한국여성의 연령별 정상폐용적 측정 (The Lung Volumes of Korean Females)

  • The normal lung volumes were determined and subdivided under sitting position in 48 middle school girls, 49 high school girls and 44 house wives. All subjects were free of pulmonary and vascular diseases. The vital capacity was measured by Mckessons spirometer and the residual volume was determined by Rahn's three breathing method. 1. The lung volumes (BTPS) of middle school girls determined were: $RV\;0.59{\pm}0.11l\;FRC\;1.45{\pm}2.22l\;VC\;2.68{\pm}0.29l$ 2. The lung volumes (BTPS) of high school girls determined were: $RV\;0.83{\pm}0.19l\;FRC\;1.9{\pm}0.25l\;VC\;3.15{\pm}0.24l$ 3. The lung volumes (BTPS) of house wives determind were: $RV\;0.95{\pm}0.61{\ell}\;FRC\;2.1{\pm}0.25{\ell}\;VC\;3.06{\pm}0.29l$ 4. The calculated residual ratio $(RV/TLC{\times}100)$ were: $17.7{\pm}2.57%$ in middle school girls and $20.6{\pm}3.65%$ in high school girls and $24.0{\pm}2.31%$ in house wives 5. The functional residual ratio $(FRC/TLC{\times}100)$ were: $43.7{\pm}5.98%$ in middle school girls and $48.8{\pm}4.41%4 in high school girls and $52.6{\pm}5.38%$ in house wives. 6. The correlation coefficients between vital capacity and total lung capacity were r=0.96 in middle school girls and r=0.986 in high school girls and r=0.856 in house wives. 7. The regression equations were obtained follows: $TLC(l) =1.105{\times}VC+0.304$ (in middle school girls) $TLC(l) =1.551{\times}VC-0.902$ (in high school girls) $TLC(l) =0.999{\times}VC+0.954$ (in house wives)

  • Objectives The purpose of this clinical trial was evaluate the safety and efficacy of Mahwangjeongcheon-tang (MJT). Methods Twenty six volunteers were recruited as subjects. They were divided into two groups, which were MJT test group and placebo group. The investigated items related to the safety were the weight, the results of renal function and liver function. The investigated items related to the efficacy are the results of the spirometer, the clinical asthma measurement scale in oriental medicine-V (CAMSOM-V) and the quality of life questionnaire for adult Korean asthmatics (QLQAKA). Results There are no difference in the weight, the results of the renal functions and the liver functions between MJT test group and placebo group. There are the some differences in the result of the questionnaires related to the efficacy between MJT test group and placebo group. The significant difference were observed in the CAMSOM-V at the 4th visit. Conclusion MJT is the safe and effective herbal prescription that is useful to the asthma patients.

Pulmonary functions of patients with isolated mandibular fractures: a preliminary report

  • Objectives: The aim of this study was to evaluate pulmonary function in patients with mandibular fractures and to determine the pattern of pulmonary functions in these patients. Materials and Methods: This was a cross-sectional study of pulmonary functions in Nigerian non-smoking patients with isolated mandibular fractures managed at our health institution from December 2015 to June 2017. Forced vital capacity (FVC), forced expiratory volume in one second (FEV1), peak expiratory flow rate (PEFR), and ratio of FEV1 to FVC (FEV1/FVC) were measured for all participants using a portable spirometer just before treatment. The pulmonary indices were compared with the predicted reference values for Nigerians to determine the respiratory pattern. Results: Forty participants consisting of six females (15.0%) and thirty-four males (85.0%) with a female to male ratio of 1:5.7 were included in this study. The mean patient age was 34.5±13.1 years (range, 17-63 years). The mean FVC, FEV1, FEV1/FVC, and PEFR were 3.8±1.2 L, 3.0±1.0 L, 74.3%±13.8%, and 5.2±2.2 L/s, respectively. Comparison of data with predicted values revealed that 17 subjects (42.5%) had normal pulmonary function pattern while 23 subjects (57.5%) had features suggestive of obstructive and restrictive pulmonary function patterns. Conclusion: Isolated mandibular fractures presented with abnormal pulmonary function pattern.

들숨근 훈련과 테이핑 동시적용이 호흡의 근력, 지구력, 폐기능 향상에 미치는 영향 (Impact of Concurrent Inspiratory Muscle Training and Tape on Inspiratory Muscle Strength, Endurance and Pulmonary Function)

