Purpose: Gait variability is defined as the intrinsic fluctuations which occur during continuous gait cycles. Increased gait variability is closely associated with increased fall risk in older adults. This study investigated the influence of attention-demanding tasks on gait variability in elderly healthy adults. Methods: We recruited 15 healthy elderly adults in this study. All participants performed two cognitive tasks: a subtraction dual-task (SDT) and working memory dual-task (WMDT) during gait plus one normal gait. Using the $LEGSys^+$ system, we measured the coefficient of variation (CV %=$100{\times}$[standard deviation/mean]) for participants' stride time, stride length, and stride velocity. Results: SDT gait showed significant increment of stride time variability compared with usual gait (p<0.05), however, stride length and velocity variability did not difference between SDT gait and usual gait (p>0.05). WMDT gait showed significant increment of stride time and velocity variability compared with usual gait (p<0.05). In addition, stride time variability during WMDT gait also significantly increased compared with SDT gait (p<0.05). Conclusion: We reported that SDT and WMDT gait can induce the increment of the gait variability in elderly adults. We assume that attention demanding task based on working memory has the most influence on the interference between cognitive and gait function. Understanding the changes during dual task gait in older ages would be helpful for physical intervention strategies and improved risk assessment.
This study was conducted to investigate the effects of kinesio taping in patients with degenerative arthritis. 40 female patients with degenerative arthritis were voluntarily recruited and randomly divided into experimental and control group. Experimental group(mean age was 73.4 years, height was 153.5cm, and weight was 59.9kg) was applied patellar inferior and medial gliding, quadriceps, tibial external rotation, gastrocnemius, and soleus using a kinesio tape. And control group(mean age was 72.7 years, height was 154cm, and weight was 58.6kg) was received a interferential current therapy(ENDOMED 582, Enraf-Nonius, Holland). And we received a consent form from all subjects. Measuring was conducted by using a 3-dimensional gait analysis system for measuring of stride length, cadence, and velocity. The results were as follow: Stride length was significantly increased after than before treatment(p<.05). Cadence was significantly increased after than before treatment(p<.05). And Velocity was significantly increased after than before treatment(p<.05). Cadence was a significant difference between experimental and control group(p<.05), but there was no significant difference stride length and velocity between two groups. So we suggest that kinesio taping has a effectiveness in patients with degenerative arthritis.
The purpose of this study was to determine the effects of FES on the clinical test patients who had disturbance gait due to cereberal vascular accident. The subjects for study were 16 hemiplegic patients (9 Male and 7 Female) who can gait with or without device. Their average age was 55 and they received average of 20.19 month of treatment collected data analysis was completed by using one-way analysis variable(ANOVA), Pearson ($-1{\leq}r{\geq}1$). The results were as follows : 1) There was difference in four variable (stride length, gait speed, gait cadence) between at the biginning and at the end of the treatment of FES (p<0.01). 2) There was relationship in capacity of activity between MAS and stride length (r = 0.751), gait speed (r = 0.689) but no relationship gait cadence (r = 0.236). 3) Age revealed relationship of stride length (r = -0.727), gait speed (r = -0.725), gait cadenc (r = -0.362). 4) There was no relationship in months post-CVA with MAS (r = 0.171), stride length (r = -0.110), gait speed (r = -0.096), gait cacedce (r = -0.154).
Purpose: The purpose of this study was to investigate the effects of three-dimensional virtual reality horse riding simulator training using a head-mounted display on gait and balance in children with cerebral palsy. Methods: Ten children with cerebral palsy were randomly assigned to the horse riding simulator (HRS) group (n=5) or the horse riding simulator with virtual reality (HRSVR) group (n=5). To evaluate balance, center of gravity (COG) sway velocity and total sway distance of each group were assessed using the Wii balance board, and gait speed and stride length of each group were assessed using a gait analysis system. Results: Intra-group comparisons between pre- and post-intervention measures revealed that there were significant changes in all gait and balance variables such as stride length, gait velocity, COG sway velocity and COG sway distance in the HRSVR group (p<0.05). In the HRS group, there were significant changes in all variables except stride length (p<0.05). In addition, inter-group comparisons showed significant differences between the two groups in stride length, gait velocity and COG sway distance except COG sway velocity (p<0.05). Conclusion: The findings of this study suggest that horse riding simulator training combined with 3D virtual reality can be a new positive therapeutic approach for improving functional performance in children with cerebral palsy.
Purpose : We investigated the effect of anti-flexion taping at the proximal interphalangeal joints on the gait in stroke patients with clawing toe. Methods : Nineteen patients (mean age $63.26{\pm}9.16$ years) with clawing toe were studied. Gait performance was measured under two different conditions : (1) non-tape (2) application of tape. Gait velocity, step time and stride length were examined with the GAITRite system Results : Compared to the non-tape control condition, step time of the hemiparetic side increased significantly after the application of tape(p=0.03). There was no significant mean differences between the taped and control conditions for stride length of the hemiparetic side and velocity. Conclusion : Whilst the anti-flexion tape at the proximal interphalangeal joints changed the walking by providing significant step time effect, positive changes were noted in stride length of the hemiparetic side after tape application. These findings indicate that anti-flexion tape procedures do not significantly alter stride length of the hemiparetic side and velocity.
