A Case of MALT Lymphoma in Parotid Gland Duct (이하선관에 발생한 MALT 림프종 1예)

  • Purpose: Primary malignant lymphomas of the salivary glands are uncommon. The parotid gland was most frequently involved, followed by the submandibular gland, minor salivary gland and sublingual gland. The most common subtype is mucosa - associated lymphoid tissue(MALT) lymphoma. We experienced a case of salivary MALT lymphoma involving parotid gland duct, so report a case with a review of the literature. Methods: A 65 year old female presented with a palpable mass on the left side of her cheek. There was no clinical or laboratory evidence of pre - existing autoimmune disease. Preoperative facial and neck CT with contrast showed $2.1{\times}1.7cm$ sized, ill defined, homogeneous low density mass near left masseter muscle, and no evidence of other enlarged lymph nodes. Results: At operation, a yellowish oval shaped mass was found slightly adhered to middle portion of the parotid gland duct, meaduring $2{\times}1.5{\times}0.7cm$. Microscopic finding showed that centrocyte - like cells, monocyte B cells and plasma cells were diffusely infiltrated. Immunophenotyping was preformed on fixed section. The majority of the small cells were immunoreactive for the B cell marker CD20. Based on the typical histological findings supported by immunostaining, the mass was defined as MALT lymphoma. Conclusion: We report that very rare case of MALT lymphoma involving parotid gland duct in 65 year old female patient was experienced with clinical characteristics, histologic features and references.

Effect of irradiation on the acinar cells of submandibular gland in streptozotocin-induced diabetic rats (방사선조사가 당뇨 백서의 악하선 선포세포에 미치는 영향)

  • Purpose: To observe the histologic changes and clusterin expression in the acinar cells of the submandibular gland in streptozotocin-induced diabetic rat following irradiation. Materials and Methods: Mature Sprague-Dawley rats were divided into three groups: control, diabetic, and diabetic-irradiated groups. Diabetes mellitus was induced in the Sprague-Dawley rats by injecting streptozotocin, while the control rats were injected with citrate buffer only. After 5 days, rats in diabetic-irradiated group were irradiated with single absorbed dose of 10 Gy to the head and neck region. The rats were killed at 1, 3, 7, 14,21, and 28 days after irradiation. The specimen including the submandibular gland were sectioned and observed using histologic and immunohistochemical methods. Results : Morphologic change of acinar cells was remarkable in the diabetic group, but was not observed in the diabetic-irradiated group. Necrotic tissues were observed in the diabetic-irradiated group. Coloring of toluidine blue stain was most increased at 14 days in the diabetic group, however there were no significant change throughout the period of the experiment in the diabetic-irradiated group. Expression of clusterin was most significant at 14 days in the diabetic group, but gradually decreased with time after 7 days in the diabetic-irradiated group. Degeneration of clusterin was observed in the diabetic-irradiated group. Conclusion : This experiment suggests that the acinar cells of submandibular gland in rats are physiologically apoptosed by the induction of diabetes, but that the apoptosis is inhibited and the acinar cells necrotized after irradiation.

Sialolithiasis Mimicking Metastatic Thyroid Cancer (갑상선암 전이병소로 오인된 타석증 증례)

  • We report a false positive case of I-131 scan due to a sialolithiasis in right submandibular gland. I-131 images showed not only remnant thyroid tissues and functional metastases in the anterior neck but also a hot uptake near right submandibular gland. We confirmed the sialolithiasis by radiologic studies. Sialolithiasis should be regarded as a false positive cause when I-131 scan shows a hot spot near salivary bed.

Malignant Mixed Tumor of Salivary Glands: A Clinical Study (악성 혼합종의 임상적 고찰)

  • For malignant neoplasms of salivary tissues. two of the better determinants of progosis are histologic classification and size of the neoplasm. Proper management of these tumors requires an accurate diagnosis by the pathologist and correct interpretation by the surgeon. Malignant mixed tumors account for between 3 and 13 precent of all cancers of the salivary glands and 2 percent of all tumors in these locations. The typical history of these tumors is that of slowly growing mass demonstrating a sudden increase in growth. The duration of onset of the tumor mass and the diagnosis of malignancy has been demonstrated to be 10 to 18 years. The risk of malignat transformation of a benign mixed tumor increases with the duration of the tumor. We analyzed retrospectively 13 cases of malignant mixed tumor who visited from Jan. 1985 to Dec. 1992. Mean age of the patients was 56.5 years. The origin of tumors were parotid gland 7 cases, submandibular gland 2 cases, and minor salivary gland 4 cases(palate 3 cases, tonsil pillar 1 case). According to the criteria of the AJCC on staging, stage I was 1 case, stage II 1 case, stage III 2 cases, and stage IV 9 cases. Histopathologically, carcinma ex pleomorphic adenoma were 12 cases and the true malignant mixed tumor was 1 case. The major treatment modalities were curative surgery, and radiation therapy followed. In conclusion, aggressive therapy of combined surgery and postoperative radiation therapy is required for these lesions, and patients with known or suspected benign tumor should be encouraged to undergo surgery early on in their disease to avoid malignant degeneration at a later dete.

