류마토이드 관절염에서 회전근 개 전층 파열과 동반되어 견관절과 견봉하 및 삼각근하 점액낭에 동시에 발생된 다발성 미립체: 관절경적 치료 및 자기 공명 영상 소견 -1례 보고- (Multiple Rice Body in both Glenohumeral Joint and Subacromial & Subdeltoid Bursa Simultaneously combined with Full Thickness Cuff Tear in Rheumatoid Arthritis: Arthroscopic Treatment & MR Appearance -A Case Report-)

  • 류마토이드 관절염이나 혈청학적 음성의 여러 관절병증에서 작은 크기의 섬유소성 미립체의 형성은 흔히 나타나는 소견이다. 그러나 류마토이드 관절염에서 견관절의 견봉하-삼각근하 점액낭염과 연관되어 생기는 다발성의 미립체는 굉장히 드물다. 지금까지, 문헌상 만성적인 류마토이드 관절염 환자에서 견관절의 회전근 개 전층 파열과 동반되어 견관절과 견봉하-삼각근하 점액낭에 동시에 발생된 다발성 미립체를 관혈적이 아닌 관절경적으로 치료한 예는 없는 것으로 사료되어 이에 보고하고자 한다. 또한 견봉하-삼각근하 점액낭에 발생한 미립체에 대한 자기 공명 영상 소견도 최근까지 단지 몇몇 보고에만 언급 되어 왔다. 그래서 다발성 미립체와 연관된 여러 가지의 활액막 증식성 질환들의 감별 진단을 위해 견봉하-삼각근하 점액낭에 생긴 다발성 미립체의 자기 공명 영상 소견을 기술하고자 한다.

견봉하 관절경 수술 후 진통 작용: 모르핀과 국소마취제 혼합제와 위약의 점액낭 내 지속 주입에 대한 전향적 이중 맹검 연구 (Analgesia After Subacromial Arthroscopy: Prospective Double-blind Study of Continuous Mixed Intra-bursal Infusion with Morphine and Bupivacaine and placebo)

  • The aim of this study is to determine if intra-bursal morphine and Bupivacaine mixed infusion provided useful analgesia after subacromial arthroscopic operation. At the end of the subacromial arthroscopy, continuous intra-bursal infusion catheter was inserted. In a double-blind randomized trial, Sixty patients were allocated to one of two groups: placebo group(n=30) received continuous saline infusion. Study group received mixed 5㎖ of 0.5% Bupivacaine, 2㎎ of morphine and 0.05㎖ of 1/1000 epinephrine as bolus and mixed solution of 40㎖ of 0.5% Bupivacaine and 8㎎ of morphine as maintenance dose with continuous infusion pump(0.5㎖ hourly). In the placebo group, two patients were discontinued due to leakage and catheter coming out each. Intensity of pain was evaluated preoperatively and postoperatively for 3 days by 10 graded visual analogue scale. Night pain, pain on motion, sleep disturbance, lying on painful shoulder and amounts of intramuscular pain killer were recorded. Analgesic effect for pain was cleared at 1st and 2nd postoperative day and there was less sleep disturbance for 3 days postoperatively in study group. There was no difference in pain on motion postoperatively. In study group, less pain killers were used in the first 48 hours postoperatively. The continuous intra-bursal infusion decreased perception of pain at resting stage and reduced supplemental analgesic requirement for 2 days postoperatively.

만성 견봉하 및 삼각근하 점액낭염에 발생한 다발성 쌀소체의 수술적 치료: 증례 보고 (Surgical Treatment of Multiple Rice Bodies in Chronic Subacromial and Subdeltoid Bursitis: A Case Report)

  • 견봉하 및 삼각근하 만성 점액낭염에 발생한 다발성 쌀소체의 치료에 대해 고찰하였다. 다발성 쌀소체의 형성은 혈청음성의 류마티스 관절염 혹은 결핵과 자주 연관된 만성 점액낭염의 합병증으로 형성된다. 또한 이것은 윤활막 뼈연골종증과 영상학적 및 임상적으로 비슷한 양상을 보인다. 저자들은 만성 삼각근하 및 견봉하 점액낭염에 발생한 다발성 쌀소체가 있어 관혈적 수술을 통해 제거한 44세 남자를 증례 보고하고자 한다. 제거술을 시행한 지 16개월 후 견관절의 운동 범위 및 근력은 정상 범위에 있었고 기능도 만족스러웠다. 일반 촬영 및 초음파 검사에서 쌀소체의 재발은 없었다. 견봉하 및 삼각근하 만성 점액낭염에 발생한 다발성 쌀소체는 점액낭을 포함한 관혈적 절제술에 의해 성공적으로 치료 될 수 있었다.

