화병 환자의 심박변이도 차이에 따른 황련해독탕의 효과: 화병에 대한 무작위 대조 비교임상시험 결과의 추가 분석 (Role of the Baseline Heart Rate Variability to the Effect of the Huanglian-Jie-Du Granule in Hwa-Byung Patients: Supplementary Analysis from the Randomized Trial Comparing Huanglian-Jie-Du Granule and Placebo for Hwa-Byung)

  • 화병은 뚜렷한 스트레스 사건과 관련되어 가슴의 답답함, 열감, 치밀어 오름, 억울하고 분한 감정 등이 나타나는 증후군이다. 본 논문에서는 화병에 대한 황련해독탕의 임상시험 결과를 교감신경의 활성도를 반영한다고 알려져 있는 심박변이도의 저주파(0.04~0.15 Hz) 영역 값을 중심으로 분석하여 다음과 같은 결과를 얻었다. 첫째, 황련해독탕 시험군에서 초기 심박변이도 저주파 값이 클수록 불면 증상의 개선 정도가 작게 나타났다. 둘째, 위약과 비교했을 때, 황련해독탕은 복용 7일 후 심박변이도 저주파 값을 유의하게 감소시켰다. 셋째, 화병 증상 중 가슴이 답답함, 치밀어 오름, 분노가 심박변이도 저주파 값과 유의미한 상관성이 있었다. 이를 바탕으로 심박변이도는 화병 환자를 평가할 때 유용할 것이라고 기대된다.

만성 스트레스로 유발된 우울증 쥐 모델에서 캡사이신의 항우울 효과 (Antidepressant effects of capsaicin in rats with chronic unpredictable mild stress-induced depression)

  • Purpose: This study was conducted to assess the antidepressant effects of capsaicin in chronic depressive rats and elucidate the mechanism underlying its effects. Methods: Male Wistar rats (280~320 g, 8 weeks of age) were subjected to depression induced by chronic unpredictable mild stresses. The rats were exposed to 8 kinds of stresses for 8 weeks. In the last 2 weeks, fluoxetine or capsaicin was injected subcutaneously. The dose of fluoxetine was 10 mg/kg (body weight), while the doses of capsaicin consisted of low (1 mg/kg), middle (5 mg/kg), and high (10 mg/kg). The forced swim test (FST) was conducted to evaluate the immobility time of rats. The immobility time indicates despair, one of symptoms of depression. The change of tryptophan hydroxylase (TPH) in the dorsal raphe was investigated using immunohistochemistry. In the hippocampus cornu ammonis (CA) 1 and 3, glucocorticoid receptor (GR) expression was measured. Results: The immobility time in the FST was significantly lower (p < .05) in the low-dose (M = 32.40 ± 13.41 seconds) and middle-dose (M = 28.48 ± 19.57 seconds) groups than in the non-treated depressive rats (M = 90.19 ± 45.34 seconds). The amount of TPH in the dorsal raphe was significantly higher (p < .05) in the middle-dose (M = 249.17 ± 35.02) and high-dose (M = 251.0 ± 56.85) groups than in the non-treated depressive rats (M = 159.78 ± 41.16). However, GR expression in the hippocampus CA1 and CA3 did not show significant differences between the non-treated depressive rats and the capsaicin-injected rats. Conclusion: This study suggests that capsaicin produces an antidepressant-like effect on chronic unpredictable mild stress-induced depression in rats via the serotonin biosynthesis pathway.

