Effects of Dual Tasks on Balance Ability in Patients with Cerebellar Ataxia

  • Purpose: The purpose of this study was to examine the effects of dual tasks on balance and postural control during standing in patients with cerebellar ataxia (CA). It was hypothesized that CA patients would exhibit different sway characteristics of the center of mass (COM) depending on the complexity of the secondary cognitive tasks compared with normal control subjects. Methods: A total of 8 patients with CA and age-matched healthy control subjects participated in this study. They were instructed to perform two balance tasks (non-dual and dual movement) with 3 different complexity of dual tasks. Range, variability, and velocity of COMs were measured. Results: According to the results CA patients showed deficits in balance and postural control with increased dual-task complexity during the static balance task in saggital sway movements. However, there was no significant difference in static balance in frontal sway. With higher difficulty in the cognitive task, CA patients took longer to stabilize their body center, while normal control subjects showed no change between conditions. In addition, CA patients had a greater COM resultant velocity during recovery in the dual-task condition compared with the single-task condition. These findings indicate that CA patients had defendable compensatory strategies in performing dual tasks. Conclusion: In conclusion, CA patients appeared to manage the priority to balance and postural control. Particularly in a situation with a postural threat such as when potential consequences of the loss of stability increase, they appeared to prioritize the control of balance and posture over the performance of the secondary task.

목뼈 및 등뼈 관절가동술이 전방머리자세 성인의 목통증, 목 관절가동범위 및 균형에 미치는 영향 (The Effect of Cervical and Thoracic Joint Mobilization on the Cervical Pain, Cervical Range of Motion and Balance in Adults with Forward Neck Posture)

  • 본 연구는 목뼈와 등뼈에 적용한 관절가동술이 전방머리자세를 가진 성인의 목통증, 목관절가동범위 및 균형에 미치는 영향을 알아보고자 실시되었다. 전방머리자세로 판명된 대학생 26명을 목뼈 관절가동술 그룹과 등뼈 관절가동술 그룹으로 13명 씩 무작위 배정하여 모든 대상자에게 각각 하루 10회, 1회당 30초 적용, 30초 휴식으로, 주 3회, 4주간 관절가동술을 적용한 후 변화를 비교하였다. 연구 결과, 통증과 관절가동범위에서 두 그룹 모두 유의한 효과가 있었고(p<.01), 그룹 간 비교에서는 유의한 차이가 없었다. 정적 균형의 그룹 내 변화에서 목뼈 관절가동술 그룹이 오른발 및 눈을 감았을 때 유의한 개선을 보였고(p<.05), 등뼈 관절가동술 그룹에서는 유의한 변화가 없었다(p<.05). 왼발 정적 균형은 두 그룹 모두 유의한 개선이 없었고(p<.05). 그룹 간 정적 균형의 비교에서도 유의한 차이가 없었다(p<.05). 동적 균형의 그룹 내 변화에서 목뼈 관절가동술 그룹이 앞쪽, 왼쪽, 오른쪽에서 유의한 개선을 보였고(p<.05), 등뼈 관절가동술 그룹이 앞쪽과 오른쪽에서 유의한 개선을 보였다(p<.05). 그룹 간 동적 균형에서는 왼쪽에서 유의한 차이가 나타났다(p<.05). 본 연구를 통하여 목뼈 및 등뼈 관절가동술은 전방머리자세 성인의 통증, 관절가동범위, 균형 능력에 대하여 유의한 효과가 있었다.

The Effects of Visual Biofeedback Balance Training on Functional Ability in Children with Cerebral Palsy : A Pilot Study

