특발성 돌발성 난청으로 오인된 청신경 종양 1례 (A Case Report of Vestibular Schwannoma Misdiagnosed as Idiopathic Sudden Sensorineural Hearing Loss)

  • 1. 청신경 종양 진단의 표준 검사는 두부 가돌리늄 조영 증강 자기 공명 영상 촬영이며, 경제적 문제 등으로 조영 증강 자기 공명 영상 촬영이 어려울 경우에는 청성 뇌간 반응(auditory brainstem response) 검사를 시행할 수 있다. 2. 특발성 돌발성 난청으로 진단 받은 후, 한의 의료기관에 내원한 환자에서도 기존 검사결과를 검토하여 후미로성 질환을 배제한 후 치료를 시작해야한다. 3. 비조영 증강 자기 공명 영상 촬영을 통해 후미로성 질환을 배제한 환자에서도 임상적으로 후미로성 질환이 의심되는 경우에는 표준 검사인 가돌리늄 조영 증강 자기 공명 영상 재촬영을 통해 청신경종을 감별하는 것이 바람직하다. 4. 청신경 종양에서 한방 치료를 표준 치료와 병행하여 제반 증상에 대한 관리를 기대해 볼 수 있다.

  • Objectives : The purpose of this study is to report the Korean medical treatment on sudden sensorineural hearing loss(SSNHL). Methods : The patient who has been diagnosed SSNHL had symptoms with hearing loss, tinnitus, and aural fullness. The patient was received outpatient to Dunsan oriental hospital for 3 months and was treated with oriental medicine(acupuncture, herbal-medicine, converting of Hominis placenta, warm needling and vapor treatment). Two approaches were used in order to evaluate the effect of the treatment. The hearing recovery was measured with pure tone audiometry, and the decrease of tinnitus and aural fullness was identified by patient's subjective expression. Results : The patient has his hearing restored fully and the symptoms of SSNHL accompanied with adjunctive symptoms including tinnitus and aural fullness were improved.Conclusions : This study shows that the Korean medical treatment effects on the SSNHL.

  • Objectives : The purpose of this study is to analyze outpatients who visited for treatment for sudden sensorineural hearing loss in ophthalmology & otolaryngology & dermatology clinic of Daejeon Korean medical hospital from January 1st, 2013 to December 31th, 2021. Methods : We retrospectively analyzed outpatients who visited for treatment for sudden sensorineural hearing loss in ophthalmology & otolaryngology & dermatology clinic of Daejeon Korean medical hospital from January 1st, 2013 to December 31th, 2021. according to year, season, gender, age, clinical symptoms, number of visits and initial degree of hearing loss. The statistical analysis performed using IBM SPSS 25.0 for Windows. Results : The results of analyzing the medical records of 194 patients are as follows. 1. The number of patients showed a gradual increase from 2014 to 2021 and spring was highest in season. Acc ording to gender, women visited more than men and the age group that visited the most was middle-aged, including 40s and 50s. 2. Patients experienced 13 symptoms and the average number was 4.06. In the analysis of the major symptoms according to gender, hyperacusis and headache were found to be more common in female. 3. The average of initial hearing loss was 52.54dB and was higher in male. The loss was relatively higher in 60s or older than in the younger age group. Conclusions : If treatment and management methods that reflect these symptoms are developed, it is thought that it will greatly helpful to improve the quality of life of patients with sudden hearing loss.

  • Objective : The purpose of this study is to know the effect of Korean medical treatments on sudden sensorineural hearing loss(SSNHL) without systemic or intratympanic steroid treatments.Method : We conducted retrospective review who was diagnosed as SSNHL and treated with Korean medicine in Dept. of Otolaryngology Kyung Hee University Hospital at Gangdong. There were two patients with SSNHL who refused to be treated with western medicine and were treated with Korean medicine including acupuncture, moxibustion, and herbal medicine.Result : One patient recovered completely after eight days of admission, and another patient partial recovered during 30 days through Korean medical treatments.Conclusion : Through these studies, SSNHL is expected to treated more effectively by conventional Korean medical treatments than corticosteroid treatments alone. But the additional study is needed to reveal the exact effects of Korean medical treatments.

심한 현훈을 동반한 돌발성 난청 치험 1례 (A Case of Korean Medical Treatment of Sudden Hearing Loss with Severe Vertigo)

  • Objectives : Sudden sensorineural hearing loss(SSNHL) is defined as an unexplainable decline in hearing which o㏄urs within a short period of time. This study describes a case of SSNHL accompanied by many unfavourable prognosis factors including severe vertigo. Methods : We treated 1 SSNHL patient who had many unfavourable prognosis factors including severe vertigo. After Korean medical treatment for 23 days, we measured the hearing recovery and decrease of vertigo. For diagnosis, pure tone audiometry was done. The degree of improvement was evaluated by Siegel's Criteria. Decrease of vertigo was assessed by Vertigo Scale. Results & Conclusions : The pure tone 5 frequency(0.25, 0.5, 1, 2, $4_{KHz}$) average improved by 25dB(slightly recovered). Vertigo score decreased from 4 to 1. We may expect that the Korean medical therapy improves the hearing recovery in the SSNHL patients who have unfavourable prognosis factors such as vertigo.

