  • This study was tested for the bending stress analysis of hand spl int for Hemiplegic Patients. Hemiplegia was represented the stroke and the cerebral palsy. Hand splints of four materials was an Orthoplast hand splint, a Polypropylene hand splint, a Yogips hand splint and an Aluminum hand splint. The Stress Analysis system was made by the electronic oscilloscope, strain gage sensors, amplifier, A/D converter, PC with C program. It will be able to using the important data in spl int design. The results of analysis were obtained various different graphes and maximum data in an Orthoplast hand splint, a Polypropylene hand splint, a Yogips hand splint and an Aluminum hand spl int.

Analysis on Constituent Elements and Microstructure of Fiberglass Splint and Cast

  • In this study, microstructural characteristics and constituent elements of fiberglass splint and cast are examined using a scanning electron microscope and an energy dispersive X-ray spectrometer. As observed by the scanning electron microscope, fiberglass splint and cast had a porous structure with many bundles of fiberglass textures well assembled. Spaces between bundles of the fiberglass splint are triangular or elliptical shaped and the long-axis diameter is measured at about 1 mm. The thickness of fiber bundles covered with plaster is measured at 600 ㎛ and the diameter of a single strand of fiberglass is up to 10 ㎛. The thickness of the fiberglass bundle of the fiberglass splint is measured at about 700 ㎛. Spaces between bundles are formed in the shape of triangles with gentle edges and long-axis diameter of up to 1.4 mm, which is larger than that of the splint. The thickness of a single strand of fiberglass of the plaster-coated cast is 11.5 ㎛, which is thicker than that of fiberglass of the splint. As a result of analyzing constituent elements of the fiberglass cast and the splint with an energy dispersive X-ray spectrometer, Ca, Si, and Al components are identically detected. This result shows that the fiberglass cast has a smoother surface with hardened plaster than the fiberglass splint. The thickness of the fiberglass bundle and the thickness of a single strand of the fiberglass are also larger than those of the fiberglass splint.

부정교합환자에서 교합안정장치의 사용이 하악과두의 위치변화에 미치는 영향에 관한 연구 (THE EFFECT OF OCCLUSAL SPLINT THERAPY ON CONDYLAR POSITIONAL CHANGES IN MALOCCLUSION PATIENTS)

  • There are evidences that occlusal splint therapy is critical to diagnose hidden akeleto-occlusal disharmonies in malocclusion patients and capable of enhancing stability after orthodontic treatment. In addition, evidences have implicated occlusal splint therapy in condylar positional changes during TMJ disorder treatment. In view of these evidences, this study was performed to investigate the effect of occlusal splint therapy on condylar positional changes in malocclusion patients and the possible clinical application of the occlusal splint as an additional orthodontic tool. For this study, 8 Angle's Class I malocclusion patients, who had centric occlusion-centric relation discrepancy within 1.0 mm and had no clinical symptoms of TMJ disorder, were selected as control group. And 22 malocclusion patients who had centric occlusion-centric relation discrepancy over 1.0 mm were selected and subdivided as Class I Malocclusion group, Class II div. 1 malocclusion group, Class II div. 2 malocclusion group, Open bite group, and Mandibular asymmetry group. For each subject the occlusal splint with mutually protected type of occlusal scheme was applied for 3 months. Condylar positions in centric relation and centric occlusion were measured using Panadent articulators and Panadent condylar position indicator (CPI) before and after occlusal splint therapy. On the basis of this study, the following conclusions might be drawn: 1, In control group, Class II div. 2 malocclusion group, and mandibular assymetry group, there were no significant differences in condylar positions before and after occlusal splint therapy. 2. In Class I malocclusion group, condyles were moved $0.27{\pm}0.45mm$ forward (p < 0.05) and $0.98{\pm}0.25mm$ upward (p < 0.01) after occlusal splint therapy. 3. In Class I malocclusion group, condyles were moved $0.24{\pm}0.21mm$ backward (p < 0.05) and $1.01{\pm}0.33mm$ upward (p < 0.01) after occlusal splint therapy. 4. In open bite group, condyles were moved $1.24{\pm}0.30mm$ upward (p < 0.01) after occlusal splint therapy. 5. In both control and experimental groups, there were no significant differences in lateral condylar positions before and after occlusal splint therapy.

