Association between the serum estrone-to-estradiol ratio and parameters related to glucose metabolism and insulin resistance in women with polycystic ovary syndrome

  • Objective: We aimed to evaluate associations between the ratio of serum estrone (E1) to estradiol (E2) and parameters related to serum glucose metabolism and insulin resistance in women with polycystic ovary syndrome (PCOS). Methods: In total, 133 women between the ages of 18 and 35 diagnosed with PCOS were enrolled in this study. All participants with PCOS underwent blood tests to determine hormonal and biochemical metabolic parameters and a standard 2-hour 75-g oral glucose tolerance test. They were divided into two groups according to the serum E1-to-E2 ratio: group 1 (E1/E2 ratio <2.0) and group 2 (E1/E2 ratio ≥2.0). Results: In the comparative analysis, the waist-to-hip ratio (WHR) was the only clinical variable that was significantly different between the two groups. Patients with a higher E1/E2 ratio showed higher fasting insulin levels, homeostasis model for insulin resistance, and postprandial glucose level at 2 hours (PPG2). In a correlation analysis, only PPG2 was significantly related to the serum E1/E2 ratio. However, after controlling for the confounding effects of body mass index (BMI) and WHR, fasting glucose was also significantly correlated with the serum E1/E2 ratio. Conclusion: Women with PCOS with a higher serum E1/E2 ratio were found to be more likely to show higher fasting insulin and postprandial glucose levels. Significant correlations were found between the serum E1/E2 ratio and both fasting and postprandial serum glucose levels after adjusting for BMI and WHR in women with PCOS.

Comparative Calorimetric Evaluation of Ammoniated Straw-Based Rations Supplemented with Low Levels of Untreated and Formaldehyde Treated Groundnut Cake and Fish Meal with Respect to Growing Buffalo Calves

  • Eighteen growing male Murrah buffalo (Bubalus bubalis) calves were divided into three groups consisting of six animals each and fed three urea ammoniated wheat straw (UAS) -based rations supplemented with concentrate mixtures (roughage: concentrate ratio 58:42) containing deoiled ground nut cake, GNC (8%), formaldehyde treated GNC (8%) or fish meal (8%) to undertake comparative evaluation of these rations in terms of their $CH_4$ production and growth (285 d duration) potential. A digestibility trial (10 d duration) was followed by a comparative calorimetric study in respiration chamber. Dry matter (DM) intake (84.3 to $89.3g/kg\;W^{0.75}d^{-1}$) did not differ between treatments. The digestibility coefficient of DM, organic matter (OM), crude protein (CP), neutral and acid detergent fiber did not differ significantly in different diets. Urinary energy loss as a percent of gross energy (GE) was not affected by diets. Average values of $CH_4$ production were 84.3, 77.6 and 99.1 g/d and $CH_4$ energy losses as percent of gross energy were 5.7, 5.2 and 6.1 percent on .GNC, formaldehyde treated GNC and fishmeal, respectively, and did not differ significantly. When expressed per unit of digestible OM intake, $CH_4$ production (g) was lower (p<0.05) on formaldehyde treated GNC (30.6) than on untreated GNC (30.6) and fish meal (31.9). Total ME intake and heat production were similar and hence the energy balances on different diets were similar. Nutritive value of rations in terms of digestible CP and ME were similar. Average daily gain calculated on the basis of regression of fortnights on cumulative liveweight gain in calves fed on concentrate containing unprotected GNC, protected GNC and fish meal were 437.1, 483.9 and 481.6 g, respectively. This indicated that the intake of energy was sufficient to meet the requirement of calves growing at 400 g per d. However, CP intake was around 150% of the stipulated standard (Kearl, 1982). Feed conversion ratios on unprotected GNC, protected GNC and fish meal were 11.60, 11.10 and 10.4 respectively. It was concluded that because significantly (p<0.05) low $CH_4$ is produced on protected GNC (8%), it is very good and sustainable protein source in comparison to poor quality fish meal and untreated GNC to be used in concentrate mixture for supplementing UAS-based diets.

  • During the 1970s, concern arose that cured meats contained high levels of residual nitrite and preformed nitrosamines. Therefore, the search for alternatives and alternative approaches to the use of nitrite have been still continued, however no complete alternative for nitrite has yet been identified. Recently, it was publicized in Korea that nitrite-containing meat products would be detrimental to health, about which consumers have been seriously concerned. Therefore, this study was carried out to inform the consumer of the safety status of nitrite and thereby to lead proper consumption of meat products. For assessing the safety of nitrite, data regarding the regulation for use, metabolism in human body, and dietary intake amounts of nitrite were collected and analyzed. The mean intake level of nitrite for Korean per capita was recently reported to be not more than 1% of ADI set by JECFA. On the contrary, a calculation indicated that the daily nitrite intake per capita from saliva by ingestion of vegetables in Korea would be about 300-fold higher than that from cured meats. In consideration of the low consumption amount of meat products per capita of Korean, that is, at least one fifth, compared to European and American, there is no particular reason to concern about the impairment of health by nitrite intake from meat products for Korean. However, any effort for the reduction of residual nitrite content in cured meats should be given with an idea to minimize the intake of nitrite even from the minor source.

