A practical approach to nasal reconstruction in Asian patients

  • Background: The study aimed to evaluate nasal reconstruction techniques customized for Asians. The currently available nasal reconstruction guidelines are based on Caucasian patients, and their applicability is limited in Asian patients due to differences in anatomical and structural features. Methods: A retrospective analysis was performed of the medical records of 76 patients who underwent nasal reconstruction at a single center between January 2010 and June 2020. A comprehensive evaluation was conducted of patients' baseline demographics and clinical characteristics, including age, sex, medical history, defect size and location, reconstructive procedure, pathological diagnosis, postoperative complications, and recurrence. Results: In 59 cases (77%), nasal defects resulted from tumor ablation, and the remaining 17 cases involved post-traumatic (20%) and infection-induced (3%) tissue damage. The most common defect location was the alae, followed by the sidewalls, tip, and dorsum. Forehead flaps were the most commonly used reconstructive technique, followed by nasolabial advancement flaps, rotation flaps, and skin grafts. Each procedure was applied considering aspects of structural anatomy and healing physiology specific to Asians. Complications included nasal deformity, hypertrophic scarring, secondary infection, and partial flap necrosis, but no cases required additional surgical procedures. Tumors recurred in two cases, but tumor recurrence did not significantly affect flap integrity. Conclusion: Nasal reconstruction techniques applied considering Asians' facial features resulted in fewer postoperative complications and higher patient satisfaction than the approaches that are currently in widespread use. Therefore, this study is expected to serve as an essential reference for establishing treatment guidelines for nasal reconstruction in Asians.

Clinical Pearls and Advances in Molecular Researches of Epilepsy-Associated Tumors

  • Brain tumors are the second most common type of structural brain lesion that causes chronic epilepsy. Patients with low-grade brain tumors often experience chronic drug-resistant epilepsy starting in childhood, which led to the concept of long-term epilepsy-associated tumors (LEATs). Dysembryoplastic neuroepithelial tumor and ganglioglioma are representative LEATs and are characterized by young age of onset, frequent temporal lobe location, benign tumor biology, and chronic epilepsy. Although highly relevant in clinical epileptology, the concept of LEATs has been criticized in the neuro-oncology field. Recent genomic and molecular studies have challenged traditional views on LEATs and low-grade gliomas. Molecular studies have revealed that low-grade gliomas can largely be divided into three groups : LEATs, pediatric-type diffuse low-grade glioma (DLGG; astrocytoma and oligodendroglioma), and adult-type DLGG. There is substantial overlap between conventional LEATs and pediatric-type DLGG in regard to clinical features, histology, and molecular characteristics. LEATs and pediatric-type DLGG are characterized by mutations in BRAF, FGFR1, and MYB/MYBL1, which converge on the RAS-RAF-MAPK pathway. Gene (mutation)-centered classification of epilepsy-associated tumors could provide new insight into these heterogeneous and diverse neoplasms and may lead to novel molecular targeted therapies for epilepsy in the near future.

Three-dimensional functional unit analysis of hemifacial microsomia mandible-a preliminary report

  • Background: The aim of this study was to present three-dimensional (3D) structural characteristics of the mandible in the hemifacial microsomia. The mandible has six distinct functional units, and its architecture is the sum of balanced growth of each functional unit and surrounding matrix. Methods: In order to characterize the mandibular 3D architecture of hemifacial microsomia, we analyzed the mandibular functional units of four hemifacial microsomia patients using the 3D reconstructed computed tomography (CT) images. And we compared the functional unit size between affected and non-affected side. Results: The length of condyle and angle showed significant differences between affected and non-affected sides. However, the length of mandibular body showed insignificant differences. The size differences between affected and non-affected side were observed at the condyle, angle, and body in descending order. Conclusions: This preliminary study suggests that the main etiopathogenic units are condyle and angle in the hemifacial microsomia mandible. Further investigation with the increased number of subjects will be helpful to establish treatment modality by etiopathogenic targeting of hemifacial microsomia.

