골관절염이 동반된 고혈압 노인의 건강관련 삶의 질 영향 요인 (Factor Influencing Health-Related Quality of Life in Korean Hypertensive Seniors with Osteoarthritis)

  • 본 연구의 목적은 제 7기 국민건강영양조사 2018년 자료를 이용하여 65세 이상 고혈압 노인의 골관절염 동반에 따른 삶의 질 정도와 영향 요인을 확인하기 위함이다. 대상자는 전국 단위 표본에서 추출된 65세 이상 노인 843명이었다. SPSS WIN 26.0 프로그램을 이용하여 복합표본 Rao-Scott 카이제곱 검정, t-검정, 다중회귀분석으로 분석하였다. 분석 결과 고혈압군에 비해 골관절염 동반 고혈압군 삶의 질이 유의하게 낮았다(t=5.07, p<.001). 고혈압군의 삶의 질 저하에 영향을 미치는 요인은 연령, 주관적 건강상태, 활동제한이었으며 설명력은 46.1%였다(F=12.33, p<.001). 골관절염 동반 고혈압군은 성별, 배우자 동거상태, 경제활동, 주관적 건강상태, 활동제한, 체질량지수, 수면시간, 스트레스 인지, 우울이었으며 설명력은 44.6%였다(F=99.82, p<.001). 연구 결과에 따라 향후 고혈압 노인의 삶의 질 향상을 위하여 건강상태의 긍정적 수용 및 활동제한 최소화를 위한 중재프로그램이 요구된다. 골관절염 동반 고혈압 노인에게는 추가적으로 체질량지수 관리를 위한 운동 및 영양교육과 정신건강을 강화할 수 있는 다학제 참여 프로그램 개발과 중재가 필요하다.

치위생과 재학생의 건강증진행위에 관한 연구 -경기지역 치위생과를 중심으로- (A Study on the Health Promotion Behavior of Dental Hygiene Stdents: the case of Gyeonggi province)

  • 본 연구는 치위생과 재학생들의 건강증진행위를 조사하여 대학생의 건강증진을 도모하기 위한 기초자료를 제공하고자 경기도에 소재한 4개 대학의 치위생과 재학생 687명을 대상으로 2007년 9월부터 10월까지 설문조사를 실시하였다. SPSS 12.0 program을 이용한 분석한 결과는 다음과 같다. 1. 구강관리행위는 1학년 학생($3.11{\pm}.57$)이 가장 낮은것으로 조사되었으며, 통계적으로 유의한 차이를 보였다(p < .001). 2. 건강증진행위는 건강상태가 나쁘다고 인식하는 경우 영양관리($3.63{\pm}.63$), 사회적지지($2.43{\pm}.60$), 건강에 대한 책임($2.40{\pm}.68$), 자신의 삶에 대한 존중($2.33{\pm}.51$)에서 관리행위정도가 낮은 것으로 조사되었으며, 통계적으로 유의한 차이를 보였다(p < .001, p < 0.01). 3. 부모님의 교육정도에 따른 건강증진행위는 아버지가 중졸이하인 경우($3.62{\pm}.52$) 영양관리가 가장 낮았으며(p < .001), 어머니가 중졸인 경우($2.52{\pm}.52$) 사회적 지지가 낮은 것으로 조사되었으며, 통계적으로 유의한 차이를 보였다(p < .05). 4. 자취를 하는 경우 영양관리($3.66{\pm}.57$)가 가장 낮았으며, 통계적으로 유의한 차이를 보였다(p < .001). 5. 월평균 가계 수입이 100만원 이하에서($2.42{\pm}.62$) 삶의 존중행위가 가장 낮게 조사되었으며, 통계적으로 유의한 차이를 보였다(p < .05). 6. 건강증진행위와 관련 변인간의 상관관계는 건강증진행위가 사회적 지지가 높을수록 삶의 존중이 높았으며, 삶의 존중이 높을수록 스트레스 관리가 잘되고 있는 것으로 조사되었으며, 통계적으로 유의한 차이가 보였다(p < .01).

