Purpose: The purpose of this study was to evaluate the effect of 6-week sensorimotoor training on balance ability and lower limb muscle activation during gait in older adults. Methods: Twenty-four community-dwelling older adults between 65 and 90 years of age participated in this study. In the older adults of the experimental group (n=12), the sensorimotor training program was performed bare feet. General exercise was performed in the control group (n=12). Then, both groups exercised three times a week for forty minutes over a 6-week period. Balance ability was evaluated by One leg stand (OLS) test for determining the static balance and Timed Up & Go (TUG) test for determining the dynamic balance. In addition, muscle activation of the dominant lower limb tibialis anterior and gastrocnemius medialis muscles were measured by surface EMG to evaluate muscle activation during gait. Results: A significant improvement was seen in the one leg standing (OLS) time after exercise in both the sensorimotor training (SMT) group and general exercise (GE) group (p<0.05) and the change in the SMT group was greater than that in the GE group (p<0.05). A significant reduction was seen in the Timed Up & Go (TUG) test time after exercise in both the SMT group and GE group (p<0.05). Also, a significant increase was seen in muscle activation of tibialis anterior muscle after exercise in the SMT group (p<0.05), but no such significant increase was seen in the GE group (p>0.05). Conclusion: These results suggest that sensorimotor training improves the balance in older adults and has a more positive effect on muscular strength and gait. Sensorimotor training provided a variance of training environment and COG exercise of the body is thought to be a more effective exercise program that improves balance and gait ability in older adults.
Background: Plyometric exercise is an exercise exerting forceful power in a brief period using isotonic activation. It is effective to improve reaction of muscle, agility, endurance and athletics performance. Weight training is an exericse improving muscular strength, endurance and respirating ability applying diversely in frequency and load of exercise Plyometric exercise and Weight training is to facilitate the athletics performance though improving the function of lower limb muscle, there is a difference that Plyometic jump squats is the way to improve agility and Weight training is the way to improve muscular strength. Therefore, it is necessary to know how this difference effects on athletics performance as measuring ankle, ROM, and jumping ability. Design: Randomized controlled trial. Method: This study was conducted with the voluntary participation of 40 university students, who were randomly assigned to jump squat and calf raise groups (n=20 per group). For each subject, we measured the range of motion of the ankle joint before and after exercise, as well as a standing broad jump and vertical jump test performance. We compared the performance indices before and after exercises using paired t-tests, and between groups using independent-samples t-tests. Conclusions: Both jump squat and calf raise exercises improved ankle joint dorsiflexion and plantar flexion, as well as standing broad jump and vertical jump height performance. However, there were no significant differences before versus after exercise, or between exercise types. Although jump squats and calf raises have different purposes, it is thought that, in combination, these exercises improve performance more effectively than either alone, and that such a combined exercise program improves the quality of training in both the general public and athletes in various sports.
Main cause of low back pain is a poor posture. Most low back pains are due to a poor posture. The poor posture induces muscle tension and finaliy low back pain. The poor posture arehabitually trained from the childhood by the environmetal factors. In general, maintaining good posture during working and sleeping hours are the first line of defence against back pam. (1) Supine posture is the easiest posture that relaxes and fixes muscles. Supine posture is thus a starting position for on exercise. Lying down releases the weight pressures of head and shoulder and thus body can be relaxed and extended which are helpful for treating back pain. However, supine posture can increase the pressure in ribcage posture aspect and disphragram due to visceral oragans. (2) Sitting in one position for a long time results in fatigue and relaxation of spinal muscies. Finally, body strength is weakened and sitting posture will become poor. If this poor posture continues for a longer time, pain will be accompanied due to overelongation of muscle ligaments. The habitual poor posture could induce intervertebral disc distortion. If the intervertebral disc is damaged, sitting in one position or movement causes pain. (3) Abnormal lumbar curve induces the tention of abdominal muscle and paravertebral muscle groups as well as tention of lower limb muscle group connected to pelvis. For a person with weak body strength, muscle relaxation increases curvature in lumbar, chest and cervical regions. This will induce a pelvic anterior tilting of the imaginary line between A. S. I. S. and P. S. I. S. Hip joint extensor muscle acts on releasing the pelvic anterior tilting. Contrections of hamstring muscle and femoral muscle recover the imaginary line between A.S.LS. and P.S.I.S. from pelvic anterior tilting. thus, contraction of rectus abdominis muscle are required to maintain the normal lumbar curve.
