불안정 지지면을 적용한 테라테이먼트 하지복합운동이 안굽이 무릎 하지 변형자의 무릎과 족저압에 미치는 영향 (Effect of Theratainment Low Extremity Complex Exercise using Unstable Surface on Knee and Plantar Pressure in Patient with Genu Varum)

  • 본 연구는 불안정 지지면을 적용한 테라테이먼트 하지복합운동이 안굽이 무릎 하지 변형자의 무릎과 족저압에 미치는 영향을 알아보고자 하였다. 안굽이 무릎 2등급 남녀 25명을 대상으로 안정 지지면 운동 그룹(SEG)과 불안정 지지면 운동그룹(UEG)으로 나누어 연구를 진행하였다. 하지복합운동 프로그램은 탄력밴드를 이용한 하지운동과 스쿼트 운동으로 구성하였으며, 총 5주간, 주 4회 40분 실시하였다. 무릎의 구조적 변화를 관찰하기 위해 무릎사이 간격과 Q-각을 측정하였으며, 정적 족저압을 평가하였다. 무릎사이 간격과 Q-각 측정 결과 각 집단에서 모두 실험 전에 비해 실험 후 무릎 사이 간격은 감소하고 Q-각은 증가되었음을 알 수 있었으며, 통계적으로 유의한 차이를 나타내었다(p<.05). 실험 후 집단 간 비교 검사 결과, 무릎사이 간격 차이에서 통계적으로 유의한 차이를 나타내었다(p<.05). 정적 족저압 평가 결과 각 집단에서 모두 실험 전에 비해 실험 후 양쪽 발의 앞쪽 비율(FFR)은 감소하고, 뒤쪽 비율(RFR)은 증가하였다. 안정 지지면 운동그룹의 왼쪽 발 앞쪽 비율(FFR)을 제외하고 통계적으로 유의한 차이를 보였다(p<.05). 이상의 결과를 통해 하지복합운동이 안굽이 무릎 하지 변형자의 무릎 구조 교정 및 정적 족저압 개선에 효과적이며, 불안정 지지면은 하지복합운동의 효과를 증가 시킬 수 있는 좋은 조건임을 알 수 있었다.

  • Background: The purpose of this study was investigated of effect of neuromuscular electrical stimulation combined with mirror therapy on static balance, weight support and ankle movement incline with chronic stroke patients. Design: Two-group pretest-posttest design. Methods: Thirty chronic stroke patients participated in this study. The study design is a two-group pretest-posttest design. A total of 30 people participated in the study, and 15 people were each assigned to the experimental group and control group. Experimental group received neuromuscular electrical stimulation combined with mirror therapy 30 minutes, and conventional physical therapy 30 minutes. Control group received conventional physical therapy 30 minutes. Both groups were conducted 5 times a week for 4 weeks. static balance and weight support was measured by force plate and ankle movement incline was measured by goniometer. Results: As a result of comparing the static balance, weight support and ankle movement incline change between experimental group and control group, statistically significant differences were found in all variables (p<.05). In the evaluation before and after the intervention, there was a statistically significant difference in all variables in the experimental group (p<.05), but there was no statistically significant difference in the control group (p>.05). Conclusion: Neuromuscular electrical stimulation combined with mirror therapy intervention improves static balance, increase paretic side weight support and ankle movement incline in chronic stroke patients. It could be an effective intervention for improve static balance, weight support and ankle movement for chronic stroke patients.

The Short Term Effects of Ankle Strengthening Emphasis with Jumping on Strength, Mechanical Properties, and Balance with and without Wearing High Heel in Ankle Instability

  • Purpose: The purpose of this study was to investigate the short term effects of ASEJ (ankle strengthening with emphasis on jumping) for 3weeks on strength, mechanical properties, and balance and to compare the balance with and without HH(high-heel) condition. Methods: ASEJ (a combined exercise of squat, heel raise up, and jumping) were performed for the subjects in 11 female ankle instability young females ($21.7{\pm}2.0yrs$ Cumberland ankle instability score $19{\pm}6.5$). To investigate the effect of ASEJ, investigator used dynamometer for measuring strength, MyotonPRO for measuring mechanical properties(tone, stiffness, and elasticity of the muscles), I-Balance test for static balance, and Y-balance test for dynamic balance between the condition with and without HH condition. All data were normally distributed and analyzed using the SPSS 22.0 statistical program. Comparing pre- and post-intervention and the condition with and without HH conditions data were examined using the paired t-test. The level of significance was chosen as 0.05 for all the analyses. Results: 3wks of ASEJ would strengthen leg muscles and increasing muscle tone and stiffness in most muscles however there was decreasing muscle elasticity of gastrocnemious. In addition, the ASEJ improves the static balance for ankle instability young females and increases the dynamic balance when wearing the heels especially. Conclusions: the ASEJ could recommend to improve the strength and balance for ankle instability young females. Also, measuring the balance with HH conditions well represents the risk of ankle damage in female.
