A scalar MSDD with multiple antenna reception of Differential Space-Time π/2-Shifted BPSK Modulation

  • In this paper, the issue of blind detection of Alamouti-type differential space-time (ST) ${\pi}/2$-shifted BPSK modulation in static Rayleigh fading channels is considered. We introduce a novel transformation to the received signal from each receiver antenna such that this binary ST modulation, which has a second-order transmit-diversity, is equivalent to QPSK modulation with second-order receive-diversity. The pre-detection combining of the result of transformation allows us to apply a low complexity detection technique specifically designed for receive-diversity, namely, scalar multiple-symbol differential detection (MSDD). With receiver complexity proportional to the observation window length, our receiver can achieve the performance 1.5dB better than that of conventional differential detection ST and 0.5dB worse than that qf a coherent maximum ratio combining receiver (with differential decoding) approximately.

  • A 61-year-old man presented with chest pain and ST elevation, and he underwent coronary angiography under the impression of acute myocardial infarction. Coronary intervention under intra-aortic balloon pumping was necessary due to his hemodynamic instability from the acute total occlusion of a large obtuse marginal branch. In spite of successful intervention, the cardiogenic shock persisted, and so extracoporeal membranous oxygenation was performed to treat this. Afterwards, the cardiogenic shock still persisted, and the auscultatory and echocardiographic findings revealed severe acute mitral valve regurgitation. Emergency mitral valve replacement was then performed. The ECMO and IABP were removed on the $2^{nd}$ postoperative day. The patient was discharged on the $48^{th}$ postoperative day.

  • In order to offer specific information needed to assist in operation of a ship with same type rudder through evaluating the maneuverability of training ship A-Ra with flapped rudder, sea trials based full scale for turning test, zig-zag test with rudder angle 10° and 20°, and spiral test at service condition were carried out on starboard and port sides around Jeju Island according to the standards of maneuverability of IMO. As a result, the angular velocity of port turn was higher than that of starboard turn. Therefore, the size of turning circle was longer on the starboard side. In addition, variation of the transfer due to various factors was more stable than those of the others. In the Z-test results, the mean of 1st and 2nd overshoot angles were 9.8°, 6.3° and 15.3°, 9.2° respectively when the port and starboard was 10°; the 1st overshoot angle were 18°, 13.7° when using 20°. Her maneuverability index T' and K' can be easily determined by using a computer with the data obtained from Z-test where K' and T' are dimensionless constants representing turning ability and responsiveness to the helm, respectively. In the Z-test under flap rudder angle 10°, the obtained K' value covered the range of 2.37-2.87 and T' was 1.74-3.45. Under the flap rudder angle 20°, K' and T' value showed 1.43-1.63, 1.0-1.73, respectively. In the spiral test, the loop width was unstable at +0.3° and -0.5°-0.9° around the midship of flap rudder. As a result, course stability was comparatively good. From the sea trial results, training ship ARA met the present criterion in the standards of maneuverability of IMO.

  • Purpose: This study aimed to identify correlations between the status of the umbilical cord and neonatal health status. Methods: In total, 172 newborns were enrolled who were admitted to the newborn nursery with a gestational age of 35 weeks or older and a body weight of 2 kg or above. Data were collected on the basic personal information of the newborns, the diameter and soft tissue status of the umbilical cord, and neonatal health status after birth. Analyses were performed using t-test, analysis of variance, χ2 test, and Fisher exact test. Results: Umbilical cord diameter exhibited a statistically significant difference by sex (t=2.71, p=.007). A thin umbilical cord diameter was associated with a 1-minute Apgar score less than 8 points (t=2.47, p=.015) and with being transferred to the intensive care unit (t=2.45, p=.015). Poor soft tissue status of the umbilical cord was associated with a 1-minute Apgar score of less than 8 points (χ2=16.68, p<.001) and with oxygen being supplied (χ2=4.81, p=.028). Conclusion: Assessing the umbilical cord diameter and status in newborns is an important tool for evaluating neonatal health status after birth, and this point also underscores the importance of professionals' careful observations in the newborn nursery.

