Piriformis syndrome is a syndrome of low back and leg pain thought to be due to chronic contracture of the piriformis muscle that causes irritation of the sciatic nerve. We have utilized an alternative approach using cauaal steroid in local anesthetics injection in the treatment of piriformis syndrome. I it was a very effective method without any complications.
Immune checkpoint inhibitor is associated with variety of immune-related adverse events. We present a case of polyneuropathy induced by immune checkpoint inhibitor, which was refractory to steroid and immunoglobulin. While high-dose steroid and immunoglobulin were not effective, we tried rituximab which is effective in other immune-mediated polyneuropathy. After rituximab treatment, patient's clinical symptom and nerve conduction study finding was markedly improved. We suggest rituximab might be effective in polyneuropathy induced by immune checkpoint inhibitor.
Purpose : Recently, clinical trials of steroid add-on therapy were reported with variable results in Kawasaki disease. We analyzed the clinical outcomes of patients at high risk of with Kawasaki disease(${\geq}4$ points of Harada score) treated by three commonly used different treatment regimens, with or without corticosteroids. Methods : Medical records of 96 children with Kawasaki disease treated with one of the threee regimens were reviewed retrospectively. Regimen 1 was aspirin(100 mg/kg/day) plus intravenous gamma globulin 2 g/kg single dose; regimen 2, aspirin(100 mg/kg/day) plus intravenous gamma globulin 1 g/kg single dose; regimen 3, regimen 2 plus prednisolone(2 mg/kg/day), followed by tapering two weeks and pulse therapy of methyl prednisolone performed in cases of retreatment. Also low dose aspirin was given in all three regimens for eight weeks after the acute phase. The cardiovascular and laboratory evaluations were performed on acute phase, immediate after acute phase, and subacute phase, eight weeks after treatment. Results : The frequency of coronary artery lesions and laboratory findings in the three different regimens were similar. The more rapid control of fever after treatment was noted in regimen 3. Furthermore the frequency of retreatment was decreased in regimen 3 compared to the other two regimens. Conclusion : Steroid add-on therapy showed some beneficial outcome compared to conventional treatment regimens. The role of steroid in the treatment of Kawasaki disease should be reassessed in systemic manner.
Background : In recent years, the glucocorticoid hormone has become a fundamental medication for asthma. However, a long period of hormone administration will result. in general. side effects on many body parts as well as hormone dependence, which has become a serious problem for western physicians. Objectives : We aimed to identify the clinical effects of Cheongsanghoha-tang and the steroid sparing effects of Cheongsanghoha-tang on. Materials and Methods : A subject group consists of 36 asthmatics who had been treated with Cheongsanghoha-tang for four weeks. Cheongsanghoha-tang is a herbal decoction, which has been used of the traditional therapeutic agent of asthma. PFT, QLQAKA, blood eosinophil, serum IgE, Serum IL-4. IL-5, IFN-${\gamma}$ were checked before and 4weeks after the treatment. Results : The only FVC% in ICSG among asthmatic patients was increased significantly compared to NICSG. Treatment of Cheongsanghoha-tang for four weeks resulted in significant increase in QLQAKA. The NICSG treated with Cheongsangboha-tang for four weeks were no significant difference in the blood eosinophil, serum IgE, IL-4 and IL-5. The PFT and QLQAKA in NICSG were increased significantly after 4 weeks treatment. But the serum IFN-${\gamma}$ in NICSG was decreased significantly after 4 weeks treatment. Discontinuation of treatment with inhaled corticosteroid in ICSG resulted in insignificant changes in PFT, the blood eosinophil, the serum IgE, IL-4, IL-5 and IFN-${\gamma}$ and significant increase in QLQAKA. As a result. 8 of 13 cases were cured with hormones completely and the rest of ICSG reduced the dose of ICS. Conclusions : This study shows that Cheongsanghoha-tang has the effects on the improvement of pulmonary function and cures asthmatic patients. These findings demonstrate that Cheongsanghoha-tang has the steroid sparing effect. Some satisfactory therapeutic results have been obtained in treating hormone-dependent asthma by Cheongsanghoha-tang. However. the concept and mechanism of hormone-dependent asthma have not been fully defined yet, and the standard for judging therapeutic effects have not been established. Obviously further researches concerning all these are still necessary.

