심근관류 SPECT에서 가역적인 병변을 보인 환자의 3차원 심장 SPECT/CTA 퓨전영상의 유용성 (Added Value of 3D Cardiac SPECT/CTA Fusion Imaging in Patients with Reversible Perfusion Defect on Myocardial Perfusion SPECT)

  • 목적: 관상동맥질환은 치료법이 개발되어 있는 매우 치명적인 질환이므로 빠른 진단과 치료방법의 결정 및 재발이나 앞으로의 위험성에 대한 평가가 중요하다. 최근에는 병태생리를 반영하는 심근관류 SPECT와 해부학적 영상인 CT의 퓨전 영상에 대한 기대가 증가되고 있다. 저자들은 심근관류 SPECT에서 가역적인 관류 변화가 있었던 환자에서 퓨전영상의 유용성에 대하여 후향적 분석을 하였다. 대상 및 방법: 다기관 공동 연구로 전국 5개 대학 병원에서 48명의 환자(남:녀=26:22, 나이: $63.3{\pm}10.4$ 세)를 대상으로 하였다. 환자들은 아데노신 약물부하로 Tc-99m 세스타미비 혹은 테트로포스민을 이용하여 휴식기 및 부하기 심근관류 SPECT와 64채널 다중검출 CT를 이용하여 CTA를 시행하였으며, 소프트웨어(CardIQ, GE healthcare)로 두 영상을 퓨전하여 두 검사의 진단 정확도를 비교하였다. 결과: 48명의 환자를 대상으로 144개의 관상동맥에 대하여 분석하였다. 이들 중에서 단일 혈관 질환 환자가 17명, 두혈관 질환 환자가 15명, 세혈관 질환 환자가 11명, 유의한 협착이 없는 환자가 3명이었고, 2명의 환자에서 좌측 주관상동맥의 협착이 발견되었다. 좌측 주관상동맥에 협착이 있었던 2명을 제외하고, 관상동맥의 협착을 진단하는데에 있어 심근관류 SPECT는 민감도 68.8%, 특이도 70.7%이었으며, 퓨전영상은 각각 82.5%, 79.3%, 이었고, 두 검사의 진단 정확도에는 유의한 차이가 있었다(p<0.05). 퓨전영상에서는 좌측 주관상동맥의 협착을 포함하여 전체 82개의 협착 혈관 중 68개의 협착을 진단하였고, 심근관류 SPECT만으로 분석할 때보다 11개의 협착 병변을 추가로 확인할 수 있었다. 또한 애매한 관류저하를 보였던 3개의 관류저하 부위를 허혈 병변에서 제외할 수 있었으며, 4개의 병변은 관류저하를 유발하는 관상동맥을 수정하였다. 3개의 허혈 병변에 대하여서는 허혈 관련 병변 분절을 정확하게 국소화할 수 있었다. 결론: 심근 관류 SPECT와 CTA의 3차원 퓨전 영상은 관상동맥 질환의 병태생리를 포괄적으로 이해할 수 있도록 도움을 주고, 진단의 정확도를 높일 수 있을 것으로 사료된다.

감압법을 이용한 가스 생산 시 하이드레이트 부존 퇴적층의 지반 안정성 및 가스 생산성에 대한 시추 공저압 및 감압 속도의 영향 (Effect of Bottom Hole Pressure and Depressurization Rate on Stability and Gas Productivity of Hydrate-bearing Sediments during Gas Production by Depressurization Method)

  • 선행 연구를 통해 우리나라 동해 울릉 분지에 천연가스 자원인 하이드레이트 부존 퇴적층의 존재가 확인되었다. 퇴적층에서 가스를 생산하기 위한 시도는 세계적으로 연구되고 있으며, 생산 메커니즘은 열-수리-역학적 현상이 동시에 발생하는 복합적인 현상이다. 하이드레이트의 생산성 및 안정성 평가는 실험실 규모로 수행되기에는 어려움이 있다. 따라서, 가스 하이드레이트의 생산성 및 퇴적층의 안정성 평가를 위해서는 전산 수치 해석이 필수적으로 수행되어야 한다. 이 연구에서는 여러 가지 가스 하이드레이트 생산 방법 중 감압법을 이용한 생산 시 목표 공저압 및 감압속도에 따른 하이드레이트 퇴적층의 안정성과 가스 생산성에 대한 영향을 전산 모사 해석을 통해 분석하였다. 연구결과 목표 공저압이 낮을수록 생산성은 향상되고 안정성은 악화되는 것을 확인하였고, 감압 속도는 가스 생산성 및 퇴적층의 안정성에 큰 영향을 미치지 않는 것을 확인할 수 있었다. 추가적으로 실제 시험 생산 시 발생할 수 있는 사질생산 현상에 대한 대응 전략 수립을 위해 감압 속도 조절에 따른 가스 생산성 및 퇴적층의 안정성 평가 해석을 수행하였다. 해석 결과 낮은 감압 속도에서 높은 감압 속도로 변경 시킬 경우 안정성 확보에 효과가 있음을 확인하였다. 또한, 가스 생산 시 하이드레이트 해리로 인한 퇴적층의 침하가 발생하고 시추 생산정 하부에서는 압력 구배로 인해 지반 융기가 발생하는 것을 확인하였다. 이 결과를 통해 감압법을 활용한 가스 생산 시 목표 공저압 선정을 위해서는 생산성 및 안정성에 대한 고려가 동시에 수행되어야 하며, 지반 변위로 인해 생산 시추공에 발생하는 응력에 대한 고려도 필수로 수행되어야 한다는 결론을 얻었다.