조선왕조실록과 해괴제등록 분석을 통한 황충(蝗蟲)의 실체와 방제 역사 (Biological Identity of Hwangchung and History on the Control of Hwangchung Outbreaks in Joseon Dynasty Analyzed through the Database Program on the Annals of the Joseon Dynasty and the Enrollment of Haegoeje)

  • 조선시대의 기록인 조선왕조설록DB와 국역 해괴제등록(國譯 解怪祭謄錄)을 통해 역사적인 해충인 황(蝗) 또는 비황(飛蝗)으로도 불리는 황충(蝗蟲)의 발생상을 분석하여 그 실체를 밝히고 황충의 방제사를 정립하고자 하였다. 황충발생기사 건수는 조선왕조실록 총 261건, 해괴제등록 65건이 있었다. 조선시대 전체를 통해서 황충의 발생기사 건수는 4차례의 피크가 있었는데, 조선초기인 태조-세종대에 가장 발생 빈도가 높았다. 조선왕조실록과 해괴제등록의 황충 기록을 비교한 결과, 해괴제등록 65건으로 조선왕조실록 37건에 비해 월등히 많았지만, 사건기록의 일치성은 낮았다. 조선시대 황충의 개념은 곡식해충뿐 아니라 소나무 등 산림해충까지 포함하는 것이었으며, 해괴제등록의 경우, 충재 중 80%가 황충으로 언급되어 있었다. 따라서 황의 개념은 Saigo (1916, 1937)의 주장이 합당한 것으로 판단되었다. 전체 황해(蝗害) 기사 261건 중 9.5%인 25건에서 황충의 분류학적 실체를 추적할 수 있었는데, 멸강나방 11건, 나방류 9건, 이화명나방 2건, 풀무치 2건, 벼멸구 1건, 벼물바구미 1건으로 Paik (1977)의 주장과 상당부분 일치하였으며, 황충=풀무치 또는 메뚜기류란 주장은 타당성이 낮았다. 조선왕조실록에서 황충 기사 중 출현보고는 173건으로 전체의 66%이고, 출현보고의 47%가 단순보고로서 황충발생의 보고 자체가 매우 중요했음을 의미했다. 황충의 방제대책은 발생보고건수의 20%(34건)로 낮았고, 주로 포획법이나 포제를 올렸으며, 단지 1건의 예방법이 세종대에 기사화 되었다. 황해로 인한 정책수행이나 계획의 변경 논의가 37건 있었는데, 주로 백성의 구휼이나 세금 감면이었고 군사훈련 중지, 축성 중지 등 백성 동원으로 인한 피해를 방지하고자 하였다. 또한 황충피해는 백성뿐 아니라 국왕에게 영향을 미쳐 성종의 경우 10건의 기사에서 황충방제의 스트레스를 언급하였으며, 정종 대에는 선위에 간접적 영향이 있음도 기록을 통해 확인할 수 있었다.

양돈장 관리시스템이 출하돈의 도체품질에 미치는 영향 (Effects of On-farm Management System on the Carcass Quality of Market Pigs)

