측모 사진상에서 Subnasale의 이상적인 위치에 관한 주관적 평가 (Subjective Evaluation about Ideal Position of the Subnasale on Lateral Photos)

  • 측모의 연조직 분석 시 Subnasale는 중요한 기준점으로 사용되지만 이상적인 위치에 대한 평가한 연구는 드물다. 본 연구의 목적은 한국인 측모에서 Subnasale의 위치 변화에 따른 심미적 안모에 대한 객관적 기준을 제시 하는 것이며, 추가적으로 전문가 집단과 일반 대중 간의 안모 선호도 인식에 어떤 차이가 존재하는지 평가 하는 것이다. 이번 연구를 위해 채득한 20대 여성 사진 중에서 전문가에 의해 결정된 적절한 측면 안모 비율로 판단되는 1명의 측모 사진을 선정하였다. 선정된 사진은 비순각을 유지하며 진성 수직선에 수직한 평면상에서 Subnasale을 전, 후방으로 변화시켰다. 합성된 사진은 전문가 집단(교정의사 9명)과 일반 대중(126명)에게 제시되어 각각의 선호도를 VAS상에 표시하도록 하였다. 다음의 결론을 얻었다. 1. Subnasale의 이상적인 위치는 Lateral canthus~Subnasale : Subnasale~Pronasale가 1.769 : 1을 이룰 때 이다. 2. 각도상 Nasion을 통과하는 진성 수직선과 Subnasale가 $5.5^{\circ}$를 이룰 때 이다. 3. 전문가 집단은 모든 비율 변화를 인지하였으나, 일반 대중은 1.571 : 1과 1.769 : 1 사이의 변화를 인지하지 못하였다.

한국인의 안면고경에 관한 계측학적 연구 (A Statistical Study on the Vertical Dimension in Korean)

  • The author measured the vertical dimension and the distances from Trichion to Nasion of 825 Korean adults (414 of male, 411 of female) above 21 years of age by Willis gauge. The following results were obtained. 1) The distances from subnasale to the bottom of chin were 71.4mm in male and 68.7mm in female, and those from Trichion to Nasion were 70.4mm in male and 68.0mm in female. The former were longer than the latter, and the distances in male were longer than those in female. 2) The differences between the distances from Subnasale to the bottom of chin were 1.0mm in male and 0.7mm in female. The frequencies of percentages, in which differences between the distances mentioned above were 0±1mm, were 43.0% in male, 42.6% in female, and 42.8% in total. 3) With the matter of age and sex differences, the distances from Subnasale to the bottom of chin and those from Trichion to Nasion were increasing to 31-40 age group by age, but those had a tendency of decreasing in the late age in both sexes.

Short-term impact of microimplant-assisted rapid palatal expansion on the nasal soft tissues in adults: A three-dimensional stereophotogrammetry study

  • Objective: The aim of this study was to evaluate changes in the nasal soft tissues, including movements of landmarks, changes in linear distances, and volumetric changes, using three-dimensional (3D) stereophotogrammetry after microimplant-assisted rapid palatal expansion (MARPE) in adult patients. Methods: Facial data were scanned using a white light scanner before and after MARPE in 30 patients. In total, 7 mm of expansion was achieved over a 4-week expansion period. We determined 10 soft tissue landmarks using reverse engineering software and measured 3D vector changes at those points. In addition, we calculated the distances between points to determine changes in the width of the nasal soft tissues. The volumetric change in the nose was also measured. Results: All landmarks except pronasale and subnasale showed statistically significant movement on the x-axis. Pronasale, subnasale, alar right, and alar left showed significant movement on the y-axis, while all landmarks except subnasale showed significant movement on the z-axis. The alar base width, alar width, and alar curvature width increased by 1.214, 0.932, and 0.987 mm, respectively. The average volumetric change was 993.33 ㎣, and the amount of increase relative to the average initial volume was 2.96%. Conclusions: The majority of soft tissue landmarks around the nasal region show significant positional changes after MARPE in adults. The nose tends to widen and move forward and downward. The post-treatment nasal volume may also exhibit a significant increase relative to the initial volume. Clinicians should thoroughly explain the anticipated changes to patients before MARPE initiation.

($14{\sim}15$세 한국인 안모의 실물사진계측분석에 의한 연구 (A PHOTOMETRIC STUDY OF MAXILLOFACIAL NORMALS IN $14{\sim}15$ YEAR-OLD KOREAN.)

