Application of Modified Vacuum Assisted Closure with Silver Materials in Chronic Infected Wound (만성 감염성 창상에 대한 변형 음압요법과 은이온 제재의 이용)

  • Purpose: Chronic infected wounds sustained over 4 weeks with exposed tendon or bone are difficult challenges to plastic surgeons. Vacuum assisted closure (VAC) device has been well used for the management of chronic wounds diminishing wound edema, reducing bacterial colonization, promoting formation of granulation tissue and local blood flow by negative pressure to wounds. But Commercial ready-made VAC device might have some difficulties to use because of its high expenses and heavy weight. So we modified traditional VAC device with silver dressing materials as topical therapeutic agents for control of superimposed bacterial wound infection such as MRSA, MRSE and peudomonas. Methods: We designed the modified VAC device using wall suction, 400 cc Hemovac and combined slow release silver dressing materials. We compared 5 consecutive patients' data treated by commercial ready-made VAC device(Group A) with 11 consecutive patients' data treated by modified VAC device combined with silver dressing materials(group B) from September 2004 to June 2007. Granulation tissue growth, wound discharge, wound culture and wound dressing expenses were compared between the two groups. Results: In comparison of results, no statistical differences were identified in reducing rate of wound size between group A and B. Wound discharge was significantly decreased in both groups. Modified VAC device with silver dressing materials showed advantages of convenience, cost effectiveness and bacterial reversion. Conclusion: In combination of modified VAC device and silver dressing materials, our results demonstrated the usefulness of managing chronic open wounds superimposed bacterial infection, cost effectiveness compared with traditional VAC device and improvement of patient mobility.

Characteristics of Noise Emission from Wind Turbine Generator According to Methods of Power Regulation (파워 조절 방법에 따른 풍력 터번 발전기의 방사 소음 특성)

  • In the development of electricity generating wind turbines for wind farm application, only two types have survived as the methods of power regulation; stall regulation and fun span pitch control. The sound measurement procedures of IEC 61400-11 are applied to field test and evaluation of noise emission from each of 1.5 MW and 660 kW wind turbine generators (WTG) utilizing the stall regulation and the pitch control for the power regulation, respectively. Apparent sound power level, wind speed dependence and third-octave band levels are evaluated for both of WTGs. It is found that while 1.5 MW WTG using the stall control is found to emit lower sound power than 660 kW one using the pitch control at low wind speed (below 8 m/s), sound power from the former becomes greater than that of the latter in the higher wind speed. Equivalent continuous sound pressure levels (ECSPL) of the stall control type of WTG vary more widely with wind speed than those of the pitch control type of WTG These characteristics are believed to be strongly dependent on the basic difference of the airflow around the blade between the stall regulation and the pitch control types of WTG. These characteristics according to the methods of power regulation lead to the very different noise emission characteristics of WTG depending on the seasons because the average wind speed in summer is lower than the critical velocity over which the airflow on the suction side of blade in the stall types of WT are separated. These results propose that, in view of environmental noise regulation, the developer of wind farm should give enough considerations to the choice of power regulation of their WTG based on the weather conditions of potential wind farm locations.

  • The effects of sideslip on the vortex over a delta wing was investigated experimentallu at a free strean velocity of 40 m/sec, corresponding to a Reynolds number of 1.76$\times$$10^6$, based on the root chord. The angles of attack ranged from $16{^{\circ}}$ to $28{^{\circ}}$, and the sideslip angles treated were $0{^{\circ}}$, $-10{^{\circ}}$, and $-20{^{\circ}}$. It was observed that the sideslip decreased the strengths of the vortices of both windward and leeward sides of the wing, and promoted the vortex breakdown on the windward side. At sideslip angle of $-10{^{\circ}}$, the vortex strength of leeward side was increased as the angle of attack increased. This asymmetric development and breakdown of vortices in sideslip condition would cause an abrubt change of the rolling moment at a high angle of of attack, which could be considered as a rolling moment instability.

