The Significance of the Strong Ion Gap in Predicting Acute Kidney Injury and In-hospital Mortality in Critically Ill Patients with Acute Poisoning (중증 급성 중독 환자에서 급성 신장 손상과 병원 내 사망률을 예측하기 위한 강이온차(Strong Ion Gap)의 중요성)

  • Purpose: A high anion gap (AG) is known to be a significant risk factor for serious acid-base imbalances and death in acute poisoning cases. The strong ion difference (SID), or strong ion gap (SIG), has recently been used to predict in-hospital mortality or acute kidney injury (AKI) in patients with systemic inflammatory response syndrome. This study presents a comprehensive acid-base analysis in order to identify the predictive value of the SIG for disease severity in severe poisoning. Methods: A cross-sectional observational study was conducted on acute poisoning patients treated in the emergency intensive care unit (ICU) between December 2015 and November 2020. Initial serum electrolytes, base deficit (BD), AG, SIG, and laboratory parameters were concurrently measured upon hospital arrival and were subsequently used along with Stewart's approach to acid-base analysis to predict AKI development and in-hospital death. The area under the receiver operating characteristic curve (AUC) and logistic regression analysis were used as statistical tests. Results: Overall, 343 patients who were treated in the intensive care unit were enrolled. The initial levels of lactate, AG, and BD were significantly higher in the AKI group (n=62). Both effective SID [SIDe] (20.3 vs. 26.4 mEq/L, p<0.001) and SIG (20.2 vs. 16.5 mEq/L, p<0.001) were significantly higher in the AKI group; however, the AUC of serum SIDe was 0.842 (95% confidence interval [CI]=0.799-0.879). Serum SIDe had a higher predictive capacity for AKI than initial creatinine (AUC=0.796, 95% CI=0.749-0.837), BD (AUC=0.761, 95% CI=0.712-0.805), and AG (AUC=0.660, 95% CI=0.607-0.711). Multivariate logistic regression analyses revealed that diabetes, lactic acidosis, high SIG, and low SIDe were significant risk factors for in-hospital mortality. Conclusion: Initial SIDe and SIG were identified as useful predictors of AKI and in-hospital mortality in intoxicated patients who were critically ill. Further research is necessary to evaluate the physiological nature of the toxicant or unmeasured anions in such patients.

The Comparison of Base Deficit, Lactate, and Strong Ion Gap as Early Predictor of Mortality in Trauma Patients (외상환자의 초기 사망 예측 지표로서의 내원 초기의 염기 결핍, 젖산 및 강이온 차이의 유용성 비교)

  • Purpose: Currently, there is a variety of systems available for predicting prognosis of trauma patients such as trauma score, Injury severity score (ISS) and acid-base variables. But it is not clear that the initial acid-base variables are predictors of prognosis in trauma patients at the emergency department. The objective of this study is to compare the base deficit, lactate and strong ion gap as an early predictor of mortality in trauma patients. Methods: Retrospective record review of 136 trauma patients needed to admit to intensive care unit via emergency department (June 2004 to February 2005). Data included age, injury mechanism, ISS, Revised trauma score (RTS), Multiple organ dysfunction score (MODS), Acute physiology and chronic health evaluation III (APACHE III), Glasgow coma scale (GCS), laboratory profiles, calculated anion gap and strong ion gap. Patients were divided into survivors and non-survivors, shock group and non-shock group with comparison by t-test;significance was assumed for p<0.05. Correlation between acid-base variables and mean arterial blood pressure (MABP) was evaluated. Results: There was a significant difference between the RTS (p=0.00), APACHE III (p=0.00), MODS (p=0.00), GCS (p=0.00) of survivors and non-survivors. There was no significant difference between the ISS (p=0.082), lactate (p=0.541), base excess (p=0.468) and SIG (p=0.894) of survivors and non-survivors. There was a significant difference between the RTS (p=0.023), APACHE III (p=0.002), lactate (p=0.000), base excess (p=0.000) and SIG (p=0.000) of shock and non-shock group. There was no significant difference between the ISS (p=0.270), MODS (p=0.442) and GCS (p=0.432) of shock and non-shock group. The base excess was most correlated to MABP (r2=0.150). Conclusion: Initial base deficit, serum lactate and SIG are not predictors of mortality in moderate to severe trauma patients. Initial base deficit, serum lactate and SIG are correlated with the mean arterial blood pressure in trauma patients in emergency department.