  • Purpose: The purpose of this study was to identify the effect of a kinesio tape on inspiratory muscle training(IMT) to improve muscle strength, endurance and pulmonary function. Methods: Healthy 20 males were divided into IMT group (control group) and IMT with tape group (experimental group). The same IMT program was applied to both groups using the Respifit S for four weeks, three times a week, a total 12 times. To exprimental group, kinesio tape was applied on the inspiratory agonist diaphragm and the accessory inspiratory muscle scalene, sternocleidomastoid, pectoralis minor. The inspiratory pulmonary muscle strength was measured by the maximal inspiratory pressure (PI max) and minute volume (MV) using the Respifit S and the pulmonary function were measured peak expiratory flow (PEF), forced vital capacity (FVC), forced expiratory volume in 1sec (FEV1), FEV1/FVC using the Spirometer and compared before and after. Results: Results showed that the PI max in the two groups increased significantly and experimental group increased more effectively than that of control group. However, only MV showed a significant increase in experimental group but was not significantly different between the two groups. PEF and FEV1/FVC are significantly increased in both groups, but they did not make much difference between two groups, and the FVC for the two groups did not increase significantly. FEV1 increased significantly only with control group, but did not make a difference with experimental group. Conclusion: These result show that the PI max value for experimental group increased significantly than that of control group. Therefore kinesio tape maximizes inspiratory muscle exercise effect on muscle strength improvement. However, because of the short experimental period and difficulty in subject control, increase values of the others did not show a significant difference. In other words, kinesio tape did not show maximizing the inspiratory muscle exercise effect to improve endurance and pulmonary function.

The Effect of Exhalation Breathing Exercise on Respiratory Synergist Muscle Activity and Pulmonary Functions in Patients with Forward Head Posture

  • Purpose: The aim of this study is to suggest an intervention method for clinical use in the future by analyzing the effect of breathing exercise on activity of sternocleidomastoid muscle and scalenus anterior muscle, which are respiratory synergist muscles, and pulmonary functions in patients with forward head posture. Methods: Prior to the experiment, 12 patients (experimental group) performed feedback exhalation exercise along with conventional deep neck exercise, and 11 subjects (control group) performed feedback deep neck exercise along with conventional deep neck exercise. The intervention programs were performed for 40 minutes once a day (three times a week for four weeks). Results: Before intervention, %RMS was measured for surface electromyography (sEMG), and FVC, FEV1, and FEV1/FVC were measured using a spirometer. After four weeks, these items were re-measured under the same condition and analyzed. In within-group comparison of the experimental group, activity of sternocleidomastoid muscle and scalenus anterior muscle showed a significant decrease (p<0.05)(p<0.001), and forced vital capacity (FVC) showed a significant increase (p<0.05). In within-group comparison of the control group, activity of sternocleidomastoid muscle and scalenus anterior muscle showed a significant decrease (p<0.05), and in between-group comparison, there were significant differences in activity of sternocleidomastoid muscle and FVC (p<0.05). Conclusion: Long-term forward head posture restrains exercise performance of the neck and leads to exercise avoidance of the neck during daily activities, thus restraint factors might be created even while breathing. To cut off this link, a constant effort is required and diversified research on the correlation between neck functions and breathing should be conducted.

Effects of Respiratory Muscle Strengthening Exercise on Respiratory Function and Activities of Daily Living in Stroke Patients

  • Purpose: This study aimed to demonstrate reduction in stroke symptoms by analyzing the changes in respiratory function and activities of daily living (ADL) after respiratory muscle strengthening exercise in patients who had a stroke and thereby, propose an efficient exercise method. Methods: Twenty patients with hemiplegic stroke were divided into two groups, with 10 patients in each. The control group (CG) received the traditional exercise therapy, and the experimental group (EG) received the traditional exercise therapy combined with expiratory muscle strengthening training. The training continued for 6 weeks, 5 days a week. Forced vital capacity (FVC) and forced expiratory volume at 1 second (FEV1) were measured with a spirometer, $SpO_2$ was measured with a pulse oximeter, and ADL were assessed by using the modified Barthel index (MBI). A paired t test was applied to compare the differences before and after the intervention, and an independent t test was used to compare the differences between the groups. The level of statistical significance was set as ${\alpha}=0.05$. Results: The changes in the FVC and FEV1 values within the group showed significant differences only in the EG (p<0.01). The between-group difference was statistically significant only for FVC and FEV1 in the EG (p<0.01). The changes in $SpO_2$ and MBI within the group showed significant differences only in the EG and CG (p<0.01). Between-group differences were statistically significant only for $SpO_2$ and MBI in the EG (p<0.05). Conclusion: The interventions with active patient involvement and combined breathing exercises had a positive impact on all the functions investigated in this study.

학동기 농촌 아동의 폐기능 정상 예측치와 호흡기 증상에 민감한 폐기능 지표 (Normal Predicted Values of Pulmonary Function of the Primary School Children in Rural Area and Sensitive Index of Respiratory Symptoms)

  • Pulmonary function tests were conducted on 815 healthy primary school children (390 males and 425 female) in rural area using Collins Survey Spirometer (Warren E. Collins, Inc., U.S.A.) with X-Y Recorder. Respiratory symptoms(cough, sputum, sore throat, chest pain, chest tightness, dyspnea, coryza) were surveyed by the interviews. Multiple regression analysis and regression diagnostics were done for prediction equations of FVC, $FEV_1,\;PEFR\;and\;FEF_{25-75%}$. FVC, $FEV_1,\;PEFR\;and\;FEF_{25-75%}$ values in 3 groups of children classified by the number of symptom were compared each other through standard variable value. FVC, $FEV_1,\;PEFR\;and\;FEF_{25-75%}$ showed highly significant correlation with age, height and weight. Prediction equations for FVC, $FEV_1,\;PEER,\;FEF_{25-75%}$ are functions of height only in both male and female children aged between 6 and 12 years old. PEFR showed a significant difference related with the number of symptom in female. These results suggest that the PEFR is sensitive PFT parameter in this study.