National-Wide Survey on Endotracheal Suctioning in High-Risk Infants (고위험신생아를 위한 기관지흡인에 대한 실태조사)

  • The goal of respiratory management in high risk infants is to maintain proper oxygenation by supporting respiration, therefore to minimize the secondary complications and to promote the maximum growth and development. While on artificial ventilator to achieve this goal, the infants require endotracheal suctioning(ETS) to remove lung secretions. However, the negative effects of ETS in neoates have been documented and include hypoxia, bradycardia, mucosal damage, increased intracranial pressure, and death result. The purpose of the study was to investigate how ETS is currently performed in NICU, which would be beneficial to develop the standardized ETS protocol and to apply it to these population. A national-wide survey on clinical protocol of ETS was performed to 149 neonatal nurses with the average of 3 years and 6 months experience in neonatal nursing, 34.2% of whom was bachelor in nursing. The results showed that about 89% of the nurses initiate En primarily based on the need of the subjects. The aseptic regulation on ETS was used in 83.9% of the subjects. There was no regulation on the length of catheter in 32.9% and on ID/OD ratio in 17.4%. Many nurses administered hyperoygenation/hyperinflation/hyperventilation based on personal knowhow, rather than scientific rationals (77.2%, 40.9%, 75.2%, retrospectively). About 41% of the nurse regulate subjectively the suction power, while 73.8% of them rotate the sub ject's head during suctioning and the half of the nurses was favorable in adapting the closed-suctioning protocol. With the findings of the study, the current clinical application of E% in neonates appears to be based on adult care practices, or personal preference, rather than scientific validation of the safety and effectiveness of the procedure. This study support the needs for developing and applying the standardized ETS protocolin conjunction with the consideration given to the physiologic characteristics of the neonates in respiratory distress.

Internal Flow Analysis of Seawater Cooling Pump using CFD (CFD를 이용한 해수냉각펌프의 내부유동 분석)

  • This research focuses on simulation and visualization of flow field characteristics inside a centrifugal pump. The 3D numerical analysis was carried out by using a numerical CFD tool, addressing a Reynolds Average Navier-Stock code with a standard k-${\varepsilon}$ two-equation turbulence model. The simulation accounts for friction head loss due to rough walls at suction, impeller, discharge areas and volumetric head loss at impeller wear ring. A comparison of performance curves between simulation and experimentation is included, and it reveals a same trend of those results with a small difference of maximum 5 %. At best efficiency point, velocity vectors are smooth but it changes significantly under off-design point, a strong recirculation appears at the outlet of impeller passages near tongue area. A relatively uniform preassure distribution was observed around the impeller in despite of the tongue. Within the volute, because of its geometry, spiral vortexes formed, proving that the flow field in this region was relatively turbulent and unsteady.

A Study on the Unsteady Flow Characteristics of a Delta Wing by 3-D Stereo PIV (3-D Stereo PIV에 의한 비정상 델타윙 유동특성에 대한 연구)

  • Leading edge extension(LEX) in a highly swept shape applied to a delta wing features the modem air-fighters. The LEX vortices generated upon the upper surface of the wing at high angle of attack enhance the lift force of the delta wing by way of increased negative suction pressure over the surfaces. The present 3-D stereo PIV includes the Identification of 2-D cross-correlation equation, stereo matching of 2-D velocity vectors of two cameras, accurate calculation of 3-D velocity vectors by homogeneous coordinate system, removal of error vectors by a statistical method followed by a continuity equation criterion and so on. A delta wing model with or without LEX was immersed in a circulating water channel. Two high-resolution, high-speed digital cameras($1280pixel{\times}1024pixel$) were used to allow the time-resolved animation work. The present dynamic stereo PIV represents the complicated vortex behavior, especially, in terms of time-dependent characteristics of the vortices at given measuring sections. Quantities such as three velocity vector components, vorticity and other flow information can be easily visualized via the 3D time-resolved post-processing to make the easy understanding of the LEX effect or vortex emerging and collapse which are important phenomena occurring in the field of delta wing aerodynamics.

