폐질환에 있어 세포검사와 조직검사의 진단 일치율 및 정확도에 대한 조사 연구 (Diagnostic Accordance Rate and Accuracy Between Cytological and Histological Test in Lung Disease)

  • Lung cancer is a type of cancer with high mortality; its 5-year survival rate is at a low 14%. Related cytological tests include sputum, bronchial brushing, bronchial washing and fine needle aspiration cytology test etc. From the test specimens in which sputum, bronchial brushing, bronchial washing, and fine needle aspiration cytology were performed, the sensitivity, specificity and accuracy between cytology test and histology test. In the sputum test, sensitivity was 27.71% and specificity was 98.02%, and the bronchial brushing test showed sensitivity of 93.33% and a specificity of 91.3%. The bronchial washing test was a sensitivity of 53.7% and its specificity was 98.9%, and the fine needle aspiration cytology test showed sensitivity and specificity were 88.46% and 72.97%, respectively. In the specimens diagnosed as normal at the sputum test, malignant diagnosis was found in 21 specimens of bronchial brushing, 30 cases of bronchial washings and 37 cases of fine needle aspiration cytology specimens. In the specimens diagnosed as normal at the bronchial washing test, malignant diagnosis was found in 5 specimens of sputum, 7 specimens of bronchial brushin and 1 cases of fine needle aspiration cytology. One specimens found to be normal in fine needle aspiration cytology turned out to maligant in sputum test. The result of this research shows that, in diagnosis lung cancer, a test method of high sensitivity and specificity should be pursued. However, depending on the location and malignancy of the illness, diagnosis may not be obtained in some cases. Therefore, we conclude that the cytological tests performed for lung cancer testing such as sputum, bronchial brushing, bronchial washing, and fine needle aspiration cytology should be carried out in a mutually complementary manner.

Comparison of Liquid-based Cytology and Conventional Smear in the Evaluation of Sputum Cytology

  • The cytology of sputum specimens, at a medical facility in Korea, is using the liquid-based cytology and conventional smear method. It confirms the diagnosis using conventional smear methods and the liquid-based cytology. In addition, the diagnosis uses two kinds of each method, compares the suitability, sensitivity and specificity for each test and tries to understand the efficient method needed. It divided sputum specimens in half and liquid-based cytology. A conventional smear method was conducted and the Papanicolaou's staining was conducted using Autostainer. Diagnosed each slide of staining produced smear slides specimens by using an optical microscope. The result of the liquid-based cytology and conventional smear method was that the liquid-based cytology was of a higher-grade than the conventional smear method. It was 36.8%, and 62.8% in the same condition. But only one case was of a lower grade. Liquid-based cytology showed 86.2% of adequacy, 31.0% of sensitivity and 97.5% of specificity. The conventional smear method showed 54.4% of adequacy, 19.6% of sensitivity and 100% of specificity. If it was conducted with two methods at the same time, the results would be similar to the liquid-based cytology. In conclusion, liquid-based cytology has a more superior adequacy and sensitivity than the conventional smear method. The combination of the two methods is similar to the result of only conducting a liquid-based cytology. Conducting a liquid-based cytology rather than the combination of the two ways of sputum cytoscopy, is to be considered as an efficient method to achieve diagnostic accuracy, reduce labor of clinical technologist and pathologist, and to reduce the expense of patients.

폐암의 세포병리학적 진단에 관한 검토 (Evaluation of Cytopathologic Diagnosis of Lung Carcinoma)

  • In order to evaluate the role of cytopathologic diagnosis of sputum, bronchial washing and bronchial blushing in the diagnosis of lung cancer, we performed this study. The patients included in this study had undergone sputum, bronchial washing and brushing cytology over the 20-month period of 1985 through 1987. The total number of specimens was 5,495 of 2,242 patients, including 4,830 sputa and 665 bronchial washing and brushings. The average number of sputa and bronchial washings and brushings per case was 2.4 and 1.2 respectively. Among them, about 10% were unsatisfactory specimen, and three-fourths were negative specimens. In sputum cytology, the diagnosis of "atypical cells" was given to 3%, "suspicious for malignancy" was given to 1 %, and "malignancy" was given to 13%. In bronchial washing and brushing cytology, the diagnosis of "atypical cells", "suspicious for malignancy" and malignancy" was given to 6%, 3%, and 20% respectively The cases diagnosed as "atypical cells" in cytology were actually malignancy in 95% and 84.8% of sputum and bronchial washing and brushings respectively, and the "suspicious for malignancy" were actually malignancy in 100% in both methods. The detection rates of malignancy were 504% and 55.2% in sputum and bronchial washing and blushing respectively, and the specificity was 100% in both methods. The accuracy of cell typing was 92% in sputum and 89.7% in bronchial washing and blushing.