  • Objectives : The purpose of this study is to report three cases of Korean medical treatment for sudden sensorineural hearing loss(SSHL). Methods : This study conducted with three sensorineural hearing loss patients who hospitalized in Ophthalmology, Otolaryngology & Dermatology Clinic of Korean Medical Hospital. Three patients were treated with herbal medicine(Sunkihwalhyeol-tang, Ikgibohyeol-tang, Gongjindan) and acupuncture. After treatment, we evaluated Siegel's Criteria with pure tone audiometry and subjective symptoms alteration. Results : All three patients were completely recovery about Siegel's Criteria and improved subjective symptoms after treatment. Conclusions : This study suggests that Korean medical treatment may be effective about SSHL.

동일기관 내 협진을 통한 돌발성 난청의 한·양방 병용치험 3례 (Three Cases of Sudden Hearing Loss Improved after East-West Medical Combined Treatment through Cooperation in a Hospital)

  • Objectives : The purpose of this study is to report the effect of east-west medical combined treatment of sudden sensorineural hearing loss(SSNHL) through cooperation in a hospital. Methods : We treated three patients diagnosed as SSNHL by combination of herbal medicine, acupuncture, pharmoacupuncture, moxibustion, cupping, laser therapy and conventional medications. We evaluated the results of this treatment with pure tone audiometry, word recognition score(WRS), the changes in distortion product otoacoustic emissions(DPOAE) and visual analogue scale. Results : One of them showed a meaningful improvement in the hearing level of the low frequency region. The others showed 'Complete recovery' in pure tone audiometry and WRS. The subjective symptoms including tinnitus and ear fullness improved in three patients. Conclusion : This study suggests that east-west medical combined treatment is effective on SSNHL patients.

Comparison of the Effects of Intratympanic Steroid Injection at Different Intervals in Sudden Sensorineural Hearing Loss

  • Background and Objectives: Systemic steroid therapy (SST) and intratympanic steroid injection (ISI) have been the treatment of choice for sudden sensorineural hearing loss (SSNHL). We studied the effect of ISI administered at different intervals on hearing outcomes in patients with SSNHL. Subjects and Methods: We performed a retrospective study of 427 patients diagnosed with SSNHL at Bundang Jesaeng Hospital, of whom 51 patients with SSNHL who received SST and four ISIs were included in this study. Patients were treated with four ISIs either every day for 4 days (group 1) or at intervals (mean duration of interval: 2.21 days) (group 2). Hearing outcomes were evaluated using the pure-tone test before the injection and 14 days, 1 month, and 3 months after the final injection. Recovery rates were classified based on Siegel's criteria. Results: The amount of improvement was 27.67 dB (±20.45) in group 1 and 32.79 dB (±21.42) in group 2. However, there were no significant differences between the two groups (p= 0.714). The recovery rates based on Siegel's criteria were 18/27 (66.7%) and 16/24 (66.7%) in groups 1 and 2, respectively, with no significant difference (p=1.000). Considering only complete recoveries in hearing recovery, the recovery rates were 15/27 (55.6%) and 14/24 (58.3%) in groups 1 and 2, respectively, with no significant difference (p=0.842). Conclusions: There were no significant differences in hearing outcomes or recovery rates after ISI administration every day or at intervals of 2-3 days.

돌발성 난청의 한방치료 (Oriental Medical Therapy for Sudden Sensorineural Hearing Loss)

  • Objectives: Sudden sensorineural hearing loss (SSHL) is considered an ENT emergency. Despite being a well-recognized condition, SSHL remains one of the most controversial issues in otology. Nowadays, more and more patients have an interest in Oriental medicine for treatment of SSHL. So, to ascertain the therapeutic effect of Oriental Medicine on SSHL, nineteen cases of SSHL patients who had taken Oriental medical therapy in Kyung Hee Oriental Medical Hospital were examined and analyzed. Methods: Nineteen patients who received over 10 times acupuncture therapy and a minimum 2 weeks of herbal medicine from Sep. 1, 2007 to Aug. 31, 2008 were examined and analyzed. The patients who were in the categories below were excluded: - within 7 days after onset - didn't fulfill 10 times acupuncture therapy - failed to recheck hearing outcome after treatment - less than 30dB at mean dB from 250Hz${\sim}$4000Hz. Results: The patients consisted of 12 men and 7 women with a mean age of 45.63 years (19${\sim}$76). Before treatment, 17 patients had tinnitus, 16 patients had pressure in the ear and 6 patients had dizziness, and mean dB of all patients was 66.89 dB. After treatment, 9 patients still had tinnitus, 4 patients felt pressure in the ear and 2 patents felt dizziness, and mean dB of all patients was 54.57dB. After treatment, 9 patients showed effectiveness in improving both hearing level and speech discrimination, 6 patients showed effectiveness only on speech discrimination and 4 patients showed no therapeutic effect. Conclusion: Oriental medical therapy had some therapeutic effects on SSHL even it was started 7 days after onset of the disease.