Left Ventricular Remodeling in Patients with Primary Aldosteronism: A Prospective Cardiac Magnetic Resonance Imaging Study

  • Objective: This study used cardiac magnetic resonance imaging (MRI) to compare the characteristics of left ventricular remodeling in patients with primary aldosteronism (PA) with those of patients with essential hypertension (EH) and healthy controls (HCs). Materials and Methods: This prospective study enrolled 35 patients with PA, in addition to 35 age- and sex-matched patients with EH, and 35 age- and sex-matched HCs, all of whom underwent comprehensive clinical and cardiac MRI examinations. The analysis of variance was used to detect the differences in the characteristics of left ventricular remodeling among the three groups. Univariable and multivariable linear regression analyses were used to determine the relationships between left ventricular remodeling and the physiological variables. Results: The left ventricular end-diastolic volume index (EDVi) (mean ± standard deviation [SD]: 85.1 ± 13.0 mL/m2 for PA, 75.9 ± 14.3 mL/m2 for EH, and 77.3 ± 12.8 mL/m2 for HC; p = 0.010), left ventricular end-systolic volume index (ESVi) (mean ± SD: 35.2 ± 9.8 mL/m2 for PA, 30.7 ± 8.1 mL/m2 for EH, and 29.5 ± 7.0 mL/m2 for HC; p = 0.013), left ventricular mass index (mean ± SD: 65.8 ± 16.5 g/m2 for PA, 56.9 ± 12.1 g/m2 for EH, and 44.1 ± 8.9 g/m2 for HC; p < 0.001), and native T1 (mean ± SD: 1224 ± 39 ms for PA, 1201 ± 47 ms for EH, and 1200 ± 44 ms for HC; p = 0.041) values were higher in the PA group compared to the EH and HC groups. Multivariable linear regression demonstrated that log (plasma aldosterone-to-renin ratio) was independently correlated with EDVi and ESVi. Plasma aldosterone was independently correlated with native T1. Conclusion: Patients with PA showed a greater degree of ventricular hypertrophy and enlargement, as well as myocardial fibrosis, compared to those with EH. Cardiac MRI T1 mapping can detect left ventricular myocardial fibrosis in patients with PA.

Effect of Dietary Bellflower(Platycodon glandiflorum) and Red Ginseng on Hypercholesterolemic Rats (홍삼 및 도라지 분말이 흰쥐의 혈청 및 지질에 미치는 영향)

  • Effects of bellflower and red ginseng on plasma and liver lipid metabolism in rats receiving dietary cholesterol were studied. During experimental period of 14 days the rats were divided into four dietary groups: standard diet: Control diet containing 1% cholesterol and 0.25% sodium cholate to the standard diet: bellflower diet containing 5% bellflower to the control diet, and red ginseng diet containing 5% red ginseng to the control diet. The contents of total cholesterol (TC), HDL-cholesterol(HC), total lipid(TL) and triglyceride(TG) in the plasma and liver were determined after 12 hours fasting. Liver weight of the rats fed control, bellflower or red ginseng diets was greater than that of the rats fed standard diet, however, kidney weight was greater at the standard group. Spleen weight was slightly greater in bellflower diet group. Rats fed control, bellflower and red ginseng diets showed significantly higher concentrations of TL, TC and TG, but lower concentration of HC in the plasma and liver than rats fed standard diet. Neither dietary bellflower nor red ginseng changed the levels of these four classes of lipid in the plasma of control diet group. But bellflower reduced the level of TG in the liver.

  • 본 논문에서는 대사증후군관련 의료 빅데이터 표준화 시스템의 환경과 소프트웨어 환경을 설계한 사항에 대해서 기술한다. 이를 위해 임상데이터를 기반으로 의료 빅데이터를 수집하고 국제 표준화인 공통 데이터 모델로 수집된 데이터를 ETL하여 통합 데이터베이스에 저장하였다. 본 연구를 통해 구축된 의료 빅데이터 표준화 시스템은 향후 의사결정 보조시스템 개발과 연계하여 효과적인 검색과 다양한 통계 분석을 지원할 계획이다. 또한 병원의 다양한 임상 연구를 지원하기 위한 주요 시스템으로 자리매김할 것으로 기대한다.

