A Case Study of One Patient Who Has Diabetic Gangrene in Foot and Finger Due to Diabetes (당뇨로 인한 우측(右側 ) 족저부(足底部) 및 좌수지(左手指) 궤양(潰瘍)을 동반한 환자 치험 1 예)

  • 1. Objectives Besides Diabetes Mellitus, its complications are matter of concerns as well. Its following complications are cardiovascular disease, glomerulopathy, retinopathy, neuropathy and diabetic gangrene. These complications can cause overall damage in major organs. In traditional oriental medicine it has been perceived that gangrene is mainly caused by "hwa(火)" but under sasang constitutional medicine its has been diagnosed as Taeumin superficial-disease(太陰人 表病) and Taeumin Jowiseungchung-tang(調胃升淸湯) was prescribed. As a result, significal improvments was noted and thefore we report the outcome. 2. Methods We have given Taeumin Jowiseungchung-tang(太陰人 調胃升淸湯) to a patient for treating diabetes complications. 3. Results This patient was treated with Sasang Constitional Medicine and acupuncture for about three weeks. After treatment, ulcer and whole body edema were improved considerbly. 4. Conclusions We have dianosed the patient as Taeumin superficial-disease(太陰人 表病) by undergoing several diagnostic method used in sasang constutional medicine. Therefore we have given Taeumin Jowiseungchung-tang(太陰人 調胃升淸湯) to the patient and has obtained satisfactory results in curing several diabetic complications especially ulcer due to diabetes.

Recurrent Pleomorphic Adenoma of the Parotid Gland

  • Pleomorphic adenoma is the most common type of salivary gland tumor and the most common tumor of the parotid gland. Because of its propensity for invasion, pleomorphic adenoma of the parotid requires superficial parotidectomy or total parotidectomy to minimize the risk of tumor recurrence. We report a case of pleomorphic adenoma of the parotid gland with repeated recurrences. A 23-year-old male patient presented with a protruding neck mass. Six years prior to this presentation, the patient had undergone superficial parotidectomy for the removal of pleomorphic adenoma of the right parotid gland at our institution. The patient experienced recurrence at 17 months after the initial resection, which required a total parotidectomy with partial resection of the facial nerve. Pathologic examination revealed histologic findings consistent with pleomorphic adenoma across the surgical specimen from all three of the operations. The patient suffered from facial nerve paralysis, with facial expressions partially recovered over a year.

Circularly Rotated Array for Dual Polarized Applicator in Superficial Hyperthermia System

  • A circularly rotated array for a dual polarized applicator in a superficial hyperthermia system is proposed. The applicator has a wider effective treatment area due to the $180^{\circ}$ phase shift. The dual polarized circularly rotated array (DPCRA) suppresses overheating at the center of the array and helps evenly distribute the heat. This array provides a more effective treatment area than a lattice array when a $2{\times}2$ dual polarized array is fitted to the treatment area. The treatment area is 71.5% of the aperture, whereas the effective treatment areas of the $2{\times}2$ dual polarized lattice array (DPLA) and the single polarized array (SPA) are 57.2% and 38.6% of the same aperture, respectively. The measurement matches the simulation results without blood circulation effects. In a $2{\times}2$ array applicator, the proposed DPCRA has more heat uniformity than the DLA and the SPA.

Toward Successful Management of Vocational Rehabilitation Services for People with Disabilities: A Data Mining Approach

  • This study proposes a multi-level data analysis approach to identify both superficial and latent relationships among variables in the data set obtained from a vocational rehabilitation (VR) services program of people with significant disabilities. At the first layer, data mining and statistical predictive models are used to extract the superficial relationships between dependent and independent variables. To supplement the findings and relationships from the analysis at the first layer, association rule mining algorithms at the second layer are employed to extract additional sets of interesting associative relationships among variables. Finally, nonlinear nonparametric canonical correlation analysis (NLCCA) along with clustering algorithm is employed to identify latent nonlinear relationships. Experimental outputs validate the usefulness of the proposed approach. In particular, the identified latent relationship indicates that disability types (i.e., physical and mental) and severity (i.e., severe, most severe, not severe) have a significant impact on the levels of self-esteem and self-confidence of people with disabilities. The identified superficial and latent relationships can be used to train education program designers and policy developers to maximize the outcomes of VR training programs.

