Prevalence and clinical relevance of the anatomical variations of suprarenal arteries: a review

  • The suprarenal arteries are arising from three sources: superior suprarenal artery, middle suprarenal artery, and inferior suprarenal artery. Variations in the arterial supply of the suprarenal glands in respect to origin and number are quite common and very frequently reported. The most common variation noted is in the inferior suprarenal artery followed by the middle suprarenal artery and the least common variations were observed in the superior suprarenal artery. Arteriogram of the inferior suprarenal artery is crucial in suprarenal tumour diagnosis but variation in the branching pattern and multiplicity of these arteries can cause hindrance in arteriography. The absence of middle suprarenal artery was seen to be associated with increased number of the inferior suprarenal artery. Variation in the multiplicity of arteries was observed more frequently in the inferior suprarenal artery and middle suprarenal artery which was more on the right side in most of the studies. Also, the variation in suprarenal arteries was often correlated to variations in inferior phrenic and gonadal arteries. The variations were observed to be more common on the left side therefore right adrenalectomy should be preferred over the left one. The loop formed by the inferior suprarenal artery around the right renal vein can cause venous obstruction. These variations of suprarenal vasculature are explained on the developmental basis, and prior knowledge of such variants is crucial for nephrologists to ensure minimum blood loss while performing laparoscopic adrenalectomy especially for large adrenal tumours and pheochromocytoma where the duration of surgery exceeds the usual.

부신 기원의 선천성 신경모세포종의 치료 경험 (Clinical Features and Surgical Outcome of Congenital Neuroblastoma of Adrenal Gland)

  • With the widespread use of the obstetrical ultrasound, identification of a fetal suprarenal mass becomes more common. Most of these masses prove to be congenital neuroblastomas (CNB) postnatally. However, the diagnosis is often confused with other benign lesions and the post-natal management remains controversial. The medical records of 13 patients that underwent primary surgical excision for an antenatally detected adrenal CNB, between January 1995 and April 2009, were reviewed retrospectively. The clinical, radiological, surgical, and pathological data on the suprarenal mass were collected. Staging evaluation was performed after histological confirmation of the CNB. Most of the CNBs were stage I (N=11), with 1 stage IV and 1 stage IV-S. Four patients (3 stage I and 1 stage IV-S) had N-myc gene amplification. The stage I patients were cured by surgery alone, and stage IV patients underwent 9 cycles of adjuvant chemotherapy and currently have no evidence of disease after 39 months of follow-up. The patient with stage IV-S is currently receiving chemotherapy. There were no post-operative complications. For early diagnosis and treatment, surgical excision should be considered as the primary therapy for an adrenal CNB detected before birth. The surgery can be safely performed during the neonatal period and provides a cure in most cases. Surgical diagnosis and treatment of CNB is recommended in neonatal period.

신(腎)과 갑상선(甲狀腺)의 상관성(相關性)에 관(關)한 고찰(考察) (A Study on the relationship between Shin(腎) with Thyroid)

  • So far we oriental medical doctors have referred to Shin(腎) as endocrine system, especially suprarenal gland, sexual gland and autonomic nervous system, thyroid etc. as well as kidney. But the sight on thyroid is weak and the relationship with Shin(腎) hasn't been suggested clearly. The purpose of this study is to find out the relationship between Shin(腎) with thyroid. The following are the results. 1. Ki Gi(氣機) of Shin(腎) is similar to the fuction of thyroid in that they are the base of human metabolism, influencing all the metabolism of human tissue. 2. Shin Yang(腎陽) and thyroid hormone are almost identical in that they are the base of Yang Gi(陽氣), the former as source of heat energy, the latter as energy hormone. 3. Shin(腎) and thyroid hormone are almost the same in that they activate growth of human body. 4. Ki Gi(氣機) of Shin(腎) such as Ju Gol(主骨), Saeng Su(生髓), Tong uh Noi(通於腦) is similar to the effects of thyroid hormone on bones, central nervous system and hair 5. The symptoms of deficiency of Shin Eum(腎陰虛) are almost identical with those of hyperthyroidism, so the process of Yang Hwa Gi(陽化氣) caused by exuberance of Yang due to deficiency of Shin Eum(腎陰虛陽亢) is similar to excessive metabolism caused by hyperthyroidism. 6. The process of Eum Seong Hyung(陰成形) caused by preponderance of Eum due to deficiency of Shin Yang(腎陽虛陰盛) is similar to the lowering of metabolism caused by hypothyroidism. 7. Deficiency of Shin Eum(腎陰虛) is similar to hyperthyroidism, deficiency of Shin Yang(腎陽虛) to hypothyroidism. But there are major difference in edema and the fuction of intestine. To conclude, Ki Gi(氣機) of Shin is similar to the function of throid in many respects. I hope that there will be further studies on the relationship beteen thyroid malfuction with deficiency of Shin Yang(腎陽虛) or of Shin Eum(腎陰虛) in the future.
