A study on the Needs for Nutrition Management Program for Elderly Who use Welfare Facilities (노인복지시설 사용자 중심의 영양·식생활관리 프로그램 요구 분석)

  • Objectives: This study was performed to investigate the nutritional status, health conditions, eating habits and experience and demand for nutrition dietary management of senior citizens. And these data are formed foundation of development of nutrition dietary management education program and contents in welfare facilities for the aged. Methods: We visited 3 public health centers, 3 senior citizens centers, and 4 welfare centers in Seoul, Gyeonggi-do, Chungnam province, and Daejeon area and carried out interview by semi-structured questionnaire for senior citizens older than 65 years who use those facilities. Results: The study included 17%, 30.7% and 52.3% of senior citizens from public health centers, seniorcitizen centers and and welfare centers, respectively. The age range of 43.9% of the population was 65-74 years and and 56.1% were older than 75 years. We observed that 83.2% of subjects took some medicines due to diseases that they have and 58.0% took prescription medicines for hypertension. The thing that they considered the most when selecting food was 'the taste'(p<0.05). Regarding the level of practice to keep the dietary life, they answered more than 'average' for most of items but answered less than 'average' for lot of salt intake, drinking, exercise. For the experience of nutrition dietary life education, only 19.8% answered 'Yes' and the service for nutrition dietary life management showed the highest score in the demand for 'provide nutritious food'. For the size of consulting group for nutrition/dietary life education, the public health center and welfare center preferred a larger group size but the senior citizens center preferred a smaller group. With regard to who will carry out the consulting, the demand for dietitian was the highest and the operation type showed the high demand in the order of consulting, education. The contents that they want to have consultation in nutrition dietary life education were diet therapy for diseases and the ordinary diet therapy for health. Conclusions: This study suggested the management of nutrition dietry life necessitates qualitative measures according to the different types of welfare facilities. For these, it is in need of development of counseling and education program included therapy for disease. Above all, the policy to secure dietitian of welfare facilities for the aged to perform these should be achived.

Decreased Concentration of Plasma Brain-Derived Neurotrophic Factor in Suicide Attempters (자살 시도자에서 혈장 Brain-Derived Neurotrophic Factor 농도 저하)

  • Objects:Some studies have suggested that brain-derived neurotrophic factor(BDNF), one of the most important neurotrophins, is involved in pathophysiology of depression and suicide. This study was performed to determine whether there is an abnormality in plasma BDNF concentration in suicidal attempters. Methods:The subjects were 71 suicidal attempters who visited emergency rooms in multi-medical centers. All subjects had been interviewed by using Structured Clinical Interview for DSM-IV(SCID), Hamilton Depression Rating Scale(HDRS), Young Mania Rating Scale(YMRS), and Positive And Negative Syndrome Scale(PANSS). The severity of the suicidal behavior was measured by Lethality of Suicide Attempt Rating Scale(LSARS) and Risk-Rescue Rating(RRR) system. Seventy-one age, sex, and diagnosis matched non-suicidal psychiatric patients who were consecutively admitted to a psychiatric ward during the same period recruited as psychiatric controls. They were drug-naive or drug-free at least more than 2 months. In addition, 80 healthy controls were randomly selected as normal controls. Plasma BDNF level was measured by the enzyme linked immunosorbent assay(ELISA) methods. Results:In overall F-test, differences of the plasma BDNF levels among the groups were statistically significant(F=20.226, p<0.001). In the multiple comparisons(Scheffe), while mean levels of plasma BDNF between normal controls and non-suicidal psychiatric patients were similar(p=0.984), the BDNF levels of suicidal attempters were lower than those of other two groups(p<0.001). LSARS and RRR did not reveal any significant correlations with BDNF levels in suicidal attempters. Conclusion:These results suggest that reduction of plasma BDNF level is related to suicidal behavior and BDNF level may be a biological marker of suicidal behavior.

A Prospective Cohort Study on the Relationship of Sleep Duration With All-cause and Disease-specific Mortality in the Korean Multi-center Cancer Cohort Study

