The Distance from Skin to Cervical Epidural Space (피부에서 경부 경막외강까지의 깊이에 대한 연구)

  • Backgrounds: Cervical epidural nerve block is useful in the management of a variety of acute, chronic and cancer related pain syndromes involving the head, face, neck and upper extrimity. To safely perfom the cervical epidural block, an appreciation of the expected distance from skin to epidural space is important. We studied the distance from skin to cervical epidural space of adults to determine if any relationship exists between patient height, weight and neck circumference and the distance from skin to epidural space. Methods: Patients 170, suffering from neck and upper extremity pain with cervical HIVD(herniated intervertebral dics) were selected. Cervical epidural block was performed at $C_{6\sim7}$ or $C_{7-}T_1$ intervertebral space. Then measured the distance from skin to epidural space and analysed the relationship between age, height, weight and neck circumference and the distance from skin to epidural space. Results: The cervical epidural depth of male $C_{6\sim7}$, male $C_{7-}T_1$, female $C_{6\sim7}$ and female $C_{7-}T_1$ groups were $5.17{\pm}0.63$, $5.47{\pm}0.59$, $4.84{\pm}0.56$ and $5.01{\pm}0.60$ cm respectively. Cervical epidural depth significantly correlated with body weight, ponderal index and neck circumference. Conclusions: The distance from skin to cervical epidural space has significant relationships with weight, ponderal index and neck circumference. Although experience is important, patient's weight and neck circumference are indicating factor, of the cervical epidural depth.

  • The purpose of this article is to argue that Korean music, Yukjabaegi (six-bit songs), helps us overcome grief. It is not untrue to say that Yukjabaegi is among the greatest types of music in Korea. This is a testament to the prominence of this statue among Han Ak (Korean music, 韓樂). Empathy is an inherent human quality involving all parts of the brain which is also linked to feelings of love. Empathy is created the moment we imagine ourselves in someone else's situation. Moreover, if empathy is good for us, the generation of empathy may actually mobilize our strength. Koreans have superior strength in overcoming grief and difficult situations by singing together. Often Koreans feel that their lives and labors are difficult, they start singing Yukjabaegi because they consider grief and anxiety wear on a person more than hard work. Listening to and singing Yukjabaegi can cheer up people who feel as limp as a rag after hard labor. Tackling reality and venting the experience via creative expression is bound to be healthy. Under a change of the rhythm pattern, Yukjabaegi thus becomes a major form of amusement for those suffering from difficult situations and comes as a true friend to share in our grief with us. Singing together via Yukjabaegi is a time to share grief. Thus, this, author believes that it is related to empathy. It helps us overcome grief by means of music therapy.

Study on the mental stress of the invalids with the four most common chronic illnesses and their congenital diseases in light of the four pillars of destiny -centering on the elderly in Seoul nursing homes (4대 중증질환자의 정서적 스트레스와 명리학적 선천성 질환에 관한 연구 -서울시 요양원 입소노인을 중심으로-)

  • The objective of this study is to analyze the correlations between the chronic illnesses and congenital diseases of the chronic elderly invalids in nursing homes, as well as to examine the impact of stress scale and the factors affecting the emotional burnout scale. This study conducted Seoul survey from March 15 to May 31, 2013, targeting 147 seniors in nursing homes who were suffering from the four most common chronic illnesses. Analyses performed in this study include frequency, correlation and simple regression analyses, t-test and one-way analysis of variance, etc. All of the empirical analyses were conducted at 5% level of significance. The result of the study shows that there is a high correlation between the chronic invalids and their congenital diseases determined by the four pillars of destiny and that, if they are found to have a chronic illness, patients experience a high level of stress. Persons who score higher on the stress scale tend to score higher in the emotional burnout scale.