두 사례에 의한 수부 스프린트 효과 (The EFFECT of HAND-SPLINT for 2 CASES)

  • 본 연구의 목적은 손 기능에 문제가 있는 환자들을 중심으로 splint의 효과를 알아보기 위한 기초조사로써 2명의 환자를 제주 한마음병원(Case1, 2)에서 2001년 6월부터 11월까지 조사하였다. 2명의 환자는 다음과 같다. i) 사례 1 : 뇌성마비 아동(4세, 남, 오른손)으로 한쪽 손목과 엄지손가락에 강직으로 인하여 잡기 기능에 제한이 있는 편마비 아동이다. ii) 사례 2 : 전기화상(56세, 남, 왼손)으로 4-5번째 손가락의 M.P joint와 I.P joint에 관절운동 제한으로 완전하게 주먹을 질 수가 없다. 연구를 위하여 손 기능의 평가는 표준화가 되어있는 Jebsen-hand function test, Total passive Motion(TPM)을 사용하였으며, 환자들의 만족도를 알아보기 위해서는 Canadian Occupational Performance Measure(COPM)을 사용하였다. i) 사례 1 : 두 가지 splint를 적용하였다. 즉 낮 동안에는 splint 1, 밤 동안에는 splint 2를 6시간 이상 착용하도록 하였으며 작업치료는 시행하지 않았다. ii) 사례 2 : 이 환자에게는 작업치료가 끝나는 동시에 splint. 3을 6시간 이상 착용하도록 하였으며, M.P joint와 I.P joint의 관절 변화를 조절하도록 특별히 고안된 splint를 적용하였다. 그 결과 사례 1의 Jebsen-hand function test시 초기에는 측정을 할 수가 없었으나, splint착용 후 크고 가벼운 물건 옮기기(44.15), 크고 무거운 물건 옮기기(42.66), 적목쌓기(44.63), 먹기 흉내내기(54.47) 등에서 처음 동작만 도와주면 측정이 가능할 수 있도록 진전을 보였다(그래프1), 사진(착용 전과 착용 후). 사례 2의 경우 Jebsen-hand function test의 적목쌓기와 먹기 흉내기가 가장 높았으며, 글씨 쓰기가 가능했다. 손의 관절가동 변화에서도 splint 착용전의 측정치와 착용 후 관절 가동 범위의 변화 폭이 약 $30_{\circ}$이를 보여주었다(그래프 2), (그래프3). 결론적으로 손 기능 회복에 직접적인 재활 치료뿐만 아니라, 치료 후에도 splint를 착용하여 가정에서도 계속적인 치료가 될 수 있도록 하는 것이 손의 기능회복에 도움이 됨을 알 수 있었다. 그러므로 각 환자의 문제점을 파악하여 적절한 splint를 제작해 줄 필요가 있다고 사료된다. 이 기초조사를 시작으로 하여 더 많은 환자들을 대상으로 splint가 손 기능 회복에 어떠한 영향을 미치는지 그 효과에 대하여 계속적인 연구가 필요할 것이다.

OCCLUSAL REPOSITIONING SPLINT에 의한 악관절음(顎關節音)의 치료(治療)에 관한 비교연구(比較硏究) (COMPARATIVE STUDY OF EFFECT OF STABILIZATION SPLINT AND ANTERIOR REPOSITIONING SPLINTS FOR CLICKING SOUND)

  • This study conducted 2 types of occlusal splint therapy to eliminate clicking sound. 15 patients who had clicking on their joints were selected, and divided, at random, into 3 groups. In the first group, 4 persons put on stabilization splint and in the second group, 6 persons put on anterior repositioning splint which had made condyle to protrude 3mm, and in the last group, 5 persons put on anterior repositioning splint which had made condyle to protrude 6mm. Patients who wore anterior repositioning splint were instructed to use the splint for all days. The evaluation of clicking was measured by occlusal soundscope. The clicking was converted to aucostic signal by the attached microphone, instead of vibrating sensor The in-put aucostic signal on the occlusal soundscope made it possible to observe the experiment's result. Anterior repositioning splint was produced in the centric occlusion state, when the model was mounted to articulator and inserted two pieces of 3mm and 6mm resin blocks each into the rear wall of articulator fossa. The observation of the patients who wore three different splints for 3 weeks has reached the following conclusions: 1. Stabilization splint produced no effect in eliminating the clicking sound. 2. Anterior repositioning splint therapy with 3mm condylar protrusion produced significant improvement in eliminating the clicking sound. 3. The 6mm protruded anterior repositioning splint caused pain on affected TMJ area as well as the clicking on unaffected joint.