  • Sagittal spinal balance means standing postural balance at sagittal plane. Postural imbalance with displacement of the patient's center of gravity can cause chronic back pain and ambulatory difficulty. The sagittal spinal balance is determined based on the deviation of the C7 plumb line, originating at the middle of the C7 vertebral body, from the posterior superior endplate of S1. The line is called as sagittal vertical axis (SVA). In the traditional Korean medicine, the meridian sinews, which are the most superficial pathways of the meridian system, associated with movement, muscle balance and defense. They too are separate from the main meridians, though they intersect the main meridians. Some creative and pioneer researchers in Korea thought that the anatomy trains, which suggested by Myers is a concept familiar to the meridian sinews. A reciprocal relationship between the superficial back line and the superficial front line used to be compared to the rigging of a sailboat. Therefore, We suggest that spine may be compared to a mast of the sailboat and that the sagittal spinal balance can be maintained with systemic reciprocal interacts between the front line muscles and the back.

Change of Venous Pressure of Superficial Vascular System during Free TRAM Flap Elevation (유리횡복직근피판에서 거상 전후의 피하정맥압의 변화)

  • Purpose: Free transverse rectus abdominis musculocutaneous(TRAM) flap is one of the most popular methods of breast reconstruction. But if fat necrosis and fatty induration occur at the reconstructed breast, they can make the breast harder and make it difficult to differentiate a tumor recurrence from them. To expect and prevent these complications, we measured the pressure change of the superficial venous system whose congestion can be the cause of them. Methods: An intraoperative clinical study was done to compare venous pressure of superficial inferior epigastric vein(SIEV) before and after the elevation of free TRAM flap. Fourteen TRAM flaps were included and the pressures of SIEV were measured two times at the beginning of the elevation and just before the division of the inferior pedicle. Results: The venous pressure in free TRAM flap was significantly higher after the flap elevation at both contralateral side and ipsilateral(p=0.005 and p=0.026 respectively). The four cases with vertical scar shower significantly greater increase at contralateral side than ipsilateral side(p=0.020). Conclusion: Intraoperative venous pressure recording can be an objective data for evaluating the congestion of TRAM flap and can help to prevent the complications of fat necrosis and fatty induration with venous superdrainage.

Clinical Features and Outcomes of Superficial Keratectomy and Conjunctival Advancement Hood Flap in Three Dogs with Different Extents of Corneal Edema

  • Three dogs with different extents of corneal edema were presented to the Dana Animal Hospital Eye Center. The dogs (3 eyes) were diagnosed with corneal endothelial degeneration with clinical signs of corneal edema, conjunctival hyperemia, and mild blepharospasm through a full ophthalmic examination. For the treatment of corneal edema, superficial keratectomy using a crescent microsurgical knife was performed, and a conjunctival advancement hood flap was applied to the stromal defects. In two cases where corneal edema and opacity were observed only in a part of the cornea, corneal edema was reduced and did not progress to other parts of the cornea and corneal transparency and vision were also well-maintained during the follow-up on days 349 and 231 after the surgery. In a case where the whole cornea was edematous and cloudy, corneal edema and opacity had not clearly improved at the last follow-up on day 275 after the surgery. In conclusion, SKCAHF relieved corneal edema and improved vision, and the prognosis tended to be better when there was less corneal edema caused by CED.

