Sacral schwannoma is a rare lesion with a tendency to reach large proportions. The benign schwannoma rarely involves the vertebral bodies extensively. The authors report a case of giant intrasacral schwannoma in 30-year-old woman who had intermittent lower back pain during 3 years period. CT and MRI showed a destructive mass lesion within the upper part of sacrum with a large mass extending into the presacral space. The patient underwent combined surgery consisted of anterior transabdominal approach and posterior sacral laminectomy and total removal of tumor. The characteristics of the lesion were discussed with a review of literatures.

Maffucci's syndrome is generally characterized by chondromatosis with vascular lesions of mesodermal origin, however, intracranial chondroma is rare. We present a case of Maffucci syndrome with paracellar chondroma and the review of literatures.

Schwannomas are uncommon neoplasms arising from Schwann cells of the neural sheath. Gastrointestinal schwannomas are rare, accounting for 1% of all malignant gastrointestinal tumors. Colonoscopic biopsy with immunohistochemical (IHC) staining is useful for confirming this tumor. We report on a patient with schwannoma arising from the ascending colon, which was detected by colonoscopy and endoscopic submucosal dissection was attempted. A 41-year-old man presented with abdominal discomfort. The patient was diagnosed with a subepithelial tumor on colonoscopy. He underwent endoscopic submucosal dissection. Histopathology and IHC staining confirmed that the colonic lesion was a benign schwannoma. However, the resection margin was positive. Therefore, laparoscopic ileocolectomy was performed.

Traumatic dislocation of the peroneal tendon is a rare injury. It should be distinguished from other conditions that can cause pain and disability of lateral aspect of the ankle joint. We experienced a case of chronic recurrent dislocation of the peroneal tendon, which was treated by deepening of the peroneal groove of the lateral malleolus and reattachment of the superior peroneal retinaculum. The clinical result was satisfactory.

The fracture and fracture-dislocation of the neck of the talus (Hawkins' type I-IV) are uncommon injuries and represent only 0.12 to 0.32% of all fracures. Authors clinically evaluated in 12 cases Whom treated fracures of the neck of the talus, at department of orthopaedic surgery, Sun General Hospital, from 1990 to 1996, and the following results are obtained. 1. Of 12 cases, there were 11 males and 1 female, average age was 30 years. 2. Causes of fracture was fall down injury in 7 cases(58%), traffic accident in 4 cases(33%), direct trauma in 1 case(8%). 3. According to the classification by Hawkins' type I in 2 cases(17%), type II in 7cases (58%), type III in 3cases(25%). 4. Associated injuries were calcaneal fracture in 3 cases, fracture-dislocation of talus in 3 cases, subtalar dislocation in 3 cases, medial malleolar fracture in 5 cases, soft tissue injury in 3 cases, femur and tibia fracture in 1 case, and lumbar Spine compression fracture in 1 case. 5. Average time to operation after injury was 2.5 days. 6. In 2 cases were treated conservatively and 10 cases were treated open reduction and internal fixation with screw or K-wire. 7. Complications were avascular necrosis in 4 cases, post traumatic arthritis in 2 cases, skin necrosis in 4 cases, and then ankle fusion was done in 2 cases. 8. High rate of complication was seen in the talar neck fracture associated with calcaneal fracture. In the analysis of above results, evaluated by Hawkins' scoring system were excellent to fair in 75%.

Pigmented villonodular synovitis (PVNS) is a slowly, progressive, proliferative disorder of synovial tissue characterized by villous or nodular changes of synovial-lined joints, bursae, and tendon sheaths and most frequently affects the large joints, with the knee and hip. A few studies have been reported that occurred PVNS in small joint, but mainly in hands. It is a very rare condition that occurs in the small joints of the forefoot. We have experienced the case, which developed in small joint of the forefoot, and performed total synovectomy. After the operation, there was no recurrence. We report a case of PVNS in forefoot with a review of the literature.

관절내 골연골 골절은 적절한 치료가 이루어 지지 않은 경우 골관절염을 초래하여 통증과 기능장애를 초래할 수 있다. 골 연골 골절의 크기가 큰 경우에는 관혈적 정복 및 금속고정 수술등으로 치료를 할 수 있으나, 골편의 두께가 2mm 이하로 얇은 경우 튼튼한 고정력을 얻기 어려워 제거 하는 경우가 많았다. 저자들은 대퇴 내과에 발생한 박리성 골연골염 환자와 슬개골 골절로 골편의 두께가 2mm이하인 환자에서 흡수성 반월상 연골나사를 이용하여 좋은 결과를 얻었기에 보고하고자 한다.

슬관절 후방십자인대의 결절종은 드물게 발생하며, 이에 대한 보고도 적다. 특별한 외상의 병력 없이 발생한 2례의 후방십자인대 결절종을 발견하고, 관절경을 이용하여 생검 및 제거를 하였기에 그 경험을 보고하는 바이다.