병원간호사의 직무 스트레스, 윤리적 딜레마 및 소진이 이직의도에 미치는 영향 (Influences of Occupational Stress, Ethical Dilemma, and Burnout on Turnover Intention in Hospital Nurses)

  • Purpose: The aim of this research was to identify the relationships between occupational stress, ethical dilemma, burnout and turnover intention and the factors influencing turnover intention in hospital nurses. Methods: The participants, 215 nurses, were recruited from two university hospitals located in two provinces of Korea. Data were collected through self-report questionnaires. Collected data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficients, and multiple regression with SPSS/WIN 22.0. Results: Participants' scores for occupational stress, ethical dilemma, burnout and turnover intention were $3.96{\pm}0.45$, $2.60{\pm}0.39$, $3.71{\pm}0.70$, and $3.50{\pm}0.90$ respectively. The results show that the higher the score for occupational stress, ethical dilemma, and burnout the higher the score for turnover intention. The most important factor influencing turnover intention was burnout (${\beta}=.42$, p<.001), followed by position (${\beta}=.21$, p=.001), educational level (${\beta}=.21$, p=.007), ethical dilemma (${\beta}=.13$, p=.020) in that order. These factors explained 43.5% of total variance in turnover intention (F=19.34, p<.001). Conclusion: Changes in hospital structural management systems and support strategies should be developed and operated, taking into account burnout, ethical dilemma, and personal characteristics of clinical nurses in order to reduce turnover.

미취학 자녀를 둔 종합병원 간호사의 경력몰입에 미치는 융합적 영향요인 (Converged Influencing Factors on the Career Commitment of General Hospital Nurses with Preschool Children)

  • 본 연구의 목적은 미취학 자녀를 둔 종합병원 간호사들의 양육역량과 양육스트레스를 파악하고, 경력몰입에 미치는 융합적 영향 요인을 규명하고자 시도되었다. 연구방법은 2016년 8월10부터 8월31일까지 G광역시에 소재한 종합병원간호사 214명을 대상으로 수집된 자료는 SPSS/WIN 18.0과 AMOS 18.0 Program을 이용하여 분석하였다. 본 연구 결과 미취학 자녀를 둔 종합병원 간호사의 양육역량은 양육스트레스에 유의미하게 직접적인 영향을 미쳤으며, 양육스트레스는 양육역량이 경력몰입에 미치는 영향관계에서 완전매개 효과를 나타냈다. 향후 미취학 자녀를 둔 종합병원 간호사의 경력몰입 증진을 위한 기초자료로 이용될 수 있을 것으로 기대되고, 양육 관련요인과 함께 직무 관련특성을 포함하여 경력몰입에 영향을 미치는 요인에 대한 추후 연구가 필요하리라 본다.

병원정보탐색 어플리케이션의 지속적 이용에 관한 연구 (A Study on the Continuous Use of Hospital Information Seeking Applications)

  • 본 연구는 후기수용모델을 통해 병원정보탐색 어플리케이션(이하 병원 앱)의 지속적인 이용과 이용 중단에 영향을 미치는 요인을 파악하고자 하였다. 병원 앱을 사용한 경험이 있는 이용자를 대상으로 2019년 10월 11일부터 18일까지 설문조사를 실시하여 125개의 유효한 데이터를 수집하였고 구조방정식 모형을 사용하여 데이터를 분석하였다. 연구 결과, 병원 앱 사용자의 만족도와 기대일치도는 지속적 이용의도와 지각된 유용성에 각각 유의미한 영향을 미치는 것으로 나타났다. 하지만 지각된 유용성은 지속적 이용의도에 유의미한 영향을 미치지 않는 것으로 나타났다. 본 연구는 기존에 많이 조명되지 않았던 병원 앱에 관한 연구를 통해 이러한 혁신적 기술의 지속적 사용여부에 영향을 미치는 변인들을 파악하였다. 연구 결과는 병원앱의 수용과 이용을 관찰하여 후기수용모델을 재검증하였으며, 응급상황과 같은 긴급한 도움탐색이나 신속함이 강조되는 정보요구를 만족시키는 병원앱의 특수성을 논의함으로써 이론적 확장을 도모하였다. 또한 소비자가 보고한 병원 앱의 유익성과 한계점을 바탕으로 병원 앱 개발자나 운영자들에게 보다 이용자 친화적인 앱을 제공하기 위한 실질적인 시사점을 제공하였다.

해녀 우울장애 환자의 임상적 특징 (Clinical Characteristics of Haenyeo with Depressive Disorders)