Study on Hand Greater Yang Skin from the Viewpoint of Human Anatomy

  • Objectives: This study was carried out to analyse Hand Greater Yang Skin in human. Methods: Hand Greater Yang meridian was labeled with latex in the body surface of the cadaver. And subsequently body among superficial fascia and muscular layer were dissected in order to observe internal structures. Results : A depth of Skin encompasses a common integument and a immediately below superficial fascia, this study established Skin boundary with adjacent structures such as relative muscle, tendon as compass. The Skin area of the Hand Greater Yang in human are as follows: The skin close to 0.1chon ulnad of $5^{th}$ nail angle, ulnad base of $5^{th}$ phalanx, ulnad head of $5^{th}$ metacapus(relevant muscle: abductor digiti minimi muscle), ulnad of hamate, tip of ulnar styloid process(extensor carpi ulnaris tendon), radiad of ulnar styloid process, 2cm below midpoint between Sohae and Yanggok(extensor carpi ulnaris), between medial epicondyle of humerus and olecranon of ulnar(ulnar nerve), The skin close to deltoid muscle, trapezius muscle, platysma muscle, inner muscles such as teres major muscle, infraspinatus muscle, supraspinatus muscle, levator scapulae muscle, splenius cervicis muscle, splenius capitis muscle, sternocleidomastoid muscle, digastric muscle, stylohyoid muscle, zygomaticus major muscle, auricularis anterior muscle. Conclusions: The Skin area of the Hand Greater Yang from the anatomical viewpoint seems to be the skin area outside the superficial fascia or muscles involved in the pathway of Hand Greater Yang meridian, collateral meridian, meridian muscle, with the condition that we consider adjacent skins.

Dual-innervated multivector muscle transfer using two superficial subslips of the serratus anterior muscle for long-standing facial paralysis

  • Recent reports have described several cases of double muscle transfers to restore natural, symmetrical smiles in patients with long-standing facial paralysis. However, these complex procedures sometimes result in cheek bulkiness owing to the double muscle transfer. We present the case of a 67-year-old woman with long-standing facial paralysis, who underwent two-stage facial reanimation using two superficial subslips of the serratus anterior muscle innervated by the masseteric and contralateral facial nerves via a sural nerve graft. Each muscle subslip was transferred to the upper lip and oral commissures, which were oriented in different directions. Furthermore, a horizontal fascia lata graft was added at the lower lip to prevent deformities such as lower lip elongation and deviation. Voluntary contraction was noted at roughly 4 months, and a spontaneous smile without biting was noted 8 months postoperatively. At 18 months after surgery, the patient demonstrated a spontaneous symmetrical smile with adequate excursion of the lower lip, upper lip, and oral commissure, without cheek bulkiness. Dual-innervated muscle transfer using two multivector superficial subslips of the serratus anterior muscle may be a good option for long-standing facial paralysis, as it can achieve a symmetrical smile that can be performed voluntarily and spontaneously.

Anatomical Study on Hand Gworeum Skin in Human

  • Objectives: This study was carried out to concrete the concept of Hand Gworeum Skin referred in Suwen of Huangdi Neijing. Methods: The Hand Gworeum Meridian was labeled with latex in the body surface of the cadaver, subsequently dissecting a superficial fascia and muscular layer in order to observe internal structures. Results: Skin histologically encompasses a common integument and a immediately below superficial fascia, this study established the skin boundary with adjacent structures such as relative muscle, tendon as its compass. The realm of the Hand Gworeum Skin is as follows: The skin close to the nipple on the 4th intercostal space, the interceps of biceps brachii muscle, the cubital surface at ulnad of bicipital aponeurosis, the anterior surface of the forearm, between flexor carpi radialis and palmaris longus(from wrist crease to 5chon above), the palm between the 3rd and 4th metacarpals on the cross part with the palm crease, the radiod from the middle finger nail(or the end of middle finger). The realm of the Hand Gworeum Skin is situated on between Hand Taeeum Skin and Hand Soeum Skin in front of arm. Conclusion: The realm of Hand Gworeum Skin from the anatomical viewpoint seems to be the skin area outside the superficial fascia or the muscle involved in the pathway of the Hand Gworeum Meridian vessel, Collateral Meridian vessel, and Meridian muscle, being considered adjacent vessels or nerves at the same time.