  • PURPOSE: The purpose of this study is to examine the impact of balance training on a three-dimensional balance trainer that provides the up-and-down vertical movement of the knee joint and left-and-right horizontal movement, along with visual feedback on the functional ability of children with spastic cerebral palsy (CPs). METHODS: 8 CPs participated in this study. The experiment was implemented for 40 minutes, three times a week for a total of six weeks. The subjects received general physiotherapy for 15 minutes in each session focused on balance and walking, as based on the neuro-developmental treatment theory. Balance training was performed for 20 minutes on a three-dimensional balance trainer where knee joint movement providing visual feedback is applied. The evaluations were conducted before and after the test, and posture sway was measured using 10 Meter Walking Test (10MWT), Timed Up & Go Test (TUG), and the Good Balance System to evaluate the functional ability and balance of the subjects RESULTS: 10MWT was not statistically significant (p>.05). On the contrary, TUG and postural sway indicate static balance showed a statistically significant decrease (p<.05). In a static balance test using the Good Balance System, the average moving speed statistically significantly decreased in the AP and ML directions (p<.05), and the mean velocity moment also significantly decreased (p<.05). CONCLUSION: These findings suggested that balance training using the three-dimensional balance trainer, with the features of visual feedback and up-and-down knee joint movement effects on increasing dynamic and static balance.

정상성인에 대한 정적 및 동적 자세균형제어의 정량적 분석 (A Quantitative Assissment of Static ann Dynamic Postural Sway in Normal Adults)

  • Postural balancing in human is known to be maintained by the complex mechanism coupled with cerebellum, equilibrium organ of ear, proprioception and other various organs. We developed a Computerized Balance Evaluation and Training system(COBET system) to evaluate postural control and to rehabilitate geriatrics and disabled patient. In addition, 55 normal adult were tested to investigate the influencing factors on balancing posture. For the analysis of static postural sway, areas of the moving center of pressure were calculated under 8 different positions of subjects. And subjects were also asked to follow the visual targets on monitor for the evaluation of the dynamic postural sway. In comparison of the first and the second sets of tests, there was test-retest reliability($\textit{p}$< 0.05). The controllability of the static pmtwn sway was decreased as the ages of subjects increase. When the ages of subject are over 60, the controllability was significantly decrease4 The dynamic postural sway was significantly greater in the age groups of 7th and 8th decade than the younger groups. It is concluded that COBET system is a reliable system in the evaluation of postural sway. The COBET system is considered to be a valuable training modality for the disabled patients as well as the elderly.

동작관찰훈련이 만성 뇌졸중 환자의 앉은 자세에서 균형과 몸통조절능력에 미치는 영향 (The Effect of Action on the Balance and the Trunk Control Ability in the Sit Position of Chronic Stroke Patients)

  • Purpose : To find out how action observation training for chronic stroke patients affects their balance and body control abilities in the posture seated in the rehabilitation of stroke. Methods : This study was conducted on 30 subjects who were diagnosed with stroke. The group conducted motion observation training through video clips, while the control group only conducted physical training, and the general physical therapy was performed equally by both counties. The static balance was measured using Biorescue and the dynamic balance was measured using Modified Functional Reach Test (MFRT), Postural Assessment Scale for Stroke, and Trunk Impairment Scale. Results : Static balance showed statistically significant difference in foot pressure (p<.05) as a result of comparison between pre and post exercise training. Dynamic balance was statistically significant (p>.05) as a result of comparing pre and post differences using modified functional reach test. The trunk control ability was statistically significant (p>.001). Comparison between the results of before and after motion observation training showed a statistically significant difference. Conclusion: This study confirmed that exercise training in sitting position was effective for static, dynamic balance ability and trunk control ability of hemiplegic patients due to stroke. These results suggest that the use of motion monitoring in stroke patients may have a positive impact on the diversity and function of rehabilitation.

A Comparison of Dizziness Handicap Inventory Scores with Stability Index and Fourier Harmony Index in Healthy Individuals

