Lateral approach for maxillary sinus membrane elevation without bone materials in maxillary mucous retention cyst with immediate or delayed implant rehabilitation: case reports

  • This case series study demonstrates the possibility of successful implant rehabilitation without bone augmentation in the atrophic posterior maxilla with cystic lesion in the sinus. Sinus lift without bone graft using the lateral approach was performed. In one patient, the cyst was aspirated and simultaneous implantation under local anesthesia was performed, whereas the other cyst was removed under general anesthesia, and the sinus membrane was elevated in a second process, followed by implantation. In both cases, tapered 11.5-mm-long implants were utilized. With all of the implants, good stability and appropriate bone height were achieved. The mean bone level gain was 5.73 mm; adequate bone augmentation around the implants was shown, the sinus floor was moved apically, and the cyst was no longer radiologically detected. Completion of all of the treatments required an average of 12.5 months. The present study showed that sufficient bone formation and stable implantation in a maxilla of insufficient bone volume are possible through sinus lift without bone materials. The results serve to demonstrate, moreover, that surgical treatment of mucous retention cyst can facilitate rehabilitation. These techniques can reduce the risk of complications related to bone grafts, save money, and successfully treat antral cyst.

  • This research focuses on the case studies for operating scheme of technology venture incubation centers located at science parks, technology parks, and foreign universities. Technology venture incubation centers in foreign countries commonly vitalize local economy, create job markets, and promote new venture industry. There is a distinct difference in many ways between foreign and domestic incubation centers. Foreign countries consider the position of incubation centers at national policy level. They characterize the operation of incubation centers into technology fields. They install the networks cooperating with experts and promote the management. Tenant companies moved in incubation centers enhance the degree of completion through industry-university cooperation. Universities and research centers utilize the results of R&D to make the business, accelerate to venture establishment with technical skills, and foster technology entrepreneurs by spin-offs. Through case studies we propose implications helpful to manage domestic technology incubation centers and management methods to be successful centers.

  • On public constructions, that the first planed projects delay than the scheduled completion day occurrence frequently because gap between the scheduled construction durations and the actual construction durations. These facts connect with not only problems of the construction during as the construction delay but also brought about the distribution costs increase as the failure of facility use plan and the people discomfort weight, failure of the production and the supply using facility then tremendous loss of each part etc. For reduce these loss, it must improve accuracy of the scheduled construction duration estimating when it order. This study try contribute to a successful manage of public project and reduce the construction duration extension frequent of the construction during, and then ensure the suitable construction duration by present the estimation model of the scheduled construction duration that include the construction duration correct element using as an index on the scheduled construction duration estimation.

  • Beginning January 2005, the BTL (Build-Transfer-Lease) project was initiated by government to provide high level of public services for the upgraded quality of life. The most important process of the BTL project is operation, because the company have to operate the building about $20{\sim}30$ years after completion. But a lot of problems have been founded so far. For example, no counterplan for the risk in operation, no total control system and the vagueness in project guideline and so on. These problems will cause serious demage to the BTL project. Hence, this paper analyzes the above issues and conducts the interviews with the professionals from the diverse sectors in order to propose the improvement and deliver the successful operation in BTL project.

  • As the construction industry gets specialized and complicated gradually, the number of participating organizations increased, and also the necessity of construction management(CM) is getting bigger because construction scale is becoming bigger. However, in spite of the increase in necessity, its activation through performance measurement and evaluation is very insufficient compared with advanced countries. Therefore, a checklist for CM by construction stage using AHP method in the current study is for efficient CM for successful completion of a project by dividing CM affairs by construction stage and checking performance degree by stage. If the CM checklist by construction stage, the final result of the study, is used by all the parties concerned such as construction managers, orders, designers, constructor, etc., it will be possibile to improve work efficiency of project management and produce a final outcome corresponding to the goals of a project.

Evaluation of the Microvascular Research Center Training Program for Assessing Microsurgical Skills in Trainee Surgeons

  • Background We established the Microvascular Research Center Training Program (MRCP) to help trainee surgeons acquire and develop microsurgical skills. Medical students were recruited to undergo the MRCP to assess the effectiveness of the MRCP for trainee surgeons. Methods Twenty-two medical students with no prior microsurgical experience, who completed the course from 2005 to 2012, were included. The MRCP comprises 5 stages of training, each with specific passing requirements. Stages 1 and 2 involve anastomosing silicone tubes and blood vessels of chicken carcasses, respectively, within 20 minutes. Stage 3 involves anastomosing the femoral artery and vein of live rats with a 1-day patency rate of >80%. Stage 4 requires replantation of free superficial inferior epigastric artery flaps in rats with a 7-day success rate of >80%. Stage 5 involves successful completion of one case of rat replantation/transplantation. We calculated the passing rate for each stage and recorded the number of anastomoses required to pass stages 3 and 4. Results The passing rates were 100% (22/22) for stages 1 and 2, 86.4% (19/22) for stage 3, 59.1% (13/22) for stage 4, and 55.0% (11/20) for stage 5. The number of anastomoses performed was $17.2{\pm}12.2$ in stage 3 and $11.3{\pm}8.1$ in stage 4. Conclusions Majority of the medical students who undertook the MRCP acquired basic microsurgical skills. Thus, we conclude that the MRCP is an effective microsurgery training program for trainee surgeons.