Determination of The Growth Performances and Meat Quality of Broilers Fed Saccharomyces cerevisiae as a Probiotic in Two Different Feeding Intervals

  • This study was conducted to determine the effect of Saccharomyces cerevisiae (SC) as a dietary probiotic and evaluated the most suitable feeding interval for this probiotic on growth performance, carcass yield, and meat quality parameters in broiler chickens. In total, 1,050 one-day-old Cobb 500 chicks were randomly assigned to one of seven dietary treatment groups, in a $2{\times}3$ factorial arrangement with 3 SC dosages (0.6%, 1%, and 1.4%) and two feeding intervals [long term (LT) for 35 days and short term (ST) for 28 days after hatching], with a negative control diet (NC; 0% SC). Triplicate experiments were performed with 50 birds per cage. Broilers fed a diet including SC showed increased (p<0.01) daily gain and feed efficiency compared to the control. Further, broilers fed the 1.4% SC supplemented diet showed a significantly increased (p<0.01) average daily gain (ADG) and feed conversion ratio (FCR) compared to broilers fed the 0.6% and 1% SC incorporated diets. Similarly, broilers fed an LT SC diet showed a greater (p<0.01) increase in ADG and FCR compared to broilers fed an ST SC diet. Moreover, broilers fed an LT SC diet displayed a reduced (p<0.05) meat pH, gizzard weight, and increased (p<0.05) meat water-holding capacity compared to broilers fed an ST SC diet. Broilers fed the 1.4% SC supplemented diet showed increased (p<0.05) thigh muscle weight compared to broilers fed the 0.6% and 1% SC supplemented diets. In conclusion, broilers fed LT SC diets showed improved growth performance and carcass quality parameters compared to broilers fed ST SC diets, and the NC diet, from hatching to day 35.

  • Purpose: To evaluate effectiveness of arthroscopic capsular release combined with manipulation in frozen shoulder. Material and Method: 15 patient who had failed to respond to physical therapy were evaluated, which were treated with arthroscopic capsular release combined with manipulation, from July 1998 to March 2000. Result: At a mean of fifteen months(range, six to twenty four) after the combined procedure, the improvement in the score of Constant and Murley averaged 45 points. The mean improvement in motion was 76 degrees for abduction; 40 degrees and 65 degrees for external rotation in adduction and abduction. Conclusion : Arthroscopic capsular release with manipulation is useful method to treatment the frozen shoulder which was not respond to conservative treatment.

Comparison of the efficacy of erector spinae plane block according to the difference in bupivacaine concentrations for analgesia after laparoscopic cholecystectomy: a retrospective study

  • Background: Laparoscopic cholecystectomy (LC) is a noninvasive surgery, but postoperative pain is a major problem. Studies have indicated that erector spinae plane block (ESPB) has an analgesic effect after LC. We aimed to compare the efficacy of different ESPB anesthetic concentrations in pain control in patients with LC. Methods: This retrospective study included patients aged 20 to 75 years scheduled for LC with the American Society of Anesthesiologists physical status classification I or II. ESPB was administered using 0.375% bupivacaine in group 1 and 0.25% in group 2. Both groups received general anesthesia. Postoperative tramadol consumption and pain scores were compared and intraoperative and postoperative fentanyl requirements in the postanesthesia care unit (PACU) were measured. Results: Eighty-five patients were included in this analysis. Tramadol consumption in the first 12 hours, second 12 hours, and total 24 hours was similar between groups (p>0.05). The differences between postoperative numeric rating scale (NRS) scores at rest did not differ significantly. The postoperative NRS scores upon bodily movement were not statistically different between the two groups, except at 12 hours. The mean intraoperative and postoperative fentanyl requirements in the PACU were similar. The difference in the requirement for rescue analgesics was not statistically significant (p=0.788). Conclusion: Ultrasound-guided ESPB performed with different bupivacaine concentrations was effective in both groups for LC analgesia, with similar opioid consumption. A lower concentration of local anesthetic can be helpful for the safety of regional anesthesia and is recommended for the analgesic effect of ESPB in LC.