  • If there are problems for us to sleep, we are faced with fatigue and dizziness in the day. Snoring and OSAS (obstructive sleep apnea syndrome) during sleeping are the main cause of sleep disorder. Treatments through surgical method and sleep splint can be performed to treat snoring and OSAS. Relapse of snoring and OSAS is common after treatment by surgical method. But, Recently sleep splint is frequently applied to treat snoring and OSAS with surgical treatment, because it is convenient and conservative. Sleep splint treat snoring and OSAS by ensuring airway through nose. As first step of fabrication occlusal bite is gained at a point that patient get feeling of increased nasal breathing in supined position, and next, the bite is transfered to sleep splint. This study surveyed the effect of sleep splint by questionnaire to the out-patients (the Dept. of Oral and Maxillofacial Surgery, Chonbuk National University Hospital) weared sleep splint and their partners, secondarily measured airflow through nose by aerophone II after wearing sleep splint and finally evaluated the effect of treatment of snoring and OSAS by sleep splint. The obtained result were as follows; 1. Though 'sleep splint' couldn't eliminate fundamental problems of snoring, it could improve the symptoms when patients were selected could using the 'Nakagawa's respiration method'. 2. Patients who used the sleep splint could breathe stably when patients are sleeping stably. Wearing a 'sleep splint' improved airflow by expanding the upper airway. 3. Even though sleep splint can be made with variable materials, the patients expressed the most satisfaction on the splint with '0.75mm hard shell'. 4. The 'Herbst' may allows the mandible to move the TMJ to relax. Nevertheless, some patients experienced a discomfort or irritation. 5. In Snoring and OSAS cases, it is recommended that patients should first explore non-surgical options prior to choosing a surgical treatment.

A Fully Digital Auricular Splint Workflow for Post-Keloid Excision

  • Ear keloids are challenging lesions to treat due to high recurrence rates postexcision. Conservative compression techniques as adjunct treatment have been reported to be effective. An innovative technique of using computer-aided design/computed-aided manufacturing to print a customized auricular splint improves efficiency and comfort level for patients compared with conventional methods. The ear is scanned using an intraoral scanning 2 weeks postsurgery. A two-piece auricular splint is designed on the digital model, incorporating perforated projections for three nylon screws for retention of the splint. The splint is printed with clear acrylic material, postprocessed, and finished. The patient is taught to assemble the components of the splint and instructed to wear for at least 8 hours daily. The surgery site reviewed for any ulceration, pain, or recurrence of keloid for 6 months. During the 6-month review, the excision scar remained flat and pink. The patient also reports unrestricted daily activities. The digital workflow increases comfort for the patient and reduces the number of hours required to produce a customized auricular splint compared with conventional methods. A fully digital workflow for a printed auricular splint should be considered for adjunctive treatment to excision of ear keloids.

Evaluation of the effect of two different occlusal splints on maximum occlusal force in patients with sleep bruxism: a pilot study

