Spontaneous Bilateral Supratentorial Subdural and Retroclival Extradural Hematomas in Association with Cervical Epidural Venous Engorgement

  • We describe a case of 36-year-old man who presented with a subacute headache preceded by a 1-month history of posterior neck pain without trauma history. Head and neck magnetic resonance imaging (MRI) studies disclosed bilateral supratentorial subdural and retroclival extradural hematomas associated with marked cervical epidural venous engorgement. Cerebral and spinal angiography disclosed no abnormalities except dilated cervical epidural veins. We performed serial follow-up MRI studied to monitor his condition. Patient's symptoms improved gradually. Serial radiologic studies revealed gradual resolution of pathologic findings. A 3-month follow-up MRI study of the brain and cervical spine revealed complete resolution of the retroclival extradural hematoma, disappearance of the cervical epidural venous engorgement, and partial resolution of the bilateral supratentorial subdural hematoma. Complete resolution of the bilateral supratentorial subdural hematoma was confirmed on a 5-month follow-up brain MRI. The diagnosis and possible mechanisms of this rare association are discussed.

Clinical Results of Mitral Valve Replacement with St. Jude Medical Valve (St. Jude Medical 판막에 의한 승모판치환술의 임상성적)

  • A total of 172 cases of MVR using the St. Jude Medical valve was conducted in the period from August 1986 to May 1996. The hospital mortality rate was 3.5% (n=6) and the late mortality rate was 3.3% (n=5). According to the follow-up of 161 surviving patients, the average length of survival was 50.23$\pm$0.27 months. Three cases of prosthetic valve related complication deaths were identified. Two cases could be ascribed to left atrial thrombi and resulting cerebral infarction, and one case was prosthetic valve endocarditis. Two cases were caused by hemorrhagic complications that we presume to have been accompanied by anti-coagulation therapy. The actuarial survival rate of all cases at 10 years was 92.3%. We conclude that good clinical results and a low complication rate could be achieved through mitral valve replacement with the St. Jude Medical valve. We also conclude that mid-term and long-term follow-ups were instrumental and necessary.

Foraminoplastic Superior Vertebral Notch Approach with Reamers in Percutaneous Endoscopic Lumbar Discectomy : Technical Note and Clinical Outcome in Limited Indications of Percutaneous Endoscopic Lumbar Discectomy

  • To describe the details of the foraminoplastic superior vertebral notch approach (FSVNA) with reamers in percutaneous endoscopic lumbar discectomy (PELD) and to demonstrate the clinical outcomes in limited indications of PELD. Retrospective data were collected from 64 patients who underwent PELD with FSVNA from August 2012 to April 2014. Inclusion criteria were high grade migrated disc, high canal compromised disc, and disc protrusion combined with foraminal stenosis. The clinical outcomes were assessed using by the visual analogue scale (VAS), Oswestry Disability Index (ODI) and modified MacNab criteria. Complications related to the surgery were reviewed. The procedure used a unique approach, using the superior vertebral notch as the target and performing foraminoplasty with only reamers under C-arm control. The mean age of the 55 female and 32 male patients was 52.73 years. The mean F/U period was $12.2{\pm}4.2$ months. Preoperative VAS ($8.24{\pm}1.25$) and ODI ($67.8{\pm}15.4$) score improved significantly at the last follow-up (VAS, $1.93{\pm}1.78$; ODI, $17.14{\pm}15.7$). Based on the modified MacNab criteria, excellent or good results were obtained in 95.3% of the patients. Postoperative transient dysthesia (n=2) and reoperation (n=1) due to recurred disc were reported. PELD with FSVNA could be a good method for treating lumbar disc herniation. This procedure may offer safe and efficacious results, especially in the relatively limited indications for PELD.

Molecular Epidemiology of Bacillus cereus in a Pediatric Cancer Center (소아 암 환자에서 발생한 Bacillus cereus 균혈증의 분자역학 분석에 관한 연구)

  • Purpose: Bacillus cereus has been reported as the cause of nosocomial infections in cancer patients. In our pediatric cancer ward, a sudden rise in the number of patients with B. cereus bacteremia was observed in 2013 to 2014. This study was performed to investigate the molecular epidemiology of increased B. cereus bacteremia cases in our center. Methods: Pediatric cancer patients who developed B. cereus bacteremia were identified from January 2001 to June 2014. The B. cereus bacteremia in this study was defined as a case in which at least one B. cereus identified in blood cultures, regardless of true bacteremia. Available isolates were further tested by multilocus sequence typing (MLST) analysis. A retrospective chart review was performed. Results: Nineteen patients developed B. cereus bacteremia during the study period. However, in 2013, a sudden increase in the number of patients with B. cereus bacteremia was observed. In addition, three patients developed B. cereus bacteremia within 1 week in July and the other three patients within 1 week in October, respectively, during emergency room renovation. However, MLST analysis revealed different sequence types without consistent patterns. Before 2013, five tested isolates were ST18, ST26, ST177, and ST147-like type, and ST219-like type. Isolates from 2013 were ST18, ST73, ST90, ST427, ST784, ST34-like type, and ST130-like type. Conclusions: MLST analyses showed variable ST distribution of B. cereus isolates. Based on this study, there was no significant evidence suggesting a true outbreak caused by a single ST among patients who developed B. cereus bacteremia.