Residues of Ampicillin and Amoxicillin in Olive Flounder Paralichthys olivaceus Following Oral Administration (양식 넙치(Paralichthys olivaceus)에 경구투여한 Ampicillin 및 Amoxicillin의 어류체내 잔류량의 변화)

  • Residues of ampicillin and amoxicillin in the muscles of olive flounder Paralichthys olivaceus were investigated after oral doses of 20 mg/kg body weight/day of ampicillin and 40 mg/kg body weight/day of amoxicillin in a first examination and 40 mg/kg body weight/day of ampicillin and 80 mg/kg body weight/day of amoxicillin in a second examination for 5-6 days. The ranges of seawater temperatures in the $1^{st}$ and $2^{nd}$ examinations were $13.4-15.6^{\circ}C$ and $16.8-21.4^{\circ}C$, respectively. Ampicillin and amoxicillin concentrations were analyzed by high performance liquid chromatography (HPLC). The average recovery rates of ampicillin and amoxicillin in fish samples ranged from 84.8-95.2% and 100.8-103.8%, respectively. Residual concentrations in the olive flounder peaked on the $5^{th}$ day, with average concentrations of 0.075 mg/kg and 0.311 mg/kg in the $1^{st}$ examination, and 0.098 mg/kg and 0.630 mg/kg in the $2^{nd}$ examination for ampicillin and amoxicillin, respectively, with maximum concentrations in muscle. In the $1^{st}$ examination, ampicillin concentrations in olive flounder after withdrawals of 1 and 3 days were 0.041 mg/kg and 0.023 mg/kg, respectively, and amoxicillin concentrations after withdrawals of 1 day and 10 days were 0.172 mg/kg and 0.023 mg/kg, respectively. In the $2^{nd}$ examination, the ampicillin concentrations in olive flounder after withdrawals of 1 and 3 days were 0.041 mg/kg and 0.023 mg/kg, respectively, and amoxicillin concentration after withdrawals of 1 day and 10 days were 0.172 mg/kg and 0.023 mg/kg, respectively. We suggest that the recommended withdrawal periods should be 3 days for ampicillin and 10 days for amoxicillin in the olive flounder.

The Anxiety and Discomfort of Platelet Donors (혈소판 공여자의 불안과 불편감 - 첫 공여자를 중심으로 -)

  • This study was designed to evaluate the degree of the anxiety and discomfort experienced by first-time donors in platelet donation. The subject of this study was 309 first-time platelet donors visiting apheresis unit of St. Mary hospital in Seoul. The data was collected from Questionnaire during period of 28 April to 10 September 1999. The statistical methods used were t-test. ANOVA and stepwise multiple regression. The results were as follows. 1. The most common group were 21-30 years of age and soldier of job. 2. The mean scores of the platelet donor's anxiety was $1.60\pm0.32$. 3. The mean scores of platelet donor's discomfort was $1.72\pm0.44$. 4. The variables influencing anxiety were the physical condition. the level of education and donation experience. 5. The variables influencing discomfort were the job. diet before donation and physical condition. As the result above. the student group of the first time platelet donor experienced highest anxiety. It is important to assess the physical condition and diet before donation for more comfortable platelet donation. Furthermore. the developments of nursing interventions are required to reduce anxiety and discomfort of platelet donors.

Factors Related to Motor Subtypes of Delirium Patients in a Surgical Intensive Care Unit (일 종합병원 외과 중환자실 환자의 섬망 아형별 관련 요인)

  • Purpose: The purpose of this study was to investigate the motor subtypes of delirium in patients in a Surgical Intensive Care Unit (SICU), and identify the factors related to the characteristics of patients according to the motor subtypes of delirium. Methods: Data were collected in the SICU of a tertiary hospital in city from October 2018 to June 2019. Delirium was detected using the Confusion Assessment Method for the ICU (CAM-ICU) and motor subtypes of delirium were measured with the Delirium Motor Subtype Scale (DMSS)-4. Patients' characteristics were obtained by using the electronic medical records. Descriptive statistics were used to analyze the data. Results: Among 1,112 patients, 172 patients showed delirium (15.5%). After excluding dementia patients and patients refusing to participate in the study, 126 patients included in the final analysis. Delirium patients were classified as hyperactive delirium (32.5%), hypoactive delirium (42.9%), mixed delirium (11.9%), and non-motor subtype delirium (12.7%). Conclusion: The study results suggest that hypoactive delirium is the most prevalent motor subtype of delirium in SICU. More application of ventilators, more administration of sedatives, more use of catheters, and higher nursing severity were reported for hypoactive delirium cases than hyperactive ones. Therefore, it is necessary to assess early the motor subtypes of delirium using structured tools and develop appropriate nursing interventions suitable for each subtype of delirium.