  • Objectives: Emerging evidence indicates that sleep duration is associated with health outcomes. However, the relationship of sleep duration with long-term health is unclear. This study was designed to determine the relationship of sleep duration with mortality as a parameter for long-term health in a large prospective cohort study in Korea. Methods: The study population included 13 164 participants aged over 20 years from the Korean Multi-center Cancer Cohort study. Information on sleep duration was obtained through a structured questionnaire interview. The hazard ratios (HRs) and 95% confidence intervals (CIs) for mortality were estimated using a Cox regression model. The non-linear relationship between sleep duration and mortality was examined non-parametrically using restricted cubic splines. Results: The HRs for all-cause mortality showed a U-shape, with the lowest point at sleep duration of 7 to 8 hours. There was an increased risk of death among persons with sleep duration of ${\leq}5$ hours (HR, 1.21; 95% CI, 1.03 to 1.41) and of ${\geq}10$ hours (HR, 1.36; 95% CI, 1.07 to 1.72). In stratified analysis, this relationship of HR was seen in women and in participants aged ${\geq}60$ years. Risk of cardiovascular disease-specific mortality was associated with a sleep duration of ${\leq}5$ hours (HR, 1.40; 95% CI, 1.02 to 1.93). Risk of death from respiratory disease was associated with sleep duration at both extremes (${\leq}5$ and ${\geq}10$ hours). Conclusions: Sleep durations of 7 to 8 hours may be recommended to the public for a general healthy lifestyle in Korea.

A Descriptive Study of Gall Stone Patient's Dietary Habits and Nutritional Status (담석증 환자의 식생활양식 및 영양 실태 조사)

  • Gallstone composition has changed over the past decades in the Korean with a prominent increase in the prevalence of cholesterol gallstone. This trend is possibly due to the westernization of dietary habits. The purpose of this study was descriptive of GB patient's health related eating behaviors and nutrient consumption patterns. One hundred and six gallstone patients who have had cholecystitis surgery enrolled in this study. Anthropomertic indices, such as height, weight, waist circumference, and hip circumference were measured and calculated BMI and WHR. As the biomarker, TC, TG, LDL-C, HDL-C, FBG level and SBP/DBP were measured and analyzed the relationship with GB stone formation. The structured checklist of health related eating behavior and the semiquantitative food frequency questionnaire after pre-test was used in the face to face interview study. The mean age was $54.9{\pm}13.3$ and gallstone disease was more frequent in the fifties and sixties. Mean BMI was $24.3{\pm}2.8$ in males, and $23.4{\pm}3.9$ in females, the average of waist circumference was $91.1{\pm}7.0cm$ in males and females were $85.4{\pm}9.6cm$. The WHR of men and women was $0.93{\pm}0.0,\;0.90{\pm}0.1$, respectively. The obesity and overweight trend was observed in gallstone patients. The mean blood sugar was researched at $114.5{\pm}35.0mg/dL$. And the ratio of both symptoms DM and gallstone was 26.4%. The rate of dramatic eating repast was significantly higher in the females (p<0.01). Males tended to more frequently consume meat, of high fat content meats and greasy food consumption when eating out. The CPF ratio was 57:16.5:26.5. There was a significant positive correlation between WC and energy (r=0.252, p<0.05), carbohydrate (r=0.255, p<0.05) and niacin (r=0.227, p<0.05). In addition, carbohydrate were significantly correlatied with TC (r=0.230, p<0.05). BMI appeared positive in correlation of protein (r=0.201, p<0.05) and fat (r=0.205, p<0.05). These findings provide a little association that dietary habits are related with cholesterol gallstone formation.

Korean's Traditional Method to Increase the Amount of Breast Milk. (전통적 모유량 증가방법에 관한 연구)

  • In recent days, most of mothers prefer bottle feeding to breast feeding. Even mothers who started with breast feeding, change to bottle feeding in a short period. Many factors were reported causing the trend, but a significant influencing latter was revealed the mothers' perception that their breast milk wasn't enough for their babies. The purpose of this study were to identify how mothers of 30 years ago kept breastfeeding longer period for their child, and what were the diet they used in order to keep adequate breast milk secretion. The subjects of this study were 95 women who are over 60 years or older. Data were gathered by 13 authors by interview using structured questionnare. There were 16 questions related to subject's demographic informations and the specific recipe which they used, and 7 questions related to breast feeding techniques they used. Data were analyzed by SPSS/PC and content analysis. Results were as follows. 1. The subjects reflected that their milk secretion was enough to feed their tables. 2. More than half of the subject started breast feeding on the 3rd day after delivery and had continued breast feeding until they were pregnant again. 3. The subjects tried to eat as much rice and seaweed (MiYuk) soup as possible and didn't take any other specific diet during the breast feeding period. 4. The subjects didn't pay specific attention to the breast, general health. emotion, home environment. The only thing they did was being careful not to press breast when not feed. 5. Many subjects perceived that breast feeding made them healthy, and only 7% of subjects responded that they had some health problem during the breast feeding period.