겨울철 염화칼슘(CaCl2) 처리에 따른 가로변 3가지 상록 관목류의 생육 및 생리반응 - 사철나무, 영산홍, 회양목을 중심으로 - (Growth and Physiological Response of Three Evergreen Shrubs to De-icing Salt(CaCl2) at Different Concentrations in Winter - Focusing on Euonymus japonica, Rhodoendron indicum, and Buxus koreana -)

  • 본 연구는 도심 내에서 공원 및 가로수 주변에 식재된 대표적인 세 가지 상록 관목류인 사철나무(Euonymus japonica), 영산홍(Rhododendron indicum), 회양목(Buxus koreana) 등을 중심으로 겨울철 염화칼슘($CaCl_2$) 처리에 따른 생육 및 생리적 반응을 살펴봄으로써, 내염성 정도와 제설제 피해지역에 대한 내성수종을 선별하기 위한 자료로 제시하고자 한다. 염화칼슘을 각각 0%(대조구), 0.5%, 1.0%, 3.0%, 5.0% 처리한 실험구에 2014년 11월에 정식한 후 이듬해 이른 봄인 2015년 3월에 수고, 엽장, 엽폭, 엽형지수, 엽수, 생체중, 건조중, 건물률, R/T율, 엽록소함량, 엽록소형광반응, 광합성률, 기공전도도, 증산율 등의 생육 및 생리적 특성을 조사하였다. 사철나무는 3.0% 이하, 영산홍과 회양목은 1.0% 이하의 처리구에서 생존이 가능하였으며, 사철나무가 영산홍과 회양목에 비해 생육적 감소율이 비교적 낮았다. 염화칼슘 처리농도가 높을수록 대조구에 비해 생리적 반응들이 감소되는 것은 동일하나, 영산홍과 회양목은 감소세가 비교적 뚜렷한 반면, 사철나무가 완만한 감소세를 보이고 있다. 특히, 처리농도가 높아짐에 따라 영산홍은 증산율을, 회양목은 광합성율과 기공전도도를 억제시키는 경향을 보였다. 이러한 결과를 통해 사철나무가 영산홍과 회양목에 비해 염해에 좀 덜 민감한 수종임을 알 수 있어 제설제 피해지역에 상록 관목류로 적용이 가능할 것으로 본다.

GIS 기법을 이용한 지형적 특성에 따른 월악산 소나무 연륜생장의 이질성 규명 (Analysis of Heterogeneous Tree-Ring Growths of Pinus densiflora with Various Topographical Characteristics in Mt. Worak Using GIS)

  • 소나무 직경생장과 기후인자(월 평균기온과 총강수량)와의 관계를 지형적 특성에 따라 분석하기 위하여 월악산국립공원에서 선정한 20개의 임분에서 각각 10여 본의 임목에 대한 연륜을 측정하였다. 각 연륜계열들은 크로스데이팅 한 후, 임령과 임분동태에 따른 임목생장 추세를 제거하기 위하여 표준화함으로써 임분별 연륜연대기를 작성하였다. 연륜의 생장경향을 이용한 집락분석의 결과 20개 임분을 4개의 집락으로 분류할 수 있었다. 집락 Ⅰ 의 사면 방향은 북쪽이었으나, 다른 집락들은 대부분 남족과 남서쪽이었다. 고도는 집락Ⅰ(1개 임분), 집락Ⅱ(10개 임분), 집락Ⅲ(2개 임분)이 305∼580 m이었으나, 집락Ⅳ(7개 임분)는 다른 집락들보다 높은 450~870 m이었다 이중 집락Ⅱ는 다른 집락보다 토심이 얕은 급경사의 암석지에 위치하였다. 지형에 따른 기후인자들과 연륜생장과의 관계를 분석하기 위해 반응함수를 집락별로 실시하였다. 집락Ⅰ은 북사면의 다소 중습한 지역에 위치하여 다른 집락보다 기후인자의 영향을 많이 받지 않았다. 집락 Ⅱ는 강수가 임목생장을 제한하는 주요인으로 나타났는데, 이는 집락 Ⅱ가 낮은 고도에 위치하면서 토심이 얕은 급경사의 암석지에 위치하여 수분에 대한 임목생장의 민감도가 증가한 것이라 생각된다. 집락Ⅲ과 집락Ⅳ는 집락Ⅱ에 비하여 임목생장 개시 이전의 겨울과 이른봄의 기온이 보다 중요한 인자로 나타났다. 이런 결과는 고도 상승에 따른 기온 감소(집락Ⅳ)나 계곡부의 미소지형적 특성에 따른 온도 저하(집락Ⅲ)로 발생되는 결과라 생각된다. 이상의 결과는 GIS를 이용하여 수치화 하여 연륜과 지형이 갖는 시-공간적 정보들을 동시에 분석한 결과이다.

