The purpose of this study was to provide data on the normative values of some clinically important soft tissue dimensions for adult Korean females with aesthetically beautiful facial profiles. Lateral cephalograms of 18 Korean female models, who were selected for their well balanced and aesthetic facial profiles, were evaluated. All cephalograms were taken with the subjects in a natural head position with the teeth in occlusion and the lips at rest. The means and standard deviations were determined and presented. In addition, comparisons with the previous studies were performed The results of the present study were as follows: 1. The upper and lower lips were posteriorly located in relation to the Ricketts' E line (Upper lip to E line: -2.08, Lower lip to E line: -0.04). 2. Both lips were more posteriorly located than those in the results of previous studies on Korean females selected by normal occlusion, but more anteriorly located than in the results of studies selected on an aesthetic basis. 3. The nasolabial angle for this sample was 101.03 degrees with a standard deviation of 8.47 degrees.
The Frumkin adsorption isotherm of the over-potentially deposited hydrogen (OPD H) for the cathodic $H_2$ evolution reaction (HER) at the poly-Ni|0.05M KOH aqueous electrolyte interface has been studied using the phase-shift method. The behavior of the phase shift $(0^{\circ}\leq{\phi}\leq90^{\circ})$ for the optimum intermediate frequency corresponds well to that of the fractional surface coverage $(1\geq{\theta}\geq0)$ at the interface. The phase-shift method, i.e., the Phase-shift profile $(-{\phi}\;vs.\;E)$ for the optimum intermediate frequency, can be used as a new method to estimate the Frumkin adsorption isotherm $(\theta\;vs.\;E)$ of the OPD H for the cathodic HER at the interface. At the poly-Ni|0.05M KOH aqueous electrolyte interface, the rate (r) of change of the standard free energy of the OPD H with $\theta$, the interaction parameter (g) for the Frumkin adsorption isotherm, the equilibrium constant (K) for the OPD H with $\theta$, and the standard free energy $({\Delta}G_{\theta})$ of the OPD H with ${\theta}$ are $24.8kJ mol^{-1},\;10,\;5.9\times10^{-6}{\leq}K{\leq}0.13,\;and\;5.1\leq{\Delta}G_{\theta}\leq29.8kJ\;mol^{-1}$. The electrode kinetic parameters $(r,\;g,\;K,\;{\Delta}G_{\theta})$ depend strongly on ${\theta} (0{\leq}{\theta}{\leq}1)$.
Background: The aim of this report is to present a new reference for aesthetic mandible surgery using three-dimensional cone-beam computed tomography-based treatment planning for orthognathic surgery which can be implemented in surgical planning and perioperative procedure. Methods: To make an objective standard for evaluating aesthetic mandibular outline, we make an aesthetic scoring criteria with consideration of asymmetry, broad mandibular border line, and prominent mandibular angle. Two maxillofacial surgeons and two orthodontists rated their aesthetical evaluation from 1 to 5. Experimental group consisting of 47 female and 38 male patients who had rotational orthognathic two-jaw surgery from 2010 to 2011 were chosen according to aesthetic scoring done by two maxillofacial surgeons and two orthodontists. A high aesthetic score (${\geq}16$) means the facial contour is symmetric, with no broad and narrow aesthetic mandible frontal profiles. Control A group consisted of ten female and ten male patients who had no orthognathic surgery experience and low aesthetic score (${\geq}10$). Control B group consisted of ten female and ten male patients who had no orthognathic surgery experience and had anaesthetic mandibular frontal profile and a high aesthetic score (${\geq}16$). The three-dimensional image of the patient