Clinical Study of Adenoid Cystic Carcinoma of the Salivary Glands (타액선에 생긴 선낭암의 임상적 고찰)

  • After analysis according to age, sex, site of origin, nodal involvement, perineural invasion. histopathology and treatment modality. authors had reached the following conclusion about the 28 cases of adenoid cystic carcinoma of the salivary gland, which were, histopathologically, diagnosed at Yonsei University Severance Hospital and Yongdong, Severance Hospital during the 8-year period from Apr. 1983 to Mar. 1991. Among the 28 cases, 13 were females and 15 were males, which implies that there isn't any sigificant distinction of gender. The age range was 24 years to 69 years with a mean age of 49 years, and the most common site of origin in order was 6 cases(21.4%) of maxillary sinus, 5 cases(17.8%) of submandibular gland, each 4 cases(14.4%) of parotid gland, palate, 3 cases(10.4%) of nasal cavity and each 2 cases(7.1%) of mouth floor, nasopharynx with each one case(3.6%) of buccal mucosa and retromolar trigone. With clinical staging by UICC, the 5-year survival of adenoid cystic carcinoma was 100% in stage I and II, 87.5% in stage III. In stage IV, all of the patient were died within 3 years. We have known that the prognosis was poorer in following cases; the cases with higher clinical stage, primary site other than the major salivary gland especially maxillary sinus and positive neural or nodal invasion.

Thunderbeat versus Harmonic scalpel in surgery of benign disease in salivary gland (양성침샘질환 수술에서 ThunderbeatTM와 Harmonic scalpel®의 유용성에 대한 비교연구)

  • Background and Objectives: Thunderbeat (TB) and Harmonic scalpel (HS) have been applied to variable head and neck surgery, which are useful for both coagulation and cutting of tissues. However, there have been no comparative studies covering the usefulness of these energy devices in surgery for major salivary gland. In this study, we analyzed the surgical outcomes of two devices in surgery for parotidectomy and submandibular gland (SMG) resection. Materials and Methods: A retrospective chart review of 90 patients including two groups (HS group, n=45 versus TB group, n=45) of patients matched for their patient factors was conducted. Clinicopathologic factors of the patients and surgical outcomes such as the operation time, intraoperative bleeding, amount and duration of drain, hospital stay and complications were compared between two groups. Results: There were no significant difference between two groups regarding the clinicopathologic factors and short-term surgical outcomes. Conclusion: Thunderbeat and HS are both effective and safe for parotidectomy and SMG resection in variable benign disease of major salivary gland.

Quantitative Assessment of Salivary Gland washout in Clinically Healthy Dogs

  • The aims of this study were to obtain the normal ranges of enhancement parameters for salivary gland in dynamic CT and to investigate the effects of fasting time on contrast enhancement in clinically normal beagle dogs. With five healthy beagle dogs, dynamic CT examination was performed according to fasting times (as fasting times, 12hours, 0 min, 20 min, 40 min, 1 hours, 6 hours, 24 hours). In normal beagles with 12hours fasting, enhancement parameters through the preliminary study were as follows: ImaxA - 472 .49 ± 19.01 HU; ImaxS - 138.95 ± 6.2 5 HU; TmaxA - 25.8 ± 1.79 sec; TmaxS - 69.0 ± 23.11 sec; Teq - 80.5 ± 6.61 sec; T-Aeq - 54.5 ± 5.51 sec (Imax - peak enhancement; Tmax - time to peak enhancement; Teq - time to equilibrium phase; T-Aeq - time between peak enhancement in the common carotid artery and onset of the equilibrium phase; A - common carotid; S - submandibular gland; HU - Hounsfield unit). Additionally, ImaxA and ImaxS were significantly increased in 40 min after eating. Because these results associated with postprandial hemodynamic changes can make the diagnosis of salivary gland diseases more difficult, sufficient fasting time is important for accurate diagnosis.

  • Primary squamous cell carcinoma occuring in the salivary glands involves a grave prognosis since the tumor exhibit infiltrative properties, metastasizes early and recurs readily. But it is so rare that the clinician may see only a few in a lifetime of practice. The following report describes a case of primary squamous cell carcinoma arising from the submaxillary gland treated by combined therapy including surgery, radiotherapy and hyperthermia. The biologic behavior of the tumor was quite abnormal that immediate recurrence was noticed in two weeks after surgery. The progress was dismal and the disease became overwhelming before long.

  • Sialolithiasis is the most commom disease of salivary gland. The main symptoms are pain and swelling of the involved gland during eating. It can occur at any age but patients in their third to fifth decade present most cases. Males are more frequently affected than females. Most sialoliths are located within the duct system of the submandibular gland. Submandibular sialoliths close to the hilum of the gland tend to become large and ovoid shape, whereas sialoliths in the duct tend to be elongated. Commonly, sialoliths measure from 1 mm to less than 10 mm, and larger than 15 mm are considered rare. In one case we have removed a giant sialolith which was located in a wharton's duct and in the other case we have removed multiple sialolith including a giant sialolith which were also located in a Wharton's duct. We report these 2 cases with literature reviews.

Culture of Rat Salivary Acinar Cells (백서 타액선 선포 세포의 배양)

  • We investigated the culture condition and effects of various growth factors on the culture of salivary gland acinar cells. Male, Sprague-Dawley rats (about 6 weeks old) were sacrificed and their submandibular, sublingual, and parotid glands were used as specimens. High oxygen level more than 90% and coating of Matrigel on culture dish were important factors to help increase the survival time of acinar cells, Proper concentration of enzymes such as collagenase and hyaluronidase during isolation steps was also important. Addition of various growth factors such as dexamethasone, insulin, transferrin, selenous acid, reduced glutathione, epidermal growth factor, isoproterenol, and putrescine in culture medium helped to increase lifetime of cultured salivary acinar cells.