  • 본 연구결과는 출하돈의 도체품질 향상과 품질인증 프로그램 구축을 위한 자료로 활용하기 위하여 2년 동안 53개 농장에서 출하된 248,787두에 대한 도체평가 결과를 모돈수에 의한 경영규모, 육성비육돈 단계의 사육밀도, 수송시간, 출하시점, 비육돈 후기사료 급여비율 및 비육돈사 형태에 따라 도체등급과 PSE 발생율, 도체 근간지방 점수, 근내지방 점수, 피하지방 점수 및 육탄력성 점수에 미치는 영향을 분석한 결과이며 요약하면 다음과 같다. 1. 양돈장 경영규모는 도체등급에 영향을 미치며, 경영규모가 커질수록 좋은 등급을 나타내었으나, PSE 출현율은 경영규모에 따른 차이가 인정되지 않았다. 도체 근간지방 점수, 근내지방 점수, 피하지방 점수 및 육탄력성 점수는 양돈장 경영규모에 따라 일정한 차이를 나타내지 않았다. 2. 육성비육돈사 사육밀도는 도체등급에 영향을 미치며, 중간 밀도에서 좋은 등급을 나타내었으나, PSE 발생율은 낮은 밀도에서 오히려 높게 나타났다. 도체 근간지방 점수와 근내지방 점수 및 육탄력성 점수는 중간 밀도에서 우수하게 나타났으나 피하지방 점수는 사육밀도에 의한 차이가 없었다. 3. 출하돈의 수송시간에 의한 도체등급 차이는 인정되지 않았으며, PSE 발생율 역시 수송시간에 의한 차이는 없었다. 그러나 수송시간은 도체 육질점수에 영향을 미치며, 1시간을 초과한 수송이 근간지방 점수와 근내지방 점수, 피하지방 점수 및 육탄력성 점수 모두 우수한 것으로 나타났다. 4. 출하시점은 출하후 계류를 통한 도축전 휴식과 관련되는데, 출하시점은 도체등급과 PSE 발생율에 영향을 미치지 않는 것으로 나타났다. 도체 근간지방 점수, 근내지방 점수 및 육탄력성 점수는 오전 10시 이전 출하가 우수한 것으로 나타났으나, 피하지방 점수는 차이가 없었다. 5. 비육돈 후기사료 급여비율 차이에 의한 도체등급 차이는 인정되지 않았으나, PSE 발생율은 후기사료 21% 이상 급여가 높은 것으로 나타났다. 도체 근간지방 점수와 근내지방 점수는 후기사료를 21% 이상 급여한 경우가 우수하게 나타났으며, 피하지방 점수는 후기사료를 20% 이내 급여가 우수하였으나, 육탄력성 점수는 비육후기사료 급여비율에 의한 차이는 인정되지 않았다. 6. 비육돈사 형태에 따른 도체등급의 차이는 무창돈사가 우수하게 나타났으나, PSE 발생율 또한 무창돈사가 높았다. 도체 근간지방 점수와 근내지방 점수는 비육돈사 형태에 의한 차이는 없었으나, 피하지방 점수와 육탄력성 점수는 개방돈사가 우수한 것으로 나타났다. 이상의 결과를 종합해 보면, 양돈장 관리시스템 및 도축전 취급이 출하돈의 도체등급과 육질점수에 영향을 미치는 구체적인 내용들을 확인할 수 있었다. 따라서 양돈장 관리시스템과 관련된 각종 환경요인과 관리요인 및 도축전 취급과 관련된 인자들에 대한 세심한 관리를 통하여 스트레스 최소화와 동물복지 향상으로 도체품질 개선과 소비자를 위한 품질인증 시스템을 구축하여야 할 것으로 사료된다.

저체중 독거노인의 질병과 건강행태 : 2014년 지역사회건강조사 자료를 중심으로 (Disease and Health Behavior of Low-Weight Elderly Living Alone : Focusing on the Community Health Survey 2014)