  • PURPOSE. The occlusal splint has been used for many years as an effective treatment of sleep bruxism. Several methods have been used to evaluate efficiency of the occlusal splints. However, the effect of the occlusal splints on occlusal force has not been clarified sufficiently. The purpose of this study was to evaluate the effect of occlusal splints on maximum occlusal force in patients with sleep bruxism and compare two type of splints that are Bruxogard-soft splint and canine protected hard stabilization splint. MATERIALS AND METHODS. Twelve students with sleep bruxism were participated in the present study. All participants used two different occlusal splints during sleep for 6 weeks. Maximum occlusal force was measured with two miniature strain-gage transducers before, 3 and 6 weeks after insertion of occlusal splints. Clinical examination of temporomandibular disorders was performed for all individuals according to the Craniomandibular Index (CMI) before and 6 weeks after the insertion of splints. The changes in mean occlusal force before, 3 and 6 weeks after insertion of both splints were analysed with paired sample t-test. The Wilcoxon test was used for the comparison of the CMI values before and 6 weeks after the insertion of splints. RESULTS. Participants using stabilization splints showed no statistically significant changes in occlusal force before, 3, and 6 weeks after insertion of splint (P>.05) and participants using Bruxogard-soft splint had statistically significant decreased occlusal force 6 weeks after insertion of splint (P<.05). There was statistically significant improvement in the CMI value of the participants in both of the splint groups (P<.05). CONCLUSION. Participants who used Bruxogard-soft splint showed decreases in occlusal force 6 weeks after insertion of splint. The use of both splints led to a significant reduction in the clinical symptoms.

외상성 치아모형에서의 레진 스플린트 시스템의 유연성 비교연구 (Flexibility of resin splint systems for traumatized teeth)

  • 목적: 본 연구의 목적은 탄성력이 높게 개발된 레진의 굽힘 강도를 평가하고, resin wire splint (RWS)와 유연성을 비교 평가하는 것이다. 재료 및 방법: 세가지 레진 G-aenial flo (GA, GC), Superbond (SB, Sun medical), G-fix (GF, GC)의 세 점 굽힘 강도는 ISO4049/2000에 따라 진행하였다. GA, SB, GF를 이용한 세개의 RS군과 GA와 교정용 016" SS wire를 이용한 RWS군간의 치아 동요도 비교는 영구치 모형을 이용하였다. 상악좌측중절치에 3도의 동요를 부여하고, 치아의 수평적인 동요도를 splint 전후에 각 3회씩 periotest vale로 평가하였다. Splint 전후 periotest value는 splinting 효과로 환산하여 평가하였다. 통계적인 평가는 one-way ANOVA와 turkey HDS test를 이용하였다(${\alpha}=.05$). 결과: SB와 GF가 GA에 비해 낮은 굴곡강도를 보였으나, 세가지 RS군은 RWS에 비해 높은 splinting 효과를 나타내었다 (P < .05). 결론: 제한된 본 연구에서 사용된 재료와 상관없이 RS는 RWS에 비해 높은 견고성을 보였으며, 이는 탈구치아 치료를 위한 flexible splint에 부적합한 것으로 보인다.

두부자세 및 교합장치에 따른 연하운동의 변화 (Effects of Head Posture and Occlusal Splint on Swallowing Movement)

  • This study was performed to investigate the effects of head posture and occlusal splint on the vertical dimension in mandibular rest position and swallowing. Thirty health dental students ware selected lot this study and BioEGNⓡ(Bioresearch Inc., USA) was used for measuring interocclusal distance during rest - swallowing - rest - tapping movement. This swallowing movements were observed in both normal head posture(NHP) and forward head posture (FHP). Thickness of occlusal splint was about 2mm at posterior molar area and even tooth contact were achieved on light biting. The four mandibular positions at which interocclusal distance measured were swallowing position, after swallowing position in which interocclusal distance was maximum, rest position follows swallowing, and tapping position after rest. Changes of distance in each position were measured for three mandibular planes, that is, sagittal, frontal, and horizontal plane, respectively. The results obtained were as follows : 1. In normal head posture, the mandible was raised 1.03mm without splint, and 0.77mm with splint on swallowing, and there was no significant difference between the two. In horizontal plane, however, mandible was displaced more anteriorly in both swallowing position and tapping position with splint. 2. In forward head posture, the mandible was less raised with splint on swallowing, but features in horizontal plane were almost same as those in normal head posture. 3. In natural dentition, significant difference between NHP and FHP were observed in horizontal plane trajectory for swallowing and tapping position. But the difference for same positions were observed in frontal trajectory with splint. 4. Total amount of mandibular movement of two groups classified with sagittal interocclusal distance of swallowing position generally showed significant difference between the higher and the lower height group in head posture without splint. 5. Correlationship among total amount of mandibular movement for three mandibular planes were observed between sagittal plane and horizontal plane, and between sagittal plane and frontal plane in head posture without splint.