was taken from dental cone-beam CT (DCT) scanning (experimental group and control A group: 6 months DCT after surgery, control B group: 1st visit DCT). Each DCT was reformatted to reorient the 3D image using 3D analyzing program (OnDemand3D, cybermed Inc, CA, USA). After selection of 12 landmarks and the construction of reoriented horizontal, vertical, and coronal reference lines, 15 measurements were taken in 3D analysis of frontal mandibular morphology. Afterwards, horizontal and vertical linear measurements and angular measurements, linear ratio were obtained. Results: Mean $Go^{\prime}_{Rt}-Me^{\prime}-Go^{\prime}_{Lt}$ angular measurement was $100.74{\pm}2.14$ in female patients and $105.37{\pm}3.62$ in male patients. These showed significant difference with control A group in both genders. Ratio of $Go^{\prime}_{Rt}-Go^{\prime}_{Lt}-Me^{\prime}$ length to some linear measurements (ratio of $Me^{\prime}-Cd^{\prime}_{Rt}Cd^{\prime}_{Lt}$ to $Me^{\prime}-Go^{\prime}_{Rt}Go^{\prime}_{Lt}$, ratio of $Me^{\prime}-Go^{\prime}$ to $Me^{\prime}-Go^{\prime}_{Rt}Go^{\prime}_{Lt}$, ratio of $Go^{\prime}_{Rt}-Go^{\prime}_{Lt}$ to $Me^{\prime}-Go^{\prime}_{Rt}Go^{\prime}_{Lt}$) showed significant difference with control A group in both genders. Conclusion: This study was intended to find some standard measurement of mandible frontal view in 3D analysis of aesthetic patient. So, these potential measurement value may be helpful for orthognathic treatment planning to have more aesthetic and perspective outcomes.

Purpose: We studied to find out apo-E genotype polymorphism in minimal change nephrotic syndrome(MCNS) and IgA nephropathy(IgAN) and to determine the relationship between apo-E genotype and clinical course of MCNS. Materials and Method: 43 MCNS patients and 15 IgAN patients were examined for apo-E polymorphism. 50 healthy blood donors were examined for apo-E genotype as control. Genomic DNA was prepared front peripheral blood leukocytes according to standard procedures. Results: As compared with control group, e4 allele frequency was significantly increased in MCNS (p<0.01). However, in IgAN e2 allele frequency, however, was 2.6 times higher than normal control (P<0.01). The frequency of e4 allele of frequent relapser group was 4.6 times higher than normal control and was 2 times higher than infrequent relapser group. Conclusion: We think that apo-E typing might be one of the parameters, which should be considered to predict the course of MCNS in children. MCNS with risky HLA profile and E4/4 genotype could indicate the need for a longer steroid administration. And apo-E genotype needs to be considered for the evaluation of therapeutic responses to other drugs. (J, Korean Soc Pediatr Nephrol 2001 ; 5 : 87-99)
Danazol, one of the IOC banned substances, is a synthetic steroid which stimulates the synthesis of cellular protein with multiple and diverse biologic effects. To confirm the androgenic effects of danazol, levels of eight endogenous steroids and its major metabolite in human urine were simultaneously analyzed by selected ion monitoring (SIM) of GC/MS after oral administration. The recovery range of this method was 71.59%~93.56% and the RSD values of precision and accuracy test were 1.87%~10.48% and 1.32%~11.25%, respectively. The limits of detection of most of the drugs were $0.01{\sim}0.05{\mu}g/mL$ and calibration was carried out using urine spiked with each endogenous steroid and ethisterone standard at a concentration of 0.1, 0.5, 1.0, 10, 20 and $50{\mu}g/mL$ and with $10{\mu}g/mL$ of internal standard. Correlation coefficients varied between 0.963 and 0.991. The endogenous steroid profiling is one of valuatle tool for decteation of anabolic steroids.
