요양병원 간호사의 환자안전문화 인식, 표준주의 지식 및 수행도 (Perception of Patient Safety Culture, Knowledge and Performance of Standard Precaution among Long-Term Care Hospitals Nurses)

  • 본 연구는 요양병원 간호사의 환자안전문화 인식, 표준주의 지식 및 수행도와의 관계를 파악하기 위해 실시된 서술적 조사연구이다. 요양병원 간호사 134명을 대상으로 2018년 1월 2일부터 6월 4일까지 자료수집이 이루어졌으며, 수집된 자료 분석은 SPSS/WIN 21.0프로그램을 이용하여 t-test, ANOVA, Scheffe's test, Pearson's correlation coefficient으로 하였다. 환자안전문화 인식은 표준주의 지식(r=.192, p=.027), 표준주의 수행도(r=.211, p=.014)와 유의한 양의 상관관계가 있는 것으로 나타났다. 요양병원 간호사의 환자안전문화 정착과 표준주의 수행 증진을 위해서는 체계적인 교육을 통해 표준주의 지식을 함양하고 긍정적인 환자안전문화를 조성할 수 있도록 하여 그 속에서 환자안전과 의료관련감염 관리의 중요성을 스스로 인식하고 실천하도록 해야 갈 것이다.

중소병원 간호사의 환자안전에 대한 태도, 도덕적 민감성이 표준주의지침 수행에 미치는 영향 (Influences of Safety Attitude for Patient, and Moral Sensitivity in Practice of Standard Precaution in Small and Medium Hospital Nurses)

  • 본 연구는 중소병원 간호사의 환자안전에 대한 태도, 도덕적 민감성, 표준주의지침 수행과의 관련성을 파악하고, 표준주의지침 수행에 영향을 미치는 요인을 규명하기 위해 시도되었다. 자료수집은 2021년 1월 11일부터 1월 22일까지 중소병원에 근무하는 150명의 간호사를 대상으로 설문조사를 시행하였다. 자료분석은 Descriptive statistics, Independent t-test, one-way ANOVA, Pearson's correlation coefficient, Multiple regression analysis를 이용하였다. 연구결과 표준주의지침 수행에 환자안전에 대한 태도 (r=.539, p<.001), 도덕적 민감성(r=.249, p=.002)에 유의한 상관관계가 있는 것으로 나타났다. 다중회귀분석결과 중소병원 간호사의 표준주의지침 수행에 영향을 미치는 요인은 환자안전에 대한 태도와 근무부서로 설명력은 33.8%이었다(F=18.47, p<.001). 연구결과를 통해 중소병원 간호사의 표준주의지침 수행을 높이기 위해서는 환자안전에 대한 올바른 태도를 정립할 수 있는 교육프로그램 개발이 필요하다.

기대권침해론에 관한 일본의 최근 동향 (Recent Trends in the Theory of Expectation Rights Violations in Japan)

  • The concept of expectation rights considers 'the expectation' that the patient should be given proper medical treatment as the benefit and protection of the law, so it would be the benefit and protection of the law due to personal rights different from 'the legal principle that has the possibility to a considerable extent' being in an extension of life and body. However, the problem how the patient's expectation of medical service sets up in order to make it the benefit and protection of the law would be still left in the vague concept of the patient's 'expectation', thus, in the first place, the medical practice following formed medical standard in every particular medical institutes should be the standard because these medical services are normally within a range of the patients' expectations. In addition, it should be naturally constituted as mental profit to get the subjective circumstances such as 'the patient's expectation' to be an object, and also, different from the profit and protection of the law such as life and body that should be absolutely protected, the origin of violation behavior should be regarded simultaneously to define the denotation of expectation rights. Therefore, the expectation rights violations would be problematic in case it fails to reach the medical standard that is expected for common doctors to practice properly. This is the concept of expectation rights that gets subjective matters such as the patient's expectation to be objectivity as medical practices that can be expected by generalized abstract doctors. This standard should be defined as the minimum standard that is naturally expected for doctors to practice, different from medical standard that decides the level of doctors.

