어깨 가쪽돌림 운동 시 운동방법과 아래팔의 자세에 따른 가시아래근과 뒤어깨세모근의 근활성도 비교 (Comparison of Infraspinatus and Posterior Deltoid Muscle Activities According to Exercise Methods and Forearm Positions During Shoulder External Rotation Exercises)

  • Background: Shoulder external rotation exercises are commonly used to improve the stabilizing ability of the infraspinatus. However, during exercise, excessive activation of the posterior deltoid compared to the infraspinatus causes the humeral head to move anteriorly in an abnormal position. Many researchers have emphasized selective activation of the infraspinatus during shoulder external rotation exercise. Objects: This study aims to delineate the optimal exercise method for selective activation of infraspinatus by investigating the muscle activities of the infraspinatus and posterior deltoid according to the four shoulder exercise methods and two forearm positions. Methods: Thirty healthy individuals participated in this study. The participants were instructed to perform shoulder external rotation exercises following four exercise methods: sitting external rotation (SIER); standing external rotation at 90° abduction (STER); prone external rotation at 90° abduction (PRER); side-lying external rotation (SLER), and two forearm positions (neutral, supinated). The electromyography (EMG) signal amplitude was measured during each exercise. Surface EMG signals were recorded from the posterior deltoid, infraspinatus, and biceps brachii. Results: EMG results of the infraspinatus and posterior deltoid in PRER, were significantly higher than that of the other exercises (p < 0.01). The EMG ratio (infraspinatus/posterior deltoid) in SIER was significantly higher than that of the other exercises. EMG activation of the posterior deltoid in SIER, PRER, and SLER was significantly higher in neutral than in supinated (p < 0.01). Furthermore, the EMG of the infraspinatus in SIER was significantly higher in neutral than in supinated (p < 0.01). The EMG ratio (infraspinatus/ posterior deltoid) in SIER was significantly higher in neutral than in supinated (p < 0.05.) Contrarily EMG ratios in PRER and SLER were significantly higher in supinated than in neutral (p < 0.05). Conclusion: The results show that clinicians should consider these exercise methods and forearm positions when planning shoulder external rotation exercises for optimal shoulder rehabilitation.

Effects of Bridge Exercise with Pelvic Compression Belt on Electromyographic Activities of Selected Lumbopelvic Muscles in Young Adults with Lumbar Instability

  • PURPOSE: This study assessed the effect of the pelvic compression belt on the electromyographic activity of erector spinae (ES), internal oblique (IO), rectus femoris (RF), and biceps femoris (BF) after bridge exercise with pelvic belt compression in subjects with lumbar instability. METHODS: Forty subjects with lumbar instability volunteered for this study. We asked them to perform the bridge exercise while wearing a pelvic belt compression for 30 minutes five times weekly over a six week period. The pelvic compression belt was adjusted just below the anterior superior iliac spines with stabilizing pressure using elastic compression bands during bridge position. Surface electromyographic data were collected from the erector spinae (ES), internal oblique (IO), rectus femoris (RF) and biceps femoris (BF). RESULTS: After the six week intervention, the experimental group improved significantly. Muscle activation significantly decreased in the erector spinae, rectus femoris, and biceps femoris, and increased in the internal oblique muscle in bridge position while wearing a PCB (P <.01). CONCLUSION: Our findings suggest that the bridge exercise with pelvic belt compression is helpful to reduce activation in superficial muscles and lower extremity muscles such as erector spinae (ES), rectus femoris (RF), biceps femoris (BF) and increase activation in deep trunk muscle such as the internal oblique (IO).

푸쉬업 플러스 동작 시 동측과 반대측 다리 들기에 따른 우측 어깨근 활성도 비교 분석 (Comparative Analysis of the Right Shoulder's Muscle Activity When Lifting Ipsilateral and Contralateral Legs during the Push Up Plus Exercise)

  • The purpose of this study is to conduct a comparative analysis of the right shoulder's muscle activity when lifting ipsilateral and contralateral legs during the Push Up Plus (PUP) exercise, which is a typical shoulder stabilizing exercise, and to provide effective data for a shoulder stabilization exercise. Upper trapezius, lower trapezius, levator scapula, supraspinatus, infraspinatus, deltoid posterior, serratus anterior and pectoralis major, which are eight main muscles of a shoulder, were analyzed for the left and right leg lifting by using an electromyogram (EMG). The study revealed that the muscle activities of the right shoulder's upper trapezius, levator scapula, supraspinatus, serratus anterior and pectoralis major were higher when lifting an ipsilateral (right side) leg, compared to lifting a contralateral (left side) leg. Therefore, lifting an ipsilateral leg can be an effective method for enhancing the maneuverability (mobility) of the right shoulder when lifting a single leg.

