Background: The aim of the present systematic review was to evaluate and compare the efficacy of warmed and unwarmed local anesthesia solutions in reduction of pain during intraoral injection administration. Methods: PubMed, Ovid SP, and Cochrane Central Register of Controlled Trials were searched from publication years 1990 to 2020 with relevant MeSH terms. Studies were screened by titles and abstracts, followed by full-texts evaluation of the included studies. Results: A total of four studies were included in the systematic review. Outcomes evaluated were subjective and objective pain during administration of the warmed local anesthesia solution in comparison with the unwarmed local anesthesia solution. Among the four studies that evaluated the self-reported pain score, three studies showed significantly lower pain scores associated with warmed local anesthesia. Only two studies evaluated the observed pain score, and both of them reported a significantly lower pain reaction with the warmed local anesthesia solution. Conclusion: Within the limits of this systematic review, warming the local anesthesia solution to body temperature (37℃) before administration seemed to reduce the discomfort during intraoral local anaesthesia administration, and more high-quality studies should be carried out to validate the same.

Objective: The purpose of this study is to examine about dental hygienists' myofascial pain syndrome, lower back pain, carpal tunnel syndrome (CTS), medial and lateral epicondylitis, hand-arm vibration syndrom and work-related musculoskeletal syndrome (WMSD) experience and hygienists' posture, motion. Methods: The self-administered questionnaire was surveyed from June 1 to September 30 of 2018 targeting 280 dental hygienists in Gyeongnam province and 266 dental hygienists' answers were analyzed. Results: The average daily working hours of a dental hygienist was more than eight hours 59.0%, with an average of 33 patients per day. The average number of patients who receive treatment for more than 30 minutes is 15. The angle of motion of the subjective evaluation was above 60%. Medical position and form of movement were more than 50% above the standard level. Symptoms of posture and motion that cause WMSD were hand-arm vibration syndrome 68.1%, myofascial pain syndrome 58.6%, lower back pain 51.1%, CTS 50.4% in order. Experience WMSD related symptoms which dental hygienists experience were myofascial pain syndrome 92.9%, CTS 57.9%, lower back pain 56.4%, medial and lateral epicondylitis 37.2%, hand-arm vibration syndrome 24.4%. Conclusion: The above results showed the posture and motion of dental hygienists and were found that the experience rate which dental hygienists experience WMSD of myofascial pain syndrome, Lower back pain, and CTS was significantly more than 56%.
This study was designed to evaluate the sex differences in the pain control effect by morphine injection to masticatory muscle pain patients. Patients with masticatory muscle pain visited the Department of Oral Medicine, Kyung Hee University Dental Hospital were recruited to this study and diagnosed by RDC/TMD. Experimental group were divided into four group each from male (n=20) and female (n=20); saline injection group (n=5), lidocaine injection group (n=5), morphine 1.5 mg injection group (n=5) and morphine 3 mg injection group (n=5). Evaluation list was the subjective pain evaluation(visual analogue scale, Mc Gill pain questionnaire, pain drawing) and the objective pain evaluation(pressure pain threshold, pressure pain tolerance) and evaluation time was injection before, after 1 hour, 24 hour, 48 hour and then it was analyzed statistically. The results were as follows : 1. The male and female were significantly different statistically morphine 3 mg group in visual analogue scale evaluation. (male: p<0.05, female: p<0.05) 2. The male and female were more significantly different statistically morphine 3 mg group than morphine 1.5 mg group in McGill pain questionnaire evaluation. (male: p<0.001, female: p<0.01) 3. The male were significantly different statistically morphine 3 mg group in pain drawing evaluation and pressure pain threshold evaluation. (PD: p<0.001, PPT: p<0.05) Therefore, it was revealed that the morphine 3 mg injection for masticatory muscle pain was effective to pain control male patients and more effect than female patients in the objective pain evaluation.