The purpose of this study was to analyze the effects of after aquatic exercise on gait in the Falls Experienced elderly. There were one group : Fall Experienced Elderly Women(n=8). They were tested on their gait (Elapse time of each phase, Stance time of limb, Stride length, Velocity of segment). we took video and analyzed their movement using Ariel Performance Analysis System and compared gait parameters. For data analysis, mean and standard deviation scores were calculated, and correspondence sample t-test and pearson's correlation analysis were used. First, after exercise is short than before exercise on Elapse time of each phase, fall-experience subjects showed meaningful total time. Second, after exercise is short than before exercise on Stance time of limb and Stride length, fall-experience subjects showed meaningful Stride length. Third, after exercise is fast than before exercise on Velocity of segment.
Purpose : The aim of this study was to determine the effect of Proprioceptive Neuromuscular Facilitation (PNF) on gait ability in hemiplegic patients. Method : The subjects of this study were 11 hemiplegic patients. Each subject was taken PNF exercise with 3 times per week for 4 weeks. Pre- and post-intervention change in gait ability were measured using an 6m walking test, stride length, and step length. The data were analyzed using the paired t-test. Results : The results of this study showed significantly improvement in 6m walking test, stride length, and step length after intervention. Conclusion : These results suggest that the PNF coordination exercise is an effective way of improving gait ability for hemiplegic patients.
Purpose: This study examined the effects of backward walking training with task orientation on the functional walking ability of children with cerebral palsy. Methods: This study was a single-blinded, randomized controlled trial with a crossover design conducted at a single rehabilitation facility with cross-over to the other intervention arm following a two-week break. For a total of 12 children with spastic hemiplegia cerebral palsy, the forward walking training group (n=6) underwent training three times a week for three weeks, 40 minutes a day, and the backward walking training group (n=6) was also trained under the same conditions. To identify the functional walking ability, variables, such as the walking speed, stride length, and step length, were measured using a walk analyzer (OptoGait, Microgate S.r.l, Italy). Results: Both groups showed significant increases in walking speed, stride length, and step length (p<0.01). The backward walking group showed more significant improvement in the walking speed from pre- to post-test (p<0.05). The gait characteristics were similar in the two groups (stride length and step length) but the walking speed in the backward walking group showed a mean difference between the positive effects higher than the forward walking group. Conclusion: Task-oriented backward walking training, which was conducted on the ground, may be a more effective treatment approach for improving the walking functions of spastic hemiplegia children than forward walk training.
Purpose : The purpose of this research was to determine the effects of Proprioceptive Neuromuscular Facilitation(PNF) and Functional Electrical Stimulation(FES) of combined on gait ability in hemiplegic gait. Methods : The subjects of this study were 13 hemiplegic patients. Each subjects was taken PNF pattern and FES of combined with 5 times per week for 4weeks. Pre- and Post-intervention change in gait ability were measured using an Timed up and Go test, stride length of the affected side, step length of the affected side. The data were analyzed using the paired t-test. Results : The results of this study were showed significantly improvement in TUG, stride length of the affected side, step length of the affected side after intervention. Conclusion : These results suggest that the Proprioceptive Neuromuscular Facilitation(PNF) and Functional Electrical Stimulation(FES) of combined exercise is an effective way of improving gait ability for hemiplegic patients.
Purpose: This study investigated the influence of attention-demanding tasks on gait and measured differences in the temporal, spatial and kinematic characteristics between young healthy adults and elderly healthy adults. Methods: We recruited 16 healthy young adults and 15 healthy elderly adults in this study. All participants performed two cognitive tasks: a subtraction dual-task (SDT) and working memory dual-task (WMDT) during gait plus one normal gait. Using the LEGSys+ system, knee and hip-joint kinematic data during stance and swing phase and spatiotemporal parameter data were assessed in this study. Results: In the elderly adult group, attention-demanding tasks with gait showed a significant decrease in hip-joint motion during the stance phase, compared to the normal gait. Step length, stride length and stride velocity of the elderly adult group were significantly decreased in WMDT gait compared to normal gait (p<0.05). In the young adult group, kinematic data did not show any significant difference. However, stride velocity and cadence during SDT and WMDT gaits were significantly decreased compared to those of normal gait (p<0.05). Conclusion: We determined that attention-demanding tasks during gait in elderly adults can induce decreased hip-joint motion during stance phase and decreased gait speed and stride length to maintain balance and prevent risk of falling. We believe that understanding the changes during gait in older ages, particularly during attention-demanding tasks, would be helpful for intervention strategies and improved risk assessment.