  • When a strong electric field is applied between a sharply curved electrode and a blunt surface, the corona may result in a gas movement in the electrode gap which is directed toward the blunt surface. That is called the corona wind. It enhances heat and mass transfer between the surface and the surrounding gas. Moreover such enhancement causes no noise or vibration, which can be applied in complex, isolated geometries, and allows simple control of surface temperatures. This paper examines the relationship between the corona wind and the relative humidity. The facility consists of high voltage power supply thin tungsten wire, plate electrode, multimeter, microammeter and flow meter. Gas velocity is a linear function of voltage, relative humidity and is proportional to the square root of the current. The maximum velocities for the positive and negative corona discharge are 1.9 m/s (2.74 CMM/m), 1.5 m/s(2.15 CMM/m), respectively.

The Electronic Structure Calculations for Hexagonal Multiferroic Materials (다중강전자 상태를 가진 육방정계물질의 전자구조 계산)

  • We have studied electronic structures and magnetic properties of $YMnO_3,\;ScManO_3$ with hexagonal structure using Full Potential Linearized Augmented Plane Wave (FLAPW) method based on LSDA method. LSDA calculation results show that multiferroic $YMnO_3$ shows energy gap due to hexagonal symmetry and magnetic interaction. Because of insulating gap and small Y ion, $YMnO_3$ shows magnetic and ferroelectric state. However, $ScMnO_3$ does not show the energy gap because of strong hybridization of Mn-O for LSDA calculation. We confirmed the stability of multiferroic state for $YMnO_3\;and\;ScManO_3$ using total energy calculations. The antiferromagnetic and ferroelectric states have the lowest energy about 100 meV.

  • The high loading combustion is accomplished by making the turbulent intensity strong and the scale small in the premixed combustor. The Da-mkoler number, which is decreased by short turbulent characteristic time or by long chemical reaction time, can make the distributed reaction flame. So we developed a doubled jet burner for high loading combustion. The doubled jet burner was designed to make the scale of the flame small by the effect of impingement and increasing shear stress with doubled jet. We investigated the turbulence characteristics of unburned mixture and visualized several flames with the typical schlieren photography. Then we studied the influence of several factors that related the scale of flame. Consequently, the doubled jet burner can make the eddy very small. And we can obtain the detail information of the flame scale through ADSF(the Average Distance between Successive Fringes) in the micro- schlieren photography. The ADSF is not a exact flame scale, but it has qualitative trend with increasing turbulent intensity. The ADSF is diminished remarkably with increasing turbulent intensity. The reason is that strong turbulent intensity makes the flame zone thick and flamelets numerous. We can confirm this fact by the signal analysis of ion currents.

Micro-discharged plasma density, electron temperature and excited xenon density for enhancement of vacuum ultraviolet luminous efficiency in alternating current plasma display panel

  • The plasma ion density in AC-PDP has shown to be increased from $5.6{\times}10^{11}cm^{-3}$ to $9.0{\times}10^{11)cm^{-3}$ as the Xe mixture ratio to neon increase from 1 % to 10 %, respectively, at fixed pressure of 400 Torr, by using the micro-Langmuir probe. It is noted that the plasma ion density is density increases as the gas pressure increases in this experiment. The electron temperature decreases from 2.3 to 1.2 eV as the Xe mole fraction increases from 1 % to 10 % at fixed pressure of 400 Torr, which is measured by the micro Langmuir probe and high-speed ICCD camera in this experiment. It is noted that the electron temperature decreases as the gas pressure increases from 150 to 400 Torr in this experiment. It is also observed that the exited Xe atom density and the plasma ion density are in strong correlation sharp between each other in this experiment. It is noted that $5.2{\times}10^{12}cm^{-3}$ in the $1s_5$ metastable state and $1.2{\times}10^{12}cm^{-3}$ in the $1s_4$ resonance state for the PDP cell with gap of 50 um distances under the fixed gas pressure of 400 Torr and Xe content ratio of 10 %.