견갑골 안정화 운동이 유방암 절제술을 한 여성의 견관절 가동범위, 통증 및 기능수준에 미치는 영향 (Effects of Scapular Stabilization Exercise on the Range of Motion, Pain, and Function in the Shoulders of Women with Breast Cancer Surgery)

  • Background: To compare the effects of scapular stabilization exercise on the range of motion, pain, and function in the shoulders of women with breast cancer surgery. Methods: Fifteen patients were performed scapular stabilizing training using dumbbells for 30 minutes daily, five times per week for 4 weeks. The range of motion, pain, and function in the shoulder were assessed. Goniometer was used for ROM of shoulder external rotation. Shoulder pain and disability index was used for pain and function of shoulder. Comparison of the pre and post intervention was calculated by a paired t-test. Results: After 4 weeks of training, range of motion, pain and function in the shoulder improved significantly in patients with breast cancer surgery (p<.05). Conclusion: These findings indicate the more favorable effects from scapular stabilization exercise in patients with breast cancer surgery.

편마비 환자에서 슬링을 이용한 체간-골반 안정성 집중 훈련이 근활성도 및 균형능력에 미치는 영향 (Effect of Intensive Trunk-Pelvic Stabilization Training Using Sling on Muscle Activity and Balance in Hemiplegia)

  • 본 연구의 목적은 슬링을 이용한 편마비 환자의 체간-골반 안정성 집중훈련 시 체간의 근활성도 및 균형능력에 미치는 효과를 알아보고자 하였다. 6개월 이상에서 1년 된 편마비로 진단 받은 입원 환자 20명을 대상으로 무작위 추출하여 두 그룹으로 나누었다. 실험은 4주간, 주 5회, 1회당 30분간 운동치료를 시행하였고, 대조군(n=10)은 일반적 운동훈련만 적용하였고, 실험군(n=10)은 일반적 운동훈련 및 슬링을 이용한 체간-골반 안전성 집중훈련을 실시하였다. 훈련 효과를 알아보기 위해 훈련 전 후에 체간조절능력, 근활성도, 균형능력을 각각 측정하였다. 훈련 후, 유의한 차이는 실험군에서 체간조절능력(TIS)(p<.001), 특히, 정적조절능력(p<.05), 동적조절능력(p<.01), 협응능력(p<.05), 근 활성도는 RA(p<.001), EO(p<.001), ES(p<.001), Multifidus(p<.05), FMA (Balance)(p<.01), MTD-Balance에서는 정적(p<.001), 동적균형능력(p<.001) 모두 각각 유의한 차이를 보였다. 이로써 슬링을 이용한 체간-골반 안정성 집중훈련은 체간의 근활성도와 균형능력 향상에 효과적인 치료임을 증명하였고, 균형에 있어서 체간-골반 안정화 및 조절의 중요성을 확인할 수 있었다.

A Comparison of Shoulder Stabilizer Muscle Activities of Therapeutic Climbing and Isometric Exercise in Patients with Shoulder Impingement Syndrome

  • Purpose: The purpose of this study was to compare therapeutic climbing exercise and general isometric exercise in patients with shoulder impingement syndrome. Methods: Among 20 adults, study subjects were arbitrarily classified into an experimental group of 10 and a control group of 10. The control group performed general isometric exercise (ISE) and the experimental group performed therapeutic climbing exercise (TC) (3 sets, 3 times per week for 8 weeks). To evaluate the effects of exercise, subjects were evaluated using a Disabilities of the arm, shoulder and hand score (DASH), a goniometer for range of motion, and shoulder activity measured serratus anterior, upper trapezius, and lower trapezius. Independent and paired t-test were used for comparison of the effect between groups. Results: DASH scores showed a significant decrease in both groups after 8 weeks of treatment (p<0.001) and significant difference was observed between the TC groups (p<0.01). Flexion and abduction were significantly increased after 8 weeks of treatment in the ISE group (p<0.001) and flexion, abduction, external and internal rotation were significantly increased after 8 weeks of treatment in the TC group (p<0.001). Serratus anterior and lower trapezius activity were significantly increased after 8 weeks of treatment (p<0.001) and upper trapezius activity was not significantly increased after 8 weeks of treatment in the ISE group (p>0.05). Serratus anterior, lower trapezius, and upper trapezius activity were significantly increased after 8 weeks of treatment in the TC group (p<0.001). Conclusion: Scapular stabilizing exercise using a therapeutic climbing exercise increases range of motion and decreases DASH, and increases activity of shoulder muscles in patients with impingement syndrome.