Study on the Development of Home Care Nursing Intervention Protocol for Stroke Patients (뇌졸중 환자의 가정간호중재 프로토콜 개발)

  • Stroke patient needs rehabilitation after receiving an acute treatment in a hospital. When stroke patient gets involved in an early discharge program, home care nurse plays a pivotal role to make them to gain a full strength and to come back to his/her prior life before he/she is sick. In spite of the importance of home care nursing intervention protocol for home care nurses to perform home care nursing autonomously, home care nursing intervention protocol for stroke patient is rarely developed. Therefore this study was conducted to develop home care nursing protocol that is applicable for stroke patients in home care nursing area. 41 home care nursing charts for stroke patients registered in home care nursing agencies from December 1st 1994 to August 31st 1999 at Y hospitals in Seoul and Won-Ju city were analyzed. 44 home care nurses who were having over three years' experience on stroke patients were participated in this study as a user validity validation group. The results of this study are as follows. 1. 28 nursing diagnoses were selected on the basis of evaluation of nursing diagnoses of stroke patients presented in a previous literature and case studies on home care nursing. 2. 17 nursing diagnoses were classified through the frequency analysis of home care nursing charts for 41 stroke patients who had received home care nursing. The order of sequence was like these: impaired skin integrity, risk for infection, nutritional deficit, impaired physical mobility, constipation, knowledge deficit, ineffective airway clearance, anxiety in family members, risk for aspiration, self care deficit, altered urinary elimination, ineffective individual coping, social isolation, risk for injury, self-esteem disturbance, impaired verbal communication, fatigue of family caregiver. 3. Based on validation on expert and user validities, 44 nursing interventions which were above ICV=.80 were chosen. 4. Nursing intervention protocols which showed above ICV=.90 were developed and were like these; pressure ulcer care, position change, preventive care for circulatory dysfunction, tube care : catheter, vital sign monitor, constipation/impaction management, artificial airway management, suction of airway secretion, environmental management : safety, and fall prevention.

  • Computational assessment of gas-dynamic characteristics is explored for a three-dimensional counter-flow thrust vector control system in a rectangular supersonic nozzle. This convergent-divergent nozzle is designed by Method of Characteristics and its design Mach number is specially set as 2.5. Performance variations of the counter-flow vector system are illustrated by varying the gap height of the secondary flow duct. Key parameters are quantitatively analyzed, such as static pressure distribution along the centerline of the upper suction collar, deflection angle, secondary mass flow ratio, and resultant thrust coefficient. Additionally, the streamline on the symmetry plane, three-dimensional iso-Mach number surface contour, and three-dimensional turbulent kinetic energy contour are presented to reveal overall flow-field characteristics in detail.

Numerical Analysis on Effects of the Boundary Layer Fence Equipped on the Hub of Rotor in the First Stage Axial Flow Gas Turbine (1단 축류 가스터빈내 동익의 허브면에 장착된 경계층 펜스의 효과에 대한 수치 해석적 연구)

  • The objective of this study is to investigate the three-dimensional turbulence flow characteristics of a rotor passage of an one-stage axial flow gas turbine and to investigate the effects of a boundary layer fence installed on the hub endwall of the rotor passage. Secondary flows occurring within the rotor passage (e.g. horseshoe vortex, passage vortex, and cross flow) cause secondary loss and reduce turbine efficiency. To control these secondary flows, a boundary layer fence measuring half the height of the thickness of the inlet boundary layer was installed on the hub endwall of the rotor passage. This study was performed numerically. The results show that the wake and secondary flows generated by the stator reduced the rotor load to constrain the development of cross flow and secondary flow reinforced by the rotor passage. In addition, the secondary vortices occurring within the rotor passage were reduced by the rotation of the rotor. Although, the boundary layer fence induced additional vortices, giving rise to an additional loss of turbine, its presence was shown to reduce the total pressure loss when compared to effects of the case without fence regardless of the relative position of blades by enervating secondary vortices occurred within the rotor passage.

Performance Simulation of Flow Control Oil Pump for Auto Transmission According to Rotating Speed (자동변속기용 유량제어 오일펌프의 회전속도 변화에 따른 성능 해석)

  • The purpose of this study is to analyze the performance of the flow control oil pump for automatic transmission. The numerical model for analysis the performance of the flow control oil pump was develop and the characteristics of the internal flow, discharge flow rate, displacement of outer ring, driving torque, generation of cavitation was investigated according to rotating speed. As a result, the cavitation generation increased as the rotating speed increased. The volumetric efficiency was 90% for 2200 rpm and it decreased rapidly, then it decreased about 81% for 5000 rpm. Besides, the cavitation generation was 20%~30% for inlet of suction part, but it reduced below 13% owing to the compression. However, it shows higher cavitation generation for high rotating speed like 5000 rpm.