  • 본 연구는 독거노인의 질병과 건강행태의 파악을 통해 독거노인의 저체중에 영향을 미치는 요인을 규명하기 위한 조사연구이다. 2014년 지역사회건강조사 원시자료를 활용하였으며, 65세 이상 저체중 독거노인 922명의 자료를 최종분석에 사용하였다. 자료의 분석은 SPSS/WIN 22.0 프로그램의 복합표본분석 모듈을 이용하여, 기술통계, Rao Scott $x^2$ test, 로지스틱 회귀분석을 통해 분석하였다. 본 연구의 결과, 독거노인들은 만성질환을 가지고 있는 비율이 높았으며, 흡연, 음주, 운동량 부족, 짜게 먹는 식습관, 틀니사용에 의한 저작 불편, 나쁜 주관적 건강상태와 같은 좋지 않은 건강행태를 가지고 있는 것으로 나타났다. 로지스틱 회귀분석 결과 독거노인의 성별에 따른 저체중 발생 위험확률은 다음과 같다. 흡연으로 인한 저체중 위험도는 여자노인에서 가끔 피우는 경우 3.004배, 걷기량으로 인한 저체중 위험도는 3일미만 걷는 사람이 저체중일 가능성은 1.420배로 유의하게 높았다. 주관적 스트레스를 많이 느끼는 경우 저체중일 가능성은 남자노인에서 2.220배, 여자노인에서 1.282배로 유의하게 높았다. 주관적 건강수준이 나쁜 사람이 저체중일 가능성은 남자노인에서 3.633배, 여자노인에서 1.590배로 유의하게 높았다. 본 연구의 결과를 토대로 저체중 독거노인의 건강행태 개선을 위한 적절한 간호중재의 수립과 관리방안의 마련이 필요하다. 또한, 저체중 독거노인에 대한 신체적, 심리적, 사회적 특성 등의 다양한 변인을 고려한 반복연구가 필요할 것이다.

Systemic analysis 방법을 활용한 국내 학교급식 위생의 주요 영향 인자 분석 연구(2005-2014) (Systemic Analysis on Hygiene of Food Catering in Korea (2005-2014))

  • Systemic analysis 기법을 이용하여 국내 학교급식의 위해관리를 위한 정보를 제공하고자 학교급식 위생 관련 문헌분석을 실시하였다. 단체급식 및 위생 분야 키워드 47개를 도출한 후, DBpia 검색엔진을 통하여 도출된 키워드를 입력하여 최근 10년간(2005-2014) 생산된 총 1,177개 논문을 검색하였다. 이후 관련논문을 수집하고 전문가리뷰를 통하여 최종 142개 논문을 선정하였다. 시설별, 이해당사자별, 외부요인, 내부요인, 직접적 요인, 간접적 요인으로 나눠 문헌을 분석하였다. 시설별로 학교(64편)가 산업체(5편)나 병원(3편)보다 단체급식 위생 관련문헌이 많았다. 학교급식의 주요요인을 분석한 결과 시스템/시설/설비(15편), 위생교육(12편), 생산/납품업체(6편), 식재료(4편), 복합적 요인(9편)이었다. 학교급식 위생관련 요인을 환경적 요인, 인적 요인, 식재료 요인, 고용 및 직무요인으로 구분하였다. 이해당사자별로 영양사, 조리종사원, 학생, 교직원, 식재료 납품업체 등으로 구분하여 분석하였다. 첫 번째, 환경적 요인으로 시설 설비 영역과 시스템 영역으로 구분하였다. 시설 설비 영역에서 대상이 영양사인 경우 급식시설의 명확하지 않은 구획 및 구분, 다량조리기기의 부족으로 인한 음식 온도관리 미흡, 위생관리를 위한 기기 구비율 저조, 조리실 온 습도 관리 미흡 등이 문제점으로 지적되었다. 대상이 학생인 경우에는 교실배식 환경을 문제로 꼽았다. 시스템 영역에서는 영양사가 대상인 경우 학교 내 구성원 간 HACCP 시스템 팀의 낮은 협력정도가 문제로 지적되었다. 조리사/조리종사원이 대상인 경우에는 과도한 업무량과 높은 노동 강도, 급식소 안전 관련 근무 조건의 열악함이 문제가 되었다. 학부모를 대상으로 조사한 결과에서는 학교급식 모니터 제도의 활동이 저조한 것이 문제로 파악되었다. 두 번째, 인적 요소 측면에서는 "위생교육 부족"이 가장 큰 문제점으로서, 교육대상이 조리종사원인 경우 형식적인 위생교육, HACCP 관련 교육 미흡, 낮은 개인위생관리수행이 문제가 되었다. 대상이 학생인 경우에는 위생교육 경험이 적고, 위생교육의 적용 및 효과가 낮은 것으로 나타났다. 세 번째, 식재료 요소 측면에서 원재료 자체의 위생문제와 신선편이식품 전처리 식재료의 불신이 문제로 파악되었다. 한편, 생산 납품업체 관련 납품업체 배송차량 관리 미흡, 생산 납품업체 직원의 위생관리, HACCP 비인증업체의 위생관리가 문제로 지적되었다. 마지막으로 고용 및 직무 요소 측면에서 영양사와 조리종사원의 고용형태, 연봉수준은 직무만족도 및 직무몰입도에 영향을 주는 것으로 나타났으며, 직무스트레스는 직무수행, 나아가 학교급식 위생에도 영향을 줄 수 있다는 것이 확인되었다. 학교급식 위생의 원인 분석을 통하여 향후 정부는 예산확보, 현장조사, 인증시스템 제도 마련, 근무 조건 개선, 위생훈련 및 점검 강화, 전문가에 의한 위생컨설팅 및 급식장 설계 컨설팅을 강화할 필요가 있다. 본 연구는 향후 급식위생안전 개선을 위한 교육 자료 및 관련 기술 개발에 활용할 수 있다.