Purpose: We evaluated the physical stress and pain to the musculoskeletal system of a dental practitioner when engaging in a dental scaling training exercise to prevent the development of musculoskeletal injuries. Methods: The 18 female (average age: 21$\pm$1 years) subjects were voluntarily picked from a group of juniors who have completed a one-and-a-half year training course that includes training exercises on the dentiform and on live subjects (other trainees). The test is done by measuring pain, activity, grip strength, and finger dexterity for each subject's hand and wrist. Before the test all subjects were confirmed to be right-handed and were informed of the study and its objective. Measuring was done before and after each subject performed dental scaling for one hour using the scaler and the curet. Results: Pain levels increased for both hand and shoulders, but hand pain was often greater than shoulder pain. Grip strength significantly declined in the right hand but not the left. For joint mobility, the flexion and the extension for the shoulder joint did not change; but the range of motion for both wrist joints significantly increased. For the dexterity test, both hands showed increased dexterity after the exercise. Conclusion: Dental scaling can affect the shoulders and wrists/hands. Therefore, a musculoskeletal injury prevention program for dental practitioners, which may include encouraging them to assume correct body posture when at work, must be sought. This study evaluated only the shoulders, wrists, and hands; but future studies should include areas such as the cervical area, the back, and the lower limbs.

In this paper, we designed and tested an ankle joint mechanism for a gait rehabilitation robot. Gait rehabilitation programs are designed to improve the natural leg motion of patients who have lost their walking capabilities by accident or disease. Strengthening the muscles of the lower-limbs and stimulation of the nervous system corresponding to walking helps patients to walk again using gait assistive devices. It is an obvious requirement that the rehabilitation system's motion should be similar to and as natural as the normal gait. However, the system being used for gait rehabilitation does not pay much attention to ankle joints, which play an important role in correct walking as the motion of the ankle should reflect the movement of the center of gravity (COG) of the body. Consequently, we have designed an ankle mechanism that ensures the safety of the patient as well as efficient gait training. Also, even patients with low leg muscle strength are able to operate the ankle joint due to the direct-drive mechanism without a reducer. This safety feature prevents any possible adverse load on the human ankle. The additional degree of freedom for the roll motion achieves a gait pattern which is similar to the normal gait and with a greater degree of comfort.

Acute transverse myelitis(ATM) is defined as an acute intramedullary dysfunction of the spinal cord, ascendng or static involving both halves of the cord and appearing without any history of previous neurological diseases due to traumatic accident, tumor of all kind, encephalitis and of course excluding all possible viral, bacterial and fungal infection. It is mainly characterized by acute motor disorder of both limbs in respect to which spinal segments are affected as well as sensory disorder and dysuria & dyschezia. The exact cause is unknown, however it is recently suggested that immunological factors are highly involved. It has been reported by several reliable sources that it is often accompanied by immunological diseases such as systemic lupus erythematosus(SLE). As treatments non steroid anti-inflammatory drugs(NSAIDS) are primarily recommended as to steroids, limited doses are injected only with the proper prescription from the physician. Operative methods are not options as traumatic accidents and tumors are excluded as factors. To enhance muscle strength and prevent articular contracture physical therapy and passive exercise is imperative. The following patient whose chief complaints were mainly about hypoesthesia of Rt. lower limb and stiffness of phalanges of both fingers as well as to weakness of lower extermity. Therefore it has been diagnosed as arthalgia syndrome. In oriental medicine factors such as wind evil heat-evil, dampness-heat evil, cold evil cause the arthalgia syndrome. In this case the patient was diagnosed as dampness-heat evil and herbal medicine Chunglijagam-Tang and Dong-Qi acupuncture was applied to treat bladder disorder.
The purpose of this study was to investigate the changes of kinetic and kinematic parameters in obstacle gaits after 12 weeks of an aquatic exercise program. Eight female elders walked in four different heights of obstacles(0, 2.5, 5.1, & 15.2cm) on their self-selected speed. The ROM of hip was significantly increased after the aquatic exercise program. Swing and Stance duration were decreased. The step length was significantly increased and the step width was decreased. After the exercise program the clearance between the right foot and the top of obstacle(except 15.2cm) increased and the crossing speed was increased. The braking force, propulsive force, braking impulse, and propulsive impulse were significantly changed after the aquatic exercise program. The 12 weeks of the aquatic exercise program resulted in lower body strength and balance gains in female elders. The improvements were associated with changes in kinetic and kinematic parameters leading to an obstacle-crossing speed and a safer lower-limb control. The aquatic exercise program is suggested as an effective intervention to promote gait ability and prevent fall-related to the injuries.

