexes, but BMI was related to EAS thickness.

항문암의 동시 화학 방사선 요법 치료결과 (Concurrent Chemoradiotherapy Results in Patients with Anal Cancer)

  • Among the 63 patients with histopathologically proven primary squamous cell anal cancer who were managed in Presbyterian Medical Center and Yonsei University Cancer from Jan. 1971 to Dec. 1991, 34 patients, who were managed with surgery alone(abdominoperineal resection) or post-operative radiotherapy and concurrent chemoradiotherapy were analysed. With mean follow up time of 81.3 months, 30 Patients(88$ \% $) were followed up from 17 to 243 months. In methods, 10 patients were treated with surgery alone. 9 Patients were treated with combined surgery and postoperative radiotherapy(50$\∼$60 Gy in 28$\∼$30 fractions). 15 patients were treated with concurrent chemoradiotherapy. Chemotherapy (Mitomycin C 15 mg/squ, bolus injection day 1;5-FU, 750 mg/squ, 24hr infusion, day 1 to 5) and radiotherapy started the same day. A dose of 30 Gy was given to the tumor and to the pelvis including inguinal nodes, in 15 fractions. After 2 weeks a boost of radiotherapy(20 Gy) to the ano-perineal area and second cycle of chemotherapy completed the treatment. The overall 5-year survival rate was 56.2$ \% $. Concurrent chemoradiotherapy group was 70$ \% $ and surgery alone group was 16.7$ \% $. According to the cox proportional harzard model, there was significant difference between survival with concurrent chemoradiotherapy and surgery alone(p=0.0129), but post-operative radiotherapy was 64.8$ \% $, which was not stastically significant(p=0.1412). In concurrent chemoradiotherapy group, the anal funtion Preservation rate was 87$ \% $ and the severe complication rate(grade 3 stenosis and incontinence) was 13.3$ \% $. In conclusion, we conclude that the concurrent chemoradiotherapy may be effective treatment modality in patients with anal cancer.

Extended Low Anterior Resection with a Circular Stapler in Patients with Rectal Cancer: a Single Center Experience

  • Background: to evaluate the outcome of stapled colo-anal anastomoses after extended low anterior resection for distal rectal carcinoma. Materials and Methods: A retrospective study of fifty patients who underwent coloanal anastomoses after extended low anterior resection was conducted at Imam Hospital from September 2007 up to July 2012. Results: The distance of the tumor from anal verge was 3 to 8 cm. Anastomotic leakage developed in 6% of patients and defecation problems in 16%. One-year local recurrence was 6% while three-year local recurrence was 4%. One-year systemic recurrence was seen in 22% while three-year systemic recurrence was seen in 20%. Conclusions: Colo-anal anastomoses after extended low anterior resection for distal rectal carcinoma can be conducted safely.

Anorectal malformation with didelphys uterus: Extremely rare anomaly and successful neoanal sphincter reconstruction with gracilis muscle flap

  • Anorectal malformation or imperforate anus is a congenital anomaly of rectum and anus. Mullerian duct anomalies are abnormal development of uterus, cervix, and vagina. Imperforate anus with double uterus is extremely rare and cannot explain by normal embryologic development. Moreover, guideline in treatment is inconclusive. We report an extremely rare case of a young adult female who presented with recurrent pelvic inflammatory disease caused by rectovaginal fistula in congenital imperforate anus and didelphys uterus, and successfully neoanal reconstruction with gracilis muscle flap. Aims for treatment are closed rectovaginal fistula, and anal sphincter reconstruction. To our best knowledge, the imperforate anus with double uterus is extremely rare anomaly. Furthermore, successfully anal sphincter reconstruction with functional gracilis muscle in the imperforate anus with double uterus has never been reported in English literature.

"Skip Area"가 있는 선천성 거대결장 (Total Colonic Aganglionosis With Skip Area)

  • Early recognition and surgical treatment of Hirschsprung's disease prevents serious mortality and morbidity from enterocolitis and obstruction. Usually this disease is characterized by a single aganglionic segment of the colon extending distally to the anal margin. In surgical treatment, the surgeon performs a frozen section biopsy to confirm whether there are ganglion cells. If there are intervening ganglionic sites in aganglionic bowel, there may be confusion in diagnosis and treatment. The authors experienced one case of total colonic aganglionosis with skip area. A transverse loop colostomy was performed on a 7 day-old male baby with colon perforation due to Hirschsprung's disease. But intestinal obstruction persisted and required two more operations to find the true nature of the disease. There were aganglionic segments from the anal margin to the terminal ileum 3.7cm proximal to the ileocecal valve. The entire transverse colon and appendix were normally ganglionated.

Goligher 분류상 1기-3기에 해당하는 항문질환 환자의 한의치료 증례군보고 (Korean Medicine Treatment for Patients with Grade I-III Hemorrhoids (Goligher Classification): A Case Series)

  • Background: Hemorrhoid surgery is a frequently performed surgery in Korea, second only to cataract surgery. However, it has serious complications, including bleeding, infection, anal stenosis, and urinary retention, so active conservative therapies are in need at an early stage. Case summary: Thirteen patients with anal disease complained of main symptoms of pain, bleeding, and prolapse, with additional complaints of itching and mucus discharge. An herbal medicine mixture of Eulja-tang and Hwangryeonhaedok-tang was administered twice daily, and dry cupping therapy, acupuncture, and hot pack placements were administered on pelvic and sacral regions once weekly for four weeks. The symptoms were evaluated before and each week after the treatments. The treatments had statistically significant therapeutic effects. Conclusion: Korean medicine treatment-effectively alleviating the symptoms of anal disease, such as pain, bleeding, and prolapse-may represent an alternative therapy for conservative treatments in the early stages of anal disease.

Surgical Repair of Atresia Ani with Rectovaginal Fistula in an African Buffalo (Syncerus caffer)

  • A three-month-old female African buffalo born at Seoul Zoo showed signs of abdominal distension, bulging of the perineal skin, and small stool volume compared to feed intake. Upon physical examination, atresia ani with rectovaginal fistula was diagnosed. This case was subjected to surgery under inhalation anesthesia after injecting a sedative. Surgery was performed in two steps: anal reconstruction and closing the rectovaginal fistula. First, a circular skin incision was made at the end of the rectal pouch to create an anus, and then the skin of the anus and the mucous membrane of the rectum were brought into apposition by simple interrupted sutures. Second, the rectovaginal fistula was ligated on both vulval and anal side. Antibiotics were administered on every alternate day and the sutures were removed at ten days surgery under sedation. The rectovaginal communication was closed and the calf was able to urinate and defecate normally. The animal grew to become a normal adult without any complications. This is the first case report of atresia ani with rectovaginal fistula in an African buffalo, that was successfully treated by surgical intervention.