This study an analyzes the effects of the P/S ratio of dietary lipids and antioxidant vitamin supplements on serum lipids level and fatty acid profile, the degree of lipid peroxidation, and the antioxidant enzyme activities in the liver of rats treated with 7,12-dimethylbenz($\alpha$) anthracene(DMBA). P/S ratio of dietary lipids was made into 0.5, 1 and 2 by mixing palm oil, soybean oil, sesame oil and perilla oil at 10%(w/w) fat level and n-6/n-3 ratio was fixed to 4. Antioxidant vitamin of $\alpha$-tocopherol or $\beta$-carotene was supplemented in addition to vitamin mixture which was given at 1 % of the standard diet. female Sprague-Dawley strain rats, about 60 days old, were divided into three groups(LP : low P/S ratio(0.5), MP : medium P/S ratio (1.0), HP , high P/S ratio(2.0)) and each group was sub-divided into three groups(S ; standard, T ; tocopherol supplemented, C : carotene supplemented): Two weeks after feeding experimental diets, all groups were treated with a single dose of DMBA(2mg/100g BW) by gastric intubation and fed experimental diet for 9 week. The results were as follows ; 1) Serum total cholesterol(TC) level was not significantly influenced by diet but tended to be lower in HP groups compared to LP and MP groups. Triglyceride level was the highest in LP groups and the lowest in $\alpha$-tocopherol supplemented groups. 2) Thiobarbituric acid reactive substance(TBARS) level, representing lipid peroxidation in hepatic microsome, tended to be increased as the unsaturation of dietary lipids increases. $\alpha$-Tocopherol supplement significantly decreased TBARS level. 3) The activities of superoxide dismutase(SOD) and glutathione peroxidase(GSHPx) in hepatic cytosol showed the tendency to be high with increasing P/S ratio of dietary lipids. SOD activity was not significantly influenced by antioxidant vitamin, but GSHPx activity was significantly increased in $\alpha$-tocopherol supplemented groups. In summary, high polyunsaturated fat diet was effective on reducing the serum level of total cholesterol and triglyceride, while it increased unsaturation and peroxidizability of serum fatty acid. With increasing P/S ratio of dietary lipids, lipid peroxidation was increased in the liver and antioxidant enzyme system was induced to inhibit lipid peroxidation against oxidative damage. $\alpha$-Tocopherol supplement was effective in lowering lipid peoxidation, but $\beta$-carotene supplement did not exhibit antioxidant effect. Background: Breast cancer is the most frequent cancer of women in Pakistan with the majority presenting with stage III or IV lesionsat initial diagnosis. Patient and health system related factors are well known determinants of delay in presentation and diagnosis. Additionally, breast cancer being a heterogeneous disease, the various molecular subtypes featuring different aggressiveness also need to be considered. The present study evaluated the association of stage at initial diagnosis of breast cancer with these two factors in local women at a tertiary level health care facility in Lahore, Pakistan. Materials and Methods: Patient and tumor features were recorded separately during diagnostic workup in Breast Clinics at INMOL and at Services Hospital, Lahore. Data were entered in MS Excel and analyzed by descriptive statistics and Chi-Square test. Results: Among the 261 patients, 64% were staged as late breast cancer (LBC), the mean age was 46.8 with standard deviation of 13 years. Some 92% had invasive ductal carcinoma (IDC), 61% had luminal types (LT) of non-aggressive tumor while 39% had the non-luminal types (NLT) of of HER2-enriched or basal aggressive tumors. While 70% of patients presented within one year of symptomatic disease (early report group "ERG"), 30% reported after a mean delay of 4 years with a standard deviation of 3.75 years. The stage distribution among ERG patients was not statistically different from those reporting late (P=0.123). Statistically larger proportion of patients with NLT presented as LBC as compared to the LT (P =0.034). Among the ERG, statistically different stage distribution of disease was observed for the NLT versus LT (P=0.047). Among those presenting late, this difference was insignificant (P=0.416). Conclusions: Breast cancer is a distinct disease in Pakistan with a high frequency of aggressive molecular types affecting younger women, with the majority presenting as LBC. Association of NLT with higher stage at diagnosis is statistically significant whereas time delay in diagnosis is not. Further research is required to define the risk profile and features in local patients. The burden of LBC can be reduced by promoting breast health awareness and by establishing easily accessible dedicated breast care set ups in the hospitals.