병원 간호사의 선호근무시간대에 관한 연구 (A Study on Hoslital Nurses' Preferred Duty Shift and Duty Hours)

  • The duty shifts of hospital nurses not only affect nurses' physical and mental health but also present various personnel management problems which often result in high turnover rates. In this context a study was carried out from October to November 1995 for a period of two months to find out the status of hospital nurses' duty shift patterns, and preferred duty hours and fixed duty shifts. The study population was 867 RNs working in five general hospitals located in Seoul and its vicinity. The questionnaire developed by the writer was used for data collection. The response rate was 85.9 percent or 745 returns. The SAS program was used for data analysis with the computation of frequencies, percentages and Chi square test. The findings of the study are as follows: 1. General characteristics of the study population: 56 percent of respondents was (25 years group and 76.5 percent were "single": the predominant proportion of respondents was junior nursing college graduates(92.2%) and have less than 5 years nursing experience in hospitals(65.5%). For their future working plan in nursing profession, nearly 50% responded as uncertain The reasons given for their career plan was predominantly 'personal growth and development' rather than financial reasons. 2. The interval for rotations of duty stations was found to be mostly irregular(56.4%) while others reported as weekly(16.1%), monthly(12.9%), and fixed terms(4.6%). 3. The main problems related to duty shifts particularly the evening and night duty nurses reported were "not enough time for the family, " "afraid of security problems after the work when returning home late at night." and "lack of leisure time". "problems in physical and physiological adjustment." "problems in family life." "lack of time for interactions with fellow nurses" etc. 4. The forty percent of respondents reported to have '1-2 times' of duty shift rotations while all others reported that '0 time'. '2-3 times'. 'more than 3 times' etc. which suggest the irregularity in duty shift rotations. 5. The majority(62.8%) of study population found to favor the rotating system of duty stations. The reasons for favoring the rotation system were: the opportunity for "learning new things and personal development." "better human relations are possible. "better understanding in various duty stations." "changes in monotonous routine job" etc. The proportion of those disfavor the rotating 'system was 34.7 percent. giving the reasons of"it impedes development of specialization." "poor job performances." "stress factors" etc. Furthermore. respondents made the following comments in relation to the rotation of duty stations: the nurses should be given the opportunity to participate in the. decision making process: personal interest and aptitudes should be considered: regular intervals for the rotations or it should be planned in advance. etc. 6. For the future career plan. the older. married group with longer nursing experiences appeared to think the nursing as their lifetime career more likely than the younger. single group with shorter nursing experiences ($x^2=61.19.{\;}p=.000;{\;}x^2=41.55.{\;}p=.000$). The reason given for their future career plan regardless of length of future service, was predominantly "personal growth and development" rather than financial reasons. For further analysis, the group those with the shorter career plan appeared to claim "financial reasons" for their future career more readily than the group who consider the nursing job as their lifetime career$(x^2$= 11.73, p=.003) did. This finding suggests the need for careful .considerations in personnel management of nursing administration particularly when dealing with the nurses' career development. The majority of respondents preferred the fixed day shift. However, further analysis of those preferred evening shift by age and civil status, "< 25 years group"(15.1%) and "single group"(13.2) were more likely to favor the fixed evening shift than > 25 years(6.4%) and married(4.8%)groups. This differences were statistically significant ($x^2=14.54, {\;}p=.000;{\;}x^2=8.75, {\;}p=.003$). 7. A great majority of respondents(86.9% or n=647) found to prefer the day shifts. When the four different types of duty shifts(Types A. B. C, D) were presented, 55.0 percent of total respondents preferred the A type or the existing one followed by D type(22.7%). B type(12.4%) and C type(8.2%). 