완전무치악 환자에서 디지털 가이드 수술 방식을 이용한 무피판절개 임플란트 식립증례: 증례보고 및 5년 추적관찰 (Flapless implant placement with digital 3D imaging and planning system in fully edentulous patient: A case report and 5-year follow-up)

  • 디지털 방식을 이용한 치과 임플란트 진료에서 가장 주목받는 분야 중 하나가 디지털 소프트웨어와 컴퓨터 단층촬영을 이용하여 수술용 스텐트를 제작, 임플란트를 식립하는 분야이다. 컴퓨터를 이용한 디지털 임플란트 치료계획 시스템과 이를 이용한 수술용 가이드는 예지성과 심미성, 기능성을 갖춘 임플란트의 식립과 수복을 가능하게 한다. 특히 이러한 가이드 수술은 임플란트 수복물의 형태와 기능을 고려한 임플란트의 식립 시 더욱 유리하다. 최근 여러 제조사에서 보철물 제작을 위해 치과용 스캔부터 제작방법까지 다양한 디지털 방식의 제작과정을 제시하고 있다. 이러한 방법들은 정확하고 예지성 있는 예후를 위해 여러 연구자들에 의해 연구되어지고 있으며, 점차 발전하고 있다. 이번 증례는 완전무치악의 여성 환자에서 임플란트 고정성 보철물을 이용한 수복증례이다. 하악은 all-on-four 방식을 적용하여 4개의 임플란트를 식립하였으며, 상악은 이에 기초하여 6개의 임플란트를 식립하였다. 최종보철물에 기초한 임플란트의 식립을 위해 임플란트 수술용 스텐트를 이용하였으며, 환자는 치료 결과에 만족하였으며, 5년 추적관찰 결과, 임플란트와 보철물 모두 안정적으로 유지되고 있다.

How image-processing parameters can influence the assessment of dental materials using micro-CT

  • Purpose: The aim of this study was to evaluate the influence of voxel size and different post-processing algorithms on the analysis of dental materials using micro-computed tomography (micro-CT). Materials and Methods: Root-end cavities were prepared in extracted maxillary premolars, filled with mineral trioxide aggregate (MTA), Biodentine, and Intermediate Restorative Material (IRM), and scanned using micro-CT. The volume and porosity of materials were evaluated and compared using voxel sizes of 5, 10, and 20 ㎛, as well as different software tools(post-processing algorithms). The CTAn or MeVisLab/Materialise 3-matic software package was used to perform volume and morphological analyses, and the CTAn or MeVisLab/Amira software was used to evaluate porosity. Data were analyzed using 1-way ANOVA and the Tukey test(P<0.05). Results: Using MeVisLab/Materialise 3-matic, a consistent tendency was observed for volume to increase at larger voxel sizes. CTAn showed higher volumes for MTA and IRM at 20 ㎛. Using CTAn, porosity values decreased as voxel size increased, with statistically significant differences for all materials. MeVisLab/Amira showed a difference for MTA and IRM at 5 ㎛, and for Biodentine at 20 ㎛. Significant differences in volume and porosity were observed in all software packages for Biodentine across all voxel sizes. Conclusion: Some differences in volume and porosity were found according to voxel size, image-processing software, and the radiopacity of the material. Consistent protocols are needed for research evaluating dental materials.

갑상샘 유두암종의 세포진단에서 형태학적 계측의 분석 (Morphometric Analysis for Cytological Diagnosis of Thyroid Papillary Carcinoma)