  • This paper presents the understandings carried out for the installation of the hydrogen reservoir of the multi-gap pseudospark switch under developing for the accelerator applications. As a cold cold cathode switch, the pseudospark switch could replace the thyratron switch which has hot cathode and being used well currently in the high power field such as laser and accelerator applications. Especially in the klystron modulator, the key component is a switch which mostly defines the jitter and the instability of the modulator system. To get the less jitter and the instability, we need to find proper range of the pressure for the gas discharge inside gas switch. This could be achieved by the understanding of the characteristic of the nonevaporable getter (NEG) which is used as a hydrogen reservoir for the switch. Therefore we verified the characteristics of the NEG (St 172, Saes) and its installation in the switch. Finally we controlled the getter to find best pressure point for the pseudospark switch.

LI-RADS version 2018 in Patients with Prior History of Hepatocellular Carcinoma: Are LR4 Observations Enough for the Diagnosis of Recurrent HCC?

  • Purpose: We evaluated the diagnostic performance of LI-RADS version 2018 using gadoxetic acid enhanced MRI for recurrent but untreated HCC in patients with prior history of HCC. Materials and Methods: We enrolled 50 consecutive patients who 1) prior history of treatment of HCC, 2) underwent liver surgery for radiological/clinical diagnosis of new HCC between 2013 to 2018, 3) had gadoxetic acid enhanced MRI within one month before surgery, and 4) did not have more than five HCCs or infiltrative tumors only. Two radiologists reviewed MRI and determined the presence of LR3, LR4 and LR5 observations except previously treated tumors based on LI-RADS version 2018 in consensus. We sub-classified LR4 into LR4m (LR4 with major features only) and LR4u (LR4 upgraded from LR3 by ancillary features). LR4u were further sub-classified into LR4ua (with arterial phase hyperenhancement) and LR4un (without arterial phase hyperenhancement). Results: PPV for LR5, LR4 and LR3 observations for recurrent HCC were 100%, 61.5% and 25.0%, respectively. 100% (3/3) of LR4m were HCC. However, PPV of LR4u was 56.5%. PPV of LR4ua and LR4un were 73.3% and 25.0%, respectively. Sensitivity of LR5 and LR5+LR4 observations as a diagnostic threshold were 32.1% and 89.3%, respectively. Sensitivity for LR5+LR4m+LR4ua observations for diagnosis of HCC were 83.7% and significantly superior to that of LR5 without significant deterioration of specificity (75.0%). Conclusion: In patients with prior history of HCC, LR4 observations by major features or with APHE may be regarded as recurrent HCCs given high sensitivity and comparable specificity/PPV to LR5 observations.

Secondary Septic Arthritis Due to Olecranon Bursitis -A Case Report- (주두 점액낭염(olecranon bursitis)에서 발생한 2차적 화농성 관절염(septic arthritis) -증례 보고 1례-)

  • Olecranon bursitis rarely Progresses to septic arthritis. In our case, the 24 year old woman was visited due to progressing right elbow pain, despite antibiotic treatment of chronic olecranon bursitis caused by elbow laceration 2 months ago. Pus draining sinus, localized heating and swelling could be seen on physical examination. Septic arthritis and pathologic fracture was diagnosed under arthroscopic examination. Arthroscopic irrigation and synovectomy for elbow joint, olecranon bursectomy and curettage of olecranon bone was done. In the operation field, the elbow and draining sinus over olecranon was communicated each other on saline irrigation test. The patient was treated for 3 weeks with intravenous antibiotics. At postoperative 4 weeks, bone graft was done. The possibility of chronic osteomyelitis and septic arthritis must be considered in a patient with chronic olecranon bursitis.

The Effects of Walking Exercise on Blood Glucose Level and Motivation of Exercise in the Diabetes Mellitus Patients (보행운동이 당뇨환자의 혈당 및 운동에 대한 동기에 미치는 효과)

  • This study was performed to explore the effects of walking exercise on blood glucose level in the diabetes mellitus patients. The design of this study was a equivalent pre-post test experiment. The subjects consisted of eighteen diabetes mellitus patients. The walking exercise consists of 22.5minutes, speed of 120walking per minute. Data were analyzed with mean, standard deviation, Wilcoxon signed rank test, Spearman correlation and percentage using SAS program. The results were as follows. 1. The blood glucose level has significantly decreased after walking exercise. 2. Strength of exercise and change of blood glucose level was revealed correlation. 3. Age and strength of exercise was revealed high correlation. 4. Satisfaction after walking exercise was revealed all patients. In conclusion, the walking exercise can decrease blood glucose level. This exercise was show a positive effect on the exercise compliance.
