장기요양시설 간호제공자의 구강건강관리에 대한 지식과 영향요인 (Knowledge of Oral Health and Its Predictors in Nursing Staff of Long-term Care Institutions)

  • Purpose: The purposes of this study were to assess the oral health knowledge of nursing staff in long-term care institutions and to identify predictors of oral health knowledge. Method: For this descriptive correlation study, a self-administered and structured questionnaire was used. Respondents were 111 nursing staff in two long-term care facilities and two long-term care hospitals located in G metropolitan area and C province in the Southern part of Korea. Descriptive statistics, t-test or ANOVA, and stepwise multiple regression analysis were used to analyze the data. Results: Participants in this study did not have many opportunities to learn about oral health care for elders in long-term care institutions. The percentage of correct answer for oral health knowledge questionnaire was 64.5%. Predictors of oral health knowledge among nursing staffs were education on oral health in long-term care institutions, type of institution, and length of time working with elders. These three variables accounted for 24.2% of variance in oral health knowledge. Conclusions: Nursing staffs should make an effort to improve their knowledge of oral health. Moreover, oral health educational program for nursing staffs working with elders in long-term care institution is need to be developed and the effectiveness of this education needs to be evaluated.

노인장기요양 인정조사원의 의사소통능력, 공감만족 및 공감피로가 소진에 미치는 영향 (The Effects of Communication Skills, Compassion Satisfaction, Compassion Fatigue on Burnout among Staff of Long-term Care Insurance for the Elderly in National Health Insurance Services in Korea)

  • Purpose: This study was a descriptive research to identify the effects of communication skills, compassion satisfaction, compassion fatigue on burnout among staff of long term care insurance for the elderly in National Health Insurance Services in Korea. Methods: A descriptive cross-sectional design was used. The participants were 191 staffs of long-term care insurance in National Health Insurance Services. Data were collected via mail from the 24 branch offices which were randomly selected among the total of 226 centers of National Health Insurance Corporation. Stamm's professional quality of life (ProQOL) and Communication Skills Test tool were included in questionnaire to detect. SPSS/WIN 20.0 was used to conduct the descriptive statistics, t-test, ANOVA, correlation analysis, and multiple regression. Results: Compassion fatigue had a significant positive effect on burnout, while Compassion satisfaction had a negative effect on burnout. The explained variance for burnout was 69% and compassion fatigue was the most significant factor in burnout among staff of long term care insurance services. Conclusion: The results indicate that the factors influencing staffs' burnout are compassion fatigue, compassion satisfaction, and education level. Therefore, strategies to decrease compassion fatigue and improve compassion satisfaction are needed to decrease burnout for staffs of long-term care insurance in National Health Insurance Services.

  • This study was conducted to the cognition and attitude for internal and external organizational behavior of health centers staff. An analytical model employed in this study was developed by modifying 'organizational behavior model'. Data was collected a mail survey of officers at health center, including 66 directors and 1,768 staffs of the health centers in southern region. The major findings are as follows : Among directors of the health centers 47% of them stated that legislators and chief executive officer(CEO) of local governments are highly concerned about the public health program. However only 15.2% of health center directors reported that legislators and CEO put public health high priority. For leadership type of directors 39.0% of them was classified as controller, 30.3% as participant, 22.7% entertaining and 7.6% comprehending. Regarding sociopsychological characteristics of the health center staffs, about a quarter of them had high degree of group cohesion, while 10.7% had low degree. Those staffs who are older, high educational level, or working in the rural health centers showed higher degree of cohesion than those who are young, low educational level, or working in urban health center. A third of them were less likely to be satisfied by what they are doing at the health centers. The higher educational level, the likely to be satisfied by what they are doing at the health centers. The higher educational level, the more satisfied with their job. Considering these results, policy implication was discussed and suggested. It is suggested that educational efforts be made to improve leadership of the health center directors and concern with public health program by the CEOs and legislators of local governments.

원주시 치과의원 종사자의 금연지도활동에 대한 실태조사 (Tobacco Cessation Activities of Dental Staffs in Wonju City)

  • 치과종사자의 금연에 대한 태도, 금연지도활동의 실태, 금연지도활동의 장애요인에 대한 자료를 조사 분석하여 금연지도활동을 확대하는데 필요한 자료를 얻기 위하여 강원도 원주시 구강보건지료기관에 근무하는 치과의사, 치과위생사, 간호조무사, 치과코디네이터를 대상으로 하여 설문조사법을 적용하여 다음과 같은 결과를 얻었다. 1. 치과종사자의 금연에 대한 태도점수의 평균 6점 만점에서 5.18점(${\pm}1.21$)으로 비교적 금연에 대해 긍정적인 태도를 가지고 있는 것으로 나타났다. 2. 치과종사자의 금연지도활동 점수의 평균은 12점 만점에서 6.67점(${\pm}1.85$)으로 비교적 낮게 나타났다. 3. 금연지도활동의 장애요인으로는 금연문제를 거론하기가 힘들다는 응답이 42.3%로 가장 큰 장애요인으로 나타났다. 4. 치과종사자의 금연지도활동 점수와 통계적으로 유의한 상관관계를 보인 변수는 금연에 대한 태도, 연령, 금여권유 시, 금연 가능성에 대한 인식이었다. 이상의 결과로부터 구강건강관리인력의 금연지도활동에 대한 자기효능감을 높이기 위해 정규교과과정이나 보수교육 프로그램의 개발을 통행 지속적으로 금연지도활동에 대한 이론 및 기술교육을 통해 제공해야 하며, 금연중재 프로그램의 적용과 관련하여 프로그램의 효과를 평가할 수 있는 연구가 필요하다.