  • Magnetic and electronic properties of reduced rutile titanium dioxide $(TiO_{2-\delta})$ thin films doped by Mn have been investigated. The present sol-gel-grown semiconducting $TiO_{2-\delta}:Mn$ films exhibit a ferromagnetic behavior at room temperature for a limited range of Mn content. The Mn-doped films have p-type electrical conductivity with the carrier concentration near $10^{19}\;cm^{-3}$. The observed room-temperature ferromagnetism is believed to be intrinsic but not related to free carriers such as holes. Oxygen vacancies are likely to contribute to the room-temperature ferromagnetism-trapped carriers in oxygen vacancies can mediate a ferromagnetic coupling between neighboring $Mn^{+3}$ ions. The energy band-gap change due to the Mn doping measured by spectroscopic ellipsometry exhibits a red-shift compared to that of the undoped sample at low Mn content. It is explainable in terms of strong spin-exchange interactions between Mn ion and the carrier.

  • One of the most useful method for increasing combustion loading of premixed flame is to strengthen the turbulent intensity of unburned mixture. It produces an important information to a design of efficient combustion equipment that analysing microstructure of strong turbulence premixed flame. The flame structure and characteristics are depend on the turbulence of unburned mixture. Therefore, to strengthen the turbulent intensity of unburned mixture make flame scale small and accomplish efficient combustion. We measured the velocity of local flame front movements, local eddy radius and local reaction zone thickness quantitatively with increasing turbulent intensity of unburned mixture. We researched the microstructure of flame using ion currents that react sensitively in the reaction zone. Consequently, the velocity of local flame front movements is depend on the velocity of unburned mixture and local eddy scale is to be small with increasing turbulent intensity. But there is no change in local reaction zone thickness with turbulence.

  • The earthquake data(M$\geq$4.0) for post-1900 in the Korean Peninsula show temporal variation with active and quiet periods. The pattern is quite similar to northeastern China and Inner Zone of Southwest Japan. Yeongweol earthquake occurred in the seismic gap region of the Korean Peninsula. This is the first medium-size earthquake in inland region of the southern peninsula since 1978. The intensity based on the felt area estimation of about 400 places shows MMI III-Ⅷ in inland region. IIon Cheju Island and Ion Ulreung Island. The isoseismal of MMI Ⅶ shows an elongated circle in the direction of NE-SW and covers some parts of Jungdong-myon, Yeongweol-kun, Sindong-eup and Nam-myun, Jeongseon-kun. There occurred quite strong shaking, numerous cracks on the walls of buildings, falling and movement of slate and tiles on the roofs, falling of tiles from the wall and falling of materials from desks, rook falling from mountain and collapse of gravel lauers on the river side. The least square fitting of the intensity data of the Yeongweol earthquake by a popular intensity attenuation relation yields the following : I=Io+1.82249 - 0.65295*InR - 0.00707*R

An experimental study on turbulence characteristics of mixture and combustion characteristics of doubled jet burner flames (미연혼합기의 난류특성과 이중분류버너화염의 연소특성에 관한 실험적 연구)

  • Premixed flame is better than diffusion flame to accomplish a high loading combustion. Since the turbulent characteristics of unburned mixture has a great influence on the flame structure, it is general that many researchers realize a high loading combustion with strengthening turbulent intensity of unburned mixture. Because turbulent premixed flame reacts efficiently on the condition of distributed reaction region, we made high turbulent premixed flame in the doubled impingement field. We investigated turbulent characteristics of unburned mixture with increasing shear force and visualized flames with direct and Schlieren photographs. And the combustion characteristics of flame was elucidated by instantaneous temperature measurement with a thermocouple, by ion currents with a micro electrostatic probe, by radical luminescence intensity and local equivalence ratio. Extremely strong turbulent of small scale is generated by impingement of mixture, and turbulent intensity of unburned mixture increased with the mean velocity. As a result of direct photographs, visible region of flame became longer due to increasing central direction flux. But as strengthed turbulent intensity, visible region of flame turned to shorter and reaction occurred efficiently. As strengthened turbulent intensity of mixture with increasing flux of central direction, maximum fluctuating temperature region moved to radial direction and fluctuation of temperature became lower. The reason is influx of central direction which caused flame zone to move toward radial direction, to maintain flame zone stable and to make flame scale smaller.