헤데라 헬릭스 식물의 적외선 열영상에 의한 저온 및 한풍피해에 관한 연구 (An Analysis Method on Injury Symptoms Utilizing Infrared Thermal Imaging under the Freezing Stress of Hedera helix L.)

  • Hedera helix의 저온에서의 피해 및 생육진단을 위하여 적외선서모그라피에 의해 측정한 적외선 열영상으로 부위별 체표면온도를 해석하였다. 노지재배품 첨단부와 기부의 엽신에 대한 실험설정온도 $-6^{\circ}C$구와 $-12^{\circ}C$구의 온도분포를 각각 비교해보았다. 결과 노지재배품 첨단부의 $-6^{\circ}C$실험구에서의 엽면온도는 $-2^{\circ}C{\sim}-7^{\circ}C$의 분포를, $-12^{\circ}C$실험구에서는 $-2^{\circ}C{\sim}-15^{\circ}C$까지의 범위에 분포하고 있었다. 한편 노지재배품 기부의 엽면온도는 $-6^{\circ}C$실험구에서의 엽면온도는 $-2^{\circ}C{\sim}-11^{\circ}C$의 분포를, $-12^{\circ}C$실험구에서는 $-1^{\circ}C{\sim}-12^{\circ}C$까지의 범위에 분포하고 있었다. 이를 보면 $-6^{\circ}C$실험구에서는 첨단부쪽이, $-12^{\circ}C$실험구에서는 기부쪽이 보다 광범위한 온도분포를 보여 주었다. 이는 대체적으로 기부 쪽보다는 첨단부 쪽의 엽신이 저온감수성이 높은 것을 의미한다. 한풍피해를 규명하기 위하여 노지재배품의 평균엽면온도를 측정한 결과 $0^{\circ}C$실험구에서 $-6.2^{\circ}C$, $-2^{\circ}C$실험구에서 $-6.8^{\circ}C$, $-4^{\circ}C$실험구에서 $-7.5^{\circ}C$를 나타내어 각각의 설정온도(공기온도) 보다도 $3.5{\sim}6.2^{\circ}C$ 저온이었으며 부위별로 온도가 다른 점으로 보아 저온감수성이 상이한 것으로 추론하였다.

감쇠보정, 산란보정 및 해상도복원이 정량적 심근 SPECT의 관상동맥질환 진단성능에 미치는 효과 (Effect of Attenuation Correction, Scatter Correction and Resolution Recovery on Diagnostic Performance of Quantitative Myocardial SPECT for Coronary Artery Disease)

  • 목적: 심근 SPECT 영상의 재구성 과정에 감쇠보정, 산란보정 및 해상도복원 시스템(ASCRR)이 적용되었을 때 관상동맥질환 진단의 진단능 변화를 정량적 심근 SPECT에서 조사하였다. 대상 및 방법: 관상동맥질환이 의심되는 환자 75명과 관상동맥질환 저위험군 20명의 환자에 대하여 휴식기 Tl-201/부하기 Tc-99m-MIBI 게이트 심근 SPECT를 시행한 후, ASCRR을 적용하여 적용 전후의 예민도 및 특이도와 정상검출율의 변화를 구하였다. 결과: ASCRR 적용 후, 전반적인 진단능에는 유의한 변화가 없었다. 혈관 영역 별로는 좌전하행동맥(LAD)이나 좌회선동맥(LCX) 영역의 병변에서는 유의한 차이는 없었고, 우측관상동맥(RCA) 영역의 병변을 진단하는 데 있어서는 ASCRR의 적용 후 특이도는 유의하게 향상되었지만, 이와 더불어 예민도는 유의하게 감소되었다(모두 p<0.05). 정상검출율은 좌전하행동맥(LAD) 영역과 좌회선동맥(LCX) 영역에서는 ASCRR 적용 전후에 유의한 차이가 없었고, 우측관상동맥(RCA) 영역에서만 유의한 증가를 보여, 전반적으로는 통계적으로 유의한 차이가 없었다. 결론: 심근 SPECT에서 감쇠보정 및 산란교정, 그리고 해상도복원시스템의 적용은 관상동맥질환의 진단에 있어서 우측관상동맥(RCA) 영역의 정상검출율 및 특이도는 증가시키지만, 전반적인 진단능은 향상시키지 못한다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms.