PURPOSE: This study aimed to quantify the effects of whole body vibration (WBV) stimulation training on the muscle thickness and gross motor function in children with spastic cerebral palsy. METHODS: Twenty children diagnosed with spastic cerebral palsy were assigned randomly to the Whole Body Vibration (WBV) group (n=10) and control group (n=10). The WBV group received vibration therapy including five different therapy, and the control group received only five general physiotherapy sessions. After 10 weeks of intervention, the muscle thickness was measured using ultrasound, and the Gross Motor Function D and E items were evaluated. RESULTS: After the intervention, both groups showed a significant increase in muscle thickness and gross motor function (p<.05). The WBV group showed a significant increase in the quadriceps femoris and tibialis anterior muscles compared to the control group, whereas no significant increase in the gastrocnemius muscle was observed (p<.05). The WBV group showed significant improvement in the Gross Motor Function D and E scores compared to the control group (p<.05). CONCLUSION: WBV training may be a useful way of improving the lower extremity muscle strength in children with spastic cerebral palsy, which may help improve the gross motor function.
The purpose of this study was to compare pregnant women's gait parameters and mechanical energies caused by changes in hormone levels and anatomical features such as body mass, body-mass distribution, joint laxity, and musculo-tendinous strength from pregnancy to postpartum. Ten subjects (height: $161{\pm}6.5cm$, mass: $62.7{\pm}10.4\;kg$, $66.4{\pm}9.3\;kg$, $68.4{\pm}7.7\;kg$, $57.2{\pm}7.7\;kg$) participated in the four times experiments (the first, middle, last term and after birth) and walked ten trials at a self-selected pace without shoes. The gait motions were captured with Qualisys system and gait parameters were calculated with Visual-3D. Pregnant women's gait velocities were decreased during the pregnancy periods, but increased after birth. Stride width and cycle time were increased during pregnancy, but decreased after birth. Thigh energy (77.4%) was greater than shank energy (19.06%) or feet (3.54%) about total energy of the lower limbs. Their feet (Left R2=0.881, Right R2=0.852) and shank (Left R2=0.318, Right R2=0.226) energies were significantly increased (positive correlation), but double limb stance time (DLST, R2=0.679) and body total energy (R2=0.138) were decreased (negative correlation) for their velocities. These differences suggest that thigh segment may be a dominant segment among lower limbs, and have something to do with gait velocities. Further studies should investigate joint power and joint work to find energy dissipation or absorption from pregnancy period to postpartum.

The purpose of study was to compare respiratory function and quadriceps muscle activity in stroke patients by applying inspiratory muscle training combined with whole body vibration. In addition, the purpose of study is to present an exercise method for improving the respiratory function of stroke patients and the function of the lower limb muscles of stroke patients. Totally, 21 patients with Stroke patients were randomly assigned to two groups through clinical sampling. 11 patients who applied whole body vibration combined with respiratory exercise were randomly assigned to Experiment Group I, and 10 patients who applied placebo exercise combined with breathing exercise were randomly assigned to Experiment Group II. And for 5 weeks, 4 days/week, 1 time/day, 4 sets/1 time intervention program was implemented. Before intervention, the respiratory function was measured with a maximum inspiratory pressure meter, the lower extremity muscle activity was measured using the surface EMG, and the balance ability was measured using a bug balance test. And after 5 weeks, the post-test was re-measured and analyzed in the same way as the pre-test. In the comparison of changes within the group of experimental group I, there were significant differences in the activity and balance of the respiratory muscle strength, the biceps femoris, and the anterior tibialis muscle (p<.05). In the comparison of the changes in the experimental group I, there was a significant difference in respiratory strength and balance (p<.05). In the comparison of changes between groups, there was a significant difference in the activity of the biceps femoris and anterior tibialis (p<.01). In the future, research on protocols for respiratory exercise and whole body vibration to improve neuromuscular function is considered to be necessary.