의료종사자의 환자안전문화와 표준주의 인식과 상관관계 (Awareness and Correlation of Patient Safety Culture and Standard Precautions among Health Care Personnel)

  • Objectives: This study was conducted as descriptive correlation research in order to survey healthcare personnel(HCP)'s awareness of patient safety culture and their recognition of standard precautions, and to examine the correlation between the two factors. Methods: The subjects were 400 HCPs including 80 doctors, 240 nurses, and 80 medical technicians from two general hospitals. The questionnaire used in the survey consisted of 9 questions on general characteristics, 44 on the perception of patient safety culture, and 21 on the recognition of standard precautions. Results: According to the subjects' general characteristics, the score was significantly higher in those aged over 40 than in those aged 30-39. In addition, it was significantly higher in managers than in practitioners, in those with work experience of less than a year than in those with 5-9 years. The score was also significantly higher in those working 8 hours a day than in those working over 10 hour a day. The number of medical accident reports according to the subjects' general characteristics was significantly larger in nurses than in doctors, in managers than in practitioners, and in those with 10 years' or longer experiences than in those with less than a year. In the awareness of standard precautions according to the subjects' general characteristics, the score was significantly higher in female workers than in male workers, and in managers than in practitioners. As a whole, the subjects' awareness of patient safety culture and their recognition of standard precautions showed a significant correlation with each other. Conclusion: From the result of this study, sufficient work force and the promotion of organizational culture for safety is needed in order to guarantee patient safety. Likewise, these results suggest that experience, job skill, and adequate working hours have a positive effect on the awareness of patient safety culture and the recognition of standard precautions.

감염예방을 위한 응급실 환자분류공간 국내외 시설기준 분석연구 (Infection Control in Triage Space of Emergency Room: Based on Analysis of Healthcare Facility Standards)

  • Purpose: Facilities for infection prevention according to the characteristics of the patients, caregivers and medical personnel are needed in the patient triage room of the emergency department as a space for the first medical examination and classification of the patients. This study focus on the patient tirage room with the highest frequency of use in the emergency department to prevent the hospital acquired infection of the emergency department. Methods:: This study analyzed the facilities standard needed for the infection prevention through interviews with the medical personnel and analyses on the facilities standard/cases of foreign hospitals and facilities standard at home and abroad Results: And based on this, it attempted to present improvement measures by analyzing the line of circulation and space used by infected patients in a hospital designated in the regional emergency medical center among hospitals whose emergency department overcrowding index is high. Implications: The facilities standard for the infection prevention among the courses for patient classification of the emergency medical centers could be identified and implemented to prevent infection.

간호대학생의 의료관련감염 관리를 위한 표준주의지침 수행도 영향요인 : 환자안전에 대한 태도, 임파워먼트, 인지도를 중심으로 (Affecting Factors on Performance of Standard Precaution for Healthcare associated Infection Control and Prevention in Nursing Students: focused on Patient Safety Attitude, Empowerment, Awareness)

  • 본 연구는 간호대학생의 환자안전에 대한 태도, 임파원먼트, 표준주의지침 인지도 및 수행도 간의 관련성을 파악하고, 표준주의지침 수행도의 영향요인을 규명하고자 시도되었다. 연구대상자는 J시 소재 일 간호대학 재학생 185명이었다. 자료수집은 2018년 9월 10일에서 21일까지 진행되었으며, 수집된 자료는 SPSS Statistics 22.0 프로그램을 이용하여 Independent t-test, ANOVA, Pearson's correlation coefficient, stepwise multiple regression으로 분석하였다. 다중회귀분석 결과 표준주의지침 수행도의 영향요인은 인지도, 임파워먼트, 환자안전행사 참여로 나타났으며 수행도를 51.0% 설명하였다. 이에 간호대학생의 의료관련감염 관리를 위한 표준주의지침 수행도를 향상시키기 위해서는 인지도를 높이고 임파워먼트를 향상시킬 수 있는 다양한 융합적 교육프로그램의 적용과 환자안전행사에 간호대학생이 적극 참여할 수 있도록 하는 방안을 모색하여야 할 것이다.