Purpose: Although several chemotherapy regimens used against advanced gastric cancer (AGC) have been studied extensively in an attempt to further improve the prognosis of patients, to date, no standard chemo-therapeutic regimens have been established. The aim of this study was to determine the anti-tumor efficacy and safety of TS-1 or TS-1 plus cisplatin (CDDP). Material and Methods: We treated 78 patients with AGC either with $80\;mg/m^{2}$ of TS-1 for 28 days, which was followed by a 2-week rest, or with $80\;mg/m^{2}$ of TS-1 for 21 days and $80\;mg/m^{2}$ of CDDP on day 8 every 5 weeks. Results: Tumor response rates in the neoadjuvant chemotherapy group and in the recurrent or post-palliative surgery group were 87.5% and 32.4%, respectively, and they were 28.6% and 48.4%, respectively, in the TS-1 group and the TS-1 plus CDDP group. The survival rates in the recurrent and the post-palliative surgery group were significantly different according to the degree of tumor response (P=0.0016), but the one-year survival rates according to the kinds of regimens (TS-1 or TS-1/CDDP group) were not significantly different. The incidences of grade 3 or 4 adverse effects in the TS-1 and the TS-1/CDDP groups were 14.3% and 36.8%, respectively. Conclusion: The anti-tumor efficacy and safety of TS-1 and TS-1 plus CDDP in Korean patients with AGC seemed to be high with modest adverse effects, thus suggesting the possible use of this regimen as a standard chemotherapy for gastric cancer.

Understanding the airflow characteristics within the canopy structure installed between closely adjacent tunnels either for light adaptation or for protection from snow hazards is required for the normal ventilation as well as safety system design. Grade, horizontal alignment, cross-sectional area and shape are known to substantially influence the fire smoke behavior and their influences raise great concern for the safety design. This paper aims at studying the effects of tunnel geometrical characteristics and canopy installation on the ventilation and fire propagation through CFD analysis. In the case of 145m long canopy, 50% opening ratio is preferred with respect to the airflow pattern and ventilation efficiency. When a 20MW fire occurs in a 1.8km-long tunnel and four 1250mm reversible jet fans are instantly turned on, smoke concentration at 40m downstream of the fire decrease 13% for the upgrade tunnel with 2% gradient and increases 20% for -2% gradient, compared to the standard horizontal tunnel. Backlayering is observed within 45m-long segment toward the entrance in 2% down-graded tunnel. In a rectangular tunnel, there is no significant difference of smoke concentration as well as velocity profile from the standard crown tunnel. Three-laned tunnel shows lower level of both profiles and backlayering is detected up to 50m upstream of the fire, while the risky situation rapidly disappears thereafter.

Background: Wedge filters are commonly used in radiation oncology for eliminating hot spots and creating a uniform dose distribution in optimizing isodose curves in the target volume for clinical aspects. These are some limited standard physical wedges ($15^{\circ}$, $30^{\circ}$, $45^{\circ}$, $60^{\circ}$),or creating an arbitrary wedge angle, like motorized wedge or dynamic wedge,${\ldots}$ The new formulation is presented by the combination of wedge fields for determining an arbitrary effective wedge angles. The isodose curves also are derived for these wedges. Materials and Methods: we performed the dosimetry of Varian Clinac 2100C/D with Scanditronix Wellhofer water blue phantom, CU500E, OmniPro - Accept software and 0.13cc ionization chamber for 6Mv photon beam in depth of 10cm (reference depth) for universal physical wedges ($15^{\circ}$, $30^{\circ}$, $45^{\circ}$, and $60^{\circ}$) and reference field $10.10cm^2$. By combining the isodose curve standard wedge fields with compatible weighting dose for each field, the effective isodose curve is calculated for any wedge angle. Results: The relation between a given effective wedge angle and the weighting of each combining wedge fields was derived. A good agreement was found between the measured and calculated wedge angles and the maximum deviation did not exceed $3^{\circ}$. The difference between the measured and calculated data decreased when the combined wedge angles were closer. The results are in agreement with the motorized single wedge appliance in the literature. Conclusions: This technique showed that the effective wedge angle that is obtained from this method is adequate for clinical applications and the motorized wedge formalism is a special case of this consideration.