  • The diagnosis of papillary thyroid cancer is generally based on the findings of intranuclear cytoplasmic inclusions and nuclear grooves. Although anisokaryosis and poikilokaryosis, in papillary thyroid cancer, are not distinct when compared to other cancers, cytological examination can provide useful preoperative information. Our study evaluated the diagnostic role of computer-assisted image analysis for the pre-surgical assessment of papillary thyroid carcinoma. Thyroid aspirates from twenty female patients who were histologically confirmed to have both papillary carcinoma and benign nodules were studied. Different populations of 50 benign cells and 50 malignant cells were analyzed. Five morphometric parameters were selected for analysis: nuclear area, perimeter, maximum length, maximum width and intensity standard variation. The values obtained for papillary carcinomas were higher than the surrounding benign nodules as follows: nuclear area 63.5 vs. 36.1 (p=0.000), nuclear perimeter were 29.4 vs. 22.0 (p=0.000), maximum length 9.6 vs. 7.1 (p=0.000), maximum width 8.2 vs. 6.3 (p=0.000), the ratio between maximal length and maximal width 1.16 vs. 1.13 (p=0.000), the standard variation of intensity 14.9 vs. 15.9 (p=0.101) respectively. Therefore, morphometric information can be helpful for the differential cytological diagnosis of papillary thyroid carcinoma.

Assessment of CT numbers in limited and medium field-of-view scans taken using Accuitomo 170 and Veraviewepocs 3De cone-beam computed tomography scanners

  • Purpose: To assess the influence of anatomic location on the relationship between computed tomography (CT) number and X-ray attenuation in limited and medium field-of-view (FOV) scans. Materials and Methods: Tubes containing solutions with different concentrations of $K_2HPO_4$ were placed in the tooth sockets of a human head phantom. Cone-beam computed tomography (CBCT) scans were acquired, and CT numbers of the $K_2HPO_4$ solutions were measured. The relationship between CT number and $K_2HPO_4$ concentration was examined by linear regression analyses. Then, the variation in CT number according to anatomic location was examined. Results: The relationship between $K_2HPO_4$ concentration and CT number was strongly linear. The slopes of the linear regressions for the limited FOVs were almost 2-fold lower than those for the medium FOVs. The absolute CT number differed between imaging protocols and anatomic locations. Conclusion: There is a strong linear relationship between X-ray attenuation and CT number. The specific imaging protocol and anatomic location of the object strongly influence this relationship.

Cell Versus Chemokine Therapy Effects on Cell Mobilization to Chronically Dysfunctional Urinary Sphincters of Nonhuman Primates

  • Purpose: A major question remaining in approaches to tissue engineering and organ replacement is the role of native mobilized native cells in the regeneration process of damaged tissues and organs. The goal of this study was to compare the cell mobilizing effects of the chemokine CXCL12 and cell therapy on the urinary sphincter of nonhuman primates (NHP) with chronic intrinsic urinary sphincter dysfunction. Methods: Either autologous lenti-M-cherry labeled skeletal muscle precursor cells (skMPCs) or CXCL12 were injected directly into the sphincter complex of female NHPs with or without surgery-induced chronic urinary sphincter dysfunction (n=4/treatment condition). All monkeys had partial bone marrow transplantation with autologous lenti-green fluorescent protein (GFP) bone marrow cells prior to treatment. Labeled cells were identified, characterized and quantified using computer-assisted immunohistochemistry 6 months posttreatment. Results: GFP-labeled bone marrow cells (BMCs) were identified in the bone marrow and both BMCs and skMPCs were found in the urinary sphincter at 6-month postinjection. BMCs and skMPCs were present in the striated muscle, smooth muscle, and lamina propria/urothelium of the sphincter tissue. Sphincter injury increased the sphincter content of BMCs when analyzed 6-month postinjection. CXCL12 treatment, but not skMPCs, increased the number of BMCs in all layers of the sphincter complex (P<0.05). CXCL12 only modestly (P=0.15) increased the number of skMPCs in the sphincter complex. Conclusions: This dual labeling methodology now provides us with the tools to measure the relative number of locally injected cells versus bone marrow transplanted cells. The results of this study suggest that CXCL12 promotes mobilization of cells to the sphincter, which may contribute more to sphincter regeneration than injected cells.