The Effects of Curl-up Exercise using XCO on Trunk Muscle Activation in Healthy Adults

  • Purpose: This study examined the effects of curl-up using XCO® on trunk muscle activation in healthy adults. Methods: This study design was a single-blind randomized controlled trial. Twelve participants were enrolled in this study. The subjects were instructed to perform curl-up exercise in STCU (straight curl-up), LTCU (left twist curl-up), RTCU (right twist curl-up), TWCU (twist curl-up), and PPCU (power push curl-up). Electromyography was used to assess the percent maximal voluntary isometric contraction (%MVIC) of the rectus abdominis (RA), external oblique (EO), internal oblique (IO), and erector spinae (ES) muscles. The data were analyzed using two-way ANOVA with a repeated measure. The statistical significance level was set to α=0.05 Results: The IO showed significant differences in the main effect of the group and the interactions between the group and exercise (p<0.05). In particular, the IO represented the interactions between group and exercise in the RTCU and PPCU (p<0.05). The RA, EO, and ES did not show significant interactions between the group and exercise (p>0.05). The RA, IO, and EO showed significant differences in the main effect of the group (p<0.05). The EO showed a significant difference in the main effect of exercise (p<0.05). Conclusion: These findings suggest that XCO® can be used to increase the muscle activation of the internal oblique, which is the lumbar stabilizing muscle, when XCO® is combined in the curl-up exercise with rotation. In the future, research on the intensity, frequency, and duration of XCO® exercise will be needed according to the individual characteristics and preferences.

카이로프랙틱 요법과 요통 운동 요법이 요추 자세 및 통증 변화에 미치는 영향 비교 - 통학버스 이용 대학생을 대상으로 - (The Effects of Chiropractic Therapy and Low Back Exercise of University Students in Commuter)

  • Objectives: The Purpose of this study was to investigate the effects of chiropractic treatment and low back exercise on lumbar lordosis angle, gravity line, range of motion, and pain degree of university students who are taking a commuter bus at least 4 hours of round trip. Methods: A group of 15 participants received chiropractic treatment around the lumbar spine region twice per week for 8 weeks. Another group of 15 participants were treated with low back exercise three times per week for 8 weeks. Results: In the present results, both chiropractic treatment and low back exercise did not affect the change of lumbar lordosis. However, chiropractic treatment also significantly improved the range of the lumbar motion, including Flexion, Extension, Right Lateral Flexion and Left Lateral Flexion(p<0.05), and consequently decreased the pain degree. Low back exercise significantly moved the lumbar gravity line to almost normal scale, and improved the range of the lumbar motion, including Flexion, and Right Lateral Flexion (p<0.05), resulting in the reduction of pain degree, although both chiropractic treatment and exercise treatment did not change the lumbar lordosis angle at the statistically significant. Conclusions: From these results, it can be inferred that chiropractic treatment might be beneficial to alleviating the low back pain of the university students using a commuter bus by improving the range of lumbar motion or stabilizing the lumbar gravity line.

복부 드로우-인 기법을 동반한 슬링 운동이 젊은 여성의 복부 근육들의 두께에 미치는 영향 (Effect of the Sling Exercise With Abdominal Drawing-In Maneuver on Thickness of Abdominal Muscles in Young Women)

  • Purpose: The purpose of this study was to explore the influence of a sling suspension exercise with abdominal drawing-in maneuver (ADIM) on the thickness of abdominal muscles. Method: Twenty healthy young women volunteered for this study, and they were randomly assigned to either the control group or experimental group. Subjects of both groups performed the ADIM in standard method. And subjects of the experimental group underwent a structured sling exercises additionally, which consists of bridging exercise on supine, elbow support and trunk control exercise on prone, and trunk flexion exercises. The exercises were performed thirty minutes per day, 3 times a week for a 4-week period. Thickness of abdominal muscles (transverse abdominis; TrA, internal obliques; IO and external obliques; EO) was measured by using real-time ultrasonography. Results: After the intervention, subjects of the experimental group appeared to be significantly increased for the thickness of the TrA (p<.05). However, significant difference weren't found for the IO and EO (p>.05). Conclusion: These findings suggest that sling suspension exercise with the ADIM may be favorably used to augment trunk stabilizing effort by increasing TrA thickness. Further studies need in this field.

다양한 자세에 따른 턱 당기기 운동이 목과 어깨의 근활성도에 미치는 영향 (Effect of Chin Tuck Exercises on Various Postures and Muscle Activity of the Neck and Shoulder)

  • Purpose: This study examined how performing chin tuck exercises in supine, sitting, and standing positions affects changes in the muscle activity of the neck and shoulder. Methods: Seventeen men and three women with forward head posture participated in the study. The subjects performed the chin tuck exercise using a pressure biofeedback unit. After determining the ideal order for performing the chin tuck exercise in supine, sitting, and standing positions through randomized controlled trials, muscle activity was measured in the dominant-side sternocleidomastoid, scalenus anterior, upper trapezius, and levator scapulae. Muscle activity was measured three times in each muscle for each position, and the average of the three measurements was used for analysis. Results: In this study, the chin tuck exercise performed in a supine position reduced muscle activity of the sternocleidomastoid, scalenus anterior, upper trapezius, and levator scapulae significantly more than performing the exercise in either sitting or standing positions (p < 0.05). No significant difference in muscle activity was observed when the exercise was performed in a sitting position versus a standing position (p < 0.05). Conclusion: Performing a chin tuck exercise in a supine position is more effective for stabilizing the neck and shoulder than performing it in a sitting or standing position, as it reduces excessive tension and fatigue in the neck and shoulder.