회전근개 질환에서 시행한 관절경적 견봉하 감압술의 결과 (The Results of Rotator Cuff Disease Treated by Arthroscopic Subacromial Decompression)

  • We reviewed the results of arthroscopic surgery in patients with rotator cuff disease. Arthroscopic subacromial decompression(ASD) was performed on 22 patients with rotator cuff disease who had not responded to nonoperative measures. In the patients who had a complete tear of the rotator cuff(four of ten Neer's stage III patients), mini-open repair also was performed. Results were determined by questioning patients about their satisfaction with the outcome of surgery and by functional assessment of the shoulder with the parameters of pain, function, and range of motion according to the UCLA shoulder rating scale. The average follow-up was 21 months. The results were sixteen(72%) excellent, three(14%) good, two(9%) fair, and one(5%) poor. The following variables were analyzed to assess their influence on final outcome; duration of preoperative symptoms, Neer's stage(extent of damage to the cuff), type of acromion. Satisfactory results were achieved in thirteen of fourteen patients(93%) who had duration of preoperative symptoms below one year, and in four of six(67%) above 2 years. And satisfactory results were achieved in eleven of twelve patients(92%) who had Neer's stage II and in eight of ten(80%) stage III and achieved in six of seven patients(86%) who had Bigliani's acromion type I, in nine of eleven(82%) type II, and in four of four(100%) type III. There are multiple factors that may influence the recovery after ASD or the eventual outcome. However, although there is still a controversy about the pathogenesis of rotator cuff disease, We found that arthroscopic subacromial decompression and mini-open repair in patients with rotator cuff disease were well enough documented to be considered a standard treatment.

극상근건염과 견봉하점액낭염 및 심각근하점액낭염으로 진단받은 견비통 환자를 대상으로 대용량 신바로 약침치료를 통한 통증경감과 가동범위 호전에 대한 증례보고 (A Case Report on the improvement of Range of Motion and Pain Relief for Patients Diagnosed with Supraspinatus Tendinosis, Subacromial Bursitis and Subdeltoid Bursitis treated with Megadose Shinbaro Pharmacopuncture)

  • Objectives : The purpose of this research is to show the effectiveness of the Korean medical treatment on patients diagnosed with supraspinatus tendinosis, subacromial bursitis and subdeltoid bursitis treated by Korean medical treatment Including megadose shinbaro pharmacopuncture. Methods : We used megadose shinbaro pharmacopuncture on patients who received treatment at Bucheon Jaseng Korean medicine hospital from December, 2016 to January, 2017. Also we checked NRS, SPADI, range of motion(ROM) and shoulder physical examination to follow the scale showing the improvement of the symptoms of the patients. Results : All 4 patientes showed the improvement of NRS, SPADI range of motion(ROM) and shoulder physical examination. Conclusion : This research showed that megadose shinbaro pharmacopuncture was effective on the treatment of shoulder pain patients diagnosed with supraspinatus tendinosis, subacromial bursitis and subdeltoid bursitis.

The effects of a single-dose subacromial injection of a nonsteroidal anti-inflammatory drug in geriatric patients with subacromial impingement syndrome: a randomized double-blind study

  • Background: As nonsteroidal anti-inflammatory drugs (NSAIDs) and steroids have similar effects, steroids can be avoided to reduce adverse effects. This study aimed to compare the differences in symptom improvement after subacromial injection of steroids or NSAIDs. Methods: Sixty patients with rotator cuff syndrome for at least 3 months were enrolled and divided into steroid and NSAID groups. The steroid group received a mixture of 1 mL of triamcinolone acetonide (40 mg/mL) and 1 mL of lidocaine hydrochloride 2%, while the NSAID group received a mixture of 1 mL of Ketorolac Tromethamine (30 mg/mL) and 1 mL of lidocaine hydrochloride 2%. The patients were assessed before and at 3, 6, and 12 weeks after the procedure. Shoulder scores from visual analog scale (VAS), American Shoulder and Elbow Surgeons (ASES), and University of California Los Angeles (UCLA) were used for evaluation. Results: Both groups showed improvements in the clinical outcomes. Overall VAS, ASES, and UCLA scores improved from 6.9, 32.7, and 16.0 before the procedure to 2.0, 1.2, and 1.1; 81.5, 87.6, and 88.5; and 29.7, 31.8, and 32.0 at weeks 3, 6, and 12 weeks after the procedure, respectively. Twenty-six patients (86.7%) in the steroid group and 28 (93.3%) in the NSAID group reported satisfactory treatment outcomes. There were no significant differences in the outcomes between the two groups (p=0.671). Conclusions: Subacromial injection of NSAIDs for rotator cuff tendinitis with shoulder pain had equivalent outcomes with those of steroid injection at the 12-week follow-up.