Effect of posterior span length on the trueness and precision of 3 intraoral digital scanners: A comparative 3-dimensional in vitro study

  • Purpose: This in vitro study measured and compared 3 intraoral scanners' accuracy (trueness and precision) with different span lengths. Materials and Methods: Three master casts were prepared to simulate 3 different span lengths (fixed partial dentures with 3, 4, and 5 units). Each master cast was scanned once with an E3 lab scanner and 10 times with each of the 3 intraoral scanners (Trios 3, Planmeca Emerald, and Primescan AC). Data were stored as Standard Tessellation Language (STL) files. The differences between measurements were compared 3-dimensionally using metrology software. Data were analyzed using 1-way analysis of variance with post hoc analysis by the Tukey honest significant difference test for trueness and precision. Statistical significance was set at P<0.05. Results: A statistically significant difference was found between the 3 intraoral scanners in trueness and precision (P<0.05). Primescan AC showed the lowest trueness and precision values(36.8 ㎛ and 42.0 ㎛;(39.4 ㎛ and 51.2 ㎛; and 54.9 ㎛ and 52.7 ㎛) followed by Trios 3 (38.9 ㎛ and 53.5 ㎛; 49.9 ㎛ and 59.1 ㎛; and 58.1 ㎛ and 64.5 ㎛) and Planmeca Emerald (60.4 ㎛ and 63.6 ㎛; 61.3 ㎛ and 69.0 ㎛; and 70.8 ㎛ and 74.3 ㎛) for the 3-unit, 4-unit, and 5-unit fixed partial dentures, respectively. Conclusion: Primescan AC had the best trueness and precision, followed by Trios 3 and Planmeca Emerald. Increasing span length reduced the trueness and precession of the 3 scanners; however, their values were within the accepted successful ranges.

Comparison of instrumental methods for color change assessment of Giomer resins

  • Objectives: The aim of this study was to compare the color change of the Giomer resin composite (Beautifil-Bulk) by using photographs obtained with a smartphone (iPhone 6S) associated with Adobe Photoshop software (digital method), with the spectrophotometric method (Vita Easyshade) after immersion in different pigment solutions. Materials and Methods: Twenty resin composite samples with a diameter of 15.0 mm and thickness of 1.0 mm were confectioned in A2 color (n = 5). Photographs and initial color readings were performed with a smartphone and spectrophotometer, respectively. Then, samples were randomly divided and subjected to cycles of immersion in distilled water (control), açai, Coke, and tomato sauce, 3 times a day, 20 minutes for 7 days. Later, new photographs and color readings were taken. Results: The analysis (2-way analysis of variance, Holm-Sidak, p < 0.05) demonstrated no statistical difference (p < 0.005) between the methods in all groups. Similar color changes were observed for all pigment solutions when using the spectrophotometric method. For the digital method, all color changes were clinically unacceptable, with distilled water and tomato sauce similar to each other and with statistical differences (p < 0.005) for Coke and açai. Conclusions: Only the tomato sauce produced a color change above the acceptability threshold using both methods of color assessment. The spectrophotometric and digital methods produce different patterns of color change. According to our results, the spectrophotometric method is more recommended in color change assessment.

방사선사진에서의 골질과 임상적으로 평가한 골질 분류의 상관관계 (Correlation of bone quality in radiographic images with clinical bone quality classification)

  • Purpose : To investigate the validity of digital image processing on panoramic radiographs in estimating bone quality before endosseous dental implant installation by correlating bone quality in radiographic images with clinical bone quality classification. Materials and Methods : An experienced surgeon assessed and classified bone quality for implant sites with tactile sensation at the time of implant placement. Including fractal dimension eighteen morphologic features of trabecular pattern were examined In each anatomical sites on panoramic radiographs. Finally bone quality of 67 implant sites were evaluated in 42 patients. Results : Pearson correlation analysis showed that three morphologic parameters had weak linear negative correlation with clinical bone quality classification showing correlation coefficients of -0.276, -0.280, and - 0.289, respectively (p<0.05). And other three morphologic parameters had obvious linear negative correlation with clinical bone quality classification showing correlation coefficients of -0.346, -0.488, and -0.343 respectively (p<0.05). Fractal dimension also had a linear correlation with clinical bone quality classification with correlation coefficients -0.506 significantly (p<0.05). Conclusion : This study suggests that fractal and morphometric analysis using digital panoramic radiographs can be used to evaluate bone quality for implant recipient sites.