PTSD 환자의 피해 사고 및 피해 사고에 기여하는 심리학적 요인 (Idea of Persecution and Psychological Factors Associated With Idea of Persecution in Patients With PTSD)

  • 연구목적 본 연구는 외상 후 스트레스 장애 환자(Post-traumatic stress disorder, PTSD) 중 Clinician-Administered PTSD Scale (CAPS) Minnesota Multiphasic Personality Inventory-2 (MMPI-2)를 시행한 환자들을 대상으로 피해 사고에 영향을 미치는 요인을 확인하고자 한다. 방 법 2013년 5월부터 2020년 4월까지 중앙보훈병원에서 CAPS와 MMPI-2를 시행한 116명의 환자를 후향적으로 검토하였다. CAPS 점수를 기준으로 하여 환자들은 PTSD군(n=63, 연령: 58.16±17.84)과 외상 노출이 있는 비PTSD군(n=53, 연령: 67.34±12.05)으로 나뉘었다. 피해사고, CAPS 및 MMPI-2의 척도들 간의 상관 관계를 확인한 후 임상적으로 관련된 위험 요인을 확인하기 위해 선형 회귀 분석을 시행하였다. 결 과 PTSD 군은 외상 노출이 있는 비PTSD 군에 비해 MMPI-2에서 정신분열증, 피해 사고, 역기능적 부정적 정서, 기태적 경험, 정신증, 부정적 정서성/신경증, 불안, 우울, 분노 척도에서 유의한 차이를 보였다. 피해 사고와 CAPS, MMPI-2의 상관 관계를 보았을 때, 피해 사고와 MMPI-2 항목들 간에는 통제 결여 항목을 제외하고 강한 관련성을 보였다. PTSD 군에서 다중선형회귀분석을 통해 역기능적 부정적 정서 및 분노 척도가 피해 사고의 위험 요인임을 확인하였다. 결 론 PTSD 군은 외상 노출이 있는 비 PTSD 군에 비해 피해 사고가 높게 나타나고, 역기능적 부정적 정서 및 분노 척도가 피해 사고의 위험 요인일 수 있어 이에 대해 평가하고 적절한 치료를 제공하는 것이 필요하다.

한국형 마음 챙김 명상에 기반한 스트레스 감소 프로그램이 만성통증에 미치는 효과 (The Effect of Korean version of Mindfulness-Based Stress Reduction Program on Chronic Pain of Workers.)

  • 본 연구의 목적은 한국형 마음 챙김 명상에 기반한 스트레스 감소 프로그램이 근로자의 만성통증완화에 미치는 영향을 연구 하는데 있다. 이러한 목적을 달성하기 위하여 만성통증을 가진 근로자 20명을 대상으로 프로그램의 효과를 알아보기 위해 사전 검사, 사후 검사, 추수 검사에서 측정한 평균점수로 혼합변량분석(Mixed ANOVA)을 실시하였다. 분석된 자료는 심리적, 신체적, 신경 생리적 반응이었다. 아울러 프로그램 후 각 효과가 지속적으로 유지될 것인가를 추가 검증하고자 3개월 후에 사후검증도 실시하였다. 본 연구 결과, 프로그램 참여 집단의 신체적 증상과 신체화, 강박증, 대인예민성, 우울, 불안, 적대감, 공포불안, 정신증 등의 반응이 프로그램 참여 전에 비해 유의미하게 감소하였고, 또한 생리 및 신경생리지표 또한 프로그램 전에 비해 유의미한 변화가 나타났으며, 3개월 후까지 이러한 변화의 효과가 유지되었다. 본 연구의 결과로 보아 마음 챙김 명상에 기반한 스트레스 감소 프로그램은 통증환자의 심리적, 신체적, 신경생리적 통증완화에 유의미한 영향력을 미치는 것으로 밝혀졌다. 따라서 본 연구결과를 통해 만성통증환자의 임상적 프로그램 개발의 기초자료를 제공하고자 한다.