  • Purpose: The purpose of this study was to determine whether the degree of dizziness affects static balance due to the disruption or absence of the senses involved in balance. To this end, the correlation between the Dizziness Handicap Inventory (DHI), which objectively evaluates dizziness, the Fourier Index (FI; Frequency bands of postural oscillation, F1, F2-4, F5-6, F7-8) and the Stability Index (ST), which evaluates static balance ability, were examined. Methods: This study investigated balance and dizziness issues in 30 healthy young adults. Participants underwent multiple tests like the DHI and tetra-ataxiometric posturography (Tetrax) under different conditions (eyes open/closed, standing on a foam-rubber pillow, and with the head in various orientations). Results: We found that F1 exhibited a weak positive correlation with dizziness under normal conditions, as well as when the eyes were closed (r=0.396, p<0.05) and the head was tilted back (r=0.375, p<0.05). Meanwhile, F5-6 showed a moderate positive correlation with dizziness in both head-back (HB: r=0.471, p<0.05) and head-forward postures (r=0.404, p<0.05). Lastly, both F7-8 and ST demonstrated a moderate positive correlation with dizziness when the head was in a forward posture (F7-8: r=0.483; ST: r=0.403, p<0.05). Conclusion: The study results indicate that the severity of dizziness affects sensory systems and balance. It also suggests that head movements, especially forward and backward, further stimulate the vestibular system, intensifying dizziness, and balance problems in affected individuals.

기구를 이용한 골반 압박이 20대의 요통 경험자와 비경험자의 체간 근지구력과 균형 능력에 미치는 영향 비교 (Comparison of the Effects of Pelvic Compression Using Instruments on Trunk Muscle Endurance and Balance Ability in Subjects in Their Twenties With or Without Low Back Pain)

  • Background: Low back pain (LBP) is a representative disease, and LBP is characterized by muscle dysfunction that provides stability to the lumbar spine. This causes physical functional problems such as decreased posture control ability by reducing the muscular endurance and balance of the lumbar spine. Pelvic compression using instruments, which has been used during recent stabilization exercises, focuses on the anterior superior iliac spine of the pelvis and puts pressure on the sacroiliac joint during exercise, making the pelvis more symmetrical and stable. Currently, research has been actively conducted on the use of pelvic compression belts and non-elastic pelvic belts; however, few studies have conducted research on the application effect of pelvic compression using instruments. Objects: This study aimed to investigate whether there is a difference in trunk muscular endurance and dynamic and static balance ability levels by applying pelvic stabilization through a pelvic compression device between the LBP group and the non-LBP group. Methods: Thirty-nine subjects currently enrolled in Daejeon University were divided into 20 subjects with LBP group and 19 subjects without LBP (NLBP group), and the groups were compared with and without pelvic compression. The trunk muscular endurance test was performed with 4 movements, the dynamic balance test was performed using a Y-balance test, and the static balance test was performed using a Wii balance board. Results: There was a significant difference the LBP group and the NLBP group after pelvic compression was applied to all tests (p < 0.05). In the static and dynamic balance ability test after pelvic compression was applied, there was a significant difference in the LBP group than in the NLBP group (p < 0.05). Conclusion: These results show that pelvic compression using instruments has a positive effect on both those with and without LBP and that it has a greater impact on balance ability when applied to those with LBP.

Changes in Back Body Surface Measurements for Dynamic Postures in the Form of Baseball Batting Motion with a 3D body Scanning

  • The purpose of this study was to analyze human upper body surface changes at the shoulder and back area. The body surface data were analyzed in terms of muscle and bone displacement in dynamic postures. Body surface data were collected with a 3D body scanner. The body surface was scanned at the static and four baseball batting postures. The body surface dimensions over the deltoids, scapulae and trapezius were measured. The results show that the vertical measurements of the deltoids increased by 20%. The horizontal measurements of the axilla of the back increased. The surface of the trapezius was elongated by over 10%, and the lower back musculature was elongated by about 50%. The results of this study showed that changes in back body surface caused by upper arm movements. It was influenced by the deltoid articulated with the humeri and the scapulae and trapezius. These body surface changes caused by muscle activities and ranges of motion can be used to design functional clothing.

  • This paper deals with a novel autocalibration method of three-axis micromachined accelerometers applied to a new digital intelligent putter for golfers. This putter can help golfers monitor and analyze their putting posture and therefore modify their putting action to get better score and enjoy their lives through golf. The micromachined accelerometers to get information of the motion are the essential part of the putter to measure the three-axis acceleration as accurately as possible. This paper presents an efficient autocalibration algorithm to find the offset and sensitivity of accelerometers by only using the static measurement data at six different positions. The experimental results on the developed putters show the validity of the proposed algorithm for the new smart putter.