8. When the condition of monetary incentives for the evening(20% of salary) and night shifts(40% of. salary) of the existing duty type was presented. again the day shift appeared to be the most preferred one although the rate was slightly lower(66.4% against 86.9%). In the case of evening shift, with the same incentive, the preference rates for evening and night shifts increased from 11.0 to 22.4 percent and from 0.5 to 3.0 percent respectively. When the age variable was controlled. < 25 yrs group showed higher rates(31.6%. 4.8%) than those of > 25 yrs group(15.5%. 1.3%) respectively preferring the evening and night shifts(p=.000). The civil status also seemed to operate on the preferences of the duty shifts as the single group showed lower rate(69.0%) for day duty against 83. 6% of the married group. and higher rates for evening and night duties(27.2%. 15.1%) respectively against those of the married group(3.8%. 1.8%) while a higher proportion of the married group(83. 6%) preferred the day duties than the single group(69.0%). These differences were found to be statistically all significant(p=.001). 9. The findings on preferences of three different types of fixed duty hours namely, B, C. and D(with additional monetary incentives) are as follows in order of preference: B type(12hrs a day, 3days a wk): day shift(64.1%), evening shift(26.1%). night shift(6.5%) C type(12hrs a day. 4days a wk) : evening shift(49.2%). day shift(32.8%), night shift(11.5%) D type(10hrs a day. 4days a wk): showed the similar trend as B type. The findings of higher preferences on the evening and night duties when the incentives are given. as shown above, suggest the need for the introductions of different patterns of duty hours and incentive measures in order to overcome the difficulties in rostering the nursing duties. However, the interpretation of the above data, particularly the C type, needs cautions as the total number of respondents is very small(n=61). It requires further in-depth study. In conclusion. it seemed to suggest that the patterns of nurses duty hours and shifts in the most hospitals in the country have neither been tried for different duty types nor been flexible. The stereotype rostering system of three shifts and insensitiveness for personal life aspect of nurses seemed to be prevailing. This study seems to support that irregular and frequent rotations of duty shifts may be contributing factors for most nurses' maladjustment problems in physical and mental health. personal and family life which eventually may result in high turnover rates. In order to overcome the increasing problems in personnel management of hospital nurses particularly in rostering of evening and night duty shifts, which may related to eventual high turnover rates, the findings of this study strongly suggest the need for an introduction of new rostering systems including fixed duties and appropriate incentive measures for evenings and nights which the most nurses want to avoid, In considering the nursing care of inpatients is the round-the clock business. the practice of the nursing duty shift system is inevitable. In this context, based on the findings of this study. the following are recommended: 1. The further in-depth studies on duty shifts and hours need to be undertaken for the development of appropriate and effective rostering systems for hospital nurses. 2. An introduction of appropriate incentive measures for evening and night duty shifts along with organizational considerations such as the trials for preferred duty time bands, duty hours, and fixed duty shifts should be considered if good quality of care for the patients be maintained for the round the clock. This may require an initiation of systematic research and development activities in the field of hospital nursing administration as a part of permanent system in the hospital. 3. Planned and regular intervals, orientation and training, and professional and personal growth should be considered for the rotation of different duty stations or units. 4. In considering the higher degree of preferences in the duty type of "10hours a day, 4days a week" shown in this study, it would be worthwhile to undertake the R&D type studies in large hospital settings.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers.