Objectives The purpose of this study is to report the effectiveness of Korean medicine treatment on 69 infant and young child atopic dermatitis patients. Methods 69 infants and young child atopic dermatitis patients who had visited oriental medical clinics from 2011.1.9 to 2012.1.31 were studied. All of them were continually treated for 3 to 9 months, and were analyzed by Objective SCORAD Index (OSI). A detailed analysis of OSI was done according to gender, age, using of topical steroid ointment, family history of atopic disease, treatment period, and intensity. Results 1. Male and female percentage of the study group was 56.5%: 43.5%. The average period of treatment was 6.09 months. 71.0% used topical steroid ointment, and 65.2% had family history of atopic disease. 2. The average period of treatment was significantly longer in patients who used topical steroid ointment, or who had family history of atopic disease. 3. 95.7% of total patients reported decrease in OSI score at the final visit. OSI was significantly lowered after 3, 6, and 9 months of treatment. The longer period of treatment, the lower average post-treatment OSI. 4. 85.5% of the study group had severe dermatitis, and 14.5% was moderate case based on the OSI intensity assessment at initial visit. These percentages got changed at the final visit as 34.8% of severe, 58.0% of moderate, and 7.2% of mild cases. The average treatment period of patients who were diagnosed as severe at the initial visit was 6.18 months, and their final OSI improvement rate was 39.58%. 58% of them were improved to be mild or moderate at the final visit. Conclusion The Korean medicine treatment is effective in treating infant and young child atopic dermatitis patients. There was significant decrease in OSI score after 3, 6, and 9 months of treatment. The difference increase with the treatment period.

Purpose: This study evaluated the results of conservative treatment for simple bone cyst. Materials and Methods: We studied 126 cases of simple bone cyst, which were treated by observation or steroid injection from February 1990 to May 2010. Their mean age at diagnosis was 14 years (${\pm}9$ years, 2-60 years) and the mean follow-up duration for this study were 41 months (${\pm}37$ months, 12-159 months). Observation and steroid injection was applied for conservative management of simple bone cyst and results was scored by modified Neer classification. Results: 31 cases (24.6%) were treated by observation and 95 cases (75.4%) were treated by steroid injection. The 95 patients were treated by a mean of 1.1 (${\pm}0.9$, 0-4) steroid injections and a single steroid injection was given in 65 cases (51.6%). The 102 cases (80.9%) were resulted in modified Neer classification I and II which could be considered as healing of simple bone cyst. Conclusion: Conservative management can be considered as an initial treatment for simple bone cyst with good result.
Urinary steroid levels were investigated in the treatment of CKD-501, a new anti-diabetic drug candidate. CKD-501 was administered orally at the dosage of 1, 2, 4 mg/day for 7 days to normal men (n=18). Urine was collected before, during and after stopping the drug administration and the urinary level of androgen, estrogen, progestin and corticoids were quantified using GC-MS (gas chromatography-mass spectrometry). Only urinary corticosteroid and an androgen, DHEA levels among all the analyzed steroids, have been found to increase progressively, reaching significant levels on the last day of drug treatment and later declined after the drug treatment is withdrawn. Therefore, it was thought that an increase in the urinary corticoid and DHEA levels could be a characteristic of CKD-501, since it prominently acts on the glucose sensitivity and suppresses the triglyceride levels. In conclusion, it was found that CKD-501, an anti-diabetic drug candidate, affects the glucocorticoid and DHEA levels and it plays a crucial role in glucose homeostasis.

Femoroacetabular Impingement (FAI) arises from morphological abnormalities between the proximal femur and acetabulum. Impingement caused by these morphologic abnormalities induces early degenerative changes in the hip joint. Furthermore, FAI patients complain of severe pain and limited range of motion in the hip, but a guideline for treatment of FAI has not yet been established. Medication, supportive physical treatment and surgical procedures have been used in the treatment of the FAI patients; however, the efficacies of these treatments have been limited. Here, we report the diagnosis and treatment for 3 cases of FAI patients. Intra-articular (IA) steroid injection of the hip joint was performed in all three patients. After IA injection, pain was reduced and function had improved for up to three months.