폐암 진단방법으로서 객담검사의 유용성 (Usefulness of Sputum Cytology as a Diagnostic Tool of Lung Cancer)

  • To analyze the accuracy and usefulness of sputum cytology as a screening method, 103 cases of histologically proven lung cancer registered from 1998 to 2000 at Kangbuk Samsung Hospital were retrospectively examined. We reviewed the original cytologic and surgical diagnoses for the cases, and the cytology slides of all cytologically negative cases. The overall sensitivity of sputum cytology was 0.83 ; the sensitivity of prebronchoscopy sputum cytology for bronchogenic carcinoma was 0.87. Central tumor location (P=0.002), tumor size (>2.4 cm), (P=0.027) and the number of sputum samples $(\geq3)$ (P=0.001) were associated with a positive cytologic diagnosis. Of the 18 cytologically negative cases, 9 cases(38% of smears) were determined to be insufficient for diagnosis, due strictly to low cellularity and saliva. After a review of the cytology slides of cytologically negative cases, we identified several atypical clusters in one case of bronchioloalveolar carcinoma. This negativity was thus attributed to an interpretation error (1/18, 5.6%). Our results suggest that its sensitivity is more strongly related to the specimen adequacy and the times of sampling than to interpretation error. In terms of sensitivity, specificity, accessibility, cost, and morbidity associated with the screening tests, sputum cytology was found to be an accurate effective screening method for lung cancer.

객담검사에서 진단된 폐효모균증 - 1 례보고 - (A Case of Unsuspected Pulmonary Cryptococcosis in Sputum Cytology)

  • Cryptococcosis is a systemic mycosis that most often involves the central nervous system, followed by subclinical or symptomatic pulmonary involvement. We describe a case of unsuspected pulmonary cryptococcosis in a 69-year-old woman, diagnosed by sputum cytology. Sputum smears revealed numerous yeast like organisms typical of Cryptococcus neoformans measuring from 7 to 17 um in diameter. Mucicarmine, PAS, and methenamine sliver stains showed positive reaction of capsule and cell wall of Cryptococcus neoformans.

폐결핵증 객담의 세포병리학적 관찰 (Observation on Sputum Cytology in Pulmonary Tuberculosis)

  • Sputum smears of 116 cases in 55 pulmonary tuberculosis patients with the demonstration of acid-fast bacilli in sputum were retrospectively reviewed to investigate the cytologic diagnosis of pulmonary tuberculosis in sputum. Epithelioid cells, multinucleated giant cells of Langhans or nonspecific type, or necrotic materials of caseation or nonspecific nature were found in 40% of the cases, but in 60% of the cases, only nonspecific findings including squamous metaplasia were found. Only in 6% of the cases, the cytologic diagnosis of tuberculosis was possible with the findings of epithelioid cells and muitlnucleated giant cells in a necrotic background.

기관지경검사에서 객담세포검사의 암양성률에 대한 연구 (Cytologic examination sputum and bronchial secretion bronchofiberscopic sampling procedures in lung cancer)

  • The value of exfoliative sputum cytology in diagnosis of lung cancer has been accepted with bronchoscopic technique and results has been much improved by foreign investigators, but there was not presented accurate reports for sputum cytologic evaluation in Korea. We tried to find indicators of cytologic tests result in our hospital. During the period between May, 1980 and August, 1982, 400 patients, tested at Department of Thoracic and Cardiovascular Surgery of Kosin Medical College, had diagnostic bronchofiberscopic examination, and the cytologic study of sputum and bronchial secretions were performed. The sputum or bronchial secretion during bronchofiberscopic examination were obtained with various methods and the name of specimen were labeled as I, ASPIRATION SPUTUM, which was collected initially endobronchial sputum as introducing of scope, II, WASHING SPUTUM, which was collected washing bronchial secretion, III, BRUSHING SPUTUM, which was washing solution of brushing instrument and endobronchial sputum after brushing of lesions, IV, POST-SCOPIC SPUTUM, which was expectorated sputum as soon as removing of scopic instrument, V, ALL SPUTUM CYTOLOGY & CELL BLOCK, which was collected all specimen of above procedures. The diagnostic results of bronchofiberscopic examination was disclosed 174 cases [43.5%] of proved lung cancer, 47 cases [11.8%] of suspected lung cancer in grossly, and 179 cases [44.8%] of others finding except cancer. Patient with bronchofiberscopically grossly evidence of lung cancer which were not confirmed histologically or cytologically were excluded from this cytologic study. Histologic and cytologic correlation in proven lung cancer, 174 cases was revealed that number of cytologic positive patients were 45 cases [38.7%] among the 117 cases of proved squamous cell carcinoma, 12 cases [38.7%] among hislogically unknown cancer 34 cases and 6 cases [33.3%] among small cell undifferentiated carcinoma 18 Gases. Total cytologic positive result was presented as 67 cases [38.3%]. The other type of lung cancer, histologically, could not comparison because of small cases. The sequence of positive cytologic result in I-V specimen were disclosed as II, WASHING SPUTUM 57.6%, and V, ALL SPUTUM & CELL BLOCK 41.4%. The I, III & IV result were 28.6%, 22.2% and 26.1% respectively.