Effects of Nutritional Supplementation of Cereal-Vegetable Diet on the Growth of Rats (I) - Especially on Protein Metabolism - (곡류.야채식이의 영양소 보완이 흰쥐의 성장에 미치는 영향 (I) - 단백질 대사를 중심으로 -)

  • This study was designed to find out the nutritional defect of general Korean diet and the effective way of nutritional supplementation. Seventy weanling Sprague-Dawley male rats weighing 51.8$\pm$0.9g were blocked into ten groups and fed ten different diets ad libitum for eight weeks: Standard groups(st gp) was given 72% sugar-20% casein diet: Cereal-vegetable group(c-v gp) was fed cereal-vegetable diet(c-v diet) composed of rice, barley, soybean, spinach and cabbage: the other eight groups were fed c-v diets supplemented with casein, vitamin $B_{2}$, calcium, vitamin A, vitamin B2 and A, vitamin A and calcium, vitamin $B_{2}$ and calcium, vitamin A and $B_{2}$and calcium, respectively, on the basis of each nutrients content of standard diet. The results were as follows: 1. Food intakes and body weight gains in all the experimental groups were significantly lower than st gp. Among experimental groups, casein gp and vit B2 gp tended to gain more body weights than c-v gp. 2. Through all the experimental period, F.E.R., pp.E.R., and NDPcal% did nod show significant differences among all the experimental groups and st gp. 3. The weights of liver, kidney, and gastrocnemius were significantly lower in all the experimental groups as compared with st gp. But brain and sex organ weights did not show differences among all the groups. 4. Nitrogen contents of total carcass, liver, and gastrocnemius in all the experimental groups tended to be increased as compared with st gp, and among experimental groups, they tended to be increased by casein supplementation and decreased by ca supplementation. 5. Apparent nitrogen digestibility, urinary and fecal nitrogen excretion, the amount of nitrogen retained, and N.P.U. did not show any significant differences among all the groups. 6. Serum total protein concentrations did not show any significant differences among all the groups.

Expression and Purification of a Functional Recombinant Aspartate Aminotransferase (AST) from Escherichia coli

  • Aspartate aminotransferase (AST; E.C. 2.6.1.1), a vitamin B6-dependent enzyme, preferentially promotes the mutual transformation of aspartate and ${\alpha}$-ketoglutarate to oxaloacetate and glutamate. It plays a key role in amino acid metabolism and has been widely recommended as a biomarker of liver and heart damage. Our study aimed to evaluate the extensive preparation of AST and its application in quality control in clinical laboratories. We describe a scheme to express and purify the 6His-AST fusion protein. An optimized sequence coding AST was synthesized and transformed into Escherichia coli BL21 (DE3) strain for protein expression. Ideally, the fusion protein has a volumetric productivity achieving 900 mg/l cultures. After affinity chromatography, the enzyme activity of purified AST reached 150,000 U/L. Commutability assessment between the engineered AST and standard AST from Roche suggested that the engineered AST was the better candidate for the reference material. Moreover, the AST showed high stability during long-term storage at $-20^{\circ}C$. In conclusion, the highly soluble 6His-tagged AST can become a convenient tool for supplying a much better and cheaper standard or reference material for the clinical laboratory.

A comparison of the aerobic cost and muscle use in aerobic dance to the energy costs and muscle use on treadmill, elliptical trainer and bicycle ergometry

  • Objective: To determine the energy consumed and muscle use during dance compared to different standard exercise devices. Design: Longitudinal study. Methods: Fifteen female subjects were evaluated to assess the energy cost and muscle activity during a 20 minute dance video compared to treadmill, elliptical track and bicycle ergometry. The later 3 forms of exercise were accomplished in four, 5 minute bouts at different intensities of exercise. Subjects were in the age range of 22-24 years old, were free of cardiovascular disease and did not have any neurological injuries. They were not sedentary and exercised at least twice a week. During the exercise, muscle activity was measured by the electromyogram recorded by surface electrodes on 6 muscle groups. A Cosmed metabolic cart was used to measure oxygen consumption during the exercise. Results: The aerobic dance video that was tested here was equivalent to a hard workout on any of the 3 exercise modalities. The dance routine was equivalent in terms of energy consumed to running at 225 watts of work or running for 20 minutes at a speed of 2 meters per second (4.47 miles per hour). Compared to the bicycle, it was equivalent to cycling at 112 watts for 20 minutes (2.25 kpm), and for the elliptical trainer, dance was equivalent to 435 watts. Concerning muscle use, the dance routine was the most balanced for upper, core and lower body muscles. Although the elliptical trainer was close, it required muscle less muscle use. Conclusion: A good dance video can be more effective than standard exercise equipment.

  • The theory of seven emotions is a unique theory in oriental medicine which describes the mutual relationship between body and mind of human. Although, the term 'Seven emotions' was not clearly indicated in ${\ulcorner}$The Yellow Emperor's Internal Classic(黃帝內經)${\lrcorner}$, it is appeared in ${\ulcorner}$A Treatise on the Three Catagories of Cause of Diseases(三因方)${\lrcorner}$ written by Chen Yan(陳言) in South-Song Dynasty. It seemed that Chen Yan explained seven emotions as the internal etiologic factor according to the classification of seven emotions of ${\ulcorner}$Ye-Gi(禮記)${\lrcorner}$ under the academic influence during Song Dynasy which emphasized more on the standard of right and wrong rather than individual emotion. Meditation or consideration modulates the function of spleen and stomach and the metabolism of blood and body fluid and it also controls the various emotions and maintains the equilibrium of human body. Human emotions are influenced by the changes of nature and deeply related to time and space including social-environmental factors. The function and strength of seven emotions: joy, anger, anxiety, worry, grief, apprehension and fright are determined by the external stimulation as the causes of illness.