정신건강의학과 직원 대상의 마음챙김명상 기반 인지치료의 효과 (The Effect of Mindfulness-Based Cognitive Therapy on Psychiatric Staff)

  • 연구목적 정신건강의학과 직원들이 겪는 임상현장에서의 과도한 업무 스트레스, 소진의 심화, 업무 만족도 감소 등으로 인해, 전반적인 근무 효율이 저하되거나 스스로의 정신건강 관리에 어려움을 겪을 가능성이 증가할 수 있다. 정신건강의학과 직원을 대상으로 한 그룹 Mindfulness-Based Cognitive Therapy(이하 MBCT)의 업무 스트레스, 소진, 자기 효능감, 회복력, 업무 만족도에 대한 효과를 알아보고자 하였다. 방법 일 종합병원 정신건강의학과 소속 직원 중 5개 직역 28명을 대상으로 연구 참여 동의 하에 그룹 MBCT를 시행하였다. MBCT 시행 전 후에 각각 업무 스트레스, 소진, 회복력, 자기 효능감, 업무 만족도를 자가보고식 도구를 이용하여 측정하였고, 그룹 MBCT의 효과를 비교하기 위해 각 척도 별 MBCT 전 후 점수를 비교하였다. 결과 각 척도 별 그룹 MBCT 시행 전 후의 업무 스트레스, 소진 척도 점수는 MBCT 시행 후 유의한 감소를 보였으며, 회복력, 업무 만족도, 자기 효능감 척도 점수는 MBCT 시행 후에 시행 전보다 유의한 증가를 나타내었다. 결론 정신건강의학과 직원 대상의 그룹 MBCT가 직원들의 업무 스트레스와 소진을 줄이고, 회복력, 자기 효능감 및 업무 만족도를 향상시키는 데에 도움이 되는 것으로 나타났다. 이는 정신건강의학과 치료환경에서 직원들이 그룹 MBCT를 통하여 본인의 정신건강을 증진시킬 수 있음을 시사한다. 정신건강의학과 직원들의 정신건강 증진은, 나아가 치료환경에 있는 환자에게도 긍정적인 영향을 끼칠 수 있을 것이다.

대학 교직원의 라이프스타일에 따른 세분시장별 대학 교직원 급식소 서비스 품질 인식 분석 (Measuring Service Quality Perception of University Faculty Members & Staffs Towards Faculty Foodservice Based on Lifestyle Segmentation)

  • Market segmentation helps providers to find better marketing opportunities and allows foodservice managers to develop the right product for each target market. Therefore, this study, taking university faculty and staff as subject, is intended to diagnose the relative value of service quality attribute, on the basis service quality scenario of faculty foodservice; to suggest price for improving customer loyalty in market segments. A questionnaire was developed ar d mailed to 600 Yonsei university faculty and staffs. A total of 385 questionnaires were usable; resulting in a 58.7% of faculty and a 69.7% of staff response rate, respectively. Statistical data analysis was completed using the SAS/Win 6.12 for descriptive Analysis, ANOVA, principal factor analysis, cluster analysis, reliability test and discriminant analysis. The results of the study are as below. Eighteen questions were selected for measuring respondents' lifestyle by AIO method and the seven lifestyle factors derived from factor analysis and aggregated distinct 4 clusters. Service quality attributes of the scenario were determined with 'food quality', 'menu variety', 'atmosphere', 'fast service', and 'clean and sanitation'. 'Food quality', 'menu variety', 'atmosphere', 'fast service', and 'clean and sanitation', in decreasing order, were identified as improving customer loyalty. However, most faculty and staffs were satisfied with the present meal price. The result of this study indicates that the relative value of service quality was differed significantly among the various market segments. 'Food quality', 'menu variety', and 'atmosphere' were determined as major service quality attributes. Thus, customer loyalty could be increased by improving food taste and quality, atmosphere, and service delivery. (Korean J Community Nutrition 8(4) : 556 ∼565, 2003)

대학 급식소의 식수예측 기법 운영 현황 (Investigation on the Performance of the Forecasting Model in University Foodservice)