Objectives The quality of life in cancer patients is important. The aim of this study is to report a case which showed symptoms improvement after treatment with modified Galgeunhaegi-tang. Methods We retrospectively reviewed the medical records. The patient's body weight and the circumference of lower extremity were measured and the subjective symptoms such as chest pain were evaluated by the NRS(Numeric Rating Scale). And the frequency of defecation was checked daily. Results The patient had received chemotherapy from March to November 2014 for 8th times. During the chemotherapy, the patient developed symptoms such as weight gain, edema of lower extremity, chest pain, headache, low back pain, and constipation. But additional evaluation and management with western medicine couldn't solve the problem. So he started to take Korean medicine. And he was transferred to our cancer center to keep continuing the treatment. After the treatment, the body weight, the lower extremity circumference and the subjective severity of chest pain, headache, low back pain were all decreased. During hospitalization at our center, the patient didn't suffer from constipation. Conclusions An elderly non-small cell lung cancer patient showed the improvement of symptoms that were continued during chemotherapy. As we can see in this case, Korean medicine can be a solution of the symptoms induced by chemotherapy, if there is no symptom improvement with western medicine.
Discomfort as well as muscular pain and musculoskeletal diseases occur in cases of stressed posture due to prolonged pressure. Therefore, the American Academy of Orthopaedic Surgeons (AAOS) and Pediatric Orthopaedic Society of North America (POSNA) recommend that bags that weigh no more than 15 - 20% of the wearer's weight. However, despite the prolonged pressure from using baby carriers, there are no recommendations and limited studies on how to protect wearers. Therefore, this study investigates subjective fatigue according to the usage of waist support and hip seat. Based on this, muscle fatigue and secondary subjective fatigue according to three types of currently commercial baby carriers (X-type, H-type, and H-hip type) were measured. Subjective comfortability was evaluated on a 5 point Likert-scale and subjective fatigue was evaluated on Borg's CR-10 scale. Objective muscle fatigue was also compared by measuring and analyzing electromyogram (EMG). The results of primary subjective fatigue showed statistically significant changes in the shoulders, waist, calves, and soles of the feet according to the usage of hip seats and waist support. Electromyogram measurements also showed less muscle fatigue at the upper trapezius muscle and thoracic erector spine muscle for the H-hip seat baby carrier, which has both waist support and hip seat, compared to an X-type baby carrier, which has neither. However, results of subjective fatigue showed opposite results at the waist despite having same results at the shoulders and beneath the shoulders. This show discrepancies between objective muscle fatigue and subjective fatigue; therefore, that both aspects must be taken into consideration when developing ergonomic baby carriers.
Objective: The purpose of this study was to analyze the risk factors of the musculoskeletal workload of mid-old-age female caregivers by identifying work postures and subjective symptoms. Method: This study was carried out on a total of 206 subjects 206 mid-old-age caregivers(45~65 years old) from November 2009 to April 2010: 206 mid-old-age caregivers (45~65 years old) at the long-term hospital. A questionnaires survey on general factors and subjective symptoms of subjects was conducted. The risk factors of the musculoskeletal workload evaluation method is based on OWAS, RULA, REBA according to occupation properties. Results: The analysis of the rates of the subjective symptoms showed that 78.6% subjects experience low back pain. The analysis of the work type showed that transfer is the highest rate(36.3%) and position change is the second(18.2%). The analysis of the work postures showed that the thrusted back with twisting position over $20^{\circ}$ is highest rate(37.4%), the bended back forward with twisting position is the second(27.5%). Conclusion: This study suggested that the need of preventive education for caregivers and workload improve. It is hoped that subsequent study on the difference of subjective symptoms between educated caregivers and non educated caregivers will be conducted.
Dural ectasia is defined as ballooning or expansion of the dural sac surrounding the spinal cord. This report describes a rare case of low back pain and sciatica, suspected as being dural ectasia. The patient was hospitalized for 45 days, and underwent integrative Korean medical treatment, including pharmacopuncture, acupuncture, herbal medicine, Chuna therapy, cupping therapy, and physiotherapy. The effect of the treatment was evaluated using the numerical rating scale, Oswestry disability index, European quality of life 5 dimensions, and subjective symptoms. After inpatient treatment, the pain the patient experienced was significantly reduced and the evaluation indices improved. This case report suggested that integrative Korean medical treatment could be an effective therapeutic choice for low back pain and sciatica, with dural ectasia. Further clinical studies are needed to support this observation.