호흡기 증상 없이 발생한 뇌 지방색전증 1례 (Fat Embolism Syndrome Which Induced Significant Cerebral Manifestation Without Respiratory Distress)

  • Fat embolism syndrome is a collection of respiratory, neurological and cutaneous symptoms and signs associated with trauma and other disparate surgical and medical conditions. The incidence of clinical syndrome is low while the embolization of marrow fat appears to be an almost inevitable consequence of long bone fractures. The pathogenesis is a subject of conjecture and controversy. There are two theories which have gained acceptance(mechanical theory, biochemical theory). Onset of symptom is usually within 12 to 72 hours, but may manifest as early as 6 hours to as late as 10 days. The classic triad of fat embolism syndrome involves pulmonary changes, cerebral dysfunction and petechial rash. The cornerstone of treatment is preventing the stress response, hypovolemia and hypoxia and operative stabilization of fractures. Corticosteroid are the only drugs which have repeatedly shown a positive effect on the prevention and treatment of fat embolism syndrome. We report a case of post-traumatic fat embolism syndrome with severe cerebral involvement without respiratory distress. A 55 years old female had a traffic accident. She sustained pelvic bone fracture and both humerus fracture. Approximately 4 hours after the accident, mental status change developed without a focal neurologic deficits. She had no respiratory symptom and sign. Her brain MRI showed multiple cerebral fat embolism lesion. The patients received supportive treatment with corticosteroid, albumin. Her neurologic status stabilized over several days. After orthopedic surgery, she was discharged 62 days after admission.

Acquisition of Thermotolerance in Transgenic Orchardgrass Plants with DgHSP17.2 Gene

  • To develop transgenic orchardgrass (Dactylis glomerata L.) resistant to high temperature, the recombinant DgHSP17.2 gene was introduced into orchardgrass plants using the Agrobacterium-mediated transformation method and expressed constitutively under the control of the CaMV 35S promoter. The results of genomic DNA PCR and Southern analysis showed a DNA band and hybridization signal on agarose gel and X-ray film in transgenic orchardgrass plants harboring the recombinant DgHSP17.2 gene, but a DNA band and hybridization signal were not observed in the wild type and empty vector control plants. The same result was also obtained in RT-PCR and Southern blot analysis, and these transgenic orchardgrass plants did not show any morphological aberration both in the culture bottle and soil mixture. When leaf discs cut from transgenic orchardgrass plants with recombinant DgHsp17.2 gene were exposed to lethal temperature (heat treatment at $60^{\circ}C$ for 50 min), 60-80% of the leaf discs showed only damage symptoms, but non-transgenic leaf discs showed a lethal condition. These results indicate that the DgHsp17.2 gene may act as a protector from heat stress in plants.

The Impact of Multiple Roles on Psychological Distress among Japanese Workers

  • Background: There has been considerable interest in Japanese society in the problem of work-related stress leading to depressive symptoms, and an increasing number of primary houseworkers maintain paid employment. The purpose of this study was to examine the differential impact of multiple roles associated with psychological distress among Japanese workers. Methods: We studied 722 men and women aged 18-83 years in a cross-sectional study. The K10 questionnaire was used to examine psychological distress. Results: The proportion of participants with psychological distress was higher in women (17.8%) compared with men (11.5%). Having three roles significantly decreased the risk of psychological distress [women: odds ratio (OR), 0.37-fold; men: OR, 0.41] compared with only one role. In working married women, there was significantly less psychological distress (OR, 0.27), and those with childrearing or caregiving responsibilities for elderly parents had significantly less psychological distress (OR, 0.38) than those with only an employment role. Similarly, working married men who had childrearing or caregiving responsibilities for elderly parents had significantly less psychological distress (OR, 0.41) than those who had only an employment role. Conclusion: The present study demonstrated that participants who had only an employment role had an increased risk of psychological distress. The degree of psychological distress was not determined solely by the number of roles. It is important to have balance between work and family life to reduce role conflict and/or role submersion, which in turn may reduce the risk of psychological distress.