A Study on the Ego-Integrity Phenomenon of the Elderly (노인의 자아통합현상에 관한 연구)

  • This study was conducted in order to attain an in-depth understanding of and describe the Ego-Integrity Phenomenon of the elderly of age 65 and older. The data was collected from June 2020 to July 2020 as an in-depth interview consisting of semi-structured questions. The data was analyzed using the Colaizzi's method, one of the phenomenology methods. All interviews were recorded and transcribed for analysis. Through analysis, 309 significant statements were extracted, and 88 formulations of meaning, 53 themes, 16 theme clusters, and 5 categories were derived. The categories and the theme clusters are as such : 'reflecting on the past life with delight' ('fruitfulness over the lifetime', 'achieving through fulfilling responsibilities', 'achieved specially and gratefully'), 'Currently lonely and distressed due to the bitter old days' ('Physical health needs and psychosocial needs are not met', 'Wasn't able to make good choices for parents and children', 'Elderly life is not easy'), 'Satisfaction from enduring tough and difficult journeys in life' ('Compensated for hard work at workplace and home', 'Problem solved through endurance and overcoming', 'Living a healthy elderly life by escaping from the habits of the past'), 'Realizing the way of life as an elderly' ('Now is the best and most satisfying moment', 'Managing self in old age', 'Living passionately, though it may be late', 'Retiring from the forefront of life with an abundant heart'), 'Passing on beautifully' ('Desires toward the later years', 'Hope during the last moments', 'Appearance and emotions when death approaches'). It was observed that during the Ego-Integrity Phenomenon of the elderly, they have come to discern the meaning of life by reminiscing the regrets and difficulties of the past, and through this reminiscence, they have come to attain a positive and transcendental hope for the rest of their lives. The Ego-Integrity of the elderly appeared to be subjective, dynamic, and occurring simultaneously rather than continuously. It is recommended to develop a variety of nursing interventions while taking into consideration the age and the type of experience of the elderly.

A Study on Effect of the Children Culture Art Education Program Activity in Local Child Care Center -Focusing on Traditional dance program- (지역아동센터의 문화예술교육프로그램에 대한 효과 연구 -전통춤 프로그램 중심으로-)

  • This study examined traditional dance programs in cultural and arts education in local children's centers. We aimed to contribute to the voluntary and active participation of children in cultural and arts education through differentiated cultural and arts educational programs. Interviews were conducted from March 2016- June, 2016 at A Regional Children's Center in Chungnam. Once a week, a 50-minute were held 16 Session as "learning traditional joyful dance". After completing the program, 25 children who participated in the program were asked about the good points and disappointments regarding the program. The interviews used open, structured questions that have been primarily used in phenomenological research. The list of questions was prepared in advance according to the research purpose. The results of the study are as follows: The evaluation of the first program was generally positive. By experiencing activities through cultural and arts education through traditional dance programs for children, children learned to be polite. It also changed positively in conversation. Children who showed negative attitudes toward traditional dance programs gradually participated in activities. Second, I examined the evaluation of lecturers who teach traditional dance. The children's response to the program was positive, with sincere and responsible lecture preparation and progress. The instructor prepared the class in advance, welcomed the children and ran the class in good faith. Instructors who taught traditional dances were proficient in dance, majored in dance and teaching children. The learning effect was high by the instruction of the good dance movement. Rich in lecture experience, he is proficient in dealing with children in class. Material was delivered according to the child's dance level.

The Decision-Making Journey of Malaysian Women with Early Breast Cancer: A Qualitative Study

  • Background: The survival outcomes for women presenting with early breast cancer are influenced by treatment decisions. In Malaysia, survival outcome is generally poor due to late presentation. Of those who present early, many refuse treatment for complementary therapy. Objective: This study aimed to explore the decision making experiences of women with early breast cancer. Materials and Methods: A qualitative study using individual in-depth interviews was conducted to capture the decision making process of women with early breast cancer in Malaysia. We used purposive sampling to recruit women yet to undergo surgical treatment. A total of eight participants consented and were interviewed using a semi-structured interview guide. These women were recruited from a period of one week after they were informed of their diagnoses. A topic guide, based on the Ottawa decision support framework (ODSF), was used to facilitate the interviews, which were audio recorded, transcribed and analysed using a thematic approach. Results: We identified four phases in the decision-making process of women with early breast cancer: discovery (pre-diagnosis); confirmatory ('receiving bad news'); deliberation; and decision (making a decision). These phases ranged from when women first discovered abnormalities in their breasts to them making final surgical treatment decisions. Information was vital in guiding these women. Support from family members, friends, healthcare professionals as well as survivors also has an influencing role. However, the final say on treatment decision was from themselves. Conclusions: The treatment decision for women with early breast cancer in Malaysia is a result of information they gather on their decision making journey. This journey starts with diagnosis. The women's spouses, friends, family members and healthcare professionals play different roles as information providers and supporters at different stages of treatment decisions. However, the final treatment decision is influenced mainly by women's own experiences, knowledge and understanding.