The aim of this study was to evaluate the efficacy of a topical 0.2% hyaluronic acid (HA) preparation in the management of wound after removal of arch bar for facial bone fracture and a suture site after orthognatic, oral cancer or oral surgery. Forty patients participated in a randomized, placebo controlled, double-blind trial to evaluate the efficacy of the topical HA and preparation. HA topically applied to the wound after removal of arch bar or stitch out, 3 times a day for 4 weeks. Evaluation is performed once a week for 4 weeks. For subjective evaluation, relative pain reduction in visual analog scale (VAS) and existence of heat sensation was accessed. For objective evaluation, gross evaluation, papilla index, existence of wound dehiscence, redness and swelling was checked. The same evaluation was performed in each arch bar group and suture group. For whole subject, 0.2% HA group resulted higher reduction than placebo group in pain of site in first week with significancy. Same findings were seen other weeks but there was no significancy. 0.2% HA group had better result than placebo in objective evaluation (papilla index, wound dehiscence, redness and swelling), but in gross evaluation placebo had better result than 0.2% HA group with no significancy. Subject was divided into suture group and arch bar group. Same aspect was seen, but only suture group had significancy not arch bar group in pain reduction score. 0.2% HA group resulted higher reduction than placebo group in pain of site in first week with significancy, especially in suture group. It reveals topical application of HA in wound especially suture site reduced pain in early stage. And 0.2% HA group had better result than placebo in papilla index, redness and swelling with no statistical significancy. In conclusion, HA has effect of pain reduction and healing promotion in the mucosal wound after oral surgery.

Objectives The purpose of this study is to systematically review the degree of pain and function improvement in acupuncture treatment for patellofemoral pain syndrome (PFPS). Methods Randomized controlled trials (RCTs) were searched from Medline/PubMed, Cochrane Library, Embase, WanFang Data, China National Knowledge Infrastructure (CNKI), Citation Information by NII (CiNII), Koreanstudies Information Service System (KISS), ScienceON, KoreaMed, and Korean Medical Database (KMbase). The methodological quality of selected RCTs' were evaluated according to Cochrane RoB2 tool. Results Total of 158 studies were identified and 7 RCTs were selected for systematic review. Frequently used evaluation method were visual analogue scale and Kujala scores. All studies showed significant effect on pain reduction and knee function between before and after treatment, but showed partial significance between the intervention and control group. Conclusions Research showed that acupuncture treatment is an effective treatment for improving the pain and function of PFPS, but it is difficult to conclude that it has a significant effect compared to other treatments. Selected studies included subjective evaluation indicators and the total of selected studies were small, so the interpretation of the results is limited. Also most of the included studies are evaluated to have some concerns of bias. Therefore, subsequent research should continously be done in more objective and systematic method.
Objectives : The objective of this study was to investigate the relationship between psychological distress and pain in cancer patients. Methods : 249 patients with cancer who visited National Health Insurance Service Ilsan Hospital between April 2013 and March 2014 were evaluated with National Cancer Center Psychological Symptom Inventory(NCC-PSI) which consisted of Modified Distress Thermometer(MDT) and Modified Impact Thermometer(MIT). Each scale was divided into 3 subscales targeting separate symptoms: insomnia, anxiety, and depression. Psychological distress was defined as positive for those who scored above the cutoff values in at least one of all six subscales. The Numeric Rating Scale for Pain(NRS-Pain) was used to assess the subjective severity of pain. Logistic regression was performed to investigate the association between psychological distress and pain. Results : Univariate logistic regression analysis showed that pain, gender, compliance, and two subscale scores of Hospital Anxiety and Depression Scale(HADS) were significantly associated with psychological distress. Multivariate logistic regression analysis showed that pain and HADS anxiety subscale score maintained a statistically significant association with psychological distress adjusted for variables including age, gender, years of education, Eastern Cooperative Oncology Group performance status, cancer stage, Charlson Comorbidity Index, compliance, and HADS depression subscale score. One point increase in pain was 1.31 times more likely to cause psychological distress. In secondary analysis, pain was significantly associated with all subscales of NCC-PSI, except MIT-anxiety subscale. Conclusions : This study suggests that NCC-PSI, a screening tool for psychological distress, reflects pain. We recommend that physicians who treat cancer patients consider the examination of psychological distress which provides comprehensive evaluation of various factors regarding quality of life.