뇌졸중 후 요실금이 일상생활에 미치는 영향 (Impact of Urinary Incontinence on Daily Life after Stroke)

  • Purpose: This study was performed to identify the prevalence and type of urinary incontinence (UI) after a stroke, to find the differences in urinary symptoms according to continence or incontinence, and to find the degree of impact of UI on daily life. Method: For data collection, we had a structured interview with a questionnaire. The subjects were 239 post stroke patients. Results: Among the subjects(mean age: $65{\pm}10$), 66.1 percent had an infarction, and 25.5 percent had a hemorrhage. And 26.4 percent of subjects were within 2 weeks and 28.9 percent from 1 year to 5 years since their episodes of a stroke. Forty five point six percent of subjects had various types of UI: urge 25.7 percent, stress 14.7 percent, functional 20.2 percent, and mixed 39.4 percent. There were significant differences in frequency, nocturia, decreased stream, and incomplete emptying between the incontinent and continent groups. Subjects reported UI influenced various aspect of daily life, 54.1 percent as cause of distress, 53.2 percent on overall quality of life, and 40.4 percent on sleep. The Mixed UI, including urge UI, had higher impact on daily life than others. Conclusion: There was a high prevalence of UI, mixed, urge, and functional type as most prevalent, and it had a strong impact on daily life of post stroke patients.

Role of dietary nucleotides to mitigate post-weaning stress in newly weaned pigs

  • This review provides an overview of dietary nucleotides as an alternative to in-feed antibiotics for weaning pigs. Dietary nucleotides are composed of DNA or RNA molecules and are normally contained in protein-rich feed ingredient, brewer's yeast, yeast extract, and milk. Weaning pigs are suffering from several stresses, such as environmental challenges (i.e. crowding, transportation, and feeding). Such stressors can damage the intestinal epithelium and cause an invasion by Escherichia coli, secondary inflammatory responses, and post weaning diarrhea. To overcome weaning disorder, people often use antibiotics which reduce symptoms and boost growth performance. However, since antibiotics were banned due to concerns of antibiotic resistant bacteria, researchers are studying alternative materials to antibiotics. Dietary nucleotides are one of the alternative materials for replacing antibiotics and can be used in abnormal conditions, such as weaning diarrhea, low digestibility, and disease condition. Nucleotides have substances that have important roles in cell division and cell growth, affecting growth performance, intestinal condition, and immunological effect at the weaning stage. However, nucleotides' composition is very different between sources and this aspect makes it difficult to utilize nucleotides at the weaning stage. Therefore, this review paper focuses on i) the characteristics and functions of dietary nucleotides and ii) the effect of dietary nucleotides on the growth performance and immune system of pigs.

동의보감에 수재된 오계(烏鷄)에 대한 생리활성 연구 (The Study on Biological Activities of Yeonsan Ogye listed on Dong-ui-bo-gam)

  • Objectives : The aim of this study is to investigate cell viability, anti-inflammatory, antioxidant, immunoenhancing activity using various extracts of Yeonsan Ogye.Methods : In order to evaluate cytotoxicity, MTT assay was performed. We investigated production levels of pro-inflammatory cytokines such as tumor necrosis factor (TNF)-αand interleukin (IL)-6, and nitric oxide(NO) in LPS-induced RAW 264.7 cells. NO production in RAW 264.7 cells was measured by using Griess reagent. Cytokines including IL-6 and TNF-αwere measured by Luminex and ROS was measured by Flow cytometry.Results : No cytotoxicity of various extracts of Yeonsan Ogye was observed in RAW 264.7 cells. Productions of ROS in RAW 264.7 cells were increased from extraction of bones and decreased from extraction of skin. Also, productions of NO in RAW 264.7 cells were increased to bone extract and decreased at skin extract. In addition, productions of pro-inflammatory cytokines (IL-6 and TNF-α) in LPS-induced RAW 264.7 cells were decreased at skin, meat extracts, respectively. Finally, the levels of immune-related cytokines (IL-6 and TNF-α) were increased compared to those of the normal group.Conclusions : It is concluded that Yeonsan Ogye extracts seem to have significant biological activities likes anti-inflammatory, antioxidant, immuno-enhancing etc. These results may be developed as a raw material for new health food and new therapeutics to ease the symptoms related with inflammatory and oxidative stress. In terms of oriental traditional medicine, we expect that it contribute to building of EBM (Evidence-Based Medicine) from the this result.