Characterization of Mouse Interferon-Induced Transmembrane Protein-1 Expression in Mouse Testis

  • Interferon induced transmembrane protein-1 (Ifitm-1) has been reported to have an important role in primordial germ cell formation, and it has expressed in female reproductive organ. In the present study, Ifitm-1 gene expression was identified in testes and all part of epididymis using western immunoblot and immunohistochemistry. Interestingly, Ifitm-1 expression was observed on the head of spermatozoa. To investigate the role of Ifitm-1 gene expression in behavior of spermatozoa after acrosome reaction, fresh sperm was incubated with calcium ionophore to induce acrosome reaction, whereas the expression of Ifitm-1 was not altered after the acrosome reaction. Then to identify the effect of Ifitm-1 in sperm motility and other seminal parameters, different concentration of Ifitm-1 antibody was incubated with spermatozoa, and seminal parameters were assessed using computer-assisted semen analysis (CASA). Interestingly, motility, progressive, and VAP were increased in the sperm with Ifitm-1 antibody treated compared to rabbit serum, however other parameters such as straightness were not changed. In order to identify the functional significance of Ifitm-1 in fertilization, capacitated spermatozoa were pre-incubated with anti-Ifitm-1 antibody and subsequently examined the ability to adhere to mouse oocytes. However, any defection or alteration in sperm-egg fusion was not found, Ifitm-1 antibody treated or non-treated spermatozoa showed a normal penetration. Although the precise role of Ifitm-1 in sperm motility and following fertilization need to be elucidated, this study suggests that the activation of Ifitm-1 on the sperm may enhance the motility of spermatozoa in mice.

Dosimetric advantages and clinical outcomes of simultaneous integrated boost intensity-modulated radiotherapy for anal squamous cell carcinoma

  • Purpose: The purpose of this study was to explore the dosimetric difference between simultaneous integrated boost intensity-modulated radiotherapy (SIB-IMRT) and three-dimensional conformal radiotherapy (3DCRT), and the clinical outcomes of anal squamous cell carcinoma (ASCC) chemoradiotherapy featuring SIB-IMRT. Materials and Methods: This study included ten patients with ASCC who underwent chemoradiotherapy using SIB-IMRT with 5-fluorouracil and mitomycin C. SIB-IMRT delivered 54 Gy to each primary tumor plus metastatic lymph nodes and 45 Gy to regional lymph nodes, in 30 fractions. Four patients received additional boosts to the primary tumors and metastatic lymph nodes; the median total dose was 54 Gy (range, 54 to 60 Gy). We additionally created 3DCRT plans following the Radiation Therapy Oncology Group 9811 protocol to allow dosimetric comparisons with SIB-IMRT. Locoregional control, overall survival, and toxicity were calculated for the clinical outcome evaluation. Results: Compared to 3DCRT, SIB-IMRT significantly reduced doses to the external genitalia, bladder, and intestine, delivering the doses to target and elective nodal region. At a median follow-up time of 46 months, 3-year locoregional control and overall survival rates were 88.9% and 100%, respectively. Acute toxicities were treated conservatively. All patients completed radiotherapy with brief interruptions (range, 0 to 2 days). No patient experienced ${\geq}grade$ 3 late toxicity during the follow-up period. Conclusion: The dosimetric advantages of SIB-IMRT appeared to reduce the toxicity of chemoradiotherapy for ASCC achieving high locoregional control in the extended period.
