A Case of Tuberculous Meningitis Combined with Acute Cervical Epidural Abscess and Cervical Spondylitis (급성 경추 경막외 농양 및 경추척추염과 동반된 결핵성 수막염 1예)

  • Neurologic sequelae of tuberculous meningitis include hemiparesis, paraparesis, quadriparesis, aphasia, developmental delay, dementia, blindness, visual field defect, deafness, cranial nerve palsies, epilepsy, and hypothalamic and pituitary dysfunction. But cervical epidural abscess and cervical spondylitis are rare. A 64-year-old woman who was diagnosed as tuberculous meningitis presented a severe neck pain and stiffness after 3 weeks of anti-tuberculous medication. Electromyography and cervical X-ray showed a cervical spondylosis with polyradiculopathy. But cervical MRI showed an acute cervical epidural abscess and mild cervical spondylitis. After continuous anti-tuberculous medication with supportive care, she showed a slow clinical improvement. But about 1 month of anti-tuberculous therapy, she presented a more aggravation of neck pain, neck stiffness, radicular pain, and neck motion limitation. Follow-up cervical MRI showed an more advanced cervical spondylitis. Afterthen she has recovered slowly by cervical laminectomy with posterior stabilization and continuous anti-tuberculous medication.

Intractable Pain Management of Lung Cancer Involving in Both Brachial Plexuses (양측 상박 신경총 침범으로 불인성 통증을 동반한 폐암환자의 통증치료 경험)

  • Severe intractable pain with paresthesia and severe dyspnea produced by lung cancer involving both brachial plexuses, refractory to ordinary pharmacologic approaches, was managed by epidural morphine and bupivacaine administration with the continuous Baxter infusion system. Chest pain, which is somatic pain in character, was well managed with the epidural morphine and bupivacaine administrations. However paresthesia and tingling sensation of the hand and forearm were poorly controlled by epidural morphine, and were finally managed by bolus epidural injections of bupivacaine. Supportive therapy included epidural steroid injection and TENS, but the effect was not satisfactory. Severe dyspnea seemed to aggrevate cancer related pain.

  • Mediastinum is the space located between the right & left lung lobes in the center of the thorax, and contains many vital organs such as heart, aorta, trachea, esophagus, vagus nerve, lymphatics. So the infection of mediastinal space causes a serious and potentially fatal process. Like other infections, surgical drainage, aggressive antibiotic therapy and supportive care are recommended for optimal outcome. Airway management in the presence of the neck, glottic edema, elevation of the tongue is a formidible problem especially in the case from odontogenic origin. We have recently encountered such a case resulting in a number of local and systemic complications.

  • This study analyzed the contents of the research papers concerning the obesity from a psychiatrical point of view. As a result, the following conclusion was drawn. 1. In obese people, negative emotions, especially depression were frequently reported. 2. Negative feelings, especially anger, anxiety, depression, induce binge eating and can lead to obesity. 3. Depression was related to the wrong body image more than to the body mass index. 4. Wrong body image affects eating attitudes, which may cause eating disorders. 5. To treat obesity or eating disorders, we must use psychosocial treatment, for example, supportive, cognitive or behavior therapy.

Cycad revoluta toxicosis in a dog

  • A 3-year-old female Cocker Spaniel dog acutely developed hepatopathy following ingestion of root of Cycad (Cycas revoluta). Seven hours after the ingestion, she showed acute continuous nausea, vomiting, and marked depression and was presented to the local veterinary clinic. At physical evaluation, the dog showed moderate weakness, pale mucous membrane, jaundice, and dehydration. Serum biochemical analysis revealed elevated alanine aminotransferase and aspartate aminotransferase, and mild azotemia. The dog was treated with intravenous fluid, antibiotics, and hepatic protectants for 10 days and recovered without any sequela. Although the ingestion of Cycad in dogs is rare, it can be treated successfully with appropriate supportive therapy.