Palliative Care Education in Gynecologic Oncology: a Survey of Gynecologic Oncologists and Gynecologic Oncology Fellows in Thailand

  • Background: The main purpose of this study was to survey the education and training of certified gynecologic oncologists and fellows in Thailand. A secondary objective was to study the problems in fellowship training regarding palliative care for gynecologic cancer patients. Materials and Methods: A descriptive study was conducted by sending a questionnaire regarding palliative care education to all certified gynecologic oncologists and gynecologic oncology fellows in Thailand. The contents of the survey included fellowship training experience, caring for the dying, patient preparation, attitudes and respondent characteristics. Statistics were analyzed by percentage, mean and standard deviation and chi-square. Results: One hundred seventy completed questionnaires were returned; the response rate was 66%. Most certified gynecologic oncologists and fellows in gynecologic oncology have a positive attitude towards palliative care education, and agree that "psychological distress can result in severe physical suffering". It was found that the curriculum of gynecologic oncology fellowship training equally emphasizes three aspects, namely managing post-operative complications, managing a patient at the end of life and managing a patient with gynecologic oncology. As for experiential training during the fellowship of gynecologic oncology, education regarding breaking bad news, discussion about goals of care and procedures for symptoms control were mostly on-the-job training without explicit teaching. In addition, only 42.9 % of respondents were explicitly taught the coping skill for managing their own stress when caring for palliative patients during fellowship training. Most of respondents rated their clinical competency for palliative care in the "moderately well prepared" level, and the lowest score of the competency was the issue of spiritual care. Conclusions: Almost all certified gynecologic oncologists and fellows in gynecologic oncology have a positive attitude towards learning and teaching in palliative care. In this study, some issues were identified for improving palliative care education such as proper training under the supervision of a mentor, teaching how to deal with work stress, competency in spiritual care and attitudes on responsibility for bereavement care.

Docetaxel and Cisplatin in First Line Treatment of Patients with Unknown Primary Cancer: A Multicenter Study of the Anatolian Society of Medical Oncology

  • Background: The overall prognosis for cancers of unknown primary (CUP) is poor, median overall survival (OS) being 6-12 months. We evaluated our multicentric retrospective experience for CUP administered docetaxel and cisplatin combination therapy. Materials and Methods: A total of 29 patients that were pathologically confirmed subtypes of CUP were included in the study. The combination of docetaxel ($75mg/m^2$, day 1) and cisplatin ($75mg/m^2$, day 1) was performed as a first line regimen every 21 days. Results: The median age was 51 (range: 27-68). Some 17 patients had multimetastatic disease on the inital diagnosis. Histopathological diagnoses were well-moderate differentiated adenocarcinoma (51.7%), undifferentiated carcinoma (27.6%), squamous cell cancer (13.8%), mucoepidermoid carcinoma (3.4%) and neuroendocrine differentiated carcinoma (3.4%). Median number of cycles was 3 (range: 1-6). Objective response rate was 37.9% and clinical benefit was 58.6%. Median progression free survival (PFS) and overall survival (OS) were 6 months (range: 4.3-7.7 months) and 16 months (range: 8.1-30.9 months), respectively. Fourteen patients (60.8%) were treated in a second line setting. There was no treatment related death. Most common toxicities were nausia-vomiting (44.6%) and fatigue (34.7%), serious cases (grade 3/4) suffering nausia-vomiting (10.3%), neutropenia (13.8%) and febrile neutropenia (n=1). Conclusion: The combination of cisplatin and docetaxel is an effective regimen for selected patients with CUP.

  • This paper describes a Q-methodological study on the Perception of comfort in hospital in-patients. The participants completed a 37-item Q sort made up of statements which could be ranked in terms of their relevance to the subjective meaning of comfort. Three interpretable types of comfort were identified through this Q study : Type I, positive medical action style, feel secure and satisfied when medical staff show a positive attitude towards them as patients. They put a high value on prompt responses from medical staff and physical aspects of care like non-invasive procedures or comfortable position. Type II, social relation style, experience a state of comfort when they perceive support or concern from medical staff, relatives or other patients suffering from similar diseases. They feel safe and secure when medical staff are kind and have a good reputation or when the size of the hospital is big enough, Type III, emotional wellbeing style, feel that hope for a healthy life or maintaining self-care activities are highly valuable. They feel safe and comfortable when their privacy is protected. They put worth on independent thinking, strong will, and emotional or psychological comfort. The results of this study can be used as a basis to develop nursing measures for comfort. Further studies on factors which influence perception of comfort and intervention strategies according to the above types of comfort need to be done.