  • The purpose of this study was to investigate the utilization level of forecasting methods in contract foodservice management companies. Questionnaires were distributed and collected from 30 foodservice management companies contracted with universities and 49 university foodservices in Seoul and Kyungki area. Statistical data analysis was performed using SPSS/WIN 10.0 based on the production records of Yonsei University foodservices and the weather reports from a meteorological observatory. The results of this study were as follows: 1) The objectives of the fore-casting systems were identified as saving costs through eliminating the leftover, meeting the customer demands, and improving efficiency in food preparation.2) All of the university foodservices were already performing the forecasting methods but in foodservice management companies as a whole,89.7 percents were applying the method and only 55.2 percents had the separate forecasting department. 3) A large number of foodservice staffs in the head office (65.5%) answered that they often utilized intuitive estimates based on the past experiences and records for forecasting while 65.3% managing staffs in the university foodservices answered the same.4) Both in the head office and university foodservices, actual number of meals served were recorded. In the head office, mostly estimated numbers and actual numbers of meals were recorded while estimated, prepared, and actual numbers of meals served were recorded for most of the cases in university foodservices. 5) The primary factors considered for forecasting were the actual production records for the last month, the customer preference for the selected menu items, and the specific day of the week.

호텔 중식 주방 한.중 조리 종사원의 갈등 요인 분석에 관한 연구 (A Study on Analysis of Conflicting Factors between Kitchen Staffs of Korean and Chinese in Chinese Restaurant of Hotel)

  • The aim of this study was to analyze conflicting factors between Korean and Chinese Kitchen staff within the Chinese restaurants if hotels, and formulate a plan for resolving problems. A closed-end questionnaire was used for survey, which was conducted between 30 April 2005 and 4 June 2005. The survey was conducted among Koran and Chinese (China/Taiwan) cooking staff at Chinese restaurants within 5 star hotels located in Seoul, Korea. 231 questionnaires were distributed to the staff members, and the data collected and analyzed Form the results, Chinese staffs that have more cooking experience were found to be in higher positions and receive higher salaries. In addition, compared to Korean staff, the Chinese staff had fewer problems with conflicts from personnel adminstration, personnel adminstration, personal/psychological areas, benefits/rewards and relationship with their outlet manager. Conversely, Korean staff experience more disagreements in each conflict factor area than Chinese staff, implying there is a divergence between Korean and Chinese staff.

우리나라의 의료서비스 증진에 영향을 미치는 조세감면 수용성에 관한 연구 (A Study on the Influence of the Acceptability of Tax Expenditures to the Improvement of Healthcare)

  • It is needed for human-being to get the better healthcare and to be educated well for happiness. In korea, they have had less interest in the healthcare than the education. In this study, the role of taxes was surveyed for the improvement of the healthcare. Many articales on the tax system of the healthcare were investigated. And it was searched also what is the key factors of tax expenditures for the improvement of the healthcare. Through a survey to the staffs of medical services, tax experts, the receiver of medical services and tax officiers, the gap of opinions in each group and the factors of tax expenditures were analysed. The findings of this study are as following: Firstly, For the improvement of the healthcare the acceptability of tax deduction is needed. Secondly, is found that there is a meaningful difference between the staffs of medical services and tax officiers in the acceptability of tax expenditures.

병원 학대피해아동보호팀의 아동학대 사망사건 개입경험 연구 (A Qualitative Study on Intervening Work Experiences of Hospital-Based Child Protection Team on Child Abuse Death Cases)

  • 이 연구는 아동학대 예방과 개입의 큰 축을 담당하는 병원 학대피해아동보호팀의 아동학대 사망사건 개입경험을 탐색할 목적으로 수행되었다. 이를 위하여 전국적으로 등록된 62개 병원 학대피해아동보호팀과 접촉하였고, 최종적으로 아동학대 사망사건을 경험한 5개 병원 학대피해아동보호팀 구성원들(의료사회복지사와 의사)과의 인터뷰를 통해 자료를 수집하였다. 수집된 자료를 질적연구방법을 사용하여 분석한 결과, 팀의 개입에도 불구하고, 학대 피해 아동이 매우 위중한 상태에서 병원에 도착했거나, 보호자의 거짓말이나 치료 거부로 인한 치료적 대응의 지연 때문에, 아동의 사망 진행을 막기 어려웠던 것으로 나타났다. 그럼에도 학대피해아동보호팀의 의심과 개입을 통해 사망 원인이 학대임을 밝혀낸 것은 팀 활동의 의미 있는 성과로 간주되었다. 이 과정에서 신고의무자 역할에 대한 인식 부족과 부담은 의료진의 적극적인 개입을 막는 장애물이며, 개입을 지속하기 위해서는 학대 인과관계 규명에 필요한 증거자료의 확보가 중요하다는 점이 확인되었다. 이러한 연구 결과를 토대로 학대 상황에 대한 조기 발견과 증거 확보 조치, 사망 원인 규명을 위한 실천적 및 정책적 방안에 대해 제언하였다.