Continuous Axillary Branchial Plexus Block -I. Modification of catheter insertion method- (지속적 액와부 상완신경총 차단술 -I. 카테테르 삽입방법의 변형-)

  • Background: Authors modified the traditional continuous axillary brachial plexus block technique of Selander for purpose of increasing success rate and decreasing complications by use of commercial epidural anesthesia set. Method: Thirty-nine patients scheduled for upper extremity operations were injected with 40 ml of anesthetic solution by axillary perivascular technique, using 23~25G immobile needle at 2 cm from the pectoralis major. Tuohy needle was immediately introduced at 4 cm from the pectoralis major and pierced the expanded neurovascular sheath at an angle of 30 degree to the skin. The "pop" was well noted well. Needle was advanced 0.5 to 3.0 cm and epidural catheter introduced through the needle. After removal of needle, occlusive dressing was done. Tip of catheter and spread of solution were demonstrated by fluoroscopy with contrast dye after completion of procedure. Result: Catheter insertion was successful at first attempt for all case. Total length of insertion was from 6 to 13($10.0{\pm}1.7$) cm. Tip of catheter was placed in infraclavicular space(66.7%), about the humeral head(17.9%) and in upper arm in 3 cases as U-shape(7.9%). Catheters were maintained for $6.7{\pm}2.6$(3-12) days. There were no complications such as: perforation of major vessels, needle trauma to nerve, infection, bleeding or hematoma. Conclusion: This study demonstrated continuous axillary brachial plexus block with epidural anesthesia set is safe, easy and convenient modification of technique of Selander.

  • In recent years, construction information management systems using Information Technology(IT) have been developed in the construction industry. Construction information management plays a significant role in successful project implementation and completion. However, there have been a number of problems in current practice that manually collects and utilizes construction information generated in site. This paper mainly describes a new approach which is able to gather, manage, and accumulate the construction information in real-time using IT tools such as PDA, barcode, RFID, biometrics identification and digital camera. It first reviews the existing applications and their limitations, and potential IT tools for construction data collection and management. It then proposes a utilization method of IT tools for an effective information management in domestic construction site. Conclusions and future research works are also made in this paper,

Perceptions held by Investigators, IRB Members and IRB Administrators on the Bioethical Oversight System of National R&D Projects (국가연구개발과제 생명의학윤리 감독체계에 대한 인식조사 및 제언)

  • Purpose: Aim of this study was to gather and evaluate perceptions of investigators, IRB members, and IRB administrators on the appropriateness of IRB review process and bioethical oversight system of national R&D (NR&D) projects. Method: Investigators, IRB members, and IRB administrators at 17 different institutions were surveyed using convenience sampling and survey questionnaires were partially group-specialized to consider any differences between the groups. Results: Participants included 29 investigators, 37 IRB members, and 17 administrators with response rate of 100% (83 of 83). According to the responses obtained, insufficient preparation time for constructing protocol and gaining IRB approval was one of the main problems in the IRB review process (investigator 79.3%, IRB administrator 88.2%). Also, discrepancy between NR&D and IRB's protocol formats was another major issue (IRB members 96.4%, IRB administrator 100%) and most investigators (89.7%) had to modify the original NR&D protocol to obtain IRB approval. Moreover, it was reported that 13.8% of investigators and 31.3% of IRB administrators did not submit midyear reports to IRB and for bioethical issues of NR&D projects, 17.2% of investigators did not include information on project status and safety issues in the annual reports. Conclusion: In conclusion, for successful and ethical completion of R&D projects, revision of both IRB review process and NR&D project protocol formats as well as implementation of appropriate bioethical oversights are necessary.

  • Since the year of 2003, the overseas plant market has grown by an average of 6% per year. It is because of the increase of oil-money, economic growth of BRICs, and the result of heavy competition among the countries which demands a great amount of resources for the purpose of resource securing. Such a situation is expected to be continued for a while. The procurement is one of critical functions for successful completion of a plant project. For effective procurement, it is required to manage vendor's information efficiently. But most of the vendor management systems deal with simple general information and performance data about vendors. In this sense, this paper proposes a vendor management system which can be used within a decision-making system. The system uses existing DB and makes the close relationship with the vendors. This paper will also reduce risks and difficulties in the process of selecting the proper vendors and will increase efficiency of procurement process in electronic bidding between companies.