폐암진단을 위한 객담세포검사 결과 - 16개월간의 전주 예수병원 객담세포검사 결과를 바탕으로 - (Results of Sputum Cytology in Diagnosis of Lung Cancer - Based on the Results Obtained for 16 months in Presbyterian Medical Center -)

  • A prospective survey of sputum cytologic specimen was performed for 16 months from Jan. 1993 to Apr. 1994 in Presbyterian Medical Center. The purpose of this study is to find the positive rate of sputum cytology in the diagnosis of lung cancer and to correlate these results with tumor location and stage. Sputum cytologic specimen were received from 104 patients among 158 patients diagnosed as lung malignancy by histologic examination. Cytologic diagnosis of "suggestive of malignancy" was made in 61 patients (59%) and dysplasia in 9 patients(9%), atypia in 14 patients(13%), benign in 15 patients(14%) and inadequate specimen in 5 patients (5%), respectively. Among 84 patients beyond the cytologic diagnosis of atypia, 51 patients (61%) disclosed a central location, while 33 patients (39%) showed peripheral lesions. All 54 patients diagnosed as suggestive of non-small cell carcinoma were stage III or over, and all 7 patients diagnosed as suggestive of small cell carcinoma were in advanced stage. These results suggest that the cytologic examination of sputum seems to be an important tool in diagnosis of lung cancer.

폐암 진단에서 bronchofiberscopy 의 임상적 의의 (Clinical Evaluation of Bronchofiberscopic Examination in The Diagnosis of Cancer of the Lung)

  • Two hundred and seventeen patients underwent diagnostic rigid bronchoscopy or bionchofiberscopy to evaluate the cytologic diagnosis in the lung cancer patient at the department of chest surgery of Yon-Sei university, college of the medicine from 1971 to 1977 year. One hundred and twenty cases of these patients were taken rigid bronchoscopy and ninety four cases of these patient were taken bronchofiberscopy. Cytologic examination of the sputum was done in 214 cases and sputum cytology was positive in 50 cases [23.4%]. Rigid bronchoscopy was made in 120 cases and this bronchoscopic cytology including bronchial washing and bronchial biopsy was positive in 34 cases [28.5%]. Bronchofiberscopy was performed in 94 cases and was positive in 45 cases [47.5%]. Histopathologically, 41 cases [43.6%] were epidermoid cell carcinoma, 8 cases [8.5%]of undifferentiated cell type, 12 cases [12.8%]of adenocarcinoma, 8 cases [8.5%]of alveolar cell type, and the 25 cases were undetermined. Cytologic examination of the sputum lacks the accuracy of the bronchoscopies in terms of both localization and accurate histologic indentification of the type of neoplasm. Rigid bronchoscope has the advantage of permitting identification of a tumor in a central location and of providing a sufficient amount of biopsy material for accurate diagnosis of carcinoma. However, it has the disadvantage of limiting examination to the larger, more central portions of the tracheobronchial tree. Bronchofiberscope had the advantage of examine upper lobe as well as other portions of the tracheobronchial tree which could not be visualized with the rigid bronchoscopy. A positive diagnosis in bronchofiberscopy was obtained in the highest rate, 47. 8% [45 cases]. A1 last, if a bronchogenic carcinoma is suspected on the basis of either symptoms of an abnormality on the chest film the diagnostic work-up-sputum cytology, bronchial washing, bronchoscopic biopsy, scalene node biopsy, thoracentesis and mediastinoscopy explothoracotomy etc-should precede in an attempt not only to obtain the higher positive diagnosis but also to obtain a tissue diagnosis and to evaluate the stage of the disease and to ascertain the appropriate mode of therapy.

기관지암종 진단에 있어서 객담과 기관지 세척액 검사의 유용성 - 기관지경 검사를 통해 확진된 예를 대신으로 - (Diagnostic Sensitivity of Sputum and Bronchial Washing Cytology in Bronchogenic Carcinomas Confirmed by Bronchoscopic Biopsy)

  • To evaluate the role of sputum and bronchial washing for the diagnosis of lung carcinoma, we studied the sensitivity of both cytologic techniques using the biopsy confirmed cases from 228 patients. Among them, 123 cases were squamous cell carcinomas, 42 cases were adenocarcinomas, 48 cases were small cell carcinomas, one case was large cell carcinoma, and 14 cases were other types of carcinoma including poorly differentiated carcinomas. Three hundreds and ninety two sputa and 173 sputa were obtained in the pre- and post-bronchoscopic periods. Bronchial washing had been taken once in each patient. The overall sensitivity of the sputum cytology was 0.52 and that of the bronchial washing 0.63, while it increased to 0.83 when a combination of both techniques. Squamous cell carcinomas were diagnosed to the great extent in which sensitivities were 0.59 and 0.74, in sputum and bronchial washing, respectively. The post-bronchoscopic sputa showed higher sensitivity (0.44) than pre-bronchoscopic sputa (0.30). The sensitivity on sputa increased from 0.34 to 0.49 when three samples were examined compared to the single examination. The accuracy of cell typing was 94.0% in sputa and 93.8% In bronchial washing. Repeated sputum examination including post-bronchoscopic sputa is warranted to Improve sensitivity and a complementary role of both cytologlc techniques can be postulated by these data.