  • The primary method of diagnosis in craniofacial deformity patient is used by gross inspection. The study for objective analysis and evaluation in Korean maxillofacial soft tissue portions is rare and our normals have been based on Caucasian such as American. But it is difficult to apply it to ours so we studied the frontal photometric measurements for maxillofacial Korean normals. The photograph were taken with the subject sitting in head position parallel with interpupillary plane to the floor. During taking pictures, facial analyser by Obwegeser in Martin was used for correct measurements. 54 male and 57 female ages 14 to 15 years were selected from a group of 111 healthy middle school students in Inchon. 1. The vertical measurements in glabella-subnasale, subnasale-stomion and stomion-menton in the male were higher than in the female subjects in $14{\sim}15$ years old Korean, so middle and lower third face were higher than those of female(p<0.05). 2. The horizontal measurements of female was higher than those of male in the rt. and It. eye width but smaller in nose width and lip width(p<0.05). 3. There was no significant difference in gender compared with soft cheek and mention angle measurements When our reports were compared with american white population, the vertical proportions in Korean normals were similar to american and the line measurements in middle facial length, nose width and intercanthal distance were higher than those of american.

의약외품 마스크의 표준 치수 규격 개발 (Designing of standard sizing system for hygienic masks)

  • This study aims to design a new sizing system for hygienic masks to protect against COVID-19-related respiratory disorders. The product sizes were collected from 70 commercially available hygienic masks, and 18 head measurements were obtained from the three-dimensional (3D) scan data of 2,048 men and women aged 20 to 69 years from the 6th Size Korea Survey. The statistical analysis was performed using the IBM Statistical Package for the Social Sciences (SPSS) 28.0 program. The "bitragion-subnasale arc" and "menton-sellion length" were chosen as the key body dimensions for the hygienic mask sizing system. The linear regressions with 18 3D head measurements were analyzed, and it was determined that the key body dimensions were useful for statistically predicting other 3D head measurements related to hygienic masks. A new sizing system was proposed for two types of masks, "masks with a tight fit" and "masks with a loose fit," taking into account the existing Korean Industrial Standards (KS) and the shortcomings of the sizes of hygienic masks on the market. The sizing system for tight-fitting masks consisted of the key body dimensions, with their sizes indicated by a pair of numbers. The sizing system for loose-fitting masks consisted solely of the bitragion-subnasale arc, with their sizes denoted by letters such as S, M, and L (denoting small, medium, and large, respectively). Future studies should consider this mask sizing system for different age groups, such as children and adolescents.

상악후퇴증 및 하악전돌증의 악교정수술예 (A Case Report of Maxillary Retrusion and Mandibular Protrusion Corrected by Simultaneous Maxillary and Mandibular Osteotomies)

  • This is a case report of orthognathic surgery for the correction of maxillary retrusion and mandibular protrusion. The summary and results are as follows, 1. The maxillary retrusion was corrected by LeFort I osteotomy. 2. The mandibular protrusion was corrected by sagittal split osteotomies in the rami. 3. And, for the correction of the discrepancy between max8llary and mandibular arches, the mandibular arch was widened by the midsymphyseal step osteotomy. 4. The ratios of horizontal changes of soft tissue to hard tissue at the points, Subnasale (Sn), Labrale superius (Ls), Labrale inferius (L9), and Supramentale (B) were 67.6%, 43.2$, 70.2% and 87.7%, respectively.

편측성 구순열 환아의 안모 계측 연구 (Anthropometric Analysis of Unilateral Cleft Lip Patient)

  • Purpose: Cleft lip and palate is one of the most frequent hereditary deformities of the maxillofacial region which can arise in facial and jaw abnormalities as well as malocclusion and speech problems. In particular, unilateral cleft lip and palate is characterized by midface deformity resulting in maxillary anterior nasal septal deviation and nasal deformity. The aim of this study is to analyze the facial deformity of untreated unilateral cleft lip patients for contribution to primary cheiloplasty. Methods: Thirty-three patients with unilateral cleft lip and palate were impressioned before operation and facial casts were made. The casts were classified into complete cleft lip and incomplete cleft lip groups and each group were classified into affected side and normal side. Anthropometric reference points and lines were setted up and analysis between points and lines were made. Results and Conclusion: The obtained results were as follows: 1. The intercanthal width had no significant difference between the incomplete and complete cleft lip groups. 2. Cleft width and alar base width were greater in the complete group, and nasal tip protrusion was greater in the incomplete group. 3. Involved alar width and nostril width were greater in the complete group and in both complete and incomplete groups, involved alar width and nostril width were greater than the non-involved side. 4. The lateral deviation of the subnasale was greater in the complete group in both involved and non-involved sides. 5. The nasal laterale was placed inferiorly in both cleft groups. 6. The subnasale was deviated to the non-involved side in both cleft groups. 7. The nose tip was deviated to the non-involved side in both cleft groups and had greater lateral deviation in the complete cleft group. 8. The midpoint of cupid's bow had no vertical difference between complete and incomplete groups, but had a greater lateral deviation in the complete group. 9. In the complete cleft group, correlation between differences in cleft width and nostril width and columella height difference were obtained.