Background The clinical results of conservative treatment options for ulnar compression at the elbow have not been clearly determined. The aim of this review was to evaluate available conservative treatment options and their effectiveness for ulnar nerve compression at the elbow. Methods In accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) recommendations, a systematic review and meta-analysis of studies was performed. Literature search was performed using Ovid MEDLINE, Embase, and Cochrane Central Register of Controlled Trials (CENTRAL). Results Of the 1,079 retrieved studies, 20 were eligible for analysis and included 687 cases of ulnar neuropathy at the elbow. Improvement of symptoms was reported in 54% of the cases receiving a steroid/lidocaine injection (95% confidence interval [CI], 41-67) and in 89% of the cases using a splint device (95% CI, 69-99). Conclusions Conservative management seems to be effective. Both lidocaine/steroid injections and splint devices gave a statistically significant improvement of symptoms and are suitable options for patients who refuse an operative procedure or need a bridge to their surgery. Splinting is preferred over injections, as it shows a higher rate of improvement.
The purpose of this experiment was to investigate the effects of adrenalectomy and PMSG treatment on reproductive organs and serum steroid hormone level in immature female rats. The animals used in this experiment were 25 days old female rats weighing a, pp.oximately 70g. They were randomly divided into two groups of intact rat group (Int-) and adrenalectomized rat group (Adx-) and each group were subdivided into two groups of Non-PMSG (-Cont) and PMSG treated (-PMSG) group. The rat of PMSG-treated group (-PMSG) was administered subcutaneously with 25 IU PMSG on first day (9 a.m.) after adrenalectomy. The adrenalectomized rat groups were su, pp.ied with saline solution through the experiment period. The rate of ovulation and vaginal opening and reproductive organ weights were observed at 8, 32, 56, 80 and 104 hours after PMSG treatment. At the same time, the serum level of estradiol-17${\beta}$ and progesterone were measured by the radioimmunoassay. The results obtained were as follows: 1. Ovulation was shown at 56 hours after treatment in Int-PMSG group and Adx-PMSG group and Adx-PMSG group. The rate of ovulation was very low in PMSG-treated groups, but it was increased in 80 to 90% at 104 hours after treatment. However, there was no ovulation in Int-Cont group and Adx-Cont group. 2. Vaginal opening was shown at 56 hours after treatment in Int-PMSG group and Adx-PMSG group and a, pp.ared in 80% at 104 hours after treatment. The rate of vaginal opening in PMSG-treated groups was very low, but Int-Cont group and Adx-Cont group had no vaginal opening. 3. The weight of ovary and uterus in two PMSG-treated groups were increased with the elapse of time after treatment and were significantly heavy in all observation time, but changes in Int-Cont group and Adx-Cont group were not recognized. The weights of ovaries and utera in Adx-Cont group were increased with the elapse of time. 4. The level of serum estradiol-17${\beta}$ was remarkably increased in PMSG-treated groups (Int-PMSG and Adx-PMSG groups) compared with Int-Cont and Adx-Cont group, and significant difference was recognized between Non-PMSG group and PMSG-treated group in the experimental period. Especially, the highest levels of Int-PMSG groups and Adx-PMSG groups were shown at 80 and 56 hours after treatment and after ward estradiol-17${\beta}$ levels of PMSG-treated groups were decreased. However, changes of the levels did not a, pp.ared in Non-PMSG groups at 104 hours after treatment. 5. The level of serum progesterone in PMSG-treated groups was significantly increased between 80 and 104 hours after treatment. With the elapse of time, the level was increased in all observed groups except for Int-Cont and Adx-Conx group. And the order from the highest level at 104 hours after treatment was Int-PMSG, Adx-PMSG, Int-Cont and Adx-Cont group.