Association between compliance with dietary guidelines and Alzheimer's disease in Korean elderly (한국 노인의 식생활 지침 실천도와 알츠하이머형 치매의 관련성)

  • Purpose: Confronting the growing burden of dementia requires understanding the causes and predictors of dementia in order to develop preventive strategies. In Korea a large proportion (71%) of dementia is Alzheimer's disease, and the risk factors have not yet been elucidated. Dietary factors may be possible risk factors, however research on the relationship between Alzheimer's disease and dietary behaviors has been insufficient. The purpose of this study was to investigate the association between compliance with the "Dietary Guidelines for Elderly (The Ministry of Health and Welfare, 2011)" and Alzheimer's disease among Korean elderly. Methods: Elderly persons who visited a University hospital or a dementia center of Seoul and agreed to participate in the examinations were selected. Among 277 subjects, 89 Alzheimer patients were selected with diagnosis and 118 subjects were assigned to the control group. Diagnosis of Alzheimer's disease was based on Alzheimer's disease criteria of Diagnostic and Statistical manual of Mental Disorders, 4th edition (DSM-IV) and criteria of National Institute of Neurological and Communicative Disorders and Stroke-Alzheimer's Disease and related Disorders Association (NINCDS-ADRDA). Information on the general characteristics, health related behaviors and compliance with the dietary guidelines for Korean elderly was collected by face to face interview using a structured questionnaire. Anthropometric variables were measured during the survey. Results: Total compliance score of dietary guidelines was significantly lower in the Alzheimer's disease group than in the control group (p = 0.0001). The odds ratio of Alzheimer's disease was significantly decreased in the group with the highest dietary guideline compliance score (OR = 0.47, 95 % CI = 0.18~1.09) compared to the group with the lowest compliance score. Conclusion: The results indicate that increasing compliance with the dietary guidelines could be an effective strategy to decrease the risk of Alzheimer's disease among Korean elderly.

A Study on Critical Factor of Selecting Online Video Flatform by Using AHP (AHP 기법을 활용한 온라인 동영상 플랫폼의 선택 속성 연구)

  • This study attempts to improve the understanding of the rapidly growing online video platform market such as Youtube and OTT, and to investigate the attributes and relative importance of them. For this purpose, the factors that influence the choice to use were derived through literature studies and the Focus Group Interview (FGI), and the priority of the factors was calculated through the analytic hierarchy process (AHP). The upper layer of the AHP structure was 'Relationship', 'Entertainment', 'Informativity', and 'Convenience', and the lower layer was structured into 13 elements. The importance priority analysis among the factors that influence the choice to use was done by teenagers, 20s, and 30s and the results are summarized as follows : First, Users consider the 'Just for fun' and 'Satisfaction of interests' as the most important factors, followed by 'Easy accessibility to use', 'Vicarious satisfaction', 'Usefulness of Information', and 'Up-to-dateness of information'. Second, the ranking of the upper layer was in the order of 'Entertainment'-'Informativity'-'Convenience'-'Relationship'.As a result of AHP,'Entertainment' was 3.6 times more important than 'Relationship'. In the comparison by age group, only teenagers regarded that 'Convenience' is more important than 'Informativity'. According to the characteristics of the age group, the lower layer of teenagers consider 'Convenient function' to be important and ranked 'Usefulness of information' in 8th. While 'Vicarious satisfaction' ranked 4th out of 13 factors in the entire age group, those in their 20s and 30s ranked 8th, showing a difference. In the case of 20s, 'Reasonable price' was ranked 4th and the 'Diversity of Information' was ranked 5th, Otherwise 30s consider 'Trustworthiness of Information' to the third. Third, unlike 'Convenience' which was the lower-rank in the upper layer AHP analysis, 'Easy accessibility to use', the lower-layer of convenience, ranked third overall in the importance analysis among the 13 lower-layer factors, and showed a similar patterns in the age groups results. In the conclusion, this study demonstrates that 'Convenience' and 'Vicarious satisfaction' factors, which were not relatively well addressed in the previous studies, are the key factors to be considered in. By presenting the results of the importance analysis on each of the selected attributes, This study has a practical implication that Industries such as on-line video service platform provider can use the importance priority in establishing the directions of future strategy.