Three-dimensional assessment of nasal changes after maxillary advancement with impaction using stereophotogrammetry

  • Objective: To evaluate the changes in the nose in three dimensions after Le Fort I osteotomy in patients with skeletal Class III malocclusion. Methods: The subjects were 40 adult patients (20 females and 20 males; mean age, 20.3 ± 3.0 years; range, 17.0 to 31.1 years) who underwent one-piece Le Fort I osteotomy with maxillary advancement and impaction treatment for maxillary hypoplasia. The mean maxillary advancement was 4.56 ± 1.34 mm, and the mean maxillary impaction was 2.03 ± 1.04 mm. Stereophotogrammetry was used to acquire three-dimensional images before and at least 6 months after surgery. Results: Alare (Al) and alare curvature (Ac) points had moved vertically and anterolaterally postoperatively. A significant increase was observed in the nasal ala width and alar base width, and no changes were noted in the columellar length, nasolabial angle, and nasal area. There was a significant relationship between maxillary impaction and nasal ala width and horizontal and sagittal positions of the bilateral Al and Ac. The only relationship found was between maxillary advancement and postoperative sagittal location of the subnasale and pronasale. Conclusions: Nasal soft tissues were highly affected by the vertical movement of the maxilla; however, the soft tissue responses were individual-dependent.

한국 성인 여성의 3차원 두형개발을 위한 머리치수 연구 (An Anthropometric Study on the Korean Female Adults Heads for the Development of 3D Craniofacial Shape)

  • The purpose of this study was to analyse craniofacial shape by 3D scanner for female adults with age groups. In this study, heads of 452 female adults were measured by 3D scanner. The obtained 25 measurements were analyzed by statistical methods. The results can be summarized as follows; 1. From the basic statistical data analysis, vertex-tragion and the length between the pupils were the longest in their twenties, and grew shorter in elderly groups. The length of nasion-subnasale and the width of mouth increased with an increased in age. 2. According to the analysis of the craniofacial proportions, the head type of female aduls was short-headed. The size of lower face increased with an increase in age. 3. The statistically noticeable differences were found in the measurement of the left and the right sides of face in the age groups of 20, 30, and 40. 4. High correlations were found in two perpendicular lengths, two horizontal lengths and two widths. 5. The order of factor analysis was as follows; the horizontal length, the perpendicular length and the width from highest.

Evaluation of the facial dimensions of young adult women with a preferred facial appearance

  • Objective: The aim of this study was to evaluate the facial dimensions of young adult women with a preferred facial appearance and compare the results with those from the general population. Methods: Twenty-five linear, nine angular, and three area measurements were made and four ratios were calculated using a sample of standardized frontal and lateral photographs of 46 young adult women with a preferred facial appearance (Miss Korea group) and 44 young adult women from the general population (control group). Differences between the two groups were analyzed using multivariate analysis of variance (MANOVA). Results: Compared with the control group, the Miss Korea group exhibited a significantly greater facial height, total facial height (TFH; trichion-menton), facial width (tragus right-tragus left), facial depth (tragus-true vertical line), and trichion-nasion/TFH ratio and smaller subnasale-menton/TFH and facial width/TFH ratios. Furthermore, the control group had smaller intercanthal and interpupillary widths. Conclusions: The Miss Korea group exhibited longer, wider, and deeper faces compared with those from the general population. Furthermore, the Miss Korea group had larger eyes, longer but less protruded noses, longer and more retruded lower lips and chins, larger lip vermilion areas, and smaller labiomental angles. These results suggest that the latest trends in facial esthetics should be considered during diagnosis and treatment planning for young women with dentofacial abnormalities.