임상시험의 표본크기 계산 (Sample Size Calculation for Cluster Randomized Trials)

  • A critical assumption of the standard sample size calculation is that the response (outcome) for an individual patient is completely independent to that for any other patient. However, this assumption no longer holds when there is a lack of statistical independence across subjects seen in cluster randomized designs. In this setting, patients within a cluster are more likely to respond in a similar manner; patient outcomes may correlate strongly within clusters. Thus, direct use of standard sample size formulae for cluster design, ignoring the clustering effect, may result in sample size that are too small, resulting in a study that is under-powered for detecting the desired level of difference between groups. This paper revisit worked examples for sample size calculation provided in a previous paper using nomogram to easy to access. Then we present the concept of cluster design illustrated with worked examples, and introduce design effect that is a factor to inflate the standard sample size estimates.

군병원 응급실 간호서비스에 대한 환자만족도 (Patient's Satisfaction with Nursing Care Services in Military Hospital Emergency Department)

  • Purpose: The purpose of this study is to provide basic data for the improvement of the quality in the emergency nursing services by investigating the expectation level and actual received level with nursing care and identifying the factors that influence patient satisfaction with the emergency nursing care in the military hospitals. Method: Using a self report questionnaire, data were collected from 150 patients who had been admitted in military hospitals emergency department. The data collection was done between April 17 and April 30, 2006. At least, 145 questionnaires were analyzed and used in this study. Results: The average sum score was 61.93 out of 76 with standard deviation 9.590. The mean of the patient's expectation levels with the overall emergency nursing services was $(3.25{\pm}.504)$ out of the highest score 4. the average sum score was 63.27 out of 76 with standard deviation 9.902. The mean score of the patient's actual levels with the overall emergency nursing services was $(3.33{\pm}.521)$ out of the highest score 4. Conclusion: nursing management strategies must be established to remove patient's dissatisfaction factors and to improve the emergency nursing care quality through continuous measurement and assessment of the patient's satisfaction with emergency nursing services in military hospitals.

Adobe Photoshop®을 이용한 가상성형수술의 유용성 (Virtual Plastic Surgery using Adobe Photoshop®)

  • Digital imaging is emerging as a standard method for patient documentation in clinical setting. Managing patient expectations before aesthetic surgery can greatly improve patient satisfaction after surgery. The patient who visited for plastic surgery wants a predicted figure after the operation. A virtual plastic surgery software is necessary in order to satisfy the desire of patients. Adobe $Photoshop^{(R)}$ is the professional standard in desktop digital imaging, offers indispensable new features for graphic and web design, photography, and video. Using imaging technology, it takes just minutes to realistically simulate the results of double eyelid operation, liposuction, rhinoplasty or any other aesthetic procedure. The aim of this study is to analyze the significance of the digital image processing and to introduce the virtual plastic surgery using Adobe $Photoshop^{(R)}$.

진료 품질 향상을 위한 환자 데이터 맞춤형 분석 프로세스 개발: 외국인 환자를 중심으로 (Development of customized patient data analysis process for quality of care improvement : focused on foreign patients)

  • Purpose: The purpose of this study was to find meaningful patient groups of disease using foreign patients data and analyze implemented test of the patient groups. Methods: The data was collected by foreign patients' EMR data of K university hospital. The author proposed tree-form patients' characteristic diagram through statistical methods that association rule, proportion test, clustering using prescription information and questionnaire information. Results: This study's analysis process was applied high blood data and diabetes data. Analysis showed other characteristic of meaningful patient groups in high blood and diabetes. In high blood, test implementation rate of patient group showed the differences. And in diabetes, test implementation rate of patient group and implemented test list showed differences. Conclusion: The result of this study can play a role as basic data that can be clinical testing standard in preventive aspect. Eventually, 5 dimensions of SERVQUAL will be improved by this study's process.