두부전방전위자세에서 두개척추각과 자세변화에 따른 폐활량의 차이 (Difference of Vital Capacity According to Cranio-Vertebral Angle and Posture Change of Forward Head Posture People)

  • Background: In this paper, the relationship between the Cranio-Vertebral Angle (CVA) and the vital capacity in each position is reviewed, and the vital capacity in the position is studied. Methods: This study targeted 20 non-smoking female students of U university, which is located in Gyeongju-si. To review the Forward Head Posture (FHP) of each subject, CVA was measured, and FVC, FEV1, and FEF (25-75%) were measured and analyzed using a spirometer. Subjects were ordered to exhale three times with ease and then inhale up to their total lung capacity. After then, they were requested to exhale longer than six seconds. Then the inspiration and expiration were repeated. The measurement was executed in three positions, including supine, prone, and sitting. In each position the measurement was repeated twice, and a one-minute break was given between each cycle, so it was measured six times in total. SPSS 14.0 for Windows was used to analyze the data. The subjects' general properties were analyzed using descriptive statistics, and the correlation between the angle and the respiration variable result in each position was analyzed. The result of the respiration variable in each position was analyzed using the one-way ANOVA, and then a Scheffe post-hoc comparison was executed. Results: According to the analysis result of the correlation between the angle and respiration variable in each position, the sitting position and FEF (25-75%) showed a positive correlation (P<0.05). The respiration variable in each position showed a significant difference in FVC (p<0.05), and the Scheffe post-hoc comparison differed in prone and sitting positions. Conclusion: To increase the FVC of FHP patients, different exercises for each position can be applied, and the result of this study can be utilized as background data for further research.

A comparison of vital capacity values and respiratory muscles activities on pelvic tilt position

  • Objective: The purpose of this study was to examine the effect on vital capacity (VC) and inspiratory muscle activation according to the anterior and posterior pelvic tilt positions. Design: One group pretest-posttest design. Methods: Twenty-six healthy adult men and women, age 19 to 27 years, volunteered to participate in this study. Forced vital capacity (FVC), and forced expiratory volume in 1 second ($FEV_1$) were measured by desktop spirometer in the pelvic positions during respiration, and muscle activation was recorded from sternocleidomastoid, upper trapezius, external intercostal, rectus abdominis, and external oblique muscles by surface electromyography (EMG) at the same time. EMG values were normalized by maximum muscle contractions (% maximum voluntary isometric contraction). Subjects were to breathe in as much air as possible and then exhale as quickly as possible in both anterior and posterior pelvic tilt positions. To measure lung capacity, inspiration was measured for 5 seconds and expiration was measured for 7 seconds with data collection taken place during the middle three seconds. Lung capacities were measured in each position three times. Results: For the results of this study, there was a significant increase in both FVC and $FEV_1$ values during the anterior pelvic tilting compared to the posterior pelvic tilting posture (p<0.05). The sternocleidomastoid, upper trapezius muscle, rectus abdominus and external oblique muscle activation was significantly increased during anterior pelvic tilt compared to the posterior pelvic tilt position (p<0.05). Conclusions: These findings suggest that pelvic anterior tilt position could be more effective for vital capacity and respiratory muscles activation during respiration.

Comparison of the Effects of Abdominal Bracing Exercises and Abdominal Hollowing Exercises on Lumbar Flexibility and Pulmonary Function in Healthy Adults

  • Background: Abdominal bracing exercise (ABE) and abdominal hollowing exercise (AHE) improve the lumbar flexibility and pulmonary function in various patients, yet the efficacy of ABE or AHE have not yet been evaluated. Objects: The purpose of this study was to compare the lumbar flexibility and pulmonary function during both ABE and AHE in healthy adults. Methods: The study included 40 healthy adults, who were randomly divided into the experimental group and control group, each with 20 subjects. All subjects performed ABE (experimental group) and AHE (control group). The lumbar flexibility such as trunk flexion test (sitting and standing position) and schober test and pulmonary function such as the spirometer including forced vital capacity (FVC) and force expiratory volume in one second ($FEV_1$) and chest circumference measurement (middle and lower chest) were measured, respectively. Two-way repeated analysis of variance was used to compare the lumbar flexibility and pulmonary function, respectively. Results: No significant effects of lumbar flexibility were observed on trunk flexion test from the sitting position (P=.478) and standing position (P=.096) in the ABE than in the AHE. However, the length of ABE was longer significantly than it of AHE (P=.024). No significant effects of lung function were observed on the FVC (P=.410) and $FEV_1$ (P=.072) in the ABE group than in the AHE group. And also, no significant effects of chest circumference measurement were observed on the inspiration (P=.468) and expiration (P=.563) in middle chest circumference and inspiration (P=.104) and expiration (P=.346) in lower chest circumference. Conclusion: This study indicated that the ABE is only more effective in lumbar flexibility by lumbar length difference than AHE in healthy adults.