The Effects of Combined Exercise Program to Intra-City Bus Driver With Chronic Neck Pain : Case Report (만성 경부통을 가진 시내버스 운전기사에게 복합 운동프로그램의 효과 : 사례연구)

  • Background: Although many of those who drive for a living are exposed to repetitive and awkward posture and whole body vibration which cause them cumulative trauma disorder, therapeutic studies related to the problems from the transport unit are still somewhat inadequate. The purpose of this case report is to identify the effect of combined exercise program on patients suffering chronic neck pain caused by long hour of intra-city bus driving. Method: A total of 4 subjects are selected as patients, all of whom have more than 10 years of experience in intra-city bus transportation services. These people have suffered from neck pain for years. We ran the combined exercise program 3 times a week for 4 weeks and respectively evaluated the results after the 2nd and 4th week. We implemented conventional physiotherapy for 40 minutes, another 40 minutes of combined exercise program, and then educated the patients to enable themselves to do active stretching program as a home program. The combined exercise program contained 3 different stages. 1st stage: active stretching program, 2nd stage combined stabilization exercise and strengthening exercise, 3rd stage: proprioceptive exercise Result: After 4 weeks of intervention, there was enhance in the range of motion. $12^{\circ}$ increase in flexion, $10^{\circ}$ in extension, $6^{\circ}$ in lateral flexion respectively, and $10^{\circ}$ in rotation on average. VAS(visual analgue scale) decreased by 33% on average, NDI(neck diability index) by 28% and fatigability by 23%. Conclusion: There were improved results in the range of motion, NDI, VAS, and in fatigability after applying combined exercise program to intra-city bus drivers exposed to whole body vibration and cumulative trauma disorder.

  • In popular music, soul music is becoming an evaluation criteria in Korea. Blues, a songer music that was singing and lamenting Filed Holler and their suffering in a vast land of black blood and sweat, which became the mother of music to understand our enthusiastic soul music. And the only spiritual haven, church music and important musicians. Especially in the 1960s Americans social background is the most important factor to understand the soul music of birth time. Our country, which has undergone tremendous colonial rule amid numerous foreign invasions, is naturally ashamed of its aging skies. For Black-Americans soul music is an expression and anger expression anger. However, many musicians, who are flooded with floods in our country, are destined to experience the essence of life. 'Soul music' the noble choice of choice music, is known to be the ultimate reminder of the misery of the eternal disaster. As soul music has become a powerful it is understandable that it has become an exaggeration.

A Qualitative Case Study on the Educational Culture Experienced by Married Immigrant Women and their Acculturation (여성결혼이민자가 체험한 교육문화와 문화적응에 관한 질적 사례연구)

  • This study investigated the educational culture experienced by married immigrant women in their own country, as well as their internalizing process of the educational culture in Korea, using a qualitative case study. The purpose of this study was to help married immigrant women solve the problem of acculturation based on their own educational and cultural experiences and to overcome limitations of previous studies, which dealt with the problem of acculturation superficially. Participants were 17 married immigrant women from eight countries who resided in Jeonnam and Jeonbuk provinces. They were selected by the sampling strategy of maximum variation. Data were collected through archives and in-depth interviews and analyzed using within-case analysis and cross-case analysis. The results showed that married immigrant women had no experience of attending private institutions and had good memories of their school life. Based on these kinds of experiences, they criticized Korean mothers who urged their children to attend private institutions. They respected their children's opinions and allowed their children to select private institutions. Despite married immigrant women apparently suffering from acculturation, they educate their children according to their own philosophy and method of education. That is, they solve the problem of acculturation actively and independently. Consequently, in order to help married immigrant women solve the problem of acculturation, it is necessary to respect the difference in the cultures and frame a system that develops the activity and subjectivity of married immigrant women.

Successful Treatment with High Dose Transdermal Fentanyl Patch for Severe Cancer Pain in a Patient with Lung Cancer (폐암 환자에서 고용량 펜타닐 첩포를 이용한 심한 암통증의 치료 경험)

  • A 60-year-old man was diagnosed with locally advanced non-small cell lung cancer. He refused treatment with a curative aim and was treated conservatively. Pain had developed on his shoulder and chest wall, which became worse as the cancer progressed. Although his pain initially appeared to be relieved with weak opioids and analgesics, it became more severe Strong opioids (transdermal fentanly patch and oxycodone), antidepressant or epidural block were introduced, However, the background pain became more intense and reached up to 8~9/10 on the visual analog scale (VAS). The dose of the transdermal fentanl patch was gradually increased to $600{\mu}g/hr$, which resulted in a dramatic improvement in his pain (9/10 of VAS) to 3/10 for most of the time. We described the successful experience with a high dose transdermal fentanyl patch for cancer pain relief, which might be an alternative option for cancer patients suffering from severe pain.