Clinical and radiological outcomes of ultrasound-guided barbotage using a spinal needle and subacromial steroid injection for calcific tendinitis of the shoulder

  • Background: Ultrasound (US)-guided techniques reported for the treatment of calcific tendinitis have mostly demonstrated good results. This study investigates the effect of US-guided barbotage using a spinal needle in patients with calcific tendinitis of the shoulder. Methods: Thirty-six patients with calcific tendinitis of the shoulder treated by US-guided barbotage with a spinal needle and subacromial steroid injection were included in the study. We evaluated clinical outcomes based on American Shoulder and Elbow Surgeons (ASES) score, Constant score, and visual analog scale (VAS) for pain score. Radiological outcomes were assessed by X-ray imaging at each visit. Results: Our results showed that US-guided barbotage and subacromial steroid injection produced good clinical and radiological outcomes in patients with calcific tendinitis of the shoulder. Of the 36 patients, only one required surgical treatment, while the others showed improvement without any complications. Compared to values before the procedure, calcific deposit size and VAS, ASES, and Constant scores showed significant improvement 6 weeks after the procedure. No significant correlation was found between the initial calcific deposit size and clinical outcomes at each time point. Conclusions: In patients with calcific tendinitis of the shoulder, US-guided barbotage using a spinal needle and subacromial steroid injection can yield satisfactory clinical and radiological results.

Immediate Effect of the Complex Rotational Stretching Method on the Proprioceptive Sensation of the Shoulder Joint, Subacromial Space, Range of Motion, Shoulder Instability and Dynamic Function

  • PURPOSE: This study compared the effects of proprioceptive sensation, subacromial space, and dynamic function according to proprioceptive neuromuscular facilitation (PNF), static stretching (SS), and complex rotational stretching (CRS). METHODS: Thirty students without any musculoskeletal disease who volunteered to participate were included in this study. The following metrics were measured to evaluate the function and stability under the normal conditions, with the PNF, SS, and CRS: special test and flexion, extension, abduction, adduction, internal rotation, external rotation (shoulder range of motion) and reaching distance on the medial (ME), superolateral (SL), inferolateral (IL), and subacromial space and proprioceptive sensation were evaluated. All measures were analyzed using one-way ANOVA and repeated measures of ANOVA. RESULTS: A clear difference in adduction in the range of motion was observed in all groups (p < .05). Significant differences could not be identified in all values in the error test, except for Ab (p < .05). Significant differences in reach were noted in all directions of the SS and PNF in the Me, SL, and IL (p < .05). After the intervention, significant differences in the average values could be identified in all groups except for the SS group after rest (p < .05). After the intervention, there was a significant difference between the CRS and SS and PNF groups (p < .05). CONCLUSION: The application of CRS is as helpful as the existing SS and PNF for improving the joint range of motion improvement, shoulder balance, and subacromial space.

견봉하 삼각근하낭염 환자에서 견봉하낭 스테로이드 주사 후 운동의 효과 (Role of Exercise after Subacromial Bursa Injection in Subacromial Subdeltoid Bursitis)

  • Objective: To find that the patients who conducted home self-exercise in conjunction with intra-articular corticosteroid injection will have better improvement in subacromial subdeltoid (SASD) bursitis symptoms than those who received only an injection. Method: A prospective, nonrandomized, comparison study was conducted in outpatient rehabilitation clinic in a tertiary university hospital. Patients diagnosed with SASD bursitis with physical examination and ultrasound evaluation were included and received ultrasound guided injection (UGI). Patients were divided into two groups according to the compliance of self-exercise: UGI-exercise group and UGI only group. Visual analog scale (VAS) was checked before, at 3 weeks, 3 months, and 6 months after the UGI. A patient's global impression of change (PGIC) survey was made at 3 months after the UGI. Results: A total of 82 patients with SASD bursitis were included. At 3 weeks after the injection, the mean VAS for the UGI-exercise group was 2.6 ± 1.7 and for UGI only group was 1.9 ± 1.3 (p=0.030). At 3 months after the injection, the mean VAS for the UGI-exercise group was 4.0 ± 1.3 and for UGI only group was 5.4 ± 1.4 (p<0.001). Conclusion: We concluded that home-based self-exercise of the shoulder provides an additional benefit for pain alleviation possibly with prolonging the effect of injection in SASD bursitis.

Extra-articular Pigmented Villonodular Synovitis of the Subacromial Space: A Case Report

  • Pigmented villonodular synovitis (PVNS) is a rare, benign, proliferating disease affecting the synovium of joints, bursae, and tendon sheaths. The knee is the most common site of involvement and accounts for up to 80% of cases. Involvement of the shoulder is extremely rare. Only 1 case of involvement of the subacromial space has been reported worldwide. We report a case of localized extra-articular PVNS of the subacromial space that was satisfactorily treated with marginal excision; the disease has not recurred during an 18-month follow-up period. The clinical manifestation, treatment, and prognosis of extra-articular PVNS are poorly understood, but if the lesion is left untreated, it can invade the surrounding soft tissue and joint. Therefore, we believe that early diagnosis and treatment are important for the satisfactory management of PVNS.