  • This retrospective study was performed in 40 cases of canine crystalluria and urinary tract symptoms. Breed, age, sex, diet history and common clinical signs were investigated for patient information. CBC, serum chemistry, urinalysis, urine culture, urine sediment and urinary stone analysis were undertaken for clinicopathological diagnosis. Crystalluria and urinary tract symptoms occurred from 6 months till 14 years in this clinical investigation and the average age was 4.68 years. It had taken an average 7.4 weeks to correct crystalluria and urinary tract symptoms treated by supportive therapy and dietary management. In conclusion, regular clinical pathologic examination, treatment and diet management should be needed to prevent the formation of urolithiasis and urinary tract diseases in dogs with crystalluria and urinary tract symptoms.

  • A 21-year-old woman ingested 1,250 mg of diphenhydramine in a single overdose. Diphenhydramine, a rare ingredient in over-the-counter medication, is used to treat insomnia in Korea. Toxicity is usually limited to anticholinergic symptoms. The standard approach to therapy for the treatment of diphenhydramine overdose is supportive care, including physostigmines and sodium bicarbonates. Here, we review the literature and for the first time report a case of acute diphenhydramine overdosage in Korea, complicated with seizures.

  • Hyperemesis gravidarum is a severe and intractable form of nausea and vomiting in pregnancy. It is a diagnosis of exclusion and may result in weight loss; nutritional deficiencies; and abnormalities in fluids, electrolyte levels, and acid-base balance. The peak incidence is at 8-12 weeks of pregnancy, and symptoms usually resolve by week 16. Interestingly, nausea and vomiting of pregnancy is generally associated with a lower rate of miscarriage. The cause of severe nausea and vomiting in pregnancy has not been identified. Extreme nausea and vomiting may be related to elevated levels of estrogens or human chorionic gonadotropin. Once the diagnosis is made, treatment consists of mainly supportive care. Until vomiting is controlled, these patients should be maintained as NPO. We treated 1 patient who had hyperemesis gravidarum and visited Kyung-Won Incheon Oriental Hospital by Herbal medication, acupuncture, moxa and nega therapy and got good result from them.

An Analogous Case of Systemic Lupus Erythematosus (SLE 유사 환자 치험 1례)

  • Systemic lupus erythematosus is a disease of unknown etiology that affects many organ system and is characterized by the presence of multiple autoantibodies that participated in immunology mediated tissue injury. A 36 years-old female patient was admitted to ward due to high fever and erythematous rash on face and hole body. She exhibited itching sense, joint pain, nausea, fatigue, sensitivity to light. The homatologic finding revealed anemia, decrease of lymphocyte, low platelet count, but LE cell, Anti nuclear antibody(ANA) were negative. In the point of Differentiation of Syndrome(辨證), SLE can be thought to be a category of Seasonal febril disease(溫病). This subject diagnosed as Domination of intense evil heat(熱毒熾盛), and was administrated with Chungonpadocyem-gamibang(淸瘟敗毒飮加味方). The clinical and laboratory findings of our patient were improved by herb medication, acupuncture therapy and general supportive measure.

Disseminated Cytomegalovirus Infection and Protein Losing Enteropathy as Presenting Feature of Pediatric Patient with Crohn's Disease

  • We report a pediatric patient admitted with abdominal pain, diffuse lower extremity edema and watery diarrhea for two months. Laboratory findings including complete blood count, serum albumin, lipid and immunoglobulin levels were compatible with protein losing enteropathy. Colonoscopic examination revealed diffuse ulcers with smooth raised edge (like "punched out holes") in the colon and terminal ileum. Histopathological examination showed active colitis, ulcerations and inclusion bodies. Immunostaining for cytomegalovirus was positive. Despite supportive management, antiviral therapy, the clinical condition of the patient worsened and developed disseminated cytomegalovirus infection and the patient died. Protein losing enteropathy and disseminated cytomegalovirus infection a presenting of feature in steroid-naive patient with inflammatory bowel disease is very rare. Hypogammaglobulinemia associated with protein losing enteropathy in Crohn's disease may predispose the cytomegalovirus infection in previously healthy children.