  • Objectives Haenyeo are Korean professional women breath-hold divers in Jeju island. The aim of this study was to investigate the clinical characteristics of depressed Haenyeo group, compared to non-Haenyeo depressed group. Methods This study included 75 Haenyeo and 340 non-Haenyeo with depressive disorders recruited from the Dementia Early Detection Program in Jeju island. Structural diagnostic interviews were performed using the Korean version of Mini International Neuropsychiatric Interview. All patients completed the questionnaires, including the Subjective Memory Complaints Questionnaire (SMCQ), the Patient Health Questionnaire-15 (PHQ-15), and the Blessed dementia scale. Depression was evaluated by the Korean version of short form the Geriatric Depression Scale (K-SGDS) and cognition was assessed by the Korean version of the Consortium to Establish a Registry for Alzheimer's Disease (CERAD) assessment packet. Results Although the mean scores of the K-SGDS were similar between Haenyeo and non-Haenyeo depressed groups, the Haenyeo group showed a higher mean score on the PSQ-15 (p < 0.001, ANCOVA adjusting for age, the K-SGDS and education). The Haenyeo group showed poorer performance on the Korean Version of Frontal Assessment Batter (p < 0.001), the Mini-Mental State Examination in the Korean version of the CERAD Assessment Packet (p < 0.018), the word fluency test (p < 0.001), and the word list memory test (p = 0.012) in ANCOVA adjusting for age and education. The mean SMCQ score was higher in the Haenyeo depressed group than in the non-Haenyeo depressed group. Conclusions The Haenyeo depressed group shows cognitive dysfunction, especially frontal lobe dysfunction, compared to the non-Haenyeo depressed group, indicating the Haenyeo depressed group may have more severe frontolimbic dysfunction due to chronic exposure to hypoxia. The Haenyeo depressed group suffers more somatic symptoms than the non-Haenyeo depressed group.

Clinical Genetic Testing in Children with Kidney Disease

  • Chronic kidney disease, the presence of structural and functional abnormalities in the kidneys, is associated with a lower quality of life and increased morbidity and mortality in children. Genetic etiologies account for a substantial proportion of pediatric chronic kidney disease. With recent advances in genetic testing techniques, an increasing number of genetic causes of kidney disease continue to be found. Genetic testing is recommended in children with steroid-resistant nephrotic syndrome, congenital malformations of the kidney and urinary tract, cystic disease, or kidney disease with extrarenal manifestations. Diagnostic yields differ according to the category of clinical diagnosis and the choice of test. Here, we review the characteristics of genetic testing modalities and the implications of genetic testing in clinical genetic diagnostics.

고출력 3색 LED를 이용한 휴대용 무영등의 개발 (Development of Portable Astral Light using the High Power 3-Color LEDs)

  • 본 연구에서는 의료용 신조명 부품으로 주목받고 있는 고출력 LED를 사용해 구강구조 확인을 위한 치과영역, 진료 및 수술실에서 환부에 대한 작은 범위(국소부위)의 무영 촬영이 가능한 휴대용 LED Light를 설계 개발하였다. 개발에 적용한 LED는 피사체 고유의 Tone에 대한 섬세한 표현력과 입체감을 부각시키기 위해 다양한 색상 구현 및 광량조절이 가능하도록 3색 LED를 사용했다. 사용된 LED의 전기적 특성 및 광학적 특성을 고려해 고효율의 Light Module를 개발했으며 휴대 사용을 위해 낮은 전압에서도 구동이 가능한 SMPS를 제작했다. 또한 PWM 제어방식을 이용해 단색광부터 백색광까지 32,768개의 다양한 색상 구현이 가능했다.

Preferences of Malaysian Cancer Patients in Communication of Bad News

  • Background: Breaking bad news to cancer patients is a delicate and challenging task for most doctors. Better understanding of patients' preferences in breaking bad news can guide doctors in performing this task. Objectives: This study aimed to describe the preferences of Malaysian cancer patients regarding the communication of bad news. Methodology: This was a cross-sectional study conducted in the Oncology clinic of a tertiary teaching hospital. Two hundred adult cancer patients were recruited via purposive quota sampling. They were required to complete the Malay language version of the Measure of Patients' Preferences (MPP-BM) with minimal researcher assistance. Their responses were analysed using descriptive statistics. Association between demographic characteristics and domain scores were tested using non-parametric statistical tests. Results: Nine items were rated by the patients as essential: "Doctor is honest about the severity of my condition", "Doctor describing my treatment options in detail", "Doctor telling me best treatment options", Doctor letting me know all of the different treatment options", "Doctor being up to date on research on my type of cancer", "Doctor telling me news directly", "Being given detailed info about results of medical tests", "Being told in person", and "Having doctor offer hope about my condition". All these items had median scores of 5/5 (IQR:4-5). The median scores for the three domains were: "Content and Facilitation" 74/85, "Emotional Support" 23/30 and "Structural and Informational Support" 31/40. Ethnicity was found to be significantly associated with scores for "Content and Facilitation" and "Emotional Support". Educational status was significantly associated with scores for "Structural and Informational Support". Conclusion: Malaysian cancer patients appreciate the ability of the doctor to provide adequate information using good communication skills during the process of breaking bad news. Provision of emotional support, structural support and informational support were also highly appreciated.